fa بررسی درمان کمک‌باروری در زنان با سن بالا از دیدگاه پزشکی و اخلاقی پژوهشي Research در سال‌های اخیر مراجعه‌ی زنان مسن به مراکز درمان ناباروری افزایش یافته است. برای استفاده از تکنولوژی‌های کمک‌باروری (ART) در زنان با سن بالا کلیه‌ی مسائل پزشکی و اخلاقی حل نشده و همواره این سؤال مطرح می‌شود که آیا باید محدودیت سنی برای ارائه‌ی درمان کمک‌باروری قائل شد یا خیر؟<br>این مطالعه از نوع اسنادی و کتابخانه‌ای است. مطالب و مقالات در پنج بخش پزشکی، اخلاقی، زوج، جامعه و فرزند حاصل بررسی و ارائه شده است. هم‌چنین قوانین، دستورالعمل‌ها و مطالب موجود در این زمینه مورد ارزیابی قرارگرفته است. در ایران دستورالعمل جامعی برای محدودیت سنی زنان جهت انجام درمان کمک‌باروری تدوین نشده است. در این‌باره کلینیک‌ها و مراکز درمان ناباروری باتوجه به سیاست مرکز و شرایط موجود، براساس آئین‌نامه‌های غیررسمی و داخلی خودشان عمل می‌کنند. بررسی پروتکل‌های درمانی در بیش‌تر مراکز نشان می‌دهد که زنان تا سن 45 سال در سیکل درمان نازایی قرار می‌گیرند و پذیرش زنان بالای 45 سال تنها با تأیید پزشک و وجود شرایط بالینی مناسب بیمار انجام می‌شود. از نظر فقهی و حقوقی نیز منعی برای درمان ناباروری زنان مسن در کشورمان وجود ندارد. اما همواره سلامت مادر و فرزند از دیدگاه پزشک اولویت اول را دارد و اعمال درمانی باید در همین راستا صورت گیرد. از آن‌جایی که نباید سن را تنها معیار پذیرش زن نابارور جهت درمان کمک‌باروری قرار دهیم، لازم است در این‌باره دستورالعمل جامعی تدوین شود.<p></p> <p>Recently larger numbers of women of advanced age are referring to fertility centers. Although there may be no age restriction for women who are naturally able to and want to be pregnant, using assisted reproductive techniques for women of advanced age is medically and ethically disputed, and this question is always raised: "Should there be an age limit for accessing assisted reproductive techniques for women?" This study was a review of ethical, legal and religious issues regarding pregnancy and reproduction in advanced age for women. We tried to cover codes, legislations and articles on the subject. There is no national legislation about age limit for women seeking assisted reproduction. In this regard, fertility clinics follow their own protocols. Most centers do not accept women older than 45 unless there is a specific clinical situation confirmed by a physician. In any case, the safety of the mother and her child must be the first concern of the physician, and all the treatment protocols should be determined accordingly. Obviously, age alone cannot be a suitable criterion for rejecting the infertile patient. Therefore, regardless of the patient's persistence and their ability to afford treatment, physicians should consider mother's safety and the welfare of the child before admission.
