fa تاثیر خارج نمودن دندان های پرمولر بر روی اختلالات مفصل گیجگاهی فکی عمومی general تحقیقی Research <p dir="rtl" align="right"><font face="tahoma,arial,helvetica,sans-serif">به عقیده برخی محققان خارج کردن دندانهای پرمولر و عقب بردن دندانهای قدامی به علت قرار دادن فک پایین در وضعیت خلفی سبب ایجاد علائم اختلال مفصل گیجگاهی فکی (TMD) می شود. هدف از این مطالعه، بررسی اثر خارج نمودن دندانهای پرمولر بر روی اختلالات مفصل گیجگاهی فکی بود. در این مطالعه مداخله ای، چهار گروه ده نفره تحت درمان ارتدنسی قرار گرفتند. درمان در گروه اول بدون خارج کردن دندان، در گروه دوم با خارج کردن دو پرمولر اول بالا، در گروه سوم با خارج کردن چهار پرمولر اول و در گروه چهارم با خارج کردن ترکیبی دندانها انجام شد. میزان Helkimo Score در تمام گروهها قبل و بعد از درمان و نیز یک و دو سال پس از درمان ارزیابی گردید. جهت آنالیز داده ها از آزمونهای ANOVA و Paired t استفاده شد. نتایج نشان داد میزان Helkimo Score پس از درمان و دو سال بعد با قبل از شروع درمان اختلاف معنی داری در همه گروهها دارد. در سال دوم پس از Debanding در مقایسه با یک سال پس از درمان ارتدنسی تفاوتها در هیچ گروهی معنی دار نبود. مقایسه نتایج درمانی بین چهار گروه اختلاف آماری معنی داری را نشان نداد؛ به عبارت دیگر خارج کردن دندانها اثر سوئی بر اختلالات مفصل گیجگاهی فکی نداشته است.</font></p> According to researches, premolar extraction and anterior teeth retraction result in temporomandibular joint disorder due to posterior position of mandible. The aim of this study was to evaluate the effect of premolar extraction on TMJ disorder. In this interventiond study, 4 groups (10 patients for each group) were treated.They were classified as 1- non extraction, 2- extraction of two upper first premolars, 3- extraction of four first premolars, 4- combined extraction groups. Helkimo scores were evaluated before treatment, after treatment, one and two years after treatment. The results were analyzed with ANOVA and paired t-test. At the end of the treatment and two years after treatment in comparison with before treatment, Helkimo score had Significant differentce in all groups (PO.001). In the second year after debanding in comparison with the first year after debanding, no significant differences were seen in each group. No significant difference was seen between the four groups in the final appraisal of TMD.
