Abstract

Increased prevalence of Helicobacter Pylori (HP) infection is a common feature in diabetics, which is attributable to the presence of diverse predisposing factors. In this study, the prevalence of HP infection has been investigated in type 1 diabetic children.
Methods: In a cross-sectional study, anti-HP antibody (IgG) was measured in 75 type 1 diabetics (aged 2-18 years) and the results were compared with 75 healthy children who were matched for age, sex and socio-economic status. In seropositive diabetic patients with gastrointestinal (GI) symptoms, gastroduodenoscopy was performed to establish the diagnosis.
Results: Sera were positive for anti-HP in 22.7% of diabetics versus 17.3% in controls (P>0.05). No significant difference was observable between seropositive and seronegative diabetic groups as regard to age, sex, age at onset of diabetes, number of outpatient visits during the last 6 months, HbAlc and insulin requirements. Gastrointestinal symptoms were more common in diabetics than the healthy controls the prevalence of these symptoms, however, did not differ significantly between seropositive and seronegative diabetics.
Conclusion: The study indicated that type 1 diabetes is not associated with increased risk of HP infection. Further studies are required to investigate the impact of HP infection treatment on the glycemic control in diabetic children.

Abstract

Background: Increased echogenicity of pancreas, due to hemosiderosis, is a frequent finding in  - thalassemic paitents. Hemosiderosis also leads to  - cell dysfunction. So diabetes and glucose intolerance are common consequences of hemosiderosis. The aim of this study was to investigate the association of increased pancreas echogenicity (IPE) with insulin sensitivity in  - thalassemic children aged 10-20 years.
Methods: After exclusion of thalassemic paitents with diabetes or familial history of diabetes, pancreas ultrasonography was performed in 42 -thalassemic children and they were divided into 2 groups with normal (21) and increased (21) pancreas echogenicity. Serum ferritin was measured, as well as serum insulin and glucose values, during an OGTT, at 0, 30, 60 and 120 minutes. A control group was selected randomly (n= 23). Insulin Sensitivity Index and Fasting Glucose/Insulin Ratio were calculated and the data were analysed using t-test and ANOVA statistical methods.
Results: Serum feritin differed significantly between 2 groups of thalassemic paitents (P<0.005), but the insulin and glucose values were not significantly different among studied population (P>0.05). Serum feritin was inversely correllated with ISI in patients IPE and 28.6% of them had IFG, as compared to patients with normal echogenicity (P<0.05).
Conclusion : Regarding the detection of all IFG cases among thalassemic paitents with IPE and the relation of feritin with ISI in this group, pancreas ultrasonography may be used to investigate the early stages of diabetes in these patients. however after conducting further studies with larger sample size and on older paitents are recommended.

Abstract

Background: Atherosclrosis is a process that initiated with hypercholestrolemia and fatty streak formation. Previous studies showed oxidative modification of LDL render immunogenic and autoantibodies to epitopes of oxidized LDL. Oxidized LDL (OX-LDL), has antigenic properties. Antibodies against oxidized LDL have been proposed to be independent predictors of atherosclerosis development. The main aims of the current study were to compare antibody titers to different types of oxidized LDL (Cu+2-LDL, Malondialdehyde-LDL) and Native-LDL between angiographically documented coronary patients, non-documented patients and healthy subjects. Correlation between autoantibodies against oxidized LDL and increased risks of cardiovascular diseases has been shown. Methods: As a case-control study, we evaluated angiographically documented coronary patients, non-documented patients and healthy subjects to measure anti-OX-LDL autoantibody levels. Enzyme-linked immunosorbent assay was used to measure anti-OX-LDL autoantibodies. ANOVA test used for statistical analysis. Results: Titers of anti-Malondialdehydo-LDL autoantibodies were 3.55±0.415, 0.361±0.20, 0.093±0.078 respectively in each group (P<0.005). There was not statistically meaningful difference, between native-LDL and Cu+2-LDL antibodies. Conclusion: It seems the titre of autoantibodies against OX-LDL considered as a predictor of progression of atherosclerosis. Our data provide further support for a role of oxidatively modified LDL in atherogenesis.

