Abstract

Background: The current article is aimed at evaluating speech recognition scores in hearing aid wearers to determine whether nonsense syllables are suitable speech materials to evaluate the effectiveness of their hearing aids.
Method: Subjects were 60 children (15 males and 15 females) with bilateral moderate and moderately severe sensorineural hearing impairment who were aged between 7.7-14 years old. Gain prescription was fitted by NAL method. Then speech evaluation was performed in a quiet place with and without hearing aid by using a list of 25 monosyllable words recorded on a tape. A list was prepared for the subjects to check in the correct response. The same method was used to obtain results for normal subjects.
Results: The results revealed that the subjects using hearing aids achieved significantly higher SRS in comparison of not wearing it. Although the speech recognition ability was not compensated completely (the maximum score obtained was 60%) it was also revealed that the syllable recognition ability in the less amplified frequencies were decreased. the SRS was very higher in normal subjects (with an average of 88%).
Conclusion: It seems that Speech recognition score can prepare Audiologist with a more comprehensive method to evaluate the hearing aid benefits.

Abstract

Materials and Methods: It is for the first time in Iran that designing and construction of phase-shifter instrument for performing Masking Level Difference have been studied. For reducing limitations of MLD test in middle ear pathologies, new methods of performing MLD has been considered.
In this method noise is conducted by bone vibrator, located on forehead. The comparison between standard method and proposed method has been obtained.Forty 20-25 year-old men referring to the Faculty of Rehabilitation Science in Iran university of Medical Sciences have been tested in the period of time between 10.30.1998 to 12.21.1998.The MLD test was performed in both method at frequencies of 250, 500, 1000, 2000 & 4000 Hz.
Results: The mean of MLD results n the standard method was 11.175 dB ranging from 10 to 13 dB that is in accordance with normative data.
- There was no significant difference between the means obtained in the methods for performing MLD.
- In both methods the MLD means decreased with frequency increments.
Conclusion: It is preferable to use proposal method in middle ear disorders due to normal cochlear sensitivity and subsequently enough noise that is conducted by bone conduction (60 dB SPL).

Abstract

Objectives: Introducing an approach for diagnosing the normal ABR independently or supplementary used to confirm the results of the other approaches.
Materials and Methods: This study was carried out in a descriptive manner on 31 male students of audiology in the Rehabilitation Faculty which were not chosen randomly. All of the samples aged between 18 to 25 years old with normal hearing and with no history of hearing disorder. They were tested in the winter of 1377 by ABR. The resulted of their ABR tests (62 curves) were in accordance with the convenient latency norms. After the preparation stage, the frequency analysis of the curves was considered through the Fast Fourier Transform function.
Results: considering the power and phase spectrum, three components are recognized, and phase for their frequency, amplitude and phase are determined.The normative values of the obtained amounts of the frequency, amplitude and phase for the frequency analysis results of the normal ABR curves are as follows:* First Component: Frequency: 340-530 Hz, Amplitude: 23-31 dB, Phase: 0-3 radian * Second Component: Frequency: 540-370 Hz, Amplitude: 20-30 dB, Phase: 0.3-2.7 radian * Third Component: Frequency: 750-1024 Hz, Amplitude: 19-30dB, Phase: 0-3.3 radian
onclusion: The results of the present study are exclusively valid for the society being considered, and their generalization requires more researches.

Abstract

Objectives :Determining the frequency distribution of hearing disorders among the student of public elementary school by Otoscopy, Puretone Audiometry, impedance Audiometry and questionnaires.
Methods and Materials: This study was can-led out in a cross - sectional descriptive survey - on 1000 students (500 girls and 500 boys) among the student of primary school of EslamAhad Gharh, academic year 1376-77
Results: 1- Otoscopy examination abnormal conditions of external ear canal was found in 13.65% of cases mostly impacted ceruman (13.3%). Abnormal conditions of external ear canal was more prevalent in girls than boys and the difference was statistically meaningful in the right (P V=0 .012) and left (PV=0.043) ear. Abnormal tympanic membrane was seen in 6.75% of cases mostly retraction (295%).
2- Impedance Audiometry: 11.05% abnormal tympanograms were observed, mostly type C (4.1%).
3- Pure Tone Audiometry: An overall 9.7% hearing loss was found in this population including 3.5% bilateral and
6.2% unilateral hearing loss. 4.15% of population suffered from SNHL observed mostly in boys and conductive. Hearing loss mostly in girls and the difference was statistically meaning in the left ear (PV=0.03).25.8% of cases were in need of medical care and surgery and six of them (l.e 0.6%) were in need of rehabilitation services.Family background showed on effects on the hearing disorders.Only 11. 4% of parents, 13.4% of teachers and 14.4% of afflicted students were aware of the problem.

