fa بررسی تأثیر بسته ABCDE بر خطر بروز عوارض در بیماران تحت تهویه مکانیکی Effect of the ABCDE bundle on the risk of complications in patients undergoing mechanical ventilation مراقبت‌هاي پرستاري Nursing Care پژوهشي Research زمینه و هدف: با توجه به پیشرفت درمانها و افزایش شیوع بیماریها، تعداد بیماران نیازمند تهویه مکانیکی در بخشهای مراقبتهای ویژه هر ساله افزایش مییابد. بسته ABCDE با رویکرد مبتنی بر شواهد، میتواند به بهبود پیش آگهی بیماران در ICU کمک نماید. مطالعه حاضر با هدف تعیین تأثیر بسته ABCDE بر خطر بروز زخم فشاری، ترمبوز ورید عمقی و دلیریوم در بیماران تحت تهویه مکانیکی طراحی و اجرا شده است. روش بررسی: این کارآزمایی بالینی یک سوکور از شهریور تا دی ماه 1400 انجام یافت. نمونه پژوهش شامل 100 بیمار تحت تهویه مکانیکی در بخش ICU بیمارستان امام رضا(ع) مشهد بود که به روش بلوکی تصادفی به گروه مداخله و کنترل تخصیص داده شدند. در گروه مداخله بسته ABCDE در بازه زمانی 7 روزه اجرا شد. گروه کنترل مراقبت معمول بخش را دریافت کردند. معیارهای بررسی خطر بروز عوارض شامل: خطر زخم فشاری با استفاده از شاخص COMHON، ترومبوز ورید عمقی با استفاده از مقیاس Wells و بروز دلیریوم با استفاده از ابزار CAM-ICU بود که در پایان هر مداخله و در روزهای 0، 1، 3، 5، 7 مورد بررسی قرار گرفتند. جهت تحلیل دادهها از نرمافزار SPSS نسخه 20 و آزمون آماری کای‌اسکوئر و تی‌مستقل استفاده شد. یافته‌ها: در انتهای روز هفتم، خطر بروز زخم فشاری در گروه مداخله به طور معناداری کاهش یافت (05/0>p). فراوانی بروز دلیریوم نیز از 4/64% به 3/13% بهطور معناداری کاهش یافت (05/0>p). همچنین در پایان روز هفتم، خطر ترمبوز وریدهای عمقی، در گروه کنترل به طور معناداری بیش‌تر از گروه مداخله بود (05/0>p). نتیجه‌گیری: استفاده از بسته ABCDE میتواند خطر بروز زخم فشاری، ترمبوز ورید عمقی و دلیریوم را در بیماران تحت تهویه مکانیکی کاهش دهد و به عنوان یک روش مؤثر و قابل انجام توسط پرستاران و سایر تیم پزشکی آموزش دیده میباشد. ثبت کارآزمایی بالینی: IRCT20190625044000N1   Background & Aim: With advancements in treatment and an increase in the prevalence of diseases, the number of patients requiring mechanical ventilation in intensive care units (ICUs) continues to rise each year. The ABCDE bundle is a multi-component, evidence-based approach designed to improve patient outcomes in the ICU. This study aimed to determine the effect of the ABCDE bundle on the risk of pressure ulcers, deep vein thrombosis and delirium in patients undergoing mechanical ventilation. Methods & Materials: This randomized, single-blind clinical trial was conducted between September 2021 and January 2022. The sample included 100 patients undergoing mechanical ventilation in the ICU department of Imam Reza Hospital in Mashhad, who were randomly assigned to either the intervention or control group using block randomization. The intervention group received the ABCDE bundle over a period of 7 days. The control group received routine care. The criteria for assessing complications included the risk of pressure ulcers and deep vein thrombosis (DVT), evaluated at the end of the intervention and on days 0, 1, 3, 5, and 7. The study utilized several assessment tools: the COMHON Index for pressure ulcers, the Wells Deep Vein Thrombosis Scale, and the Delirium Evaluation Tool in the Intensive Care Unit (CAM-ICU). Data analysis was performed using SPSS software version 20, employing chi-square, Mann-Whitney, and independent t-tests. Results: A decrease in the average pressure ulcer risk score was observed in the intervention group on day seven (P<0.05). In this group, the incidence of delirium fell decreased from 64.4% to 13.3% (P<0.05), indicating a statistically significant reduction. Additionally, the risk score for deep vein thrombosis in the control group significantly increased on day seven compared to the intervention group (P<0.05). Conclusion: The use of the ABCDE bundle significantly reduces the risk of pressure ulcers, deep vein thrombosis, and delirium in patients undergoing mechanical ventilation and represents an effective strategy that can be implemented by nurses and other trained healthcare professionals.
