Abstract
Background: Studies of the metabolic effects of Ramadan fasting on patients with type 2 diabetes mellitus are inconclusive.
Methods: Fifty-seven volunteers with type 2 diabetes underwent anthropometric and biochemical evaluation before and on the 14th and 28th days of Ramadan. Biochemical markers were measured by standard laboratory methods. Anthropometric measurements followed WHO criteria. Statistical analysis was by ANOVA for repeated measurements and Friedman’s two-way ANOVA, using SPSSv6 software.
Results: Daily cholesterol intake increased in all subjects (p<0.03). Body mass index increased (p<0.03) in women, but body mass index (BMI) and waist-to-hip ratio both decreased (p<0.01) in men. Blood pressure, fasting blood glucose and serum fructosamine did not change during the study. Plasma insulin (p<0.05), C-peptide (p<0.01) and insulin resistance (p<0.01) decreased only in men. Total and LDL cholesterol increased significantly in all subjects during the study.
Conclusion: Ramadan fasting does not alter carbohydrate metabolism or tissue insulin sensitivity in type 2 diabetes patients, given appropriate dietary education and rescheduling of oral hypoglycaemic medication. Lipid profile is unfavourably altered due to changes in both diet and biochemical response to starvation. Anthropometric indices improve in men but not women, possibly because of reduced physical activity in the latter.

Abstract
Introduction: In order to determine the relationship between serum lipid profile and diabetes mellitus as well as other cardiovascular risk factors, we carried out a cross-sectional study of 1255 oil industry workers at the Kharg Island oil terminal.
Methods: Increased levels of total cholesterol (TC≥200mg/dl), triglyceride (TG≥200mg/dl), and low-density lipoprotein (LDL≥130mg/dl) were seen in 32.7%, 39.5% and 18.5% of the workers, respectively. Decreased levels of high-density lipoprotein (HDL<40mg/dl) were seen in 13.1% of workers. 61.3% of the workers had a body mass index (BMI) ≥25kg/m2. Visceral obesity (waist circumference >100cm) was seen 32.4% of workers.
Results: There was a direct and significant relationship between TC levels and BMI (p<0.001), and between TG levels and BMI (p<0.0001). The same correlation existed between waist circumference and both TC (p<0.02) and TG (p<0.06). There was also a direct correlation between LDL levels and waist circumference (p<0.04). 10.6% of workers had some disorder of glucose metabolism. There was a direct correlation between diabetes and both TC and HDL levels (p<0.04 and p<0.05, respectively). 42.4% of workers smoked and 57.4% had a sedentary lifestyle. There was a trend toward lower blood glucose and cholesterol levels as the level of physical activity increased (p=0.1 and p=0.08, respectively). There was a significant difference between blue- and white-collar workers in the prevalence of excess weight, visceral obesity, diastolic hypertension, diabetes and cigarette smoking, but not serum lipid profile and level of physical activity.
Conclusion: The significant prevalence of dyslipidaemia and other cardiovascular risk factors in the oil workers of Kharg Island requires systematic preventive interventions to reduce cardiovascular mortality and morbidity in this population.

Abstract
Introduction: In patients with type 2 diabetes, triglyceride (TG) is usually increased, HDL decreased, and LDL normal. This pattern is associated with an increased risk of coronary heart disease. More recently, dense-particle LDL has been identified as an important risk factor for coronary heart disease.
Methods: 298 patients with type 2 diabetes attending the diabetes clinic at Doctor Shariati University Hospital underwent anthropometric and biochemical assessment. Anthropometric measurements followed WHO criteria. Biochemical indices (apoB100, TG, cholesterol, LDL, LDL particle size, HDL, and apoA1) were measured using standard laboratory methods. One-way ANOVA was used to analyse data with SPSSv6 software.
Results: Mean patient age was 55±13.2 years. Mean duration of diabetes was 9.5±6.1 years. The majority of patients were moderate to severely overweight. 73.2% of patients had some form of hyperlipidaemia. 20.7% had isolated hypertriglyceridaemia, 21% isolated hypercholesterolaemia, and 31.5% mixed hyperlipidaemia. Mean apoA1 concentration was higher than normal in this group, and mean apoB100 concentration lower. LDL particle size generally followed a small and dense pattern.
Conclusion: This study shows that LDL particles in both men and women with type 2 diabetes undergo both qualitative and quantitative changes. 35.5% of patients had smaller, denser LDL particles than normal. It appears that dyslipidaemia and B-pattern LDL particles are important risk factors for atherosclerosis in this group.

