fa بررسی سهم هزینه‌های دارو و ملزومات مصرفی پزشکی از کل هزینه‌های نظام سلامت: یک مطالعه در سطح ملی Examining the Share of Pharmaceutical and Medical Consumable Costs in Total Health System Expenditure: A National Study in Iran مدیریت تسهیلات (فیزیکی ، تاسیساتی و تجهیزاتی ) در بیمارستان مدیریت تسهیلات (فیزیکی ، تاسیساتی و تجهیزاتی ) در بیمارستان مقاله ی اصیل Original Article زمینه و هدف: پیشرفت سریع در زمینه‌های گوناگون علمی، اقتصادی، اجتماعی و فرهنگی، صنعت بهداشت و درمان را نیز دستخوش تغییرات عظیمی کرده است. سازمان بهداشت جهانی، توسعه تجهیزات و لوازم و ملزومات پزشکی را برای کاهش نابرابری‌ها بهداشتی و دستیابی به اهداف نظام سلامت ضروری دانسته و به کشورهای عضو توصیه کرده است تا بخش اعظمی از هزینه‌های نظام سلامت را به این امر اختصاص دهند. لذا مطالعه حاضر با هدف تعیین سهم هزینه‌های دارو و ملزومات مصرفی پزشکی از کل هزینه‌های نظام سلامت ایران انجام شده است. مواد و روش‌ها: این مطالعه از نوع توصیفی، مقطعی و گذشته‌نگر است که به جمع‌آوری و تحلیل داده‌های سوابق پزشکی بیماران سرپایی و بستری در طول سال‌های 1397 تا 1399 پرداخته است. هزینه‌های وسایل و لوازم مصرفی ترکیبی در نظام سلامت، از داده‌های حساب‌های ملی سلامت و داشبورد SPAS استخراج شد. سپس تحلیل آماری پیشرفته با استفاده از نرم‌افزار اکسل برای تعیین سهم هزینه‌های بهداشتی درمانی و هزینه‌های دارو و لوازم مصرفی ترکیبی در نظام سلامت انجام شد. نتایج: در طی سال‌های 1397 لغایت 1399، هزینه‌های دارو و لوازم مصرفی پزشکی برای بیماران بستری 12% از هزینه‌های کل سلامت را تشکیل می‌دهد که به طور سالانه 8/1% کاهش داشته است. همچنین هزینه‌های سرپایی 5/19% از کل هزینه‌ها را به خود اختصاص داده که به طور سالانه 7/0% افزایش دارد. بیمه سلامت پایه حدود 3/10% از بودجه خود را برای این هزینه‌ها تخصیص داده است که به طور کلی برابر با 5/31% از هزینه‌های کل نظام بهداشتی درمانی کشور است. نتیجه‌گیری: مدیریت مؤثر زنجیره تأمین و برنامه‌ریزی استراتژیک برای کاهش وابستگی به واردات، پاسخگویی مناسب به تقاضاهای فزاینده بهداشتی درمانی، نظارت و پایش بر قیمت‌های دارو و ملزومات مصرفی پزشکی و کاهش اثرات سوء تورم، تحریم و کمبود نقدینگی، افزایش سهم بودجه بهداشت و درمان از تولید ناخالص داخلی و گسترش پوشش بیمه‌ای به منظور ایجاد حفاظت مالی از افراد جامعه و دستیابی به پوشش همگانی سلامت را در نظام سلامت بسیار حیاتی است. Background and purpose: The healthcare sector is undergoing a profound transformation driven by rapid scientific, economic, and social advancements. The World Health Organization (WHO) emphasizes the necessity of medical equipment and supplies for reducing health disparities and achieving strategic health system objectives. However, there is a notable gap in the literature regarding this cost component within the Iranian health system. This study, therefore, aims to quantify the share of pharmaceutical and medical consumable costs in Iran's total health system expenditure. Methods: This retrospective, cross-sectional study analyzed outpatient and inpatient medical records from 2018 to 2020. Data on the costs of pharmaceuticals and medical consumables were extracted from Iran's National Health Accounts (NHA) and the SPAS dashboard. The collected data were statistically analyzed to determine the proportional costs of these items relative to total national health expenditure. Results: Between 2018 and 2020, inpatient pharmaceutical and medical consumable costs constituted 12% of the total health expenditure, demonstrating an annual decrease of 1.8%. In contrast, outpatient costs accounted for 19.5% of the total, with an annual increase of 0.7%. Basic health insurance allocated approximately 10.3% of its budget to cover these items, which corresponds to 31.5% of the nation's total healthcare spending. Conclusion: The findings highlight the critical need for strategic interventions within Iran's health system. Key implications include the necessity for effective supply chain management, strategic planning to reduce import dependency, and robust mechanisms for monitoring prices. Mitigating the adverse effects of inflation and sanctions is paramount.
