fa بررسی سطوح سرمی پروتئین شبه کیتیناز 3 و پروتیئن واکنش کننده‌ی C‏ دربیماران مبتلا به دیابت نوع دو دارای تنگی عروق کرونر در مقایسه با افراد دیابتی و سالم EVALUATION OF SERUM CHITINASE-3 LIKE 1 PROTEIN AND C-REACTIVE PROTEIN LEVELS IN TYPE 2 DIABETES MELLITUS WITH CORONARY ARTERY DISEASE COMPARED TO DIABETIC AND HEALTHY CONTROL تخصصي Special پژوهشي Research مقدمه: ابتلا به دیابت که با افزایش قندخون، لیپیدها وآنزیم‌های کبدی همراه است، ممکن است منجر به تغییرات تولید سایتوکاین‌های التهابی شامل پروتئین شبه کیتیناز 3 (YKL-40)و پروتیئن واکنش کننده‌ی C با حساسیت بالا (hs-CRP) شود که در ایجاد و پیشرفت عوارض دیابت ازجمله، آترواسکلروز نقش دارند. هدف این مطالعه تعیین و مقایسه‌ی سطح سرمی YKL-40- و hs-CRP در بیماران مبتلا به دیابت نوع دو دارای تنگی عروق کرونردر مقایسه با افراد دیابتی و سالم بود. روش‌ها: این مطالعه به‌صورت تحلیلی- مقطعی بر روی 87 نفر در چهارگروه شامل: 23 نفر افراد سالم (گروه یک)، 22 بیمار مبتلا به دیابت نوع دو(گروه دو)،20 بیمارتنگی عروق کرونر (گروه سه) و 22 بیمار مبتلا به دیابت نوع دو و تنگی عروق کرونر(گروه چهار) انجام شد. سطح سرمی قند ناشتا (FBS)،کلسترول، تری‌گلیسیرید، لیپوپروتیئن با دانسیته‌ی کم (LDL-C)، لیپوپروتیئن با دانسیته‌ی بالا(HDL-C) ، آسپارتات آمینو ترانسفراز (AST)، آلانین آمینو ترانسفراز (ALT)، YKL-40 وhs-CRP تعیین شدند. یافته‌ها: میانگین سطح سرمی در پارامترهای: FBS (گروه یک با گروه‌های دو و چهار وگروه دوبا گروه سه، 001/0P=)، کلسترول (گروه یک با گروه سه، 03/0P=)، تری‌گلیسرید (گروه دو با گروه سه، 027/0P=)،  HDL-C(گروه یک با گروه‌های سه وچهار، به‌ترتیب 02/0P=، 01/0P=)، ALT (گروه یک با گروه چهار، 03/0P= و گروه دو با گروه چهار، 02/0P=) و AST (گروه دو با گروه چهار، 009/0P=) معنی‌دار بود. میانگین سطح سرمیYKL-40  در بین گروه‌های یک (27/1±81/4 نانوگرم بر میلی‌لیتر) با گروه دو(61/4±52/15نانوگرم بر میلی‌لیتر) (01/0P=) و گروه سه (75/2±20/19نانوگرم بر میلی‌لیتر،017/0P=) و نیز بین گروه‌های یک و چهار (17/4±10/16نانوگرم بر میلی‌لیتر،04/0P=) ارتباط معنی‌داری را نشان می‌داد. سطح سرمیhs-CRP  در بین گروه‌های سه (53/1±49/4 میکروگرم بر میلی‌لیتر) و چهار (43/0±28/1 میکروگرم بر میلی‌لیتر)، به‌طور معنی‌داری متفاوت بود (028/0P=). نتیجه‌گیری: سطح سرمی YKL-40 در افراد مبتلا به دیابت نوع دو و بیماری عروق کرونر افزایش می‌یابد. تعیین سطح سرمی این مارکر در این افراد توصیه می‌شود. Background: Diabetes mellitus, which is associated with elevated blood glucose, lipids and liver enzymes, may results in alterations in the production of inflammatory cytokines, including chitinase-3-like protein 1 (YKL-40) and high-sensitivity C-reactive protein (hs-CRP), which induce and promote complications of diabetes such as atherosclerosis.The aim of this study was to evaluate the serum level of chitinase-3 like 1 protein (YKL-40) and high-sensitive C-reactive protein (hs-CRP) in type 2 diabetes with coronary artery disease compared to diabetic and healthy control. Methods: This cross-sectional study was performed on 87 subjects in four groups, including: 23 samples as healthy control (Group 1), 22 patients with type 2 diabetes (Group 2), 20 patients with coronary artery disease (Group 3) and 22 patients with type 2 diabetes and coronary artery disease (Group 4). Serum Fasting blood sugar (FBS), Cholesterol, Triglyceride, Low density lipoprotein (LDL-C), High density lipoprotein (HDL-C), Aspartate aminotransferase (AST), Alanine aminotransferase (ALT), YKL-40 and hs-CRP were determined. Results: The mean serum levels of FBS (Group 1 with 2 and 4, Group 2 and 3, P = 0.001), Cholesterol (Group 1 and 3, P = 0.03), Triglyceride (Group 2 and 3,p=0.027), HDL-C (Group 1 with 3 and 4, P = 0.02, P = 0.01 respectively), ALT (Group 1 and 4, P = 0.03, Group 2 and 4, P = 0.02) and AST (Group 2 and 4, P = 0.009) were significantly different..The mean serum levels of YKL-40 were significantly difference in the group 1(4.81±1.27 ng/ml) and group 2 (15.52±4.61 ng/ml) (P=0.01), group 3 (19.2±2.75 ng/ml, P=0.017) and group 4 (16.1±4.17 ng/ml, P=0.04). Also, The mean serum levels of hs-CRP in the group 3 (4.49± 1.53 μg/ml) and group 4 (1.28 ± 0.43 μg/ml) was significantly difference (P= 0.028). Conclusion: Serum levels of YKL-40 are increased in people with type 2 diabetes and coronary artery disease.