Abstract

3- Pure Tone Audiometry: An overall 5.5% hearing loss was found in this population including 3% bilateral and 1.25% unilateral hearing loss. Only conductive hearing loss was found in this population (2.7% in right ear and 3.5% in left ear), PV=0.9.Hearing loss observed mostly in girls but the difference was slight.Family background showed no effects on the hearing disorders.9.8% of cases were in need of medical care and 0.3% were in need of rehabilitation services.Only 28.8% of parents, 36.3% of teachers and 40.9% of afflicted students were aware of the problem.

Abstract

Studying auditory discrimination in children and the role it plays in acquiring language skills is of great importance. Also the relationship between articulation disorder and the ability to discriminate the speech sound is an important topic for speech and language researchers.Previous event- related potentials (ERPs) studies have suggested a possible participation of the visual cortex of the blind subjects were asked to discriminate 100 couple Farsi words (auditory discrimination tack) while they were listening them from recorded tape.The results showed that the blinds were able to discriminate heard material better than sighted subjects. (Prro.05)According to this study in blind subjects conical are as normally reserved for vision may be activated by other sensory modalities.This is in accordance with previous studies. We suggest that auditory cortex expands in blind humans.

Abstract

Objective: This research is designed to study palatographic specification of lingual- palatal consonants in Persian language. Since there is not any research about the lingua-palatal contacts in Persian language, this research is designed to determine these "normal" contact patterns and variability of Persian consonant production.
Materials and Methods: In this study 16 consonants have been studied. Subjects were 4 normal adults. Each person repeated every consonant 10 times in (CVCV) syllable with vowel /a/. Frame of maximum contact for each consonant was determined. Individual and overall representative frames obtained from those frames.
Results: There are different patterns of variability between Persian consonants.
Consonants /n/, /ſ/, and /l/ display more variable articulation and stops and affricates display less variable articulation than others.
Conclusion: Persian consonants have different contact patterns. These findings are in accordance with previous studies in other languages.

Abstract

Objective: Comparison of Air conduction (AC) and Bone conduction (BC) auditory brain stem response (ABR) latencies.
Materials and Methods: In this cross-sectional study, 34 (17 males, 17 females) normal-hearing young subjects tested with AC- and BC-ABR .Wave V latencies of both procedures were compared
Results: BC- wave V latency significantly prolonged about 0.5 ms. That prolongation was less with decreasing intensity. Females&apos AC-and BC - ABR latencies were less than males&apos.
Conclusion: AC- and BC- ABR appeared to be useful to determine sensory-neural acuity in children with atresia or difficult to test subjects specially newborns.

Abstract

Background and Aim: Hyperbilirubinemia is one of the most common abnormalities during the neonatal period. Approximately 60% full-term and 80% preterm neonates suffer from hyperbilirubinemia. Hearing loss is one of the most prevalent side-effects of hyperbilirubinemia. This happens when the serum concentration of indirect bilirubin increases dramatically. The main purpose of this study is to evaluate the hearing status and to diagnose the probable site of lesion in affected children.
Materiads and Methods: In this cross-sectional study, 33 newborn to two year old subjects uffering from hyperbilirubinemia were evaluated using auditory brainstem response (ABR) and transient evoked otoacoustic emissions (TEOAE) tests In the Audiology department of the Faculty of Rehabilitation Sciences Iran University of Medical Sciences sampling method was accessible.
Results: 21% of the subjects had severe to profound hearing loss and 9% of subjects had mild to moderate hearing loss. In 51% of the subjects there was no response to ABR and TEOAE tests indicating cochlear and/or retrocochlear lesion. The other 15% manifest only abnormal ABR test indicating the presence of the auditory neuropathy.
Conclusion: TEOAEs test has its own limitations in these subjects, that is the hearing status of such patients can not be completely evaluated by using just TEOAE test. Therefore, to detect hearing loss both ABR and TEOAE tests must be performed in these subjects.