Abstract

Background: The goal of dietary recommendations in children with type 1 diabetes mellitus is to achieve moderate to good control of blood glucose regarding to decrease in diabetic long-term complications. Although educational programs on appropriate dietary habits are offered in Isfahan Endocrine and Metabolism Research Center for diabetic patients, there is no information about their dietary habits . This study has been performed to investigate dietary intake in children with diabetes mellitus type1 and to compare the results with that of non-diabetic ones.
Methods: A cross-sectional study was performed in two groups of healthy and diabetic children. Data were collected by questionnaire and interview. The validity and reliability of the questionnaire were determined by content validity and pilot study. Data were described and analyzed by t-student and Pearson correlation tests in four age groups, using SPSS software.
Results: Findings showed that in “A” age group the intake of total calorie and fiber, in “B” age group the intake of fiber, fat, protein, carbohydrate, sugar and SFA, also fat, protein, carbohydrate, sugar and SFA-derived calorie consumption, in “C” age group the intake of sugar and fiber, also PUFA and sugar-derived calorie consumption and in “D” age group the intake of fiber, total calorie, protein, carbohydrate and sugar were higher in diabetic children as compared with the control group.
Conclusion: In all subjects, the intake of carbohydrate and total fat was the same as standard recommendations, while protein intake was a little higher which increases the risk of diabetic nephropathy. In all study groups, fiber intake has been decreased with increasing the age, so it is suggested to have dietary educational programs for general population with the goal of changing the life style of people, which itself can result in less social isolation in school-aged diabetic children.

Abstract

Background: Several studies and research have shown a higher prevalence of celiac disease (CD) and elevation of serum antibody (AGA-IgA & IgA-tTG & EMA-IgA ) in patient with diabetes mellitus type I (T1DM) in versus general and non-diabetic population. Thus screening of CD is recommended in T1DM). This study was conducted to compare frequency of celiac disease in patients with T1DM and healthy persons.
Methods: As a case-control study,60 patient with T1DM that reffered to endocrine clinic of Qazvin’s Boo-Ali hospital , in nearly one year period were enrolled as case group. 60 non-diabetic healthy subjects with age and sex matched, were selected as control group. Blood levels of Total IgA, AGA-IgA and IgA-tTG were measured in all of them, subjects who had elevated of both AGA-IgA and IgA-tTG underwent an upper GI endoscopy and biopsy was done from distal part of duodenum.
Results: Any one in case group hadn't IgA deficiency. 14 subjects in control and 12 subjects in case group had positive AGA-IgA that there was no significant difference between them . 2 subject of case group had positive IgA-tTG. Duodenal biopsy in 1 of 14 cases who had elevated AGA-IgA )1 of 2 cases who had elevated IgA-tTG) , revealed total villous atrophy indicating CD (Type IIIC with revised Marsh criteria 2001) and in other cases pathologic findings were normal or with nonspecific changes.
Conclusion: Frequency of CD in T1DM in our study is 1.67%. There is not any significant difference between case and control groups in prevalence of Celiac disease . But we conclused that screening with AGA-IgA is not a reliable screening test for CD , because there is not significant difference between T1DM and general population .

Abstract

Background: Diabetes mellitus can have variable and sometimes profound effects upon the oral tissues. It has been shown that uncontrolled or poorly controlled diabetics have particularly a greater incidence of severe recurrent bacterial or fungal infections and periodontal diseases. The purpose of this study was to determine thee relationship between oral manifestations in diabetic patients and duration of disease, and glycemic control.
Methods: We studied 116 diabetic outpatients referred to Diabetes Clinic of Shahid Bahonar Hospital of Kerman Province. We evaluated the following parameters: dental caries, periodontal status, xerostomia, burning sensation and mucosal alterations. We also recorded information about history of diabetes and glycemic control. Data were subjected to SPSS and X, t- test.
Results:
The most frequent alterations in this study were gingivitis (96.3%), periodontitis (90.2%), and xerostomia (97.24%). Burning sensation and periodontal diseases were significantly correlated to duration of diabetes (P=0.002, P=0.012 respectively).The results of the present study showed that periodontal diseases occur more frequently in moderately and poorly controlled diabetic patients than in those with good glucose control, but this difference was not statistically significant.
Conclusion:
In this study, dry mouth and periodontal diseases are common complaints among diabetic patients. Therefore management of oral diseases in diabetic patients should include a comprehensive evaluation of salivary function and periodontal status.

Abstract

Gangliosidosis GM1, is a lipid storage disorder due to deficiency of the lysosomal enzyme &beta-galactosidase. This deficiency leads to lysosomal accumulation of gangliosidosis GM1 to be, which contributes to swelling, cell damage and dysfunction of the organs. Gangliosidosis GM1a rare neurodegenerative disorder. Mongolian spots are significant clinical sign in some of the metabolic diseases, such as gangliosidosis GM1, but limited information is available. Hepatomegaly, hypotonia, edema, seizures, and skeletal malformations and Cherry red spot in the macula of the eye are of other symptoms of the disease. In this paper, we present a 10-month-old child with gangliosidosis GM1 type 1 with extensive mongolian spots who was referred the Hajar Medical Educational Therapeutic Center Shahrekord.

