fa مقایسه‌ی فراوانی آنتی‌نوکلئر آنتی‌بادی در مبتلایان به پمفیگوس‌ولگاریس با گروه شاهد عمومى General پژوهشي Research <strong>زمینه و هدف:</strong> پمفیگوس یک بیماری تاولی خودایمن اختصاصی پوست و غشاهای مخاطی است که در مطالعات پیشین همراهی آن با بیماری‌های بافت همبند مشاهده شده است. با توجه به این مسئله، وجود اتوآنتی‌بادی‌هایی از قبیل Anti Nuclear Antibodies (ANA) در مبتلایان دور از ذهن نمی‌باشد. هدف از این مطالعه بررسی فراوانی ANA مثبت در بیماران پمفیگوس‌ولگاریس و مقایسه‌ی آن با گروه کنترل بوده است. <br><strong>روش اجرا:</strong> در این مطالعه‌ی مورد ـ شاهدی گروه بیماران از میان بیماران مبتلا به پمفیگوس‌ولگاریس بستری در بیمارستان رازی تهران و گروه شاهد از میان مراجعین غیرمبتلا انتخاب شدند. ابزار جمع‌آوری اطلاعات فرم ثبت داده‌ها بود که درآن متغیرهایی مانند سن، جنس، فنوتیپ بیماری و نتیجه‌ی آزمایش ANA ثبت می‌شد. در این مطالعهANA به روش ایمونوفلورسانس غیرمستقیم (HEP2) اندازه‌گیری شد. <br><strong>یافته‌ها: </strong>در 26.7% بیماران (8 نفر از 30 نفر) مبتلا به پمفیگوس‌ولگاریس و 10% افراد گروه کنترل (3 نفر از 30 نفر) ANA مثبت گزارش گردید (0.095=P). شایع‌ترین الگوی ANA در بیماران الگوی هموژن و در افراد گروه کنترل الگوی نقطه‌ای بود (0.206=P).<br><strong>نتیجه‌گیری: </strong>در این مطالعه، فراوانی موارد مثبت ANA در گروه شاهد مشابه با مطالعات پیشین بود، اما در گروه بیماران مبتلا به پمفیگوس‌ولگاریس فراوانی به‌دست‌آمده با آمار موجود در برخی مطالعات قبلی متفاوت بود. تفاوت در میزان حساسیت کیت‌های آزمایشگاهی و آستانه‌ی مثبت درنظرگرفتن ANA و تفاوت در نحوه‌ی گزینش بیماران مورد مطالعه، از جمله عواملی است که می‌تواند توجیه‌کننده‌ی این اختلاف باشد. پی‌گیری بالینی بیماران مبتلا به پمفیگوس‌ولگاریس و درخواست ANA درصورت وجود علایم بیماری‌های بافت همبند توصیه می‌شود.<p></p> <p></p> <p style="text-align: justify"><strong>Background and Aim:</strong> Pemphigus vulgaris (PV) is an autoimmune blistering disease which is specific for skin and mucosal membranes. Its association with connective tissue diseases has already been reported. Considering this association, presence of Anti Nuclear Antibodies (ANAs) in PV patients will not be surprising. The aim of this study was to compare the frequency of ANA positive cases in patients suffering PV with a control group.<br><strong>Methods:</strong> In this case-control study, the cases were selected from the patients with PV whom were hospitalized at Razi Hospital, Tehran. The controls were chosen from patients who did not have PV. The data were collected using a questionnaire, which was designed for gathering information on participants' age, sex, PV phenotypes, and the result of ANA test. ANA positivity was assessed using indirect immunofluorescence, HEP2.<br><strong>Results:</strong> In 8 (26.7%) of 30 PV patients and 3 (10.0%) of 30 controls ANA was positive (P=0.095). The most common ANA positive patterns among cases and controls were homogeneous and speckled patterns, respectively (P=0.26).<br><strong>Conclusion:</strong> Although in this study the frequency of positive ANA result among controls was similar to what were reported in previous studies, the frequency of this finding among PV patients was different from the previous reports. Differences in the sensitivity of the laboratory kits used in different studies as well as in the threshold for ANA positivity, and differences in the patients' eligibility criteria in different studies may explain the observed discrepancies.
