fa مقایسه اثربخشی و ایمنی 3 محصول بوتولینوم توکسین نوع A تولید ایران در بهبود خطوط اخم متوسط تا شدید Comparison of the efficacy and safety of three Iran-made botulinum toxin type A products in improving moderate to severe glabellar lines عمومى General پژوهشي Research زمینه و هدف: در ایران در سال‌های اخیر، با افزایش تقاضا برای درمان‌های زیبایی و نیاز به محصولات داخلی با کیفیت، شرکت‌های متعددی اقدام به تولید بوتولینوم توکسین نوع A کرده‌اند. این مطالعه به بررسی و مقایسه اثربخشی بالینی و ایمنی 3 مورد از این محصولات پرداخته است. روش اجرا: در بخش اول مطالعه اثربخشی کلی فراورده‌های بوتولینوم توکسین نوع A مورد مطالعه، برای خطوط اخم، بر روی 230 شرکت‌کننده دریافت‌کننده یکی از 3 محصول داخلی، (سن 21/9±69/42 سال) در روز 30 ام بعد از تزریق در مقایسه با 230 دریافت‌کننده درمان استاندارد (Dysport ®) (سن 79/9±50/42 سال) مورد مقایسه قرار گرفت. در بخش دوم، مقایسه اثربخشی این سه فراورده در همین بازه زمانی و ایمنی آن‌ها تا ماه چهارم پس از تزریق انجام شد. یافته‌ها: پاسخ به درمان (حداقل دو گرید بهبودی در خطوط اخم) در مصرف‌کنندگان فراورده‌های داخلی (6/68 درصد) نسبت به مصرف‌کنندگان دیسپورت (8/52%) در مطالعات هم ارزی برتری معنادار داشت (01/0P<). در مقایسه 3 گروه درمانی داخلی، تنها در درصد افراد با 2 گرید یا بیشتر بهبودی (پزشک مستقل)، توکسین شماره 2 بهبود معناداری را نسبت به دو محصول دیگر نشان داد. نوع و شیوع عوارض جانبی گزارش‌شده (به‌جز سردرد) با مونوگراف داروی استاندارد قابل مقایسه است. نتیجه‌گیری: نتایج حاکی از اثربخشی و ایمنی مناسب فراورده‌های بوتولینوم توکسین A ساخت ایران در مقایسه با فراورده‌های مرجع دارد. جز در موارد محدود، اثربخشی و ایمنی این فراورده‌ها تا روز 30 تقریبا مشابه هستند. Background and aim: In recent years, increasing demand for aesthetic treatments in Iran and the need for high-quality domestic products have encouraged several companies to produce botulinum toxin type A. This study aimed to evaluate and compare the clinical efficacy and safety of three such Iranian products. Methods: In the first part of the study, the overall efficacy of the botulinum toxin type A products for treating glabellar lines was assessed in 230 participants who received one of the three Iranian formulations (mean age 42.69 ± 9.21 years), and compared with 230 participants who received the standard treatment (Dysport®) (mean age 42.50±9.79 years). Outcomes were evaluated on day 30 post-injection. In the second part, the comparative efficacy of the three domestic products during the same period and their safety profile up to month four post-injection were assessed. Results: A clinically meaningful response (at least a two-grade improvement in glabellar line severity) was achieved in 68.6% of recipients of the Iranian products, compared with 52.8% of Dysport recipients, demonstrating a statistically significant superiority in equivalence studies (P<0.01). Among the three domestic products, only product No. 2 showed a significantly higher proportion of participants achieving ≥2-grade improvement (assessed by an independent physician). The type and frequency of adverse events (except for headache) were comparable to the standard drug monograph. Conclusion: The findings indicate acceptable efficacy and safety of Iran-made botulinum toxin type A products compared with the reference product.
