fa اثربخشی رویکرد درمانی " شناخت‌گرا به عملکرد کاری روزمره" بر عملکرد حرکتی کودکان 10-7 سال با نقص توجه همراه با بیش فعالی عمومى General پژوهشي Research <strong>زمینه و هدف:</strong> اگرچه مشکلات حرکتی به عنوان ملاک‌های تشخیصی اختلال نقص توجه همراه با بیش فعالی در <a name="OLE_LINK2"></a><a name="OLE_LINK1">ویرایش چهارم راهنمای تشخیصی و آماری اختلال‌های روانی </a>مطرح نشده است، عموماً این اختلال همراه با این کودکان مشاهده می‌گردد. رویکرد "شناخت‌گرا به عملکردکاری روزمره" (Cognitive Orientation to daily Occupational Performance) یا به اختصار CO-OP رویکردی است که اهداف عملکردی کودک را مخاطب قرار می‌دهد. در این رویکرد درمانگر کودک را حمایت می‌کند تا از راهبرد‌های شناختی استفاده کند و طی فرآیند کشف راهنمایی شده و استفاده از راهبردهای خاص حوزه مشکلات عملکرد کاری را بهبود دهد. از اهداف CO-OP بهبود کارکرد حرکتی برای از بین بردن موانع، و در نتیجه تسهیل فعالیت‌های کودک و ارتقاء مشارکت وی در فعالیتهای روزمره می‌باشد. <p dir="rtl"><strong>روش بررسی:</strong> در این تحقیق از "مطالعه تک موردی تجربی" استفاده شد تا اثربخشی مداخلهCO-OP  را در کودکان با نقص توجه همراه با بیش فعالی بررسی کند. بر این اساس شش کودک برای شرکت در مداخله انتخاب شدند. مقیاس‌های ارزیابی شامل مقیاس کانادایی عملکرد کاری(Canadian Occupational Performance Measure: COPM) و مقیاس کفایت حرکتی برونینکز اوزرتسکی بود. در ابتدا و قبل از آغاز مداخله، COPM برای تعیین اهداف عملکردی و رتبه‌دهی عملکرد و رضایت برای اهداف انتخابی، توسط کودک و والدین به صورت مجزا اجرا شد. مداخله درمانی بر اساس برنامه درمانی CO-OP که پلاتاجکو و همکاران در سال 2001 طراحی کردند، انجام گردید. در طول مداخله، کودک راهبرد کلی حل مسئله را یاد می‌گیرد و برای کشف راهبرد‌های خاص حوزه راهنمایی می‌شود. برنامه درمانی شامل 12 جلسه مداخله به قرار هفته‌ای دو جلسه، هر جلسه 60-45 دقیقه در مرکز توانبخشی شهداء جلائی پور بود. ارزیابی‌های حرکتی به صورت دوره‌ای طی جلسات درمانی اجرا شد و نتایج ثبت گردید، همچنین مقیاس COPM بعد از مداخله مجددا اجرا شد.</p> <p dir="rtl"><strong>یافته ها:</strong> رتبه‌دهی والدین و کودک به عملکرد و رضایت اهداف عملکردی بر اساس COPM و آنالیز دیداری نتایج برونینکز اوزرتسکی در دو مرحله پایه‌ای و مداخله اثربخشی درمان را نشان داد، و کاربرد CO-OP را در کودکان با نقص توجه همراه با بیش فعالی تأیید کرد.</p> <p dir="rtl"><strong>نتیجه گیری:</strong> ارائه پروتکل درمانی که ممکن است موجب بهبود مشکلات حرکتی کودکان با نقص توجه همراه با بیش فعالی و همچنین هدایت کننده مطالعات آینده در جهت به کاربردن CO-OP بر پایه نتایج درمان CO-OP در درمان مشکلات حرکتی کودکان با نقص توجه همراه با بیش فعالی باشد.</p> <strong>Background and Aim</strong><strong>:</strong> Although motor difficulties are not considered as a diagnostic criterion in Diagnostic and Statistical Manual of Mental Disorders: However its association with Attention Deficit Hyperactivity Disorder (ADHD) is commonly reported. CO-OP is a problem-solving approach developed to help children with Developmental Coordination Disorder and addresses their motor-based problems.  In this approach, therapist supports the children to use their cognitive strategies in a process of guided discovery to solve occupational performance problems. The objectives of CO-OP are to improve the motor performance of children with ADHD by helping them to discover various procedures to remove barriers and enhance supports by facilitating activities and participation. This study was designed to examine the influence of CO-OP on motor-based occupational performance of children with ADHD.<strong> </strong> <p><strong>Materials and Methods:</strong> A single case experimental design (multiple baseline) was used to examine the influence of a 12 weeks intervention of CO-OP (twice a week) on Six children with ADHD. Each<em> </em>child selected three different goals to practice during intervention. Outcome measures included the Canadian Occupational Performance Measure (COPM), and the Bruininks-Oseretsky Test of Motor Proficiency and Performance (BOTMP). At first and prior to the intervention, the COPM was administered to the children and parents separately to obtain a rating of performance and satisfaction of the child in selective goals. The CO-OP intervention protocol was based on Polatajko and colleagues' work in 2001. During intervention, children were taught the global problem solving strategies helping them to discover and perform domain-specific strategies. Therapist and family also supported children to achieve the performance of selected goals.</p> <p><strong>Results: </strong>The results of this study demonstrated improvements in both goals and motor performance in the participants due to the intervention.
