fa بررسی اپیدمیولوژیک و رابطه سنجی دلایل تقاضای انتقال بین بیمارستانی بیماران استان گیلان با راهبری مرکز پایش مراقبت های درمانی در سال 1401 Epidemiological and correlational investigation of the reasons for requesting inter-hospital transfer of patients in Gilan province under the guidance of the Medical Care Monitoring Center in 1401 ارتقاء سلامت و درمان بیماران در بیمارستان ارتقاء سلامت و درمان بیماران در بیمارستان مقاله ی اصیل Original Article زمینه و هدف: انتقال بین بیمارستانی به جابه جایی بیماران جهت دریافت خدمات تشخیصی، درمانی و پاراکلینیک که میتواند مانع آسیبهای بیشتر و یا درمان موثرتر گردد اطلاق میشود. مطالعه حاضر با هدف جمعآوری و تجزیه و تحلیل اطلاعات مرتبط با بیماران اعزام شده و شناسایی عوامل مسبب انتقال بیماران، طراحی گردید. مواد و روشها: دادههای این مطالعه به صورت مقطعی و ثانویه از روی 11362 فرم درخواست اعزام بیماران در دانشگاه علوم پزشکی گیلان ثبت شده بود، اجرا گردید. دادهها توسط نرم‌افزار آماری Spss نسخه 22 و آزمون آماری Chisquer  تجزیه و تحلیل گردید. نتایج: میزان درخواست انتقال بیماران در سال 1401 نسبت به سال 1400، معادل 9% درصد افزایش داشته است.  بین تقاضای انتقال بیماران با توزیع نامناسب نیروی انسانی متخصص، فقدان سازوکارهای بازخوردی تقاضا، برنامهریزی غیر اثربخش کشیکهای آنکالی و مقیمی پزشکان در بیمارستان مبدأ، مدیریت ناکارآمد منابع و تجهیزات، کمبود امکانات تشخیصی، پزشکی و دارویی و همچنین با فصول سال رابطه معنادار وجود دارد (0/05>p). بین عدم پذیرش بیماران با فقدان یا کمبود تخت مورد نیاز بیمار در بیمارستان مقصد رابطه معنادار وجود دارد (0/05>p). نتیجهگیری: توزیع نامناسب نیروی انسانی متخصص با توجه به نیاز منطقه، برنامهریزی غیر اثربخش کشیکهای آنکالی و مقیمی، مدیریت ناکارآمد منابع در سطح بیمارستان و دانشگاه به عنوان دلایل اصلی تقاضا برای انتقال بیماران شناسایی شده است. تشکیل کارگروه ویژه جهت شناسایی، ارزیابی، برنامهریزی و نظارت بر رفع موانع به صورت متمرکز و تشکیل حلقههای بهبود کیفیت میتواند راهگشا باشد. Background and purpose: Inter-hospital transfer refers to the transfer of patients to receive diagnostic, therapeutic and paraclinical services that can prevent more injuries or more effective treatment. The present study was designed with the aim of collecting and analyzing information related to sent patients and identifying the factors that cause the transfer of patients. Methods: This cross-sectional study was conducted on 11,362 patient referral forms registered at Gilan University of Medical Sciences. Data were analyzed by Spss statistical software version 22 and Chisquer statistical test. Results: The amount of patient transfer requests in 1401 compared to 1400 has increased by 9%. There is a relationship between the demand for transfer of patients with inappropriate distribution of specialized human resources, lack of demand feedback mechanisms, ineffective planning of nursing staff and residency of doctors in the hospital of origin, inefficient management of resources and equipment, lack of diagnostic, medical and pharmaceutical facilities, as well as with the seasons of the year. There is significant (p>0.05). There is a significant relationship between non-acceptance of patients and the lack or lack of beds needed by the patient in the destination hospital (p<0.05). Conclusion: Improper distribution of specialized manpower according to the needs of the region, ineffective planning of resident guards, inefficient management have been identified as the main reasons for the demand for patient transfer.