Abstract

Background and Aim: For better using of electropalatography in assessment and treatment procedures it is important to know normal tongue to palate contact patterns. Several factors can change consonant&aposs articulatory patterns. One of them is voicing. In addition to determine amount of spacial indices of studied consonants, this research aimed to study tongue behavior related to change in voicing.
Materials and Methods: This is a descriptive analytic study. Consonant linguapalatal contacts were studied using reading electropalatography by recording 4 normal Persian speaking adults (aged between 21-25) repeating CVCV syllables (C=/s,z,t,d/, V=/a/) ten times. Contact patterns were examined for total number of electrodes contacted and other spatial indices. Comparisons were made across voicing contrast.
Results: In this study amount of spacial indices of studied consonants determined. It&aposs found that there was not significant difference between voiced and voiceless cognate according to number of contact and other special indices.
Conclusion: The findings of this study are in according with previous studies in other languages about the effect of voicing upon linguapalatal contact patterns. Difference in voicing does not make clear change in spacial indices related to tongue contact pattern. Compared to other parts, Tip of the tongue seem to be more sensitive for change in voicing and oral air pressure.

Abstract

Background and Aim: One of the most common complaints, particularly among patients with normal or near normal low-frequency hearing, is that their voice sounds "hollow". This is associated with "Occlusion effect". The aim of this study was to comparison of occlusion effect in normal hearing subjects via Real Ear Measurement and Audiometric Bing test.
Materials and Method: Twenty volunteers with normal hearing (10 male, 10 female), with ages ranging from 18 - 24 years were enrolled in this study. First, audiometric Bing test was performed in frequencies of 250, 500, 750 and 1000 Hz. Then in real ear measurement, subjects were instructed to vocalize /â/ and /i/ for 5 seconds. Probe microphone was recorded the sound pressure in the ear canals. The degrees of occlusion effect unit were obtained for further analysis.
Results: The means of occlusion effect were present in all subjects. There was no correlation between the results of audiometric Bing test and real-ear measurement. No significant difference was obtained between genders and monaural or binaural occlusion in real-ear measurement. But in audiometric Bing test, the difference between monaural or binaural occlusion was significant(p<0.05).
Conclusion: The peak of occlusion effect varies significantly among hearing aid users, so the hearing aid must be tuned. Probe- microphone measures will assist in determination how much gain is enough, and where frequency- specific adjustments are needed.

Abstract

Background and Aim: phonological awareness skill is an integrated ability that is manifested in 4 skills including syllable, rhyme, and phoneme awareness and alliteration. The aim of the present study is to explore development of syllabic elision skill as an aspect of syllabic awareness.

Methods: It was a descriptive-analytic cross-sectional study. Thirty normal Persian-speaking children, 15 girls and 15 boys, were randomly selected throughout Tehran kindergartens. The participants were monolingual. Children should eliminate a syllable from any word presented, and to state the remaining section of the word. The responses were analyzed using SPSS software.

Results: Ability of 5 to 6 year-old children in eliminating a syllable in two syllabic words was significantly higher than those of three and four syllabic words and syllabic elision of three and four syllable words was significantly different (p=0.000). Two, three and four syllabic task correlations were statistically significant and the correlation between syllabic elision ability and the syllabic construct and the positioning of elision syllable was significant (p<0.05).

Conclusion: Syllabic elision skill blossoms in 5 to 6 year-old Persian-speaking children. The maximum ability of syllabic elision evolves after phonological recoding and after learning reading and writing skills. Syllabic elision ability is also influenced by the syllabic construct and the positioning of elision syllable.

Abstract

Background and Aim: Evaluation of hearing-impaired children&aposs auditory skills by a valid test is very important before starting auditory training. There are many different tests available for languages other than Persian, and some limited Persian tests for children above 5 year of age as well. Accordingly considering the importance of early intervention, we aimed to design a test for evaluating auditory skills of 3-4 year-old hearing-impaired Persian (Farsi) children, which is not available to date.
Methods: The study method was test (scale) construction. The test named Tavana includes four subtests: detection, discrimination, identification and sentence comprehension. For determining content validity of the test, the vocabulary of 3-4 year-old normal-hearing and hearing-impaired children and also the comments of seven specialists were applied. We classified 10,10,8 and 6 sub skills for 1st to 4th subtests, respectively.
Results: The content validity of Tavana test&aposs subskills were 100%, 83%, 96% and 95%, respectively (mean=94%). Mean time for test performance was 82 minutes.
Conclusion: Content validity of Tavana test was 94% which was statistically valid compared to Modarres test (97%) and Mazaheryazdi test (88%).

