Abstract

This study is a descriptive -analytical research which is designed to evaluate clinical skills of Nurses in coronary care units in affiliated hospitals of Tehran medical sciences University.The specific objectives of this research are as follows: Determining clinical skills of nurses in coronary care unit in the fields of patient assessment, intrepretation of Electrocardiogram, and ABG test administration, Iv therapy, educating the clients , caring the patients under cardiac catheterization and resuscitation ofcardio-pulmonary disorders. The population involved 50 nurses who were working in the coronary care units. The sample and population were the same. The researcher collected the data during a period of 3 months and observed the subjects minimum two full working shifts.The results indicated that nursing skills of the majority of the subjects towards the patientassessment was at weak level, one half of them had high level skill ofEKG interpretation(+90%), Meanwhile 2/3 of the subjects (68%) had a very weak skill (0-20%) in the field of ABGtest interpretation. One half of them had a medium skill for administration ofthe IV drugtherapy and all of them were at very weak levelforpatienteducation skills. More than one halfof these nurses had a good skill about nursing care of the patients under cardiaccatheterization. Finally, the last objective (Cardiac resuscitation skill) was not analysed due to afew cases.

Abstract

For diagnosing PROM, the need has been acutely felt for special care in obstetrics. Coordinated management for patient care between the clinician and midwives is the custom in several obstetric care centers. Diagnosis and treatment by the physician has greatly facilitated the work of midwives, to the extent that they now play an important role in the special nursing of women in solving obstetrics. This article reviews important topics such as: epidemiology, physiology, pathology and patho-physiology. It also covers preventive measures, objective and subjective evaluation, diagnostic tests and management of PROM, as well as the psychological aspects of hazards for motherhood, fetus and childbirth.

Abstract

Introduction: Death of a patient is an unavoidable stage of nursing and in many cases nurse is the only person who takes care of the patient at the time of death. An experienced and committed nurse obliges her/himself to care for the patient before and after death in the best way possible.           

Materials and Methods: This research is a descriptive study that carried out in order to determine knowledge and attitude of the nurses about pre and post-death cares. The samples were 227 nurses of Imam Khomeini Complex Hospital. The number of subjects chosen from each hospital was proportional to the number of nurses of that hospital. The samples were selected randomly. Data were collected by a questionnaire, validity of which was confirmed by content validity and reliability of it was confirmed by test-retest method. Data were analyzed with descriptive statistics.

Results: Results showed that in majority of the nurses level of knowledge about pre-death cares was moderate and was poor about post-death cares. The majority of the nurses had a positive attitude about pre-death cares and a relatively positive attitude about post-death cares. There was not any sample with negative attitude about pre or post-death cares.

Conclusion: Regarding the undesirable level of knowledge and positive attitude of the nurses under study concerning pre and post-death cares and considering that the necessity for improving the knowledge is training and education, the results of this research can help authorities to provide training and educational programs for enhancing nurses’ knowledge regarding pre- and post-death cares.

Abstract

Introduction: Evaluation of health care services is an essential step in improving health care quality and without it such an improvement could not be achieved.

Material and Methods: In this descriptive study, parent’s satisfaction of child’s health care in the health and treatment centers affiliated to Tehran University of Medical Sciences was evaluated. Four hundred parents who had attended 25 health and treatment centers were participated in this research. A questionnaire was completed through the interview with parents of 1 to 12 months old children. Analytical and descriptive statistics were used for the analysis of data.

Results: The finding of this study showed that the majority of parents had average levels of satisfaction concerning communication’s skills of health care providers and high levels of satisfaction about availability of these cares. Satisfaction of parents in the section of the vaccination was high and majority of samples had average levels of satisfaction about growth monitoring of children. There was a relationship between infant&aposs age and parent’s satisfaction.

Conclusion: Based on these findings recommendations have been made for future research. The findings of this study can be utilized in public health planning in order to improve the quality of the infant’s health care services.

