Abstract

Many of the CP angle tumors are acoustic neuroma, vestibular schowanoma or 8th nerve tumor. This kind of tumor is benign histologically. Big size ones can cause neurological symptoms such as cerebellar imbalance, edema and cranial nerves dysfunction. Acoustic neuroma is mostly unilateral and audio logical findings manifest a unilateral hearing loss. Although big size tumors can lead to bilateral audio logical symptoms which can affect the findings of hearing assessment. Here, a 31 year-old patient suffering right ear vestibular schowanoma have been reported. changes in left ear pure tone results, acoustic reflex measurements and ABR in addition to hearing loss in the right ear have been demonstrated.

Abstract

A 34-year-old female with a history of a sudden mild left-sided hearing loss and high-pitched tinnitus along with fullness in the same ear was referred to the audiology clinic. Hearing fluctuation and variability in other symptoms were also reported. Although no complain of any vertigo or balance disorder was mentioned, light headedness was implied. Differential diagnosis of endolymphatic Fistula and retro cochlear lesion was demanded. Audiologic and radiologic findings of this patient are presented in the article to discuss findings regarding the diagnosis of the suspected disorder.

Abstract

Vestibular Schowanoma is one of the most common Intracranial Tumors which has elicited from the Schwann cells in the upper part of vestibular branch of 8th nerve and can affect the cochlear branch too. This benign capsulated tumor  is mostly unilateral. Schowanoma is relatively slow growing.In the current article we have presented the case of a 51 year-old woman complaining Right ear Tinnitus accompanying with numbness of the right side of the face without any sign of vertigo or imbalance. MRI and Auditory Brain stem Responses (ABR) studies demonstrated right side vestibular Schowanoma next to cerebellopontine Angle (CPA). The audiological and imaging findings have been brought here in details.

Abstract

Arachnoid cysts are cerebrospinal fluid-filled sacs that are located between the brain or spinal cord and the arachnoid membrane, one of the three membranes that cover the brain and spinal cord. They are usually located within subarachnoid space with a membrane consisting of arachnoid cells and collagen fibers and are filled with CSF. It is naturally considered as a congenital cyst because of its common appearance in the infancy. Typical symptoms of an arachnoid cyst around the brain include headache, nausea and vomiting, seizures, hearing and visual disturbances, vertigo, and difficulties with balance and walking. MRI and CT can help in diagnosis. Arachnoid cysts are benign and the vast majority remains asymptomatic throughout life. If they are deemed to be causing symptoms then surgery can be contemplated. Here, we are aimed at describing a case of Arachnoid cysts involved cerebellopontine angle that has been reffered to audiology clinic in Tehran University of Medical Sciences. The experiments and results of radiographic techniques has been brought here.

Abstract

This case can be placed in a neurologic classification. The patient is a child with learning disability in school activities. He was first referred to an audiological clinic because of a central auditory processing disorder (CAPD). His mother has noticed several hearing problems and this led us to the evaluations for his central auditory processing disorder. He has problems in hearing speech in noise and speech processing and need his friends repeat words during communication. no vestibular disorder was noticed nor any localization problem. The child has a good progress in school and only suffered problems in reading tasks. Intelligence quotient(IQ) was also normal.

Abstract

Objective: OAE an audiologic study for diagnosis of exteracochlear auditory neuropathy.
Method & Material: Immitance audiometry, OAE and ABR and rehabilitation Intervention was performed for 3 children suffered from kernictrus.
Conclusion: 1) OAE in the from of screening, distorted product and transient evoked-otoacoustic emission has to be added to our audiology test battery. 2) OAE studies have their own merit when we are clinically involved with the diagnosis of especial clinical entities like neonatal hypoxia, kernicterus, multiple sclerosis, meningitides and meningoencephalitis. 3) Case selection for cochlear implantation needs a knowledge of patients' OAE results. 4) In the case of intracerebral auditory neuropathy the need for other rehabilitative measures will be better understood.

Abstract

Background: Facial schwannoma is a rare tumor arising from any part of the nerve. Probable symptoms are partial or facial weakness, hearing loss, visible mass in the ear, otorrhea, loss of taste, rarely pain, and sometimes without any symptoms. Patients should undergo a complete neurotologic history, examination with documentation of facial and auditory function, specially C.T. scan or M.R.I. Surgery is the only treatment option although the decision of when to remove facial schwannoma in the presence of normal facial function is difficult.
Case: A 19-year-old girl with all above symptoms in the right side except loss of taste is diagnosed having facial schwannoma with full examination, audiometric, and radiological tests. She underwent surgery. In follow-up facial function were mostly restored.
Conclusion: The need for careful assessment of patients with Bell's palsy cannot be overemphasized. In spite of the negative results if still there is any suspicoin, total facial nerve exploration is necessary.