Abstract

Background and Aim: Reflux laryngitis is extremely common among patients with voice disorder. Medical therapy approaches are not efficient enough. The main goal of this study is to assess the acoustic and laryngeal characteristics of patients with dysphonia before and after medical or voice therapy, and to evaluate the effectiveness of each.
Methods: In this retrospective study, 16 reflux laryngitis patients were assessed. Five received complete voice therapy, tow ceased voice therapy and nine received medication. Perceptual voice evaluation was performed by a speech-language pathologist, the severity of voice problem was calculated, based on the affected acoustic and laryngeal characteristics pre- and post-treatment.
Results: Post-treatment evaluation in patients who received complete voice therapy indicates 80 percent improvement in the severity of disorder and 100 percent improvement in the perceptual voice evaluation. After medical therapy, we observed that voice disorder and perceptual voice evaluation are improved 44 and 66 percent respectively. The improvement was statistically significant in both treatment approaches: complete voice therapy (P=0.039) and medical therapy (p=0.017).
Conclusion: In patients with reflux laryngitis, most acoustic and laryngeal characteristics were normal and satisfying after the treatment. It can be concluded that the proficiency of voice therapy in improving the acoustic and laryngeal characteristics is comparable to medical therapy.

Abstract

Background and Aim: The voice of human being changes during lifetime with different patterns in males and females. In addition to assessment of changes due to aging, some studies examined the voice changes among various languages and ethnical groups. This study is performed to evaluate the fundamental frequency changes in normal 9-50 year-old Persian (Farsi) speaking individuals.
Methods: In this cross-sectional study, 320 voice samples in normal voiceless environment were recorded. The mean of fundamental frequency of vowels and counting is measured by Dr Speech software (real analysis program). Data analysis was performed by MANOVA test and the profiles of their changes were plotted.
Results: The profile of mean of fundamental frequency changes was different in males and females. The mean of fundamental frequency was equal in both genders until 13 years old and the samples of voice had low pitch (Mean: higher than 200 Hz). MFF after 13 years old significantly decreased in males specially in vowels (p<0.001). The changes in vowels and counting were similar. The mean of fundamental frequency in both genders, between 20-40 years old had  relative constancy.
Conclusion: The mean of fundamental frequency changes of vowels and counting in females and males result from diverse dimensions of larynx and other anatomical differences. Significant changes of mean of fundamental frequency in both genders after 13 years old result from puberty in males. The proportional consistence of mean of fundamental frequency between 20-40 years old is probably due to the uniformity of body in this age range.

Abstract

Background and Aim: Rhyme awareness is an important criterion for speech, language and writing processing. In Iran, there is no auditory tasks of rhyme awareness available for assessment of 5-6 year-old Persian-speaking children. The aim of this study was to develop and to evaluate Persian rhyme awareness tasks in 5-6 year-old Persian-speaking children.
Methods: A hundred normal 5-6 year-old Persian-speaking children (50 girls and 50 boys) were randomly selected. The two rhyme awareness tasks of this study were rhyme detection and rhyme production. The validity and reliability of these tasks were studied by content validity index, intraclass correlation coefficient, and standard error of measurement. The correlation between tasks was studied by Pearson correlation and the mean scores of two sections were compared by easiness level method. The comparison of mean scores of girls and boys were evaluated by independent t-test.
Results: The results showed that the proposed tasks had appropriate validity and reliability. The ability of children in production section was significantly more than in detection section. The correlation of each of two sections with total tasks was significant (p<0.05). In addition, the mean scores of girls and boys were not statistically different (p>0.05).
Conclusions: The proposed tasks are appropriate for assessment of rhyme awareness skills with appropriate validity and reliability in both genders. Two Sections of the tasks had significant correlations and influence on together. The abilities of girls and boys were the same.

