Abstract

Background: All contemporary methods of insulin administration are non-physiological. Insulin is not absorbed from the gastrointestinal tract because of its peptide nature. The aim of the present study was to examine the absorption of oral insulin from gastrointestinal tract, using novel oral formulation- adding a delivery agent superporouse hydrogel (SPH) and SPH composite (SPHC) in combination with insulin.
Methods: Capsules containing insulin and SPH &SPHC were administered orally, to 15 non-diabetic subjects in order to assess its biological effects and safety. Plasma glucose, insulin and c – peptide serum levels were determined, at timed intervals up to 4 h.
Results: In the present study, we showed that AUC of exogenous insulin in polymer -insulin group was higher than sub-cutaneous regular insulin group. It means that addition of SPHC polymer caused increase in insulin absorbtion.In addition, Insulin Tmax in polymer group was longer than sub-cotaneaus insulin group. Blood glucose AUC in sub-cotaneaus group was higher than polymer group.AUC C-peptide serum level in polymer group was higher than sub-cutaneous group.
Conclusions: Insulin in combination with a novel delivery agent, SPH and SPHC, given orally is absorbed through the GI tract in a biologically active form. This was demonstrated by suppression of endogenous insulin secretion.

Abstract

Objective: Health of old people is one of the critical issues in many societies that needs to accurate health policy making and adjust planning in health issues of elderly. The present study designed to compare the efficacy of the two traditional and electronic training methods in nutritional issues in order to life style modification in old people to develop of healthy elderly. Methods: This survey was a quasi-experimental method via applying the Pre-test and Post-test among 65+ years old people in a retirement association of Sari province, Mazandaran state. By simple random sampling method, 60 old people were selected, and then trained in two separated classes to 15 persons as women and men (30 persons) via film technique. The other method was a traditional which applied in the two separated classes of 15 persons as women and men (30 persons) by book technique. The questionnaire applied to measure of nutritional knowledge made up by the health center. Data were described by using SPSS software in accordance to descriptive statistics indexes (mean and standard deviation), and inferential statistics indexes (independent T and dependent T) were used in order to generalize results. Findings: The mean score of increasing knowledge in training group by film was 18/57 and the mean score in training group by book was 16/90 (T =5/299, df= 58). To satisfaction evaluating, the mean score in training group by film was 59 and the mean score in training group by book was 35/73 (T =1/96, df=58). Conclusion: Based on results training of old people by electronic method as a film is more effective than traditional method to increasing of nutritional knowledge in elderly. On the other hand, they had more satisfaction to life style modification by using film method in comparison with traditional method as book reading technique. Also, the training cost of the elderly via educational film is lower than training by book. So, should be suggested to consider in training by film rather than book as an efficient and effective method to improve of elderly health.

Abstract

Background: Diabetes is a chronic and prevalent disease. In recent years, its prevalence has increased because of lifestyle changes, immobility and subsequent obesity. The increase in diabetes prevalence has provoked multiple and scattered interventions by specialists and health care providers in order to control and regulate blood sugar and its related factors. The purpose of this study was to collect and integrate the results of these interventions in two fields of psychological and sport interventions and to investigate the impact of them on control of Glycosylated haemoglonin (HbA1c) in patients with diabetes by meta-analysis method.

Methods: For this purpose 38 acceptable studies (19 sport interventions and 19 psychological interventions), in terms of methodology, were selected and meta-analysis performed on them. This study is based on 38 effect sizes and research instrument is checklist of meta-analysis.

Results: The study findings showed that the effect size of psychological interventions on control of Glycosylated haemoglonbin is 0.67 (p<0.00001) and the effect size of sport interventions is 0.79 (p<0.00001).

Conclusion: according to Cohens chart, the effect size of both psychological and sport intervention on control of Glycosylated haemoglonin in patients with diabetes is medium. However, the effectiveness of sport interventions have been obtained more than the psychological intervention