Abstract

Introduction: Pregnancy is a natural phenomena and basis of birth but can induce various dangers to mother and fetus. Since mother is the central part of family, maternal mortality would be an irreparable damage to the community. On average 10100 live births and 12 maternal mortalities occur in Kurdestan province each year the maternal mortality rate is therefore 118/100000 live births which in comparison to similar figure in whole country (37/100000 live births) is three times higher.

Materials and Methods: This research is an analytic case-control study. Cases and controls were matched for place of residence and age at gestation. Cases (n=55) were chosen by census and controls (n=220) were chosen by random sampling. The tool used to collect data was questionnaire, validity and reliability of which was tested by content validity and test-retest method. The statistical testing used in this study were Chi square and odds ratio. Data were analyzed by SPSS .11 software.

Results: Most of maternal mortalities had occurred in women 18-35 years of age, illiterate and living in rural areas. Marivan had the highest maternal mortality compared to other surveyed cities (29.1%). Prenatal care (OR= 22.7), parturition agent (OR= 9.85), use of one of the method of family planning (OR= 2.5) and parturition method (OR= 2.3) had meaningful relationship with maternal mortality.

Conclusion: According to findings of this research, it is possible to decrease the maternal mortality by improving prenatal care and family planning and preventing parturition by uneducated midwives and avoiding unnecessary cesarian sections.

Abstract

Introduction: According to WHO, prolonged labour is one of the most important causes of maternal and newborn mortality. This study was carried out to assess the impact of education of partogramm on quality of midwifery’s care.

Materials and Methods: This survey was a quasi experimental study with one group pretest-posttest design which was carried out in some of governmental hospitals in Tehran in 2002. Subjects were 53 midwives who were working in different hospitals. The data was collected with an special questionnaire, WHO’s partogramm, and a check list to assess quality of midwifery care in labour. Result were analyzed with SPSS software.

Results: The findings showed that after education of partogramm a statistically significant improvement occurred in recording of twelve of fifteen variables including fetal heart rate, condition of amniotic fluid, vaginal bleeding, vital signs of mother, urine examination, drugs and fluid use, correct plotting of cervical dilatation, correct movement of dilatation graph to alert line, number of vaginal exams, recording time of rupture of membranes and time of amniotomy (p<0.0001).

Conclusion: The findings of this study indicate education of partogramm has a positive effect an quality of midwifery’s care and emphasis important of its education to midwifery’s students

Abstract

Pregnancy and delivery are special events in women’s and their family’s life. Although pregnancy is a natural phenomenon and not a disease, sometimes it can be associated with several dangers to the mother and her fetus. Maternal death is a global finding and its five most important causes are bleeding, infection, hypertension, prolonged labor and abortion in unsanitary condition. Early diagnosis of abnormal labor progression and prevention of prolonged labor can reduce the risk of postpartum bleeding and infection. The chart of labor called “Partograph” has been introduced and applied since 1970 to help diagnosis of the abnormal labor and cephalopelvic disproportion. Partograph is an early alerting system which can prevent probable difficulties in different stages of labor through recording of all of the observations and examinations on one chart, so that mother can be referred as soon as possible to higher levels of health services.

Abstract

Introduction: In patients who have a chronic disease such as end stage renal disease for which cure is not a realistic goal, maximizing functioning and well-being should be a primary objective of care. However, functioning status in the dialysis patients is often lower than normal.

Materials and Methods: This study is aiming to determine the effect of self–care educational programs on decreasing problems and improving quality of life of dialysis patients. This study was a quasi-experimental research in one group (pretest-posttest) accomplished in the educational hospitals of Iran and Tehran medical sciences universities from 2003 to 2004. A total of 32 patients were chosen on the basis of a non-randomized sampling. Each patient was assessed in two sessions before starting the dialysis treatment by questionnaires about educational requirements and quality of life and a checklist of common problems in dialysis patients. In order to validate the results, the laboratory variables were analyzed as a two months average prior to the education. The authors designed an educational program on the basis of existing problems and then determined the characteristics of each person under study and the required education was presented in two sessions. After the educational period was ended, the laboratory variables (average two months) and quality of life two months after education were analyzed and also the checklist were studied in two stages with a time interval of two weeks (third and fifth weeks).

