fa بررسی تأثیر اسپلینت استاتیک cock up c-bar  بر  قدرت، اسپاستی سیته، دامنه حرکتی و عملکرد دست غالب کودکان فلج مغزی دایپلژی اسپاستیک عمومى General پژوهشي Research <strong>زمینه</strong> <strong>و</strong> <strong>هدف:</strong> کودکان فلج مغزی از مراجعین اصلی به مراکز کاردرمانی هستند.دست به عنوان ابزاری مهم در عملکرد روزمره زندگی به حساب می آید. در اکثر کودکان فلج مغزی عملکرد دست دچار اختلال می شود. ارتزها و اسپلینت­ها به منظور بهبود پوزیشن، دامنه حرکتی، کیفیت حرکت و عملکرد دست مورد استفاده قرار می گیرند. هدف از این مطالعه بررسی تأثیر اسپلینت استاتیک cock up c-bar بر  قدرت، اسپاستی سیته، دامنه حرکتی و عملکرد دست غالب کودکان فلج مغزی دایپلژی اسپاستیک 8-12 ساله می باشد. <p dir="rtl"><strong>روش</strong> <strong>بررسی:</strong> روش مطالعه در این پژوهش از نوع مداخله ای و به صورت قبل - بعد می باشد. 13 بیمار فلج مغزی دایپلژی اسپاستیک 8 تا 12 ساله از میان افرادی که در مدارس جسمی حرکتی شهر تهران تحصیل می کردند و معیار های ورود را داشتند انتخاب شدند. بیماران از یک اسپلینت استاتیک cock up c-bar ( با زوایای 10 درجه اکستانسیون مچ دست، پالمار ابداکشن و آپوزیشن شست) به مدت 2 ماه، 2 ساعت در روز  و 4 تا 6 ساعت در شب استفاده کردند. در این مطالعه از تست جبسون تیلور برای ارزیابی عملکرد دست، مقیاس اشورث اصلاح شده برای ارزیابی اسپاستی سیته شست، مچ دست و آرنج، گونیامتر برای ارزیابی دامنه ی حرکتی شست، مچ دست و آرنج و داینامومتر برای ارزیابی قدرت دست استفاده شد.</p> <p dir="rtl"><strong>یافته­ ها:</strong> نتایج ارزیابی­ها نشاندهنده ی بهبود معنادار در عملکرد دست (P <0/001)، اسپاستی سیته مفصل آرنج   (P<0/008)، مفصل مچ دست (P<0/001)، پالمار ابداکشن شست (P<0/002) و دامنه حرکتی مفصل مچ دست (P<0/001)، دامنه حرکتی پالمار ابداکشن دست (P<0/001) و قدرت گریپ (P<0/001) بود اما تأثیر معناداری بر روی دامنه حرکتی آرنج (P<0/160 ) نداشت.</p> <p dir="rtl"><strong>نتیجه</strong> <strong>گیری:</strong> اطلاعات حاصل از تحقیق حاضر نشان می­دهد که استفاده از اسپلینت استاتیک cock up c-bar به مدت 2 ماه، 2 ساعت در روز و 4 تا 6 ساعت در شب، می­تواند روش مؤثری جهت بهبود قدرت و عملکرد دست، اسپاستی­سیته آرنج، مچ دست و شست و دامنه­ی حرکتی شست و مچ دست کودکان فلج مغزی دایپلژی اسپاستیک باشد.</p> <strong>Background and Aim</strong>: Cerebral Palsy (CP) children are the most clients referred to occupational therapy settings. Hand function is important in Activities of Daily living .There is a hand dysfunction in most cerebral palsy children. Orthosis and splints are commonly used to improve the position, range of motion, quality of movement and function of a person's arm or hand. The aim of this study was to investigate the effects of static cock up c-bar splint on strength,spasticity, range of motion, and dominant hand function in spastic diplegic children with 8 to 12 years old.<strong> </strong> <p><strong>Materials and Methods: </strong>This interventional study is in a before-after design. Thirteen spastic diplegic cerebral palsy children with 8 to 12 years old were selected through students who studied in physical disabled schools in Tehran city and had the inclusion criteria. Patients used a static cock up c-bar splint (wrist in 10 degrees of extension and thumb in palmar abduction and opposition) for two months, 2 hours daily and 4-6 hours at night. In this study the Jebsen Taylor test was used to evaluate hand function, the Modified Ashworth Scale was used to assess spasticity of thumb, wrist and elbow, ROM of thumb, wrist and elbow were tested with goniometer and power grip was evaluated with dynamometer.   <br></p> <p> <strong>Results</strong> <strong>:</strong> Results showed a significant improvement in hand function (p<0.001), in wrist`s spasticity (p<0.001), elbow`s spasticity(p<0.008), palmar abduction of thumb spasticity (p<0.002), wrist`s ROM (p<0.001), Palmar abduction of thumb ROM(p<0.001) and power grip(p<0.001).
