fa تطابق اهداف ابتدایی و ساختار سازمانی فعلی در نظام ارائه مراقبت بهداشتی اولیه در ایران: مطالعه‌ی مروری نظام‌مند مدیریت خدمات بهداشتی درمانی Hospital Managment پژوهشی اصيل Original Research <p dir="RTL" style="text-align: justify; margin-right: -0.35pt;"><strong>زمینه و هدف:</strong> در سالهای اخیر، اصلاحات نظام سلامت ایران در حوزه بهداشتی در قالب طرح پزشک خانواده و نظام ارجاع در حال انجام می‌باشد. با توجه به تغییرات اساسی در اهداف و راهبردهای کلان، لزوم تغییر در ساختار سازمانی که متناسب با این اهداف باشد، بیش از گذشته احساس می‌شود. در این مطالعه‌ی مروری تلاش خواهد شد به کلیه موارد در زمینه‌ی ساختار سازمانی و چالش‌های آن در نظام ارائه مراقبت‌های بهداشتی ایران که در طی پانزده سال اخیر منتشر شده است، اشاره شود و نتایج آنها جمع‌بندی گردد.</p> <p dir="RTL" style="text-align: justify; margin-right: -0.35pt;"><strong>روش بررسی:</strong> این مقاله یک مطالعه مروری نظام‌مند می‌باشد. فهرست کاملی از شواهد مرتبط، از طریق جستجوی نه پایگاه داده‌ای الکترونیکی، فهرست منابع مقالات یافت شده، پایگاه‌های اطلاعاتی نظام سلامت و تماس با کارشناسان، جمع‌آوری شد.</p> <p dir="RTL" style="text-align: justify; margin-right: -0.35pt;"><strong>یافته‌ها: </strong>در بررسی 14 مورد مقاله و گزارش نهایی که در دو گروه مقالات و خط‌مشی‌های بالادستی و قوانین تقسیم شده‌اند، نتایج به دست آمده نشان‌دهنده اثربخشی پایین ساختار سازمانی نظام ارائه مراقبت‌های بهداشتی در سطوح مختلف می‌باشد. اغلب مطالعات، به اصلاح ساختار سازمانی موجود، متناسب با اهداف کلان و راهبردهای نظام سلامت توصیه کرده‌اند. از بین ابعاد مختلف ساختار سازمانی، بعد تمرکز و فرایند تفویض اختیار بیشتر از بقیه مورد توجه قرار گرفته است.</p> <p dir="RTL" style="text-align: justify; margin-right: -0.35pt;"><strong>نتیجه‌گیری:</strong> در شرایط فعلی، با توجه به تغییر اهداف و راهبردهای نظام سلامت در کشور ایران، اصلاح ساختار سازمانی حوزه بهداشتی به ویژه در سطوح محیطی امری ضروری و غیر قابل اجتناب است.</p> <p style="text-align: justify;"><strong>Background and Aim:</strong> In recent years, the family physician plan has been implemented as a main strategy of the health system in Iran. Therefore, the necessity to reform organizational structure based on new goals and strategies is felt more than before. The aim of this study is to review and summarize all cases about Iran&rsquo;s organizational structure and its challenges in primary healthcare system.</p> <p style="text-align: justify;"><strong>Materials and Methods:</strong> This study is a systematic review. All relevant databases, bibliography of related papers, and laws were searched using appropriate search strategies and keywords. To evaluate the quality of selected papers, CASP tool was applied by two experts, and their choices were discussed to reach a final decision.</p> <p style="text-align: justify;"><strong>Results: </strong>Fourteen final cases were categorized in two groups: papers and upstream policies and laws. The results revealed the ineffectiveness of current organizational structure at different levels. The majority of the findings suggested proper reforms in the system.