Abstract

Background: Insulin resistance and progressive β-cells failure are the key factors in type 2 diabetes mellitus (T2DM) pathogenesis. Many studies support a primary role of RBP4 in insulin resistance and suggest that genetic variations which alter the expression level of RBP4 might influence the risk of T2DM and its complications. Diabetic foot is one of the main complications of diabetes leading to disability and hospitalization. In addition, it reduces quality of life and imposes great cost to patients. The purpose of this study was to evaluate the correlation between two single nucleotide polymorphisms (rs10882273 and rs10882283) of RBP4 genes with diabetic foot ulcer in order to identify a biomarker for prediction of diabetic foot ulcer.

Methods: This is a case-control study. Two single nucleotide polymorphisms of RBP4 genes were genotyped by hit Tetra ARMS PCR technique. In this study, 100 and 133 diabetic patients with and without foot ulcer were selected as the cases and controls, respectively.

Results: The Chi-square test revealed no significant difference in frequency of TT, CC and TC alleles of rsl0882273 between case and control groups (P=0.414). Also, Comparison of AA, CC and AC alleles of rsl0882283 in both groups did not show significant difference (P=0.85).
Conclusion: According to this study, there is no relationship between two single nucleotide polymorphisms of RBP4 genes (rs10882273 and rs10882283) with diabetic foot ulcer in type2 diabetes patients.

Abstract

Background: Diabetes is the most common metabolic disorder. Progression of diabetes complications mostly due to lack of blood sugar control. The purpose of this study was to test a hypothesized model of individual and social factors affecting blood sugar control.

Methods: Present study is a cross-sectional study and patients were selected using sex stratified random sampling. Path analysis was conducted to investigate effects variables of the diabetes knowledge, patient and doctor relationship, self care, the number of education years, household income and duration of diabetes on HbA1c. The following goodness-of-model fit were used: χ2, RMSEA, CFI and SRMR test-statistics.

Results: The estimated model demonstrated good model fit, (χ2 = 5, p=0.4), (RMSEA=0.006), (CFI =0.99) and (SRMR=0.022). Total effect and total indirect effect of age on HbA1c were significant. Age through duration of diabetes had significant indirect effect on HbA1c. Total effect of duration of diabetes on HbA1c was significant. With increasing duration of diabetes HbA1c also had increased. Total effect of waist-to-hip ratio on HbA1c was significant. Waist-to-hip ratio had significant direct effect on HbA1c.

Conclusion: In the current study the effect of individual factors such as age, waist-to-hip ratio, depression and duration of diabetes on glycemic control were detected. According to the capabilities used in this method can providing results of this study with patients and local policy makers in order to improve the care of patients with diabetes

Abstract

Background: Whole-grain foods have been reported to affect serum levels of inflammatory cytokines. However, we are aware of no study examining the effect of whole-grain intake on inflammatory biomarkers among children

Objective: The present study aimed to determine the effect of whole grain intake on serum levels of inflammatory biomarkers in overweight or obese children.  

Methods: In this randomized cross-over clinical trial, 44 overweight or obese (BMI>85th percentile for age and sex) girls aged 8-15 y participated. After a 2-wk run-in period, subjects were randomly assigned to either intervention or non-intervention groups. Subjects in the intervention group were given a list of whole grain foods and were asked to obtain 50% of their grain servings from whole grain foods each day for 6 weeks. Individuals in the non-intervention group were also given a list of whole-grain foods and were asked not to consume any of these foods during the intervention phase of the study. A 4-wk washout period was applied following which subjects were crossed over to the alternate arm for an additional 6 wk. Fasting blood samples were taken before and after each phase of study to quantify markers of systemic inflammation.

Results: Mean (±SD) age of study participants was 11.2±1.49 years. Mean weight and BMI of subjects was 51.2±10.2 kg and 23.5±2.5 kg/m2, respectively. No significant effect of whole-grain intake on weight and body mass index (BMI) was seen compared with the non-intervention group. We found a significant effect of whole grain intake on serum levels of hs-CRP (changes from baseline in intervention group: -0.55 vs. 0.20 mg/L in non-intervention group, P=0.03), soluble inter-cellular adhesion molecule-1 (-121 vs. 23 μg/L, P=0.02), serum amyloid A (-0.59 vs. 0.32 mg/L, P=0.02) and leptin (-11.5 vs. 36.8 ng/L, P=0.02) after 6 weeks. A trend toward the significant effect of whole grain intake on serum levels of sVCAM-1 (-166 vs. -32 μg/L, P=0.07) was also observed.

Conclusion: This study provides evidence supporting the beneficial effects of whole-grain foods on biomarkers of systemic inflammation in obese children.