Results: There was a significant decrease in serum urea, uric acid, serum creatinine, K, weight gain, systolic blood pressure, edema, skin itching and local vascular problems. Also, there was a significant improvement in the patients’ quality of life.

Conclusion: Self-care educational programs have a positive influence on decreasing the problems and improving the quality of life of dialysis patients.

Abstract

Background & Aim: This paper investigate the application of some management’s new theories in nursing care in order to determine the similarities between management and nursing. The need of Health Services to be effectively managed was a prominent features of developed countries health policy in the 1980 s and early 1990. In the recent years, four management’s new theories has identified: organizational excellence, total quality management, organizational culture and empowerment. Each of these new theories has key role in nursing care. This paper identifies similarities and convergence’s between management profession with nursing care, using application of management’s new theories.

Conclusion: The comparison between nursing care and management’s new theories has been provided the collaboration and cooperation between nurses and management professions to deliver health care services to patients effectively.

Abstract

Background & Aim: Family history of one member suffering from hepatitis B of family is one of the most important ways of illness transmission in Iran. Thus attention to quality of self care of patients of hepatitis B family has vital role in prevention and control in family and society.

Methods & Materials: This research is descriptive study. Samples of this research included 250 patients which 160 persons belonged to simplex hepatitis B families and 90 persons belonged to multiplex hepatitis B families. Patients refer to the center of blood transfusion organization from all of area of Gillan province. In this study data has collected by patens’ questionnaire include: demographic qualifications that were designed in two parts 1- individual qualifications and illness qualifications and questions were about quality self care of drug regimen meal regimen, addictions, precautions standard, fallow up disease and diagnostic quality self care in these groups has been analyzed in SPSS statistical soft ware by statistical tests such as 2 test and fisher test. (p<0.005).

Results: Our results show that (%64) patients of simplex family and (85.6%) patients of family multiplex have not suitable quality self care. In response to hypothesises (there is relationship between quality of self care of patiens members in family and increasing of HBSAg cases) findings show that exists meaningful relation between self care about hepatitis B and increasing cases HBSAg in families (p<0.05).

Discussion: Regarding to dimensions of quality selfcare about hepatitis B disease has important role in preventing from increasing cases HBSAg in families. Particularly regarding to results of research which indicate unsuitable quality self care of patients about drug regime “diet or therapy” “addictions” follow up disease. Therefore in order to control the hepatitis B in family according to the results of this study it is suggested further efforts should be down. The results of this study can be used for other patients infectious such as hepatitis c and Aids.

Abstract

Background & Aim: Decreasing mechanical ventilation and early extubation after cardiac surgery are the important scientific subjects that their clinical and financial benefits had been demonstrated. There are some variables that are associated with extubation time so, determination of them will help nurses to plan appropriate care aimed at doing an early and safe extubation.

Methods & Materials: The purpose of this descriptive-analytic study was to determine whether any preoperative variable had a significant effect on extubation time after CABG. The research was conducted in one of hospitals in Tehran city. In this retrospective study, data were collected by reviewing of files of eligible patients who were undergoing CABG from December 2003 to March 2005. 93 files reviewed by convenience sampling method. Collecting data tool was a checklist consists of three parts: 1) demographic characteristics 2) health and disease history 3) physiologic status. Samples divided into two groups: early and delayed extubation (>6h). Data analysis was performed by descriptive methods, chi square, fisher exact test and regression analysis.

Results: Mean and standard deviation of duration time of intubation was 7.19±3.00 with range of 3-18.25 hours in all samples. Totally 43% of subjects extubated in ≤6h and 57% of them in >6h. Age was the only preoperative variable that was found to be statistically significant to extubation time (p=0.01).