Abstract

Background and Aim: Vestibular-Evoked Myogenic Potentials (VEMP) is a new vestibular function test that assesses the vestibulospinal pathway.
Although many studies have investigated different inner ear pathologies by using VEMP, the role of this test in brainstem pathologies and demyelinating processes of central nervous system is still remained to be defined and discussed.
The aim of this study was to compare VEMP in multiple sclerosis patients and normal individuals.
Methods: We carried out a cross-sectional study in 21 patients (15 female, 6 male) fulfilling diagnostic criteria of clinically definite multiple sclerosis, and 20 normal individuals (15 female, 6 male). ages ranging from 17 to 50 years old.
The VEMPs was recorded using 500 Hz short tone burst (STB). The p13 and n23 latencies and p13-n23 peak-to-peak amplitude were the studied parameters.
Results: VEMP results were abnormal in 47.6% of patients. P13 latency and p13-n23 peak to peak amplitude in both ears showed significant difference between the two groups (p<0.05). P13 latency was delayed in 14 ears (of nine patients). VEMPs were unilaterally absent in two patients.
Conclusion: VEMPs thus represent a new method of the brainstem in multiple sclerosis disease. VEMP abnormalities in these patients might be due to impairment in vestibulocollic pathway.

Abstract

Background and Aim: The dichotic auditory-verbal memory test (DAVMT) is useful in detecting auditory-verbal memory deficits and differences in memory function between the brain hemispheres. The purpose of this study was to prepare the Persian version of dichotic auditory-verbal memory test and to obtain its results in 18- to 25-year old normal individuals of both genders in Iran.
Methods: The Persian version of dichotic auditory-verbal memory test like the original one consisted of 18 lists of ten words. The lists were arranged such that 10 words were presented to one ear, with the reverse words simultaneously were presented to the other ear, constituting a dichotic test situation. After preparing the 18 lists, content validity was assessed by a panel of eight experts and then, the words of each list were recorded on CD in a dichotic mode. Thereafter, it was performed on one hundred and ten normal individuals with ages ranging from 18 to 25 years (mean age: 21.3 SD 1.9 years) and the scores were recorded on a specially designed scoring sheet.
Results: There was no significant difference between mean scores of 18 lists of the dichotic auditory-verbal memory test. The overall mean of CVI was 96% for the 18 lists and the mean score of dichotic auditory-verbal memory test was 6.5 SD 1.0 in the normal group.
Conclusion: Based on the obtained results, the Persian version of dichotic auditory-verbal memory test has a good content validity and can be used in detecting the auditory-verbal memory deficits.

Abstract

Background and Aim: The words are the symbolic tools for communication. Nouns are the initial and the most frequent components of language that children acquire and use during the procedure of language development. Therefore, this study aimed to determine the noun perception and expression abilities in Persian-speaking children, as well as gathering the normative data for two picture-pointing and picture-naming tests.
Methods: In this cross-sectional study, 110 normal Persian-speaking children (54 girls and 56 boys), 2.5 to 4 year-old in four groups, were selected randomly throughout Arak kindergartens. Two pictorial perception and expression tests were performed in this study.
Results: Mean scores of perception and expression was statistically different between all age groups, except 3 and 3.5 year-old groups (p<0.05). There was no statistically difference between mean scores of girls and boys. In addition, a significant positive correlation between mean scores of perception and expression was shown (p=0.001, r=0.896).
Conclusion: The perception and expression abilities are different in 4 age groups. The two picture-pointing and picture-naming tests can show these differences. Regardless of rapid development of perception, perception and expression abilities develop in parallel, and the ability of girls and boys shows similar trends.

Abstract

Background and Aim: Voice disorders assess with the help of laboratory equipments and perceptual evaluation. Voice samples involve vowel prolongation, text-reading or conversational speech. In Iran, Rainbow text and Grandfather text, were developed for assessing voice.
Methods: This study was cross-sectional and descriptive-analytical. Mean and fundamental frequency variations range in 120 normal males and females aging from 20-59 years old were studied. Voice samples of reading both texts were gathered and compared.
Results: Mean fundamental frequency in males in Grandfather text was 137.93 with SD 1.2 and in Rainbow text was 135.09 with SD 1.1 (p>0.05), in females Grandfather and Rainbow texts were 206.07 with SD 0.9 and 204.23 with SD 2.1, respectively (p>0.05). Fundamental frequency variations range in Grandfather text in males was 88.10 with SD 0.8 and in females was 116.77 with SD 1.3 and in Rainbow text in males was 90.92 with SD 1.1, in females was 116.08 with SD 1.2 (p>0.05). Mean fundamental frequency of males in each text didn&apost show significant difference in age groups and just in 50-59 age group of the female population showed significant difference. Fundamental frequency variations range didn&apost show significant difference in males and females in both texts.
Conclusion: These two texts were the same in measurement of average and fundamental frequency variations range in adults. Significant difference showed between 50-59 age group of the female population and other groups.
