fa اثربخشی فتوتراپی موضعی در درمان بیماران با پوستولوز کف دست و پا عمومى General پژوهشي Research <div style="direction: rtl"><b>زمینه و هدف:</b> پوستولوز کف دست و پا بیماری پوستی پوستولی مزمن و محدود به کف دست و پا می‌باشد که مشخصه‌ی آن مقاومت به درمان و میزان بالای عود است. درمان‌های مختلفی با اثربخشی متفاوت در درمان این بیماری به‌کار رفته است. هدف این مطالعه بررسی اثربخشی bath PUVA به‌عنوان یک روش درمانی کم‌عارضه در کنترل پوستولوز کف دست و پا است.</div><div style="direction: rtl"><b>روش اجرا:</b> در این مطالعه‌ی گذشته‌نگر، بیماران پوستولوز کف دست و پا مراجعه‌کننده به کلینیک اشعه‌ی بیمارستان رازی طی سال‌های 1384 تا 1388 که تحت درمان با bath PUVA local قرار گرفته بودند ازنظر میزان بهبودی بررسی شدند. یافته‌ها با آزمون‌های آماری مربع کای و آزمون t مورد تجزیه و تحلیل قرار گرفت.</div><div style="direction: rtl"><b>یافته‌ها:</b> 95 بیمار با میانگین سنی 44.17 بررسی شدند. از کل بیماران 49.5% مرد و 50.5% زن بودند. میانگین تعداد جلسات درمان 20.9±42.5 جلسه و میانگین (<span style="line-height: 26px text-align: right">±انحراف معیار) مقدار تجمعی اشعه J/cm24/203<span style="line-height: 26px text-align: right">± 8/251 بود که 16.8% پاسخ درمانی بسیارخوب، 35.8% پاسخ درمانی خوب، 29.5% پاسخ درمانی متوسط، 9.5% بیماران پاسخ به درمان ضعیف و در 8.4% بیماران شکست کامل درمان مشاهده شد. مقدار تجمعی اشعه در بیمارانی با پاسخ درمانی بسیار خوب به‌طور معنی‌داری بیش از سایر بیماران بود </span></span><span style="line-height: 26px">(P=0.016)</span><span style="line-height: 26px text-align: right"><span style="line-height: 26px text-align: right">. تعداد جلسات درمانی نیز در این افراد به‌طور معنی‌داری بیش از سایر بیماران بود </span></span><span style="line-height: 26px">(P<0.05)‌بیست‌درصد</span><span style="line-height: 26px text-align: right"><span style="line-height: 26px text-align: right"> بیماران به‌علت عوارض درمان از مطالعه خارج شدند که در همه موارد با قطع درمان عوارض بهبود یافتند و عارضه‌ی طولانی‌مدت مشاهده نشد.</span></span></div><div style="direction: rtl"><span style="line-height: 26px text-align: right"><span style="line-height: 26px text-align: right"><b>نتیجه‌گیری:</b> این نتایج نشان می‌دهد local bath PUVA در درمان پوستولوز کف دست و پا روش مؤثر و کم‌عارضه‌ای می‌باشد.</span></span></div> <b> Background and Aim:</b> Palmoplantar pustulosis is a chronic dermatosis of palms and soles, which is associated with a high recurrence rate and resistance to treatment. Various treatments with different success rates have been used.The purpose of this study was to evaluate the efficacy of local bath PUVA in the treatment of palmoplantar pustulosis.<div><b>Methods:</b> In this retrospective study, patients with palmoplantar pustulosis who were treated with local bath PUVA in Phototherapy clinic of Razi Hospital between 2005 and 2009 were evaluated for the therapeutic efficacy of local bath PUVA. Data were analyzed using t and chi-square tests.</div><div><b>Results:</b> Ninety-five patients with a mean age of 44.17 years included in this study. 49.5% of patients were males and 50.5% were females. Mean treatment cessions of patients were 42.5 sessions and mean cumulative dose was 251.8 J/cm2. In 16.8% of cases have very good response was observed. Good, moderate and poor responses were observed in 35.8%, 29.5% and 9.5% of cases, respectively. Treatment failure was observed in 8.4% of patients. Mean cumulative dose in patients with very good response was significantly higher than other response groups, (P=0.016). Mean number of treatment sessions was also significantly higher in that group with comparison with other groups, (P<0.05). Twenty percent of patients withdrawed from the study because of complications.
