fa استفاده از بلوک داخل مفصلی در آرتروسکوپی زانو: بررسی 41 بیمار <p dir="rtl" style="text-align: justify"><!--stripped--> 800x600 <!--stripped--> <strong>زمینه و هدف</strong>: در طی پیشرفت آرتروسکوپی، در طی سه دهه اخیر، از بلوک داخل مفصلی که با تزریق ماده بی‌حس کننده به داخل مفصل صورت می‌گیرد، برای انجام آرتروسکوپی استفاده شده است. در این مطالعه، تأثیر بلوک داخل مفصلی زانو به وسیله ترکیب بوپیواکائین، لیدوکایین و آدرنالین در آرتروسکوپی زانو بررسی شده است. <strong><br></strong></p><p dir="rtl" style="text-align: justify"><strong>روش بررسی</strong>: در 41 بیمار فاقد سابقه بیماری زمینه‌ای (محدوده سنی: 18 تا 55 سال)، آنستزی جهت انجام آرتروسکوپی تشخیصی زانو، با تزریق 10 میلی‌لیتر بوپیواکائین 5/0 درصد به اضافه 10 میلی‌لیتر لیدوکائین دو درصد همراه با آدرنالین 100000/1 به داخل مفصل صورت گرفت. درد و نیاز به ضد درد در حین انجام آرتروسکوپی، مدت زمان انجام عمل، حجم مایع استفاده شده در آرتروسکوپی، مدت زمان بی‌دردی و نیاز به ضد درد بعد از انجام آرتروسکوپی، میزان درد بیمار بر اساس Visual Analogue Scale (VAS) در حین ترخیص از بخش مراقبت بعد از عمل و رضایت بیمار و جراح از روش آنستزی، مورد ارزیابی قرار گرفت. <strong><br></strong></p><p dir="rtl" style="text-align: justify"><strong>یافته‌ها</strong>: 68% بیماران درجه درد خفیف و 29% درجه درد متوسط را گزارش نمودند و تنها یک مورد درجه درد شدید داشت، به همین دلیل برای وی بیهوشی عمومی در حین انجام آرتروسکوپی به ‌کار برده شد. میانگین VAS معادل 78/2 حاصل شد. 78% بیماران و در 80% موارد جراح، میزان رضایت عالی و خوب را گزارش نمودند.</p><p dir="rtl" style="text-align: justify"> <strong>نتیجه‌گیری</strong>: با توجه به میزان رضایت بالای بیمار و جراح و میانگین VAS حاصل شده، ساده بودن انجام بلوک داخل مفصلی و همچنین عوارض مختلف مربوط به روش‌های آنستزی دیگر، روش آنستزی داخل مفصلی می‌تواند به عنوان یک روش ساده، مؤثر و کم‌خطر در انجام آرتروسکوپی زانو مطرح باشد.</p> <p><!--stripped--> Normal 0 false false false EN-GB X-NONE AR-SA MicrosoftInternetExplorer4 <!--stripped--><!--stripped--> <!--stripped--><!--stripped--> <style> /* Style Definitions */ table.MsoNormalTable {mso-style-name:"Table Normal" mso-tstyle-rowband-size:0 mso-tstyle-colband-size:0 mso-style-noshow:yes mso-style-priority:99 mso-style-parent:"" mso-padding-alt:0cm 5.4pt 0cm 5.4pt mso-para-margin:0cm mso-para-margin-bottom:.0001pt mso-pagination:widow-orphan font-size:10.0pt font-family:"Times New Roman","serif"} </style> <!--stripped--></p> <p style="text-align: justify"><!--stripped--> Normal 0 false false false EN-US X-NONE AR-SA MicrosoftInternetExplorer4 <!--stripped--><!--stripped--> <!--stripped--> <!--stripped--> <style> /* Style Definitions */ table.MsoNormalTable {mso-style-name:"Table Normal" mso-tstyle-rowband-size:0 mso-tstyle-colband-size:0 mso-style-noshow:yes mso-style-priority:99 mso-style-qformat:yes mso-style-parent:"" mso-padding-alt:0cm 5.4pt 0cm 5.4pt mso-para-margin:0cm mso-para-margin-bottom:.0001pt mso-pagination:widow-orphan font-size:11.0pt font-family:"Calibri","sans-serif" mso-ascii-font-family:Calibri mso-ascii-theme-font:minor-latin mso-fareast-font-family:"Times New Roman" mso-fareast-theme-font:minor-fareast mso-hansi-font-family:Calibri mso-hansi-theme-font:minor-latin mso-bidi-font-family:Arial mso-bidi-theme-font:minor-bidi} </style> <!--stripped--> <strong>Background</strong><strong>:</strong> During recent three decades, parallel to the improvement of arthroscopic techniques, intra-articular block by direct injection of anesthetics into the joint has been used in knee arthroscopy. In this study the efficacy of intra- articular block by complex of bupivacaine, lidocaine and adrenaline in knee arthroscopy has been assessed. <br><strong>Methods</strong><strong>:</strong> Forty one healthy adults (age range: 18-55 years) with knee problems selected for diagnostic arthroscopy. Anesthesia was induced by direct injection of 10ml 2% bupivacaine, 10ml 0.5% lidocaine plus 1/100000 adrenaline into the knee joint.  Duration of operation and volume of serum used for irrigation during the procedure pain and analgesics requirement, during and after arthroscopy VAS (Visual Analogue Scale) score, at time of discharge from recovery and also patient's and surgeon's satisfaction were assessed. <br><strong>Results</strong><strong>:</strong> Sixty eight percent and 29% of cases reported mild and moderate degree of pain perception during arthroscopy, respectively, and only one case for which general anesthesia was performed, reported severe pain. VAS mean was 2.78.
