other فراوانی انواع عوارض چشمی داروهای ضد فاکتور نکروز توموری آلفا در بیماران مبتلابه بیماری‌های روماتیسمی Frequency types of ocular side effects of anti-TNF-α drugs in patients with rheumatic diseases مقاله اصیل Original Article زمینه و هدف: بیماری‌های التهابی خودایمنی اغلب با عوارض شدید و مزمنی همراه هستند و تقریبا 4/9%-6/7 از جمعیت جهان را تحت تاثیر قرار می‌دهند. مطالعه حاضر با هدف بررسی فراوانی عوارض چشمی داروهای ضدفاکتور نکروز توموری آلفا در بیماران مبتلابه روماتیسم انجام گرفت. روش بررسی: در این مطالعه مقطعی، 122 بیمار مبتلابه بیماری‌های روماتیسمی مورد بررسی قرار گرفتند که تحت درمان با داروهای مهارکننده فاکتور نکروز توموری آلفا بودند و به بیمارستان گلستان اهواز در فاصله زمانی 1398 تا 1401 ارجاع شدند. عوارض چشمی پس از شش ماه درمان با داروهای مهارکننده فاکتور نکروز توموری آلفا توسط چشم پزشک ارزیابی شد. یافته‌ها: از 122 بیمار، 59 نفر (36/48%) به آرتریت روماتوئید و 36 نفر (51/29%) به اسپوندیلیت آنکیلوزان مبتلا بودند. در 38/7% از عوارض چشمی گزارش شده، یووئیت قدامی شایع‌ترین عارضه (28/3%) بود. نوع داروی ضدفاکتور نکروز توموری آلفا با بروز عوارض چشمی ارتباط معنا‌داری نداشت و عوارض چشمی بیشتر در بیمارانی مشاهده شد که اتانرسپت مصرف می‌کردند، اما تفاوت معنا‌داری در مصرف دو داروی دیگر مشاهده نشد (1/0=P). میانگین طول مدت بیماری در بیماران با عوارض چشمی بالاتر از گروه بیماران بدون عوارض چشمی بود (13/4±5/47 در برابر 58/2±3/22 سال) و (03/0=P). ارتباط معناداری بین طول مدت مصرف داروهای ضدفاکتور نکروز توموری آلفا و عوارض چشمی مشاهده نشد (66/0P=). نتیجه‌گیری: باتوجه به بروز بیشتر عوارض چشمی در بیمارانی که داروی اتانرسپت مصرف می‌کردند و همچنین وجود ارتباط معنادار میان طول مدت بیماری با بروز عوارض چشمی، نظارت بر درمان طولانی مدت و فالوآپ بیماران روماتیسمی که داروی ضدفاکتور نکروز توموری آلفا را مصرف می‌کنند از اهمیت بالایی برخوردار است.   Background: Autoimmune inflammatory diseases, which are often associated with severe and chronic complications, affect approximately 7.6-9.4% of world's population.  The present study was conducted with the aim of investigating the frequency of ocular side effects of anti-TNF-α drugs in patients with rheumatic diseases. Methods: In this analytical epidemiological study, 122 patients with rheumatoid arthritis, psoriatic arthritis and ankylosing spondylitis who were treated with TNF-α inhibitory drugs (including Infliximab, Adalimumab and Etanercept) referred to Golestan Hospital of Ahvaz, Iran, in 2019-2022 were examined. Ocular complications including anterior uveitis, macular edema, anterior ischemic optic neuropathy, diplopia with paired neurological involvement, anterior uveitis with diplopia and keratitis sicca were evaluated by an ophthalmologist after six months of treatment with TNF-α inhibitory drugs. Infliximab was used on day zero, week two and four, and then every 6 to 8 weeks with a dose of 3 to 5 per kilogram of body weight. Adalimumab dosage was 40 mg every other week and Etanercept was taken at a dose of 50 mg weekly. Results: From 122 patients, 59 (48.36%) and 36 (29.51%) had rheumatoid arthritis and ankylosing spondylitis, respectively. Among 7.38% of reported ocular complications, anterior uveitis was the most common complication (3.28%). Type of anti-TNF-α drug had no significant correlation with the occurrence of ocular complications and eye complications were mainly observed in patients who used etanercept, but there was no significant difference compared to the other two drugs (P=0.1). The mean duration of disease in patients with and without eye complications was 5.47±4.13 and 3.22±2.58 years, respectively (P=0.03). No significant relationship was observed between the duration of anti-TNF-α drugs use and eye complications (P=0.66).
