fa مقایسه میزوپروستول زیرزبانی و اکسیتوسین وریدی در کنترل خونریزی بعد از زایمان <p dir="rtl" style="text-align: justify"><!--stripped--> Normal 0 false false false EN-US X-NONE AR-SA MicrosoftInternetExplorer4 <!--stripped--><!--stripped--> <!--stripped--> <!--stripped--> <style> /* Style Definitions */ table.MsoNormalTable {mso-style-name:"Table Normal" mso-tstyle-rowband-size:0 mso-tstyle-colband-size:0 mso-style-noshow:yes mso-style-priority:99 mso-style-qformat:yes mso-style-parent:"" mso-padding-alt:0in 5.4pt 0in 5.4pt mso-para-margin:0in mso-para-margin-bottom:.0001pt mso-pagination:widow-orphan font-size:11.0pt font-family:"Calibri","sans-serif" mso-ascii-font-family:Calibri mso-ascii-theme-font:minor-latin mso-fareast-font-family:"Times New Roman" mso-fareast-theme-font:minor-fareast mso-hansi-font-family:Calibri mso-hansi-theme-font:minor-latin mso-bidi-font-family:Arial mso-bidi-theme-font:minor-bidi} </style> <!--stripped--><strong>زمینه و هدف</strong>: خونریزی بعد از زایمان از علل مهم مورتالینی و موربیدیتی مادران باردار است. کنترل فعال مرحله سوم زایمان یک روش شناخته‌شده پیشگیری از خونریزی بعد از زایمان است این مطالعه مقایسه بین دو داروی یوترونیک در پیشگیری از خونریزی پس از زایمان می‌باشد. <strong></strong></p> <p dir="rtl" style="text-align: justify"><strong>روش بررسی</strong>: این کارآزمایی بالینی تصادفی بر روی 542 بیمار حامله‌ نخست‌زا در طی سال‌های ۸۵ تا ۸۷ انجام شد. مادران به صورت تصادفی به دو گروه تقسیم شدند. در یک گروه 400 میکروگرم میزوپروستول زیرزبانی و در گروه دیگر 20 واحد اکسیتوسین وریدی بلافاصله بعد از تولد نوزاد تجویز شد. داد‌ه‌ها با استفاده از نرم‌افزار institute, USA) JMP  (Version 4, SASو آزمون‌های ² cو تی مستقل آنالیز گردید. <strong></strong></p> <p dir="rtl" style="text-align: justify"><strong>یافته‌ها</strong>: خونریزی پس از زایمان در بیماران تحت درمان با میزوپرستول به میزان معنی‌داری کمتر از گروه تحت درمان با اکسی‌توسین بود. طول مرحله سوم زایمان نیز در گروه میزوپروستول کمتر از گروه اکسی‌توسین بود (45/6 دقیقه در گروه میزوپروستول در قیاس با 9/6 دقیقه در گروه اکسی‌توسین. مقایسه تفاوت هموگلوبین‌ قبل و بعد از زایمان بین دو گروه نشان داد که خونریزی در گروه میزوپرستول کمتر بود و این گروه افت هموگلوبین کمتری داشت. عوارض جانبی در گروه مصرف‌کننده میزوپرستول، بالاتر بود، ولی خطرناک نبودند. شایع‌ترین عارضه در گروه میزوپروستول لرز (66/35%) و در گروه اکسی‌توسین سردرد (63/9%) بود.</p> <p><strong>نتیجه‌گیری</strong>: میزوپروستول در کنترل خونریزی بعد از زایمان موثرتر از اکسی‌توسین است. <br></p> <p style="text-align: justify"><!--stripped--> Normal 0 false false false EN-US X-NONE AR-SA MicrosoftInternetExplorer4 <!--stripped--><!--stripped--> <!--stripped--> <!--stripped--> <style> /* Style Definitions */ table.MsoNormalTable {mso-style-name:"Table Normal" mso-tstyle-rowband-size:0 mso-tstyle-colband-size:0 mso-style-noshow:yes mso-style-priority:99 mso-style-qformat:yes mso-style-parent:"" mso-padding-alt:0in 5.4pt 0in 5.4pt mso-para-margin:0in mso-para-margin-bottom:.0001pt mso-pagination:widow-orphan font-size:11.0pt font-family:"Calibri","sans-serif" mso-ascii-font-family:Calibri mso-ascii-theme-font:minor-latin mso-fareast-font-family:"Times New Roman" mso-fareast-theme-font:minor-fareast mso-hansi-font-family:Calibri mso-hansi-theme-font:minor-latin mso-bidi-font-family:Arial mso-bidi-theme-font:minor-bidi} </style> <!--stripped--> <strong>Background</strong><strong>:</strong> Postpartum hemorrhage is a major cause of maternal mortality and morbidity. It has been identified that active management of third stage of labor is an effective way in preventing postpartum hemorrhage. This randomized controlled trial was conducted to compare sublingual misoprostol versus intravenous oxytocin in the management of postpartum hemorrhage in nulliparous women.<br><strong>Methods</strong><strong>:</strong> In this randomized controlled trial conducted in Arash hospital from 2006 to 2009, Five hundred forty two nulliparous pregnant women were enrolled. They were randomized to receive either 400 microgram sublingual misoprostol or 20 IU oxytocin intravenously, immediately after the birth of newborn. <br><strong>Results</strong><strong>:</strong> Post partum Hemorrhage was significantly lower in women who received sublingual misoprostol (p<0.0001). Patients who received misoprostol had shorter length of third stage of labor (6.45 minute in misoprostol Vs 6.9 minute in oxytocin group, p=0.003). Comparison of hemoglobin levels in two groups before and after delivery showed that there is a significant lesser hemoglobin drop in misoprostol group p=0.046. Side effects were more common in misoprostol group (p<0.0001).