Conclusion: According to our findings age is associated with postoperative intubation time after CABG. It means older patients need to be under long mechanical ventilation. So, nurses should make a decision on extubation in older patients, carefully.

Abstract

Background & Aim: One of the responsibilities of nurses is to identify of effective factors on sleeping, because identification of these factors prevents from occurrence of sleep disorders, improves sleeping, decreases duration of hospitalization, and reduces use of hypnotic drugs.

Methods & Materials: This research is a comparative descriptive study. The population under research was included 50 nurses who were working in CCU wards and 50 patients who were hospitalized in CCU wards that were selected by interviewing and information gathering tools was a questionnaires which consisted of tow parts and for each group one questionnaire was used. The first part was included demographic specification. Second part is consisted of 56 questions (four rating) related to effective factors on patient&aposs sleeping in the domains such as environmental factors, personal (physical and mental) factors, pre-sleeping habits and an extra question (to explain other factors with the except of factors that mentioned in sleeping). Gathered data is processed by SPSS software, 12&aposTh version, and for achieving to research goals, descriptive and perceptive statistical methods (such as t-test, ANOVA test, and Pearson coefficient of correlation) were used. Then descriptive statistic was used in data analysis and statistical t-tests were used to compare of these two groups opinions.

Results: The results of this research showed that environmental factors such as turned on light, pain, anxiety due to loss of job, fears of outcome of disease, connection to monitoring systems are the important effective factors on sleeping according to the nurses points of view however patients believe that phone ring, pain, anxiety from loss of job, fears of outcomes of disease, connection to monitoring systems are important.

Conclusion: According to the research results, the most important effective factors on sleeping are "turned on light", "phone ring" "pain", "anxiety from loss of job", "fears of outcome of illness", "connection to monitoring systems". The foundation of this schedule is based on identification of effective factors on sleeping according to viewpoint of patients and then eliminating the disturbing factors.

Abstract

Background & Aim: Knowledge of nursing is necessary for caring people suffering from pain. Regarding pain management and nursing role in order to put into practice exact pain management and availing comfort, possessing knowledge and positive attitude toward nursing care in this population have an important role.

Methods & Materials: This descriptive (cross- sectional study) determines cancer nurses knowledge and attitudes regarding pain management in hospitalized patients in related wards in Tehran University of medical sciences. Total of 113 nurses (from 132 questionnaires were sent, 113 of them were completed) who work in cancer wards in Tehran University of medical science hospitals were selected by convenience sampling (accidental sampling). For data gathering, questionnaires were used. Statistical analysis was performed using 2 test, t-test and Pearson correlation coefficient. To compare results and for describing the results, descriptive analysis were used. Results:

Results indicates that knowledge and attitude of samples is in a high school (level of knowledge 76% and level of attitude 86.6%). The correlation between knowledge and attitudes is (R=0.350), (p=0.000) that means there is a direct relation between nurses knowledge and attitudes and by increasing one of them، the other will increase.

Conclusion: Some of features like level of education, taking part in continuing education, working in chemotherapy and radiotherapy ward, operating room and facing to cancer patients, influence on nurses knowledge and attitudes.

Abstract

Background & Objective: According to Iranian national statistics, every year 400 to 500 thousand unwanted pregnancies occur in Iran. The emergency contraceptive methods are easily accessible and affordable for the women of this region. Regarding the important role of health care providers in representing these methods, this study is to determine the knowledge and attitudes of these staff toward required contraceptive methods.

Methods & Materials: In this semi-experimental study, 216 health personnel working in south of Tehran health centers were randomly selected. Two 30- minute educational sessions were presented to the staff. The methods of education included face to face, and group discussion. At the end of each session a pamphlet was given to the staff. Data were gathered using a questionnaire before and 2-6 weeks after the education. Statistical analysis was carried out using statistical package for social sciences (SPSS).

Results: The results showed that there were significant statistical differences between the knowledge and attitudes (P<0.001) of personnel about the emergency contraceptive methods before and after education. There was no significant relationship between demographic characteristics and attitudes of personnel. There was a significant relationship between sex (P<0.001), educational level (P<0.003), educational major (P<0.001), with knowledge after intervention.

