fa مرور ملاحظات اخلاقی و نقد میزان خطر قابل قبول در کارآزمایی بالینی بر داوطلب سالم پژوهشي Research عرصه‌ی پژوهش‌های پزشکی به موازات رشد دانش پزشکی وسعت خیره‌کننده‌ای داشته و به موازات این رشد با چالش‌های جدید و جدی متعددی روبه‌رو شده است. کارآزمایی‌های بالینی از جمله مهم‌ترین حوزه‌های پژوهش در عرصه‌ی علوم پزشکی و هم‌چنین پرچالش‌ترین این حوزه‌ها از نظر ملاحظات اخلاقی است.استفاده از داوطلب سالم در کارآزمایی‌های دارویی امری اساسی و ضروری است، موضعی که با ملاحظات اخلاقی متعدد و جدی اخلاقی روبه‌روست. اگرچه مسائل اخلاقی بهره‌گیری از داوطلب سالم تنها به مطالعات مداخله‌ای به‌طور عام و کارآزمایی بالینی به‌طور خاص محدود نیست، در این نوشتار تلاش شده است با تمرکز بر این نوع از پژوهش‌های پزشکی ضمن مروری بر اهمیت موضوع و سیر تاریخی آن، مهم‌ترین چالش‌ها و ضرورت‌های اخلاقی در چهار محور رضایت آگاهانه، انتخاب منصفانه‌ی سوژه، جلب همکاری داوطلبان و ارزیابی سود و زیان مورد بررسی قرار گیرد.به‌نظر می‌رسد معیار خطر زندگی روزمره به‌عنوان میزان قابل پذیرش خطر در پژوهش‌های مداخله‌ای معیاری مبهم و در بسیاری از موارد غیر عملی است. پیشنهاد نویسندگان مقاله، استاندارد معقول به‌عنوان معیار مد نظر کمیته‌ی اخلاق در پژوهش است. بر این اساس، این کمیته به‌عنوان هیأت‌ منصفه­ به نمایندگی از جامعه قابلیت پذیرش عمومی و معقول بودن میزان خطر مطالعه را بررسی می‌کند.<strong></strong><p></p> Healthy volunteers are the first group who take part in experimental studies on the efficacy of new drugs. Parallel with expanding the boundaries of medical science, medical research has shown rapid growth which has caused new and critical ethical challenges in medical research.A clinical trial is one of the essential methods in clinical research and a very challenging method from the ethical viewpoint.Recruiting healthy volunteer participants is necessary in clinical trials of drugs, and it requires special and careful ethical considerations. Although recruiting healthy volunteers is not limited to clinical trials, we have focused our discussion on ethical issues of research on healthy volunteers in this kind of study. In this paper, ethical challenges of involving healthy volunteers in clinical trials have been discussed in four domains of risk-benefit assessment, fair subject recruitment, incentives, and informed consent. The authors believe and argue that using the daily life risks standard as the acceptable risk for healthy volunteers is impractical. We suggest defining a reasonable risk that is acceptable to the research ethics committee.