Abstract

Background and Aim: Auditory neuropathy is an hearing disorder in which peripheral hearing is normal, but the eighth nerve and brainstem are abnormal. By clinical definition, patient with this disorder have normal OAE, but exhibit an absent or severely abnormal ABR. Auditory neuropathy was first reported in the late 1970s as different methods could identify discrepancy between absent ABR and present hearing threshold. Speech understanding difficulties are worse than can be predicted from other tests of hearing function. Auditory neuropathy may also affect vestibular function.

Case Report: This article presents electrophysiological and behavioral data from a case of auditory neuropathy in a child with normal hearing after bilirubinemia in a 5 years follow-up. Audiological findings demonstrate remarkable changes after multidisciplinary rehabilitation.

Conclusion: auditory neuropathy may involve damage to the inner hair cells-specialized sensory cells in the inner ear that transmit information about sound through the nervous system to the brain. Other causes may include faulty connections between the inner hair cells and the nerve leading from the inner ear to the brain or damage to the nerve itself. People with auditory neuropathy have OAEs response but absent ABR and hearing loss threshold that can be permanent, get worse or get better.

Abstract

Background: Fanconi anemia (FA) is an autosomal recessive genetic disorder wich characterized by progressive pancytopenia, multiple congenital anomalies,  increased susceptibility to acute myelogenous leukemia and epithelial cancers specially in head and neck and GUT. Characteristic feature of FA include short stature, café-au-laitspots, small eyes, mental retardation, skeletal and ear anomalies.
Case: A 23-year-old man who was a known case of FA since he was ten reffered to audiology clinic because of severe hearing loss. His initial diagnosis was pneumonia. Audilogic evaluation revealed  bilateral profound hearing loss.
Conclusion: One of the anomalies in FA are  ear anomalies. These included conductive haering loss, external auditory canal stenosis and auricular malformation and progressive sensoryneural hearing loss. In this report external auditory canal and tympanic membrane were normal. The result of tympanometry were type An and audiometry were bilateral profound hearing loss that is a rare finding  in FA patient.

Abstract

Background and Aim: The purpose of the present study was to investigate the effect of vestibulo-proprioceptive stimulations of sensory integration theory on the development of gross and fine motor, language and personal-social functions in a child with agenesis of the corpus callosum.

Case: We report a 10.5 month old boy with agenesis of the corpus callosum. The intervention was administered based on sensory integration theory an hour a week for 20 weeks. The exercise intervention consisted of proprioceptive and linear, sustained and low frequency vestibular stimulations on suspension device and physio roll. A Denver Developmental Screening- II and milestones skill testing was completed pre-intervention and monthly. Post-intervention, age of gross motor, fine motor adaptive, language, and personal-social functions significantly improved. Based on milestones skills, maintenance of gross motor functions (e.g. sitting and quadruped position) improved. The child could roll from side to side and released objects voluntarily. The reaction time to auditory stimulations became less than 2 seconds.

Conclusion: vestibulo-proprioceptive stimulations using the neuroplasticity ability of the central nervous system is effective for development of gross and fine motor, language, and personal-social functions. These exercises can be administered for a child with agenesis of the corpus callosum.

Abstract

Background and Aim: Asking questions is one of the basic skills for getting information. "What", "Who", "Where", "Why", "How much", "How, etc. are question words frequently used for asking questions. Studies on the sequence of acquisition of question words in English speaking children follow a unique sequence. "What" is the first question word used by these children. We aimed to study whether Persian speaking children follow a unique pattern in the acquisition and use of question words as well.
Case: Two normal Persian speaking children (a boy and a girl) who were sent to the kindergarten of Faculty of Rehabilitation Sciences, Iran University of Medical Sciences by their parents were investigated. Children's spontaneous speech from 12 to 36 months was videotaped, and question words were extracted from phonetic transcription of both children's speech. In order to ask a question, both children initially used rising tone with /m/ or /hum/ before saying the first question word. Two first question words were "what" and "where" in both children's speech samples. Asking about the time with the question word /when/ was not appeared in their speech samples until 36 months. These two Persian speaking children also used Wh-question sequentially.
Conclusion: The result of this study was consistent with the results of other studies on English speaking children.

