fa بررسی مقایسه‌ای الگوی تجویز آنتی بیوتیک مورد استفاده در کنترل عفونت‌های ادنتوژنیک بین دندانپزشکان عمومی سطح شهر تهران و الگوی پیشنهادی جراحان فک و صورت دانشگاه علوم پزشکی تهران جراحی فک و صورت Oral and Maxillofacial Surgery تحقیقی Research <div style="text-align: justify;"><strong>زمینه و هدف: </strong>تجویز نادرست آنتی ‌بیوتیک در عفونت‌های ادنتوژنیک می‌تواند جان بیماران را به خطر اندازد. این پژوهش با هدف تعیین الگوی تجویز آنتی ‌بیوتیک توسط دندانپزشکان عمومی تهران و مقایسه آن با الگوی پیشنهادی جراحان&nbsp;فک و صورت دانشگاه علوم پزشکی تهران در سال ۱۳۹۹ انجام شد.<br> <strong>روش بررسی: </strong>این مطالعه مقطعی بر روی ۱۴۱ دندانپزشک تهران به ‌صورت تصادفی انجام شد. دندانپزشکان با تکمیل پرسشنامه‌ای، آنتی ‌بیوتیک‌های تجویزی خود را در شرایط مختلف عفونت‌های ادنتوژنیک گزارش کردند. فراوانی&nbsp;آنتی ‌بیوتیک‌های تجویزی با توجه به شدت عفونت، حساسیت به پنی ‌سیلین، وضعیت بارداری و شیردهی زنان و کودکان محاسبه و با دستورالعمل‌های پیشنهادی ۱۵ متخصص جراحی فک و صورت دانشگاه علوم پزشکی دانشگاه تهران&nbsp;در سال 1399 مقایسه شد. از آنالیز توصیفی و آزمون رگرسیون لجستیک باینری استفاده شد. تحلیل داده‌ها با نرم ‌افزار SPSS نسخه ۲۵ انجام شد.<br> <strong>یافته‌ها:</strong> در بیماران حساس به پنی ‌سیلین با عفونت شدید، 61% از دندانپزشکان نسخه صحیح تجویز کردند. میزان تجویز صحیح در بیماران با عفونت شدید صفر درصد، در زنان باردار و شیرده با عفونت خفیف 82/24%، در کودکان با عفونت خفیف 24/65%، در بیماران با عفونت خفیف و حساس به پنی‌سیلین 41/62%، در کودکان با عفونت شدید 69/36% و در زنان باردار و شیرده با عفونت شدید 82/33% بود. نتایج نشان داد که با افزایش سن دندانپزشکان، میزان احتمال تجویز&nbsp;آنتی بیوتیک صحیح در عفونت‌ها ادنتوژنیک به طور معنی داری افزایش می‌یابد (036/0P=، 28/0B=).<br> <strong>نتیجه‌گیری:</strong> دندانپزشکان عمومی تهران در تجویز صحیح آنتی ‌بیوتیک برای کنترل عفونت‌های ادنتوژنیک عملکرد مطلوبی نداشتند. بنابراین، برگزاری دوره‌های آموزشی برای بهبود عملکرد آنان ضروری به نظر می‌رسد.</div> <div style="text-align: justify;"><strong>Background and Aims: </strong>Incorrect prescription of antibiotics for odontogenic infections can endanger patients&#39; lives. This study aimed to determine the pattern of antibiotic prescription by general dentists in Tehran and to compare it with the guidelines recommended by maxillofacial surgeons at Tehran University of Medical Sciences in 2019.<br> <strong>Materials and Methods:</strong> A cross-sectional study was conducted on 141 randomly selected dentists in Tehran. The dentists reported their antibiotic prescriptions for various odontogenic infections by completing a questionnaire. The frequency of prescribed antibiotics was calculated considering the severity of the infection, penicillin sensitivity, pregnancy and lactation status of women, and children. These were compared with the guidelines suggested by 15 maxillofacial surgery specialists at Tehran University of Medical Sciences in 2019. Descriptive analysis and binary logistic regression were used. Data analysis was performed using SPSS software version 25.<br> <strong>Results: </strong>Among patients sensitive to penicillin with severe infection, 61% of the dentists prescribed the correct medication. The correct prescription rate was 0% in patients with severe infection, 24.82% in pregnant and lactating women with mild infection, 65.24% in children with mild infection, 62.41% in patients with mild infection and penicillin sensitivity, 36.69% in children with severe infection, and 33.82% in pregnant and lactating women with severe infection. The results showed that older dentists were significantly more likely prescribe antibiotics correctly for odontogenic infections (P=0.036, B=0.28).<br> <strong>Conclusion:</strong> General dentists in Tehran did not optimally prescribe antibiotics for odontogenic infections.
