fa اثر پانکراتکتومی ساب توتال و تقریبا کامل در درمان شیرخواران مبتلا به هیپوگلیسمی هیپرانسولینمیک مرکز طبی کودکان، 79-1364 <p align="right" dir="rtl"><font face="tahoma,arial,helvetica,sans-serif"><strong>مقدمه:</strong> در درمان هیپوگلیسمی هیپرانسولینمیک قبلا از جراحی ساب توتال استفاده می شده که به دلیل عود بالا از جراحی نزدیک به کامل (near total) استفاده نگردیده که این مطالعه استفاده از این روش را در نزدیولاستوزیس نشان می دهد. </font></p><p align="right" dir="rtl"><font face="tahoma,arial,helvetica,sans-serif"><strong>مواد و روشها:</strong> مطالعه در 15 بیمار مرکز طبی کودکان که دچار هیپوگلیسمی هیپرانسولینمیک بوده اند که در سال 1364 تا 1379 در این مرکز تحت جراحی قرار گرفته اند. </font></p><p align="right" dir="rtl"><font face="tahoma,arial,helvetica,sans-serif"><strong>یافته ها:</strong> از 15 موردیکه تحت جراحی قرار گرفتند 3 مورد ساب توتال پانکراتکتومی انجام شد که هر سه مورد عود کرد ولی در 12 بیماری که جراحی نزدیک به کامل انجام شد عودی وجود نداشت. در مجموع فقط یک مورد مورتالیتی بعلت سپسیس وجود داشت و عوارض بعد از عمل نیز نسبتا کم بود و شایعترین یافته بعد از عمل هیپوگلیسمی گذرا بعد از عمل بود. </font></p><p align="right" dir="rtl"><font face="tahoma,arial,helvetica,sans-serif"><strong>نتیجه گیری و توصیه ها:</strong> انجام جراحی نزدیک به کامل به دلیل بهبودی و کنترل بهتر هیپرانسولنییسم نتایج عالی بهمراه دارد و در صورت مهارت جراح و دقت کافی یک روش safe و مطمئن با عوارض کم برای درمان هیپوگلیسمی هیپرانسولینمیک می باشد. </font></p> <p> <strong>Background:</strong> There are many surgical approaches for hyperinsulinemic hyperglycemic treatment. Subtotal pancreatectomy which has been used for many years, is abounded know due to high recurrence rate, and has been replaced by near total resection. This study focuses on “near total resection”. </p><p><strong>Materials and Mehods:</strong> In this study 15 cases of hyper insulinemic hyperglycemia patients which has been operated since year 1985 till 2000 in the Children's Medical Center, has brought into consideration and described as a case series study. </p><p><strong>Results:</strong> From the patients, three cases were operated by subtotal pancreatomy and all of them have had recurrence. In the remaining twelve cases, the method used was different and “near subtotal pancreatomy” were used. The recurrence rate in this group was zero and there were not also any case of sepsis.
