fa تخمین تابع هزینه بیمارستانهای تحت پوشش دانشگاه علوم پزشکی شیراز طی سال های 1395-1394 Estimation of the cost function for hospitals affiliated to Shiraz University of Medical Sciences during 2015-2016 مدیریت مالی و اقتصاد در بیمارستان مدیریت مالی و اقتصاد در بیمارستان مقاله ی اصیل Original Article زمینه و هدف: تخمین تابع هزینه ابزاری مناسب در راستای تخصیص بهینه منابع در بیمارستان‌هاست. مطالعه حاضر با هدف برآورد تابع هزینه بیمارستان‌های تحت پوشش دانشگاه علوم پزشکی شیراز انجام شد. مواد و روش‌ها: در مطالعه مقطعی حاضر داده‌های مربوط به منابع، هزینه‌ها و اطلاعات تولید 28 بیمارستان بصورت ماهانه از سال 1394 تا 1395 از سامانه اطلاعات بیمارستانی استخراج شد. برای برآورد تابع هزینه، از مدل کاب-داگلاس و روش‌های اقتصادسنجی داده‌های پانل استفاده شد. تحلیل داده‌ها با نرم‌افزار STATA14 انجام شد. نتایج: کشش هزینه بیمارستان‌ها نسبت به پذیرش بستری و سرپایی در بیمارستان‌های بزرگ به ترتیب 0.555، 0.058 و در بیمارستان‌های کوچک به ترتیب 0.769 و 0.099 (P<0.01) بود. در میان منابع بیشترین تأثیرگذاری بر هزینه کل بیمارستان‌ها مربوط به هزینه سرانه پزشک با ضریب 0.175 در بیمارستان‌های بزرگ و با ضریب 0.481 در بیمارستان‌های کوچک (P<0.01) بود. هزینه متوسط معادل 3640648 و هزینه نهایی برابر با 2654802 تومان بود. شاخص صرفه‌های ناشی از مقیاس در بیمارستان‌های کوچک 868/0، در بیمارستان‌های بزرگ 613/0 و درکل بیمارستان‌ها 729/0 برآورد شد. تعداد تخت فعال، طول مدت اقامت و همچنین آموزشی بودن بیمارستان تأثیر مثبتی (P<0.01) بر هزینه بیمارستان‌ها داشت. نتیجه‌گیری: جهت کاهش هزینه بیمارستان‌ها مدیریت مؤثرترین عوامل ایجادکننده هزینه شامل هزینه‌های پزشکان و دارو کمک‌کننده خواهد بود. همچنین بیمارستان‌ها جهت رسیدن به نقطه بهینه تولید باید با ظرفیت‌ها و منابع تولیدی موجود خدمات بیشتری ارائه دهند. Background: Estimation of cost functions is an appropriate tool for optimal resource allocation in hospitals. The present study aimed at estimating cost function for hospitals affiliated to Shiraz University of Medical Sciences.                                      Materials & Methods: In the current cross-sectional study, inputs, costs and resources data for 28 hospitals were gathered from hospital statistics and information system. Required data were collected monthly for two years (2015-2016). Then, The Cobb-Douglas cost function was estimated using panel data analysis. STATA 14 was used for data analysis.   Results: Estimates showed that hospitals' cost elasticity concerning the inpatient and outpatient admission were 0.555 and 0.058 for large hospitals, and 0.769 and 0.099 for small hospitals (P<0.01). Among hospitals inputs, the cost of a physician with coefficient 0.175 in large hospitals and 0.481 in small hospitals (P<0.01) was most cost driver. In studied hospitals, average cost (36,406,480 Rial) was higher than marginal cost (26,548,020). Moreover, scale economies index were in small, large, and all hospitals were 0.868, 0.613 and 0.729, respectively. The number of active bed, length of stay and teaching activity had a positive effect on hospitals cost (P<0.01).   Conclusion: According to the findings, to control costs, the hospital should manage most cost drivers, such as physicians and pharmaceutical costs.