Conclusion: Knowledge and attitudes toward emergency contraceptive methods increased significantly after education. This finding reveals the importance of continuous education for health care staff such as midwives and physicians to facilitate successful guidance of their patients towards effective family planning.

Abstract

Background & Aim: Due to the fact that family centered care have a crucial role in pediatric nursing, it seems compatibility of nurses&apos and parents&apos perception about parents&apos needs can lead to deliver higher quality care. The aim of this study was to compare parents&apos and nurses&apos perceptions about needs of hospitalized children&aposs parents.

Methods & Materials: This is a cross-sectional survey. One hundred and fifty parents and 80 nurses were selected using convenient sampling method. Data was gathered using Kristjánsdóttir&aposs "parental needs of hospitalized children" questionnaire. Data were analyzed using descriptive and analytical (Chi-squared & Mann-Whitney U tests) methods.

Results: Research findings showed that there was significant difference between both parents&apos and nurses&apos perceptions about parents&apos needs (P=0.012). Both groups believed that parents&apos needs are met partially in hospitals. There was not meaningful difference between the parents&apos and nurses&apos perceptions concerning meeting parental needs (P=0.666). Based on their perceptions, understanding of parents&apos needs help them to meet their requirements in health care facilities. There was a significant difference between parents&apos and nurses&apos perceptions regarding parents&apos need (P=0.018).

Conclusion: Parents and nurses recognized all of the parents&apos needs to be important there were also deficits in meeting these needs. Therefore, parents&apos cooperation with health care team is necessary to facilitate the recovery of sick children as well as meeting parents&apos needs.

Abstract

Background & Aim: Family-centered care, which has become the cornerstone for pediatric nursing practice, supports the integrity of child and family health. It brings families a unique care and improves child and family health. Families are recognized as an essential part of a specialized child care during illness. They are expected to be skilful in child care both in and out of health care facilities. This study was carried out to determine health caregivers&apos attitude toward parent participation in the care of their hospitalized children at Madani pediatric hospital- Khoramabad in 2004.

Methods & Materials: This is a descriptive-analytical study. One hundred and sixty nine caregivers were selected through convenient sampling method. Data were gathered using a questionnaire including demographic and parents&apos attitude questions. The questionnaire had 36-item. The data were analyzed via SPSS computer software and using descriptive and analytical methods.

Results: Results indicated that the average of participants&apos attitude score was 92.28±12.55. Most of subjects had neutral attitudes toward parents&apos participation in their children&aposs care. Positive attitude were significantly related with marital status (P<0.04), having children (P<0.04) and being a nursing student (in comparison with medical students) (P=0.04).

Conclusion: The caregivers&apos attitudes toward parents&apos participation in the care of their hospitalized children were neutral. The students&apos and staffs&apos attitudes should be modified by taking part in continuous training programs.

Abstract

Background & Aim: Self-care activity is a key factor in managing Rheumatoid Arthritis (RA) in order to reduce disability caused by the disease. Predisposing factors including knowledge, attitude, and self-efficacy are introduced as determinants of self-care behaviors in the forth phase of PRECEDE model, i.e., ecological and educational assessment. This study aimed to assess the relationship between the predisposing factors and self-care behaviors in patients with RA in Yazd, Iran.

Methods & Materials: In this cross-sectional study, a convenience sampling was used to recruit 181 outpatients with RA referred to a rheumatology clinic in Yazd, Iran. Data were collected by conducting interviews with the participants using a survey questionnaire included knowledge, attitude, and self-efficacy, as predisposing factors and self-care behaviors items. Reliability and validity of the instruments were examined and approved. The statistical tests included bivariate correlations, Mann-Whitney U test, and linear regression.

