fa بررسی بیان ژن تلومر از انسانی (hTERT) در فازهای مختلف لوسمی میلوژن مزمن (CML) مدیریت خدمات بهداشتی درمانی Hospital Managment پژوهشی اصيل Original Research <strong>زمینه و هدف : </strong>لوسمی میلوژن مزمن (CML) یک بیماری کلونال سلول بنیادی خون ساز است که با جابجایی متقابل کروموزوم های 9 و 22 (کروموزوم فیلادلفیا) مشخص می شود. تلومراز آنزیمی است که با اضافه کردن توالی های تکراری تلومریک به انتهای کروموزوم ها باعث حفظ انسجام و یکپارچگی کروموزوم ها می گردد. نشان داده شده است که بیان زیر واحد کاتالیتیکی این آنزیم (hTERT) در اکثر سلولهای سرطانی و نئوپلازی ها افزایش بیان دارد. از این روی در این مطالعه به بررسی بیان ژن hTERT در فازهای مختلف بیماری CML پرداخته شد.<br><strong>روش بررسی :  </strong>مطالعه انجام شده از نوع بررسی مقطعی (Cross sectional) بود. 52 نمونه به صورت تصادفی از 26 بیمار CML در فاز مزمن قبل از درمان (26 نمونه) و فواصل زمانی 3 ماه پس از درمان (26 نمونه)، 9 بیمار در فاز شتاب گیرنده و 12 بیمار در فاز بلاستیک که به بخش خون بیمارستان شریعتی تهران مراجعه کرده بودند، مورد مطالعه قرار گرفتند. بیان ژن hTERT در 73 نمونه بیماران فوق با روش RT-PCR (Reverse Transcriptase- Polymerase Chain Reaction) مورد بررسی و مقایسه قرار گرفت. جهت تحلیل نتایج نرم افزار SPSS 15 استفاده شد.<br><strong>یافته ها : </strong>از 45 بیمار مطالعه شده 33 نفر(73%) مرد و 12 نفر(27%) زن بودند. بیماران مورد مطالعه در 3 گروه سنی 29-17، 40-30 و 75-41 ساله تقسیم بندی شدند. بطور کلی از 73 نمونه بررسی شده، 43 نمونه(9/58%) از نظر RT-PCR ژن hTERT مثبت و 30 نمونه(1/41%) منفی شدند. در فاز مزمن 2/69%، پس از درمان 5/38%، فاز شتاب گیرنده 6/55% و فاز بلاستیک 3/83% از نتایج مثبت بودند؛ که این اختلاف از نظر آماری معنی دار بود (p<0.05). <br><strong>نتیجه گیری : </strong>تفاوت معنی دار بیان ژن hTERT بین فازهای مزمن، پس از درمان و بلاستیک می تواند نشانگر مولکولی مناسبی در پیگیری درمان و عامل پیش آگهی دهنده در سیر بیماری باشد.</p> </span> <p style="text-align: justify"><strong>Background and Aim:</strong><strong> </strong>Chronic Myelogenous Leukemia (CML) is a clonal hematopoietic stem cell disorder characterized by a translocation between chromosome 9 and 22 called Philadelphia Chromosome. Telomerase- essential enzyme that adds telomeric repeats into the telomeres- maintains integrity of  chromosomal ends. Most normal somatic cells do not express hTERT (catalytic subunit of telomerase) but most neoplasia and cancer cells express it. In this study we evaluated the hTERT expression in patients with CML at different phases of the disease.<br><strong></strong></p> <p style="text-align: justify"><strong>Materials and Methods:</strong><strong> </strong>In this cross sectional study, 73 samples of 45 patients with CML were studied. Twenty six of samples were taken from patients in chronic phase before therapy and 26 samples three month after therapy. Nine samples were taken in accelerated phase and 12 in blastic phase. hTERT expression was studied by RT-PCR and the results were analyzed using SPSS 15.<br><strong></strong></p> <p style="text-align: justify"><strong>Results:</strong> Thirty three (73%) of patients were male and 12 (23%) were female. Patients were divided into three age groups 17-29, 30-40 and 41-75 years. Of 73 samples, 43 samples (58.9%) were positive for hTERT and 30 samples (41.1%) were negative for this gene. In chronic phase (before therapy) 69.2% of the samples were PCR positive, but after therapy only 38.5% of samples were PCR positive. In accelerated and blastic phases, 55.6% and 83.3% of samples were PCR positive respectively.
