fa بررسی میزان رعایت حقوق بیماران و برخی عوامل دموگرافیک و بیمارستانی مؤثر بر آن، با رویکرد مدل‌های رگرسیون خطی دوسطحی، در بیمارستان‌های تحت پوشش دانشگاه علوم پزشکی تهران Investigating Patient Rights and Associated Demographic and Hospital Factors Using Approach of Two-Level Linear Regression Models In the Hospitals of Tehran University of Medical Sciences اخلاق بالینی Medical Ethics پژوهشي Research <div style="text-align: justify;">یکی از موضوعات اساسی در بیمارستان‌ها، بررسی میزان کارایی و اثربخشی است. به این منظور، شاخص‌هایی متعدد ارائه شده است که به شاخص‌های عملکردی، معروف‌اند. اگر عملکرد بیمارستان‌ها، از لحاظ رعایت حقوق بیمار، به مشخصات دموگرافیک او وابسته باشد، نادیده‌گرفتن اثر شاخص‌های بیمارستانی، به‌عنوان عامل مؤثر سطح بالاتر بر میزان رعایت حقوق بیمار، ممکن است موجب نتیجه‌گیری‌هایی نادرست درباره‌ی این روابط شود. در چنین مواردی که داده‌های مورد مطالعه، به‌طور ذاتی، چندسطحی است، استفاده از مدل‌های آماری چندسطحی برای بررسی این نوع داده‌ها، مفید خواهد بود. نمونه‌ها، در طی سال 1394، از هشت بیمارستان دانشگاه علوم پزشکی تهران، به‌صورت خوشه‌ای دومرحله‌ای، جمع‌آوری شده‌اند. اطلاعات مربوط به میزان رعایت حقوق بیمار و اطلاعات دموگرافیک، از طریق پرسش‌نامه، از 375 بیمار جمع‌آوری گردید. میزان رعایت حقوق بیمار، توسط روش آماری چندمتغیره‌ی تحلیل عاملی، اندازه‌گیری شد. برای بررسی رابطه‌ی بین رعایت حقوق بیمار با برخی اطلاعات دموگرافیک، از مدل‌های رگرسیون خطی دوسطحی استفاده شد که سطح اول آن را عوامل دموگرافیک بیمار و سطح دوم را عوامل بیمارستانی تشکیل می‌دهند. میزان رعایت حقوق بیمار در بیمارستان‌ها، در سطح متوسط، رعایت شده بود. حدود شانزده درصد از واریانس متغیر وابسته‌ی میزان رعایت حقوق بیمار که مقداری زیاد است، به دلیل تغییرات در سطح بیمارستان است و بقیه‌‌ی تغییرات، در سطح فردی است؛ همچنین، متغیرهای درصد اشغال تخت، درصد شکایت و درصد فرار از بیمارستان، در سطح بیمارستان، آثاری شایسته‌ی توجه بر میزان رعایت حقوق بیماران دارند.</div> <div style="text-align: justify;">One of the main issues in hospitals is evaluation of efficiency and effectiveness. For this purpose, several indicators are presented, which are known as functional indicators. If the performance of hospitals depends on the patient's demographic characteristics, overlooking effect of hospital indicators, as an effective factor at a higher level on the patient rights, may lead to inaccurate conclusions about these relationships. In such cases, where the data are intrinsically multilevel, the use of multi-level statistical models for this type of data is useful. Samples were collected from eight hospitals of Tehran University of Medical Sciences during 2013 in a two stage cluster. Data on patient rights and demographic information were collected from 375 patients by a questionnaire. The patient's rights rate was measured by multivariate statistical analysis and factor analysis. Two-level linear regression models were used to examine the relationship between patients' rights and some demographic information. The first level was demographic factors and the second level was hospital factors. The results showed that rate of considering patients' rights in hospitals were in moderate level. About 16% of the variance of the dependent variable of patient's rights, which was significant, is due to variations at the higher level of the hospital and other variations at the individual level.