Results: Knowledge, attitude, and self-efficacy scores were 71.7%, 66.9%, and 47.5%, respectively. The total predisposing factors score was 62.3%. There was a significant positive correlation between the self-care behaviors with attitude and self-efficacy. However, there was no significant correlation between the patients&apos knowledge and self-care behaviors. Totally, the measures explained 39.3% of total self-care behaviors variance within which the self-efficacy was the strongest predictor (β=0.41).

Conclusion: Regarding the high power of predisposing factors, especially self-efficacy, in predicting self-care behaviors and the low scores of self-efficacy, health promotion planners should pay more attention to predisposing factors of self-care behaviors especially the self-efficacy in designing health promotion programs.

Abstract

Background & Objective: Prenatal period is a unique opportunity to assist the women to adopt healthy behaviors. The nutritional behaviors of pregnant women affect not only their own health but also the fetal growth and development. The purpose of this study was to determine the effect of nutrition education on nutritional behaviors in pregnant women.
Methods & Materials: In this randomized trial, 272 primigravida women were recruited using stratified cluster sampling method from eight family health clinics in Semnan. The participants were allocated in two groups randomly. The intervention group received the nutrition education through two 90-minute sessions with one week interval. Data were gathered using a questionnaire including demographic characteristics and nutritional behaviors at baseline and six weeks after the education. Data were analyzed in the SPSS.
Results: The results showed that there was not any significant difference in the mean score of the nutritional behaviors between two groups before the intervention. Significant difference was found between two groups after the intervention. The mean score in the intervention group was higher than in the control group (P<0.001).
Conclusion: The results of the study showed that nutrition education affected the nutritional behaviors in pregnant women. Therefore, the nutrition education should be integrated in educational topics provided for pregnant women.

Abstract

Background & Aim: Regarding the importance of determining the determinants of diabetes self-care, this study aimed to examine two major determinants of self-care activities, i.e. perceived barriers, and perceived self-efficacy among type two diabetic patients in Tehran, Iran.
Methods & Materials: In this cross-sectional study, 128 patients were recruited to the study using convenience sampling. The validated diabetes instruments were completed by the patients. The included participants aged older than 18 years. Data were analyzed using bivariate correlation and path analysis.
Results: The mean score of the perceived barriers and perceived self-efficacy were 3.05 and 2.18, respectively. The maximum score of self-care was 4.18. There was a positive correlation between self-efficacy and the self-care (r=0.22 P<0.01). Positive correlations were also found between education with self-efficacy and self-care (r=0.28 P<0.05), (r=0.29 P<0.05). There was a negative correlation between perceived barriers and self-care behaviors (r=-0.68 P<0.000). Using serial multiple regressions and path analysis, we had a final model (RMSEA=0.05 GFI=0.95 CFI=0.98 P=0.44 and Chi-Square=26.5). This model showed that perceived barriers were the strongest predictor for self-care behaviors. Perceived barriers mediated the effect of self-efficacy on the self-care behaviors.
Conclusion: The intervention efforts that address the studied determinants related to self-care behavior could potentially impact type two diabetic patients&apos self-care activities. These determinants should be mentioned in the program developing.

Abstract

Background & Objective: Poor physical and psychological health status in chemical warfare victims could result in decreasing quality of sleep. This study was conducted in order to evaluate the effect of using Continuous Care Model on sleep quality in chemical warfare victims with bronchiolitis obliterans.

Methods & Materials: A total of 62 chemical warfare victims with bronchiolitis obliterans were recruited to the study. The participants were randomly allocated to experimental and control groups. The Continuous Care Model was used to experimental group for a period of two months. The quality of sleep was measured using Pittsburgh Sleep Quality Index (PSQI) before and after the intervention in both study groups. Data were analyzed using independent sample t-test, and paired t-test in the SPSS-version 15.

Results: There were significant differences between experimental and control groups in the means of Subjective Sleep Quality, Sleep Disturbances, Use of Medications, and global scores after the intervention (P<0.05).

Conclusion: Using Continuous Care Model had positive effect on sleep quality in chemical warfare victims suffering from bronchiolitis obliterans.