Abstract

Background: Treacher-Collins syndrome is a congenital craniofacial disorder with multiple anomalies. This syndrome affects the maxilla, mandible, eyes, middle and outer ears, and soft palate. Conductive hearing loss due to the deformities of the middle and external ears is prevalent. The characteristics of this syndrome include multiple and serious threats to normal communication development in children. In this study, speech and language features of a Persian speaking child with this syndrome are presented.
Case: The case was an 8-year old girl with Treacher-Collins syndrome and bilateral moderate conductive hearing loss due to atretic canal. In language and speech assessments, moderate hypernasality, numerous compensatory errors and morphosyntactic deficits were observed. There were 13 phonemes that were incorrectly produced at least in one position. Besides, she used 22 types of phonological processes that were abnormal and disappear before the age of three in normal Persian speaking children.
Conclusion: Moderate hearing loss, velopharyngeal incompetency, malocclusion and dental anomalies, attention deficit/hyperactivity disorder (ADHD) and environmental factors resulted in severe speech and language disorders in this case. These disorders affected her academic performance as well. Moderate hypernasality, numerous compensatory errors, and excessive and abnormal use of phonological processes were not presented as prevalent characteristics of Treacher-Collins syndrome in other resources.

Abstract

Background: Phonemic awareness is one of the most important predictors of reading skills that has been taught by different procedures. One of the procedures is implementation of direct instruction in instruction of phonemic awareness. Current study is one of the unique studies in Iran that investigate impact of direct instruction in phonemic awareness on reading achievement of students with reading disorder.
Case: Three male second grade elementary students with reading disorder in a regular school in district six of the office of education in Tehran were selected. Multiple-baseline across subjects was selected as a research design. The following tests were used as diagnostic criteria: reading and dyslexia test and Wechsler intelligence scale for children-revised. Moreover, a reading inventory consisting of 100 words was developed by researchers to assess the reading ability of the subjects. Data were collected in three phases: baseline, intervention, and follow-up. During the intervention phase, the intervention strategies were used while during baseline and follow-up, data were collected without any intervention. Comparing three phases of the study, we may conclude that intervention package consisting of direct instruction of phonological awareness was an effective strategy in reading achievement of all three students. In addition, follow-up data indicated that the effects of the intervention procedures were stable across time.
Conclusion: Direct instruction of phonological awareness was effective in reading achievement of students with reading disorder in elementary school and increasing their abilities in reading.

Abstract

Background: Children usually produce their first words when they are 10-15 months old and go through simple to complex speech by passing some stages. One of the criteria for examining development of children's language is mean length of utterance (MLU). The main purpose of this study was calculating mean length of utterance and grammatical morphemes used by Farsi-speaking children during their 12-60th months of life.
Case: It was a longitudinal descriptive study. Every month during children's 12 to 60th months of life, 120-minute spontaneous speech samples of two children in kindergarten were videotaped and transcribed. The girl said her first word at the age of 12, and the boy said his first word at the age of 16 months. Combining words and constructing two-word utterances started at 18 and 20th months of the girl's and the boy's lives respectively. First grammatical morpheme appeared before the 24th month of children's lives and when mean length of utterance was lower than 2 morphemes. Singular verb suffixes were acquired sooner than the plural ones. Both children started using six subject identifiers of Farsi language before 36th month of their lives.
Conclusion: The speech development in Farsi-speaking children follows the same pattern as other children, starting from one-word utterances and complicates gradually by increasing the number of words, word combinations and using grammatical morphemes. The important point is that Farsi-speaking children started to use grammatical morphemes when their mean length of utterance was lower than two morphemes.

Abstract

Background: Hyperlexia is a super ability demonstrated by a very specific group of individuals with developmental disorders. This term is used to describe the children with high ability in word recognition, but low reading comprehension skills, despite the problems in language, cognitive and social skills. The purpose of this study was to assess the effectiveness of improving the semantic aspect of language (increase in understanding and expression vocabulary) on reading comprehension in an autistic child with hyperlexia.
Case: The child studied in this research was an autistic child with hyperlexia. At the beginning of this study he was 3 years and 11 months old. He could read, but his reading comprehension was low. In a period of 12 therapy session, understanding and expression of 160 words was taught to child. During this period, the written form of words was eliminated. After these sessions, the reading comprehension was re-assessed for the words that child could understand and express.
Conclusion: Improving semantic aspect of language (understanding and expression of vocabulary) increase reading comprehension of written words.

Abstract

Background: Balance disorders are common in patients following stroke. There are a number of physiotherapy modalities to treat balance impairments, one of which is vibration. The effects of vibration on balance disorders after stroke have not been investigated. This case report demonstrates the short-term effects of plantar vibration on the balance impairment of a patient with stroke.

Case presentation: A 72-years-old man with right hemiplegia resulted from stroke was admitted for vibration therapy after not responding to standard rehabilitation interventions. He complained of balance problems despite walking independently. Main studied outcomes were timed up and go test and posturography. Foot sensation, ankle plantar flexor spasticity, and the passive range of ankle dorsiflexion were assessed, as well. Vibration with a frequency of 100 Hz was introduced to the sole of the affected foot for 5 minutes. After intervention, all tests were improved compared to the baselines.

Conclusion: This case showed that the vibration was effective for improving balance disorder after stroke.
