fa ارزیابی بالینی تأثیر درمان ریشه در بهبود نسوج پریودنتال در مبتلایان به پریودنتیت مزمن پیشرفته عمومی general تحقیقی Research <strong>بیان مسأله:</strong> عقاید ضد و نقیضی در مورد ارتباط بیماریهای پالپ و بیماریهای پریودنتال وجود دارد.  تأثیر متقابل این دو نسج بر روی هم می‌تواند در طرح درمان دندان مورد نظر, اهمیت بسزایی داشته باشد.</p> <p dir="rtl"><strong>هدف:</strong> این تحقیق با هدف بررسی کلینیکی اثر درمان ریشه در بهبود نسوج پریودنتال در دندانهای مبتلا به پریودنتیت مزمن پیشرفته انجام شد.</p> <p dir="rtl"><strong>روش بررسی:</strong> در این مطالعه کارآزمایی بالینی کنترل‌شده تصادفی, 32 دندان که بر اساس آزمونهای حیات پالپ دچار نکروز یا پالپیت غیرقابل برگشت بودند، به صورت Split Mouth از 7 بیمار مبتلا به پریودنتیت مزمن پیشرفته بامعیارهای خاصی انتخاب شدند. دندانها به طور تصادفی به دو گروه مورد و شاهد تقسیم شدند و شاخصهایی چون عمق پاکت، سطح چسبندگی کلینیکی، لقی و نوع تحلیل استخوان در هر دو گروه ثبت شد. ریشه دندانهای گروه مورد درمان شدند. هر دو گروه وارد مرحله اول درمان پریودنتال (جرمگیری) شدند. شاخصهای عنوان شده در فواصل یک و سه ماه بعد نیز در هر دو گروه اندازه‌گیری شدند. به منظور مقایسه عوامل مورد بررسی از آزمونهای آماری مناسب هر بخش شامل Paired t, Wilcoxon و Mann-Whitney استفاده شد.</p> <p dir="rtl"><strong>یافته‌ها:</strong> در گروه مورد سطح چسبندگی کلینیکی در دوره یک ماهه بر خلاف دوره سه‌ماهه کاهش معنی‌داری را نشان داد؛ در گروه شاهد سطح چسبندگی کلینیکی در دوره یک ماهه کاهش یافت ولی از نظر آماری معنی‌دار نبود؛ افزایش این شاخص در فاصله زمانی یک تا سه ماه در گروه شاهد رخ داد امّا از نظر آماری معنی‌دار نبود. در این زمینه در مقایسه بین دو گروه اختلاف معنی‌داری در دوره سه ماهه و فاصله زمانی یک تا سه ماه ایجاد شد (05/0P<). شاخصهای دیگر اختلاف معنی‌داری را درهر گروه و بین گروهها نشان ندادند.</p> <p dir="rtl"><strong>نتیجه‌گیری:</strong> معنی‌دار شدن تغییرات شاخص سطح چسبندگی در مقایسه بین گروهی می‌تواند نشانگر تأثیر احتمالی پاتوژن‌های با منشأ پالپ در پیشرفت بیماری پریودنتال باشد؛ بنابراین در نمونه‌های پیشرفته پریودنتیت بالغین، ارزیابی حیات پالپ و احتمال نیاز به انجام درمان ریشه برای حذف پاتوژن‌های مخرب قبل از درمانهای پریودنتال می‌تواند مؤثر باشد.</p> </span> <p>Statement of Problem: There is a controversy about the relationship between pulpal and periodontal diseases. The interrelationship between pulp and periodontium could have an important effect on the treatment plan of the tooth. </p><p><strong>Purpose:</strong> The aim of the present research is to evaluate root canal therapy effects on periodontal healing of teeth with chronic advanced periodontitis. </p><p><strong>Materials and Methods:</strong> In this randomized controlled clinical trial 32 single rooted teeth which had necrotic pulp or irreversible pulpitis in 7 patients with chronic advanced periodontitis were selected based on specific criteria. Using a split mouth design, teeth were randomly put in two groups of test and control. In the test group root canal therapy ,scaling & root planing were done.In the control group, only scaling & root planing were performed. Clinical parameters including Pocket Depth (PD), Clinical Attachment Level (CAL), mobility, pattern of bone destruction and plaque index (PI) were evaluated in two groups at base line, 1 and 3 months after treatment. Appropriate tests such as paired Wilcoxon and Mann-Whitney were performed. </p><p><strong>Results:</strong> Statistically significant reductions were found in the test group when comparing baseline and one-month post treatment values for Clinical Attachment level (CAL) but not after 3-months. In the control group the CAL reductions were not statistically significant between baseline and one month post-treatment, but a increase were observed between one month and three months after treatment. There was a statstically significant difference between the test and the control groups. Other parameters didn’t show any significant differences in each group and between two groups. </p><p><strong>Conclusion:</strong> Since clinical attachment level was the most important parameter we found it can high lighted the role of pathogene with pulpal origin in progression of periodeontal disease and it is concluded that beside periodontal treatment in some advanced periodontal cases pulp therapy maybe an effective procedure for eleminating destructive pathogens of pulp and causing periodontal healing. </p> پریودنتیت پیشرفته مزمن،حیات پالپ،درمان ریشه،مرحله اول درمان پریودنتال،ترمیم نسوج پریودنتال Advanced chronic periodontitis,Root canal therapy,Periodontal healing,Scaling and root planning 83 91 http://jdm.tums.ac.ir/browse.php?a_code=A-10-25-362&slc_lang=fa&sid=1 R.
