fa الگوی ارزیابی واقع‎بینانه از اصول تولید مشترک برنامه‎های بهداشتی‌ و درمانی A Realistic Evaluation Pattern of Co-Production Principles in Healthcare Programs سایر سایر مقاله ی اصیل Original Article زمینه و هدف: بیماری‌های غیر واگیر عامل 76% مرگ‌ومیر در ایران و مدیریت آن وابسته به توانمندسازی جامعه و یکپارچه&lrm;سازی ساختارها در ارائه خدمات پایدار است. تولید مشترک راهبردی نوین و بین&lrm;رشته&lrm;ای است که به حل نیازهای چندجانبه کاربران می&lrm;پردازد. هدف پژوهش حاضر، شناسایی مضامین کلیدی تولید مشترک در مدیریت بیماری&lrm;های مزمن از دیدگاه مدیران اجرایی با تلفیق دو رویکرد اصول تولید مشترک و چارچوب ارزیابی واقع&lrm;بینانه برای دستیابی به الگوی کاربردی برای بهبود سیاست&lrm;گذاری و ارائه راهکارهای زمینه محور است. مواد و روش‌ها: این پژوهش با به‌کارگیری روش&lrm;شناسی کیفی و استراتژی تحلیل مضمون، به بررسی داده‌های حاصل از مطالعات کتابخانه&lrm;ای و مصاحبه‌های نیمه ساختاریافته با ۱۷ مدیر اجرایی بهداشتی و &lrm;درمانی به‌عنوان ذینفعان کلیدی خبره از طریق نمونه‌گیری هدفمند در 1403 به دست آمد. نتایج: یافته&lrm;ها نشان &lrm;داد که این الگو شامل 71 مضمون پایه، 20 مضمون سازمان&lrm;دهنده و 6 مضمون فراگیر است. توسعه فرآیندهای مشارکتی در مداخلات بالینی، ایجاد خدمات جانبی عدالت&lrm;محور، شناسایی گروه&lrm;های گوناگون، قابلیت ارائه‌دهندگان، واکنش کاربران و منافع مشترک بازیگران، مدیریت بیماری&lrm;های مزمن را عملیاتی می&lrm;کند. نتیجه&lrm;گیری: این پژوهش، با تلفیقی نوآورانه، به بازتعریف مفاهیم اولیه و رفع ابهامات نظری، هم&lrm;چنین ارائه الگویی زمینه محور برای مدیریت بیماری&lrm;های مزمن پرداخت. پیشنهادات کاربردی شامل طراحی مشترک خدمات ترکیبی برای رفع چالش&lrm;های چندجانبه، توسعه شیوه&lrm;های تعاملی و اختیارات در سطوح مختلف ساختار اجرایی برای رفع تمرکزگرایی، پایش هم‌زمان شاخص&lrm;های بالینی رفاهی از طریق پلتفرم&lrm;های امن داده و ایجاد مشوق&lrm; در تمامی سطوح ساختاری است. پیشنهادات تئوریک شامل پژوهش&lrm;های طولی برای ارزیابی پیامدها در بلندمدت و به‌کارگیری سایر رویکردهای کیفی برای شمول دیدگاه&lrm; بیماران و کارشناسان اجرایی است. Background and purpose: Non-communicable diseases (NCDs) account for 76% of mortality in Iran, where effective management hinges on community empowerment and integrated service delivery. Co-production, an innovative strategy involving shared power between service users and providers, is a promising approach for addressing the multifaceted needs of patients with chronic conditions. This study aimed to develop a context-sensitive framework for implementing and evaluating co-produced NCD management programs. We achieved this by integrating the core principles of co-production with a realist evaluation lens, drawing on the perspectives of senior healthcare managers. Methods: Employing a qualitative methodology, this study conducted a thematic analysis of data derived from a comprehensive literature review and semi-structured interviews. A cohort of 17 senior healthcare managers, selected via purposive sampling in 2025, participated as key informants. Results: The analysis yielded a hierarchical thematic structure comprising 71 basic themes, 20 organizing themes, and 6 overarching global themes. These global themes, which form the core of the proposed framework, are: (1) Developing Participatory Clinical Processes, (2) Establishing Equity-Oriented Support Services, (3) Recognizing Diverse User Groups, (4) Enhancing Provider Capabilities, (5) Understanding User Responses, and (6) Fostering Mutual Benefits among Stakeholders. The dynamic interplay of these themes is crucial for operationalizing effective NCD co-production. Conclusion: By innovatively integrating co-production principles with a realist evaluation framework, this study offers a context-sensitive model that clarifies theoretical concepts for practical application in NCD management. A key implication is the necessity for monitoring systems that track both clinical and well-being outcomes, supported by multi-level incentive structures. This research provides a foundational model for policymakers to develop, implement, and evaluate more effective and equitable co-produced healthcare programs.