Abstract

Background: Type 2 diabetes is a metabolic disorder characterized by high blood sugar levels that can damage nerves. Many organs are affected ،especially the foot that leading to loss of sensation. These factors make favorable conditions for the development of diabetic foot ulcers. Polymorphisms (Thr399Ile) of Toll Like Receptor4 (TLR4) gene due to malfunction of TLR4 protein which plays an important role in immunity. The purpose of this study was to determine the parameters which are affecting the imbalance resulting in chronic inflammation and wound healing. By showing the relationship between single nucleotide polymorphism (Thr399Ile) of TLR4 gene with diabetic foot ulcer we can identify a biomarker for prediction of diabetic foot ulcer.

Abstract

Background: Type 2 diabetes is the most common type of diabetes, and a complex endocrine metabolic disorder that one of its main complications is diabetic foot ulcers. Matrix metalloproteinases (MMPs) are one of the key enzymes in the reconstruction of extracellular matrix which have proteolytic activity. The aim of this research is based on evaluating relationship between -1562 C>T allele at MMP-9 gene promoter with diabetic foot ulcer in type II diabetic patients. If such correlation proves, it can be used as a prognostic biomarker in patients with high-risk.
 Methods: This is a case-control study. The single nucleotide polymorphism of -1562C>T allele of MMP9 gene promoter was genotyped by hit Tetra ARMS PCR technique in 100 diabetic patients with foot ulcer grade 1 or 2 as the case group according Wagner classification and in 100 diabetic patients without foot ulcer as the control group. Results: The Chi-square test revealed significant difference in genotype frequency of CC, CT and TT alleles of -1562C>T allele of MMP9 gene promoter between case and control groups (P=0.000).
Conclusion: According to this study, there is a relationship between -1562C>T allele of MMP9 gene promoter with diabetic foot ulcer in type2 diabetes patients. Thus we can introduce this biomarker for evaluation of risk and prognosis of diabetic foot ulcers.

Abstract

Background: In this study, we investigated the diagnostic power of ultrasound in the diagnosis of malignancy in thyroid nodules in Iranian patients. For this purpose, we examined the relationship between ultrasound findings and pathology findings.
Methods: The present study is a retrospective study. The patients with a diagnosis of thyroid nodules who underwent ultrasound and FNA, were included in this study. To assess the relationship between the results of FNA and the ultrasound characteristics of nodules, we classified the results of FNA into malignant and benign groups and then compared ultrasound characteristics between the two groups. In cases which the FNA results were indeterminate (AUS/FLUS or FN/SFN), the postoperative pathology result was considered (if thyroid surgery was done and the result was available).
Results: In total, 201 nodules were included in this study. The results showed that hypoechogenicity, irregular/ill-defined margin, microcalcification, malignant flow pattern in Doppler sonography and concurrent cervical lymphadenopathy with suspicious features were significantly associated with malignant thyroid nodules. However, other ultrasound findings, such as the size and location of the nodule, presence of a cystic components within the nodule, the presence of a Halo sign, and the presence of a taller-than-wide shape, could not distinguish between benign and malignant nodules. Finally, the results of the present study showed that the accuracy of ultrasound in the diagnosis of malignancy in thyroid nodules is high.
Conclusion: This study suggests that the use of ultrasound can be very effective in diagnosing malignancy in thyroid nodules.

Abstract

Background: Control of Type 2 Diabetes Mellitus (T2DM) is influenced by lifestyle. The aim of this research was to determine the predictors of treatment adherence behaviors among T2DM based on Social Cognitive Theory (SCT).
Methods: This descriptive-analytical research was done among 500 T2DM patients in Tehran during 2021. Simple random sampling was performed among patients based their medical records in the Diabetes Control Clinic of Tehran's District Five. The data collection tools were written questionnaire based on the SCT determinants. Data were analyzed using descriptive statistics (frequency, percentage, mean, standard deviation) and analytical statistics (Pearson's correlation and linear regression) in SPSS software version 16. Results: The mean score of treatment adherence behaviors was 56.34 [SD: 10.41], ranged from 16 to 80. The outcome expectations, perceived self-efficacy, and social support accounted for 43% of the variation in the outcome measure of the treatment adherence behaviors. Self-efficacy was the strongest predictor (Beta = 0.430 and P<0.001). Increasing age (ß= -0.126 and P= 0.004), insulin treatment (ß= -0.250 and P<0.001) and smoking (ß= -0.146 and P= 0.001) conversely and having health insurance (ß= 0.181 and P<0.001) were positively predictive of treatment adherence behaviors.
Conclusion: The development, implementation and evaluation of educational interventions based on SCT with emphasis on promoting perceived self-efficacy can lead to useful findings in promoting treatment adherence behaviors among T2DM patients.
