fa ارزیابی و مقایسه ایمنی و اثربخشی 5 فیلر هیالورونیک اسید تولید ایران برای اصلاح خطوط خنده (nasolabial folds) با شدت متوسط و شدید Clinical safety and efficacy assessment of 5 Iranian hyaluronic acid-based dermal fillers for improvement of moderate to severe nasolabial folds عمومى General پژوهشي Research زمینه و هدف: در سال‌های اخیر شرکت‌های متعدد ایرانی، اقدام به تولید فیلرهای هیالورونیک اسید کرده‌اند. این مطالعه مروری به بررسی و مقایسه ایمنی و اثربخشی بالینی 5 مورد از این محصولات می‌پردازد. روش اجرا: در بخش اول مطالعه، ایمنی و اثربخشی کلی فیلرهای هیالورونیک اسید مورد مطالعه، برای اصلاح خطوط خنده بر روی 150 شرکت‌کننده (سن 97/9±30/44 سال) با استفاده از ارزیابی بالینی و دستگاهی در هفته‌های 2، 12 و 24 بعد از تزریق مورد ارزیابی قرار گرفت. در بخش دوم مقایسه ایمنی و اثربخشی 5 فیلر هیالورونیک اسید مورد مطالعه با توجه به همین پارامترها انجام شد. یافته‌ها: به‌طور کلی، در هفته‌های 2، 12 و 24 به‌ترتیب در 6/80%، 3/63% و 66/60% درصد از شرکت‌کنندگان، یک گرید یا بیشتر کاهش در شدت چین‌های نازولبیال مشاهده شد. در تمامی ویزیت‌ها، متغیر سطح و حجم خطوط نازولبیال و هم‌چنین دانسیته درم در سونوگرافی، بهبود معنی‌دار نسبت به پایه از خود نشان دادند. در ارزیابی‌های دستگاهی، دو فیلر با غلظت هیالورنیک اسید 24 میلی‌گرم بر میلی‌لیتر ـ که یکی از آن‌ها حاوی مانیتول بود ـ بهترین اثربخشی دستگاهی را نشان داده‌اند درحالی که، در ارزیابی‌های بالینی فیلر حاوی مانیتول به تنهایی بهترین عملکرد را در کاهش شدت چین‌های نازولبیال از خود نشان داد. نتیجه‌گیری: فیلرهای هیالورونیک اسید ساخت ایران روشی ایمن و مؤثر برای اصلاح خطوط خنده متوسط تا شدید محسوب می‌شود ولی میزان اثربخشی آنها متفاوت گزارش شده است. Background and Aim: Recently, many Iranian companies have started manufacturing hyaluronic acid based dermal fillers. Current review aimed to assess and compare the clinical safety and efficacy of 5 Iranian hyaluronic acid based dermal fillers. Methods: The first part of the study reported, the overall safety and efficacy of 5 Iranian hyaluronic acid based dermal fillers for improvement of nasolabial folds in 150 participants (age 44.30±9.97 years) using clinical and objectives assessment methods. In the second part, we performed a comparative study among 5 tested dermal fillers. Results: Generally, at weeks 2, 12, and 24, 80.6%, 63.3%, and 60.66% of the participants, showed one grade or more improvement in the severity of nasolabial folds respectively. In all visits, the nasolabial folds biophysical parameters as well as the echo density dermis improved significantly compared to baseline. In comparative study, two fillers with the hyaluronic acid concentration of 24 mg/ml, showed the best objective efficacy, while in clinical evaluations, the filler containing mannitol has the best performance in reducing the severity of nasolabial folds showed.
