fa تامین مالی بیمارستان از طریق بازار سرمایه؛ تبیین عوامل زمینه‌ای موثر در سیاست‌گذاری تامین مالی و دستورگذاری آن Hospital financing through the capital market; Explain the underlying factors in financing policy and its ordering سیاست گذاری ، برنامه ریزی و رهبری و مدیریت در بیمارستان سیاست گذاری ، برنامه ریزی و رهبری و مدیریت در بیمارستان مقاله ی اصیل Original Article زمینه و هدف: بیمارستان بزرگترین و مهمترین واحد نظام سلامت، بیشترین سهم هزینهها را به خود اختصاص داده، بطوریکه دولتها نمیتوانند همه هزینههای آن را تأمین کنند، لذا انجام اصلاحات به منظور تامین مالی اجتنابناپذیر است. در این راستا پژوهشگران به تبیین عوامل زمینهای موثر در سیاستگذاری تامین مالی و دستورگذاری آن پرداختهاند. مواد و روشها: مطالعه حاضر توصیفی که بصورت کیفی و کمی در سال 1397 انجام شده است. جامعه آماری شامل خبرگان بخش سلامت، بازارسرمایه و سازمان تامین اجتماعی بود. از روش هدفمند برای نمونهگیری استفاده شد. ابزار گردآوری داده&rlm;ها فیش کارتها و پرسشنامه نیمهساختاریافته برای انجام مصاحبه با خبرگان بود. برای تحلیل مصاحبهها از نرم افزار MAXQDA,11 و برای معتبرسازی از تکنیک دلفی استفاده گردید. برای تحلیل سیاستگذاری و دستورگذاری از مدلهای والت گیلسون و کینگدان استفاده گردید. نتایج: عوامل زمینهای در سیاستگذاری و دستورگذاری تامین مالی بیمارستانها از بازار سرمایه، عوامل کلان(حاکمیتی و قانونی، سیاسی، اجتماعی، اقتصادی) و عوامل خرد (تولیتی، تامین مالی و فنی) میباشند. جریان مشکل بیمارستانها که عمدهترین آن تامین مالی است، جریان سیاستی که شامل اجرای مدلهای مشارکت بخش دولتی خصوصی است و جریان سیاسی که با تحریمها شروع شد، پنجره فرصتی را برای تامین مالی از بازار سرمایه میگشاید. نتیجهگیری: با وجود اسناد بالادستی و برنامههای توسعه که اجازه خصوصیسازی سازمانهای دولتی را میدهد، بیمارستانها میتوانند بغیر از بودجه دولت و درآمدهای اختصاصی، از استراتژی تامین مالی بخش خصوصی استفاده نمایند، که در این میان بازار سرمایه با توجه به ویژگیهای آن مانند تامین مالی آسان و ارزان قیمت، شفافیت و پاسخگو بودن استراتژی مناسبی میباشد. Background: Hospital, as the largest and most important health system unit, has the highest costs. Governments cannot finance all of the costs of this sector, so reforms are inevitable. In this regard, researchers have explained the underlying factors in financing policy and its commanding. Materials & Methods: This study is a descriptive study done by qualitatively and quantitatively method in 2019. The statistical population included experts from the health sector, the capital market, and the Social Security Organization. A purposive sampling method was used. Data collection tools were receipts and a semi-structured questionnaire for interviews. MAXQDA 11 software was used to analyze the interviews, and the Delphi technique was used for validation. Walt Gilson and Kingdon models were used for policy analysis and commanding.  Results: Underlying factors in policy-making and commanding the financing of hospitals from the capital market are macro-factors (governance and legal, political, social, and economic factors) and micro-factors (stewardship, financing, and technical factors). The financing problem stream of hospitals, the policy flow that includes public-private partnership models, and the political flow that began with sanctions open a window of opportunity to finance hospitals through the capital market.  Conclusion: Upstream documents and development plans allow the privatization of governmental organizations; therefore, in addition to government budgets and dedicated revenues, hospitals can use private-sector financing strategies.
