fa سند سیاستی مدیریت و پیشگیری از پزشکی تدافعی در نظام سلامت ایران اخلاق بالینی Medical Ethics پژوهشي Research <div style="text-align: justify;"><span style="font-size:14px;"><span style="font-family:yekanYW;"><span style="direction:rtl"><span style="unicode-bidi:embed"><span lang="FA">پزشکی تدافعی، انجام‌دادن اقداماتی است که اندیکاسیون پزشکی و منفعتی برای بیمار ندارد (پزشکی تدافعی مثبت) یا خودداری از انجام‌دادن اقداماتی پرخطر است که دارای اندیکاسیون پزشکی و منفعت برای بیمار است (پزشکی تدافعی منفی). پزشک این اقدامات را، صرفاً با انگیزه‌ی محافظت از خود در برابر شکایت‌ها یا تنش‌هایی مانند اعتراض بیمار یا همکاران انجام می‌دهد و معمولاً موجب واردشدن آسیب جسمی، روانی یا اقتصادی به بیمار یا نهاد پرداخت‌کننده‌ی هزینه‌ی درمانی مانند سازمان‌های بیمه‌گر می‌شود که می‌تواند عواقبی را به لحاظ کیفیت مراقبت افراد و بهره‌وری از منابع محدود سلامت در پی داشته باشد. عواملی، ازجمله نگرانی پزشک از شکایت و دادرسی، ممکن است به رفتارهای تدافعی منجر شود. این مقاله به ارائه‌ی پیشنهاداتی برای مدیریت این‌گونه رفتارها و پیشگیری از آن‌ها، شامل سه دسته‌ی کلی راهکارهای مرتبط با اصلاح سیستم رسیدگی به شکایت بیماران، راهکارهای اجتماعی مدیریت و پیشگیری از پزشکی تدافعی و راهکارهای مدیریتی‌سازمانی پرداخته است. این راهکارها بر اساس یافته‌های منتج از تحقیقی ترکیبی، شامل مرور غیرنظام‌مند منابع و مطالعه‌ی کیفی با روش برگزاری مصاحبه‌های نیمه‌ساختاریافته تنظیم شده‌اند؛ همچنین، گروه اخلاق پزشکی فرهنگستان علوم پزشکی ایران، نتایج آن را بررسی کرده‌اند.<span style="color:red"></span></span></span></span></span></span></div> <div style="text-align: justify;"><span style="line-height:2;"><span style="font-size:14px;"><span style="font-family:Tahoma;">Defensive medicine is performing actions that have no medical indication and benefit for the patient (positive defensive medicine) or refraining from performing risky actions that have a medical indication and benefit for the patient (negative defensive medicine). These actions are carried out by the physicians with the sole motive of protecting themselves against complaints or tensions such as the protest of the patient or colleagues and usually cause physical, psychological, or economic harm to the patient or the institution that pays the treatment fee, such as insurance organizations. It can have consequences in terms of the quality of care and the efficient use of limited health resources. Factors such as the physician&rsquo;s concerns about lawsuits and proceedings may lead to defensive behaviors. This study presented suggestions for the management and prevention of such behaviors, including three main categories related to the strategies for the reformation of the patient complaint handling system, social strategies for the management and prevention of defensive medicine, and managerial-organizational strategies. These strategies are based on the findings of a mixed-methods research including an unsystematic review of resources and a qualitative study conducted using semi-structured interviews.