Abstract

Background: Diabetes is a common disease and one of the biggest health problems in the country. These patients, in addition to physical problems, also suffered from mental problems which the disorder of body image is one of the most prominent problems. This study compared mental image of patients with type 2 diabetes treated with oral medication and those with insulin.
Methods: This study was conducted in a descriptive-analytic way in 1394. The study population included patients with type II diabetes are treated in diabetes center of Aran and Bidgol in 1394. A sample of 168 people was randomly chosen. Needed data was collected by demographic and multi-dimensional questionnaire using a person's attitude about their body image (MBSRQ) Dry reliable 0.73 and were analyzed with SPSS 22 software by using independent t tests, chi-square and Mann-Whitney and Fisher
Results: Base on the duration of disease, the most common was among patients with oral treatment (82.1 percent) and treated with insulin (58.3 per cent) of less than 10 years. Average rating of orientation to fit oral treatment was for patients with 41.79 and 40.15 for insulin therapy, in health assessment section for patients with 17.58 oral treatment and 16.71 for insulin. State of disease trends was in 63/1 percent of patients with moderate oral treatment and 46.4 percent with severe insulin treatment. Independent t test results showed a significant difference between the scores of two patients in the above sections (P<0.05) but significant differences in other aspects of mental image between the two groups was observed.
Conclusion: Generally, average mental image in patients treated with oral medication was not more than patients treated with insulin. Because one of the negative effects of diabetes on patients is psychiatric disorder, therefore, to improve the mental state of these patients and improve health and to control disease in these patients, research findings is expected to be the use.

Abstract

Background: Diabetes is a chronic disease with consequences affecting many aspects and dimensions of life. Patients experience relates to many aspects of their lives because the disease involves their whole life.
The present study aims at defining the role of life style in patient's life. Reviewing their lived-experience can provide basic solutions to reform the life style of patients with diabetes.
Method: in this phenomenology qualitative research, sampling was done among patients referred to Isfahan Diabetes Charity Center in a purposeful manner with the most possible variety. A number of 33 women with diabetes in the age range of 35 to 69 years with an experience of at least one year of living with diabetes and without a history of acute mental disease were chosen. Gradual sampling was conducted from January 2017 to March 2017. Patients took part in 45 to 50 minute semi-structured interviews. The interviews were recorded and then the Colaizzi method format was analyzed.
Results: analysis process of data gathered from these patients' lived-experiences, resulted in extracting four themes including physical health, psychologic health, spiritual health and social health.
Conclusion: In order to reform the life style of patient's' with diabetes, a general comprehensive attitude in way of living is needed. Understanding the lived-experience and analyzing the life style experiences results in the field of health among patients with diabetes can assist the health care staff in providing better services and prevent returns to the inappropriate life style in the future.

Abstract

Background: Diabetes type 2 is a metabolic disorder that it caused by insulin resistance. The aim of this research was to study the effect of eight weeks of aerobic exercise with ginger supplement on FGF21, Irisin and insulin resistance in women with type 2 diabetes.
Methods: In a quasi-experimental study with pretest posttest design, 24 women with type 2 diabetes were selected available and purposely as sample. The subjects divided randomly into two groups of control (placebo and exercise) and experimental (supplement and exercise). The experimental group take part in an exercise with moderate intensity 65-70 percent of target heart rate and three session per week. Ginger supplementation was performed at the same time for training with daily consumption of 1 g of ginger extract (250 g capsules) and four servings daily. The control group (placebo) also used similar capsules containing roasted flour. To evaluate serum levels of variables, blood sampling was taken from all subjects 24 hr before and after exercise and supplement intervention at 10hr fasting state.
Results: Data were analyzed by using covariance test (p<0.05). The results showed that eight weeks of aerobic training and ginger supplementation lead to increase in Irisin levels (p=0.0001), FGF21 (p=0.0001) and decrease in insulin resistance index (p=0.0001).
Conclusion: Results indicated that using aerobic exercise and ginger supplement versus aerobic exercise could have positive effects on regulatory factors of energy in people with type 2 diabetes.

Abstract

Background: Obesity is a disease whose incidence and burden on societies around the world is increasing. Sexual performance in these patients is an important aspect that is often ignored. Therefore, the aim of the present study was to investigate the effect of a period of exercise  along with electrical stimulation on the gene expression of some sex markers in testicular tissue of fasted obese rats.
Methods: In this experimental study, with a control group of 40 male rats  after induction of obesity and weight gain, randomly divided into 5 groups: control, fasting-obese, fasting-endurance exercise, fasting-electrical stimulation and Fasting-endurance training-electrical stimulation were divided. Intervention groups for a period of 4 weeks under endurance sports activity (with a speed of 10 to 20 m/min and duration of 20 to 40 minutes), electrical stimulation (foot shock device for 0.5 mA and 20 minutes) and fasting were placed. After exercise and anesthesia, brain tissue was done and after molecular processes, gene expression was measured using Real time-PCR machine.
Results: The results showed that training led to a significant increase in the expression of 5-alpha reductase and aromatase genes in samples compared to the fasted obese group (P=0.0001). Also, electrical stimulation and its combination with endurance training showed a significant increase in the 5-alpha reductase gene (P=0.0001).
Conclusion: It seems that endurance training and combined electrical stimulation of both during fasting by increasing the expression of 5-alpha reductase and aromatase genes in obese samples are effective in their sexual health and lead to its improvement.
