fa لیشمانیوز احشایی همراه با پریتونیت سلی: گزارش موردی <p dir="rtl" style="margin: 0in 0in 0pt direction: rtl unicode-bidi: embed text-align: justify" class="MsoNormal"><strong><span style="font-size: 8pt">زمینه و هدف</span></strong><span style="font-size: 8pt">:</span><span style="font-size: 8pt"> </span><span style="font-size: 8pt">لیشمانیادونووانی یک انگل اجباری داخل سلولی است و در داخل سلول‌های سیستم رتیکولواندوتلیال به‌صورت جسم لیشمن یا آماسیتگوت </span><span style="font-size: 8pt">Leishman body Amastigote</span><span style="font-size: 8pt"> بدون تاژک و فرم تاژک‌دار آن پروماستیگوت با لبتوموناد </span><span style="font-size: 8pt">Promastigot lebtomonad</span><span style="font-size: 8pt"> <span lang="AR-SA">در بدن پشه خاکی و محیط کشت دیده می‌شود. کالاآزار در استان‌های فارس و آذربایجان غربی به‌صورت اندمیک و در سایر نقاط به‌صورت اسپورادیک و در روستا دیده می‌شود. عامل کالاآزار در ایران لیشمانیا اینفانتوم و ناقل آن پشه خاکی (فلبتوموس) می‌باشد. <br></span></span></p><p dir="rtl" style="margin: 0in 0in 0pt direction: rtl unicode-bidi: embed text-align: justify" class="MsoNormal"><span style="font-size: 8pt"><span lang="AR-SA"></span></span><strong><span style="font-size: 8pt">معرفی بیمار:</span></strong><span style="font-size: 8pt"> </span><span style="font-size: 8pt">دختر بچه چهار ساله‌ای مبتلا به لیشمانیوز احشائی همراه با پریتونیت سلی گزارش می‌شود که در زمان بستری درد و بزرگی شکم، کاهش وزن و تب، </span><span style="font-size: 8pt">IFA</span><span style="font-size: 8pt"> با تیتر </span><span style="font-size: 8pt">1280/1 و جسم لیشمن در مغز استخوان و مایع آسیت اگزوداتیو با اکثریت لنفوسیت داشت</span><span style="font-size: 8pt">.</span><span style="font-size: 8pt"> </span><span style="font-size: 8pt">با شروع درمان ضد </span><span style="font-size: 8pt">TB</span><span style="font-size: 8pt">، بیمار به‌درمان باگلوکانتیم پاسخ داد.</span><strong><span style="font-size: 8pt color: rgb(255, 102, 0)"> <br></span></strong></p><p dir="rtl" style="margin: 0in 0in 0pt direction: rtl unicode-bidi: embed text-align: justify" class="MsoNormal"><strong><span style="font-size: 8pt color: rgb(255, 102, 0)"></span></strong><strong><span style="font-size: 8pt">نتیجه‌گیری</span></strong><span style="font-size: 8pt">: در کالاآزار لمفوسیت‌های </span><span style="font-size: 8pt">T</span><span style="font-size: 8pt"> حفاظتی (</span><span style="font-size: 8pt">Tht</span><span style="font-size: 8pt">) با پاسخ ضعیف یا عدم پاسخ ایمنی سلولی اختصاصی داریم در نتیجه تست مانتو تا موقع بهبودی ساپرس می‌شود. بیمار ما هم به‌علت کاهش سیستم ایمنی سلولی و سوء تغذیه و طولانی شدن بیماری با فعال‌شدن سل شکمی مانع پاسخ به درمان کلاسیک کالاآزار شده و تا شروع درمان آنتی </span><span style="font-size: 8pt">TB</span><span style="font-size: 8pt"> بیمار به درمان با گلوکانتیم پاسخ ندارد. </span></p> <p style="text-align: justify"><strong>Background</strong><strong>:</strong> Leishman Denovani is an obligatory intracellular parasite that is seen such as Leishmanbody or Amustigote in intra reticolo-endothelial system. Leishmanenios is seen as sporadic-endemic or epidemic in many places in the world. In Iran in Fars state and west Azarbayjan is endemic and in other places are in sporadic form and is found in rural areas.</p> <p style="text-align: justify"><strong>Case report:</strong> A four year-old girl was admitted with visceral Leishmaniasis and Subsequently developed peritoneal tuberculosis. The patient who lived in Dashte- Moghan, complained of abdominal pain and distention and weight loss from 1.5 years ago. The titre of IFA test for leishmansis was 1/1280. Leishman body was seen in bone marrow aspiration specimen. Bone marrow culture for leishmania was negative. The specimen of acsities fluid revealed sero- fibrino- purulent exudate with lymphocyic dominancy (over 90%). No response to classic lishmanisis treatment had been started unless the patient treated with anti tuberculoid regimen. </p> <p style="text-align: justify"><strong>Conclusion:</strong> The function of the T-helper (Tht) lymphocytes will decrease in Kala-azar disease. This is why there is no skin reaction to Manteaux (PPD) diagnostic test the patient. The patient have been suffering from long-term malnutrition with its consequent immune defect. There was no evidences of cure in our patient during classic Kala-azar therapy.