Abstract
Introduction: There is as yet no consensus as to the effect of Ramadan fasting on fasting blood glucose. We carried out a study to help clarify the situation.
Methods: This was a semi-experimental (pre- and post-) study of 115 healthy volunteers (67 men and 48 women), who fasted for at least 25 days during Ramadan. Blood samples were taken 7 days before Ramadan (at 7am after a 8-hour overnight fast), and on the 14th and 28th days of Ramadan (1 hour before sundown). The mean duration of daily fasting was 11.5±0.5 hours. Plasma glucose was measured by an enzymatic assay. Statistical analysis was by the paired-t and ANOVA functions on SPSS10.0 software.
Results: Fasting plasma glucose in the group as a whole decreased from 88.4±9.0mg/dl pre-Ramadan to 75.4±15.3mg/dl on day 14 and 62.9±7.7mg/dl on day 28 (p<0.001). Both men [87.5±8.8mgdl (pre-Ramadan) 60.8±6.4mg/dl (day 28)] and women [89.7±9.3mgdl (pre-Ramadan) 65.7±8.4mg/dl (day 28)] experienced a significant decrease in fasting plasma glucose levels (p<0.001 in both). There was a reduction in calorie intake during Ramadan in every volunteer (p<0.001), and there was a direct correlation between reduction in calorie intake and fasting plasma glucose (p<0.01).
Conclusion: Fasting plasma glucose decreases with Ramadan fasting and is associated with a reduction in calorie intake. The decrease in plasma glucose does not seem to be accompanied by any serious adverse effects in healthy volunteers, however.

Abstract
Diabetes is a common endocrine disease with complications such as retinopathy, nephropathy and neuropathy which has its monitoring through biomarkers desirable. At present, glycosylated hemoglobin (HbAic) is used for monitoring the long term control of glucose levels in diabetic patients. However, absence of a standardized range, has led to investigations that recently have suggested insulin-like growth factor-I (IGF-I) as a good biomarker for monitoring blood glucose levels in diabetics. The aim of this study was to examine the correlation between IGF-I and HbAic in Type 1 diabetes.
Methods: We designed a cross-sectional case-control study. The study composed of 26 newly diagnosed patients with Type 1 diabetes (15 male and 11 female mean age, 23.7±9.1 years) and 26 healthy controls (9 male and 17 female mean age, 24.1±4.4 years). Levels of fasting plasma glucose (FPG), HbA]C) IGF-I and IGF-binding protein-3 (IGFBP-3) were measured in both groups. FPG was measured by enzymatic glucose oxidase method and the colorimetric method was used to measure HbAlc. Determination of total serum levels of IGF-I and IGFBP-3 were done using immunoassay methods. P-value<0.05 was considered as statistically significant.
Results: The mean value of IGF-I concentrations in type 1 diabetics was significantly lower than controls (p< 0.05). A reverse correlation was observed between IGF-I and HbAic. Conclusion: The study indicates that in poorly controlled diabetics, levels of FPG and HbAic rise concurrent with a drop in levels of IGF-I decreases. Our study also showed a significant correlation between IGF-I and HbAie. Therefore, IGF-I could be indirectly used as a biomarker for controlling glucose levels in diabetics.

Abstract
Hyperlipidemia is a major risk factor for cardiovascular diseases (CVDs). A hundred and sixty six people die each day due to cardiovascular diseases in Iran which accounts for 12500 annual deaths. Several studies have revealed that changes in lifestyle are keys to the prevention of CVDs. This urges the role of continuous and comprehensive educational programs, which should be designed and performed by medical staff specially education nurses. In this study the effect of patient education thorough group discussion or an educational booklet is
compared on knowledge and practice of patients with hyerlipidemia.
Methods: This was an interventional study. Participants were randomly assigned to the "interactive discussion" group or "booklet" group. Knowledge and practice of participants in both groups were assessed before and after intervention using a standardized questionnaire. A total number of 304 patients with hyperlipidemia were studied which were equally divided into "booklet" or "Interactive
discussion" groups.
Results: "Interactive discussion" group showed %27 rise in knowledge and practice. This figure was %26 in the "booklet" group and no significant difference was observable.
Conclusion: It was concluded that interventional sessions and educational booklets had comparable efficacies. We suggest that these sessions be held on regular Intervals so as to achieve better results.

Abstract
Background: Currently different criteria are used to diagnose Gestational Diabetes. ACOG (American college of obstetric and Gynecology) accepted NDDG's (criteria National Diabetes Data Group) Criteria and ADA (American Diabetes Association) accepted Carpenter Caustan’s. Although both of these criteria have been achieved by O Salivan and Mahan’s reaserches, the number of patients has been diagnosed are different .The aim of this study was to compare Gestational Diabetes prevalence according to Carpenter Caustan’s and NDDG’s Criteria.
Methods: 1200 pregnant women were screened in a prenatal care clinic. Patients with definite diabetes were excluded. According to universal GDM screening method, for all of the patients GCT and GTT (if GCT > 130) were preformed. The results evaluated according to Carpenter Caustan’s and NDDG’s criteria.
Results: In this screening, 377 participants had positive GCT, according to Carpenter and Caustan’s criteria 83 women (6.9%), and according to NDDG criteria 50 women (3.6%) had Gestational Diabetes Mellitus. If the cut of point of GCT was 140 mg/dl instead of 130 mg/dl, according to Carpenter and Caustan’s criteria 16 women (18.1%), and according to NDDG 4 patients (9%) were missed.
Conclusion: Regarding the significant difference between two methods in diagnosis of gestational diabetes mellitus, assessment of each method value in improving prognosis and outcomes is necessary. According to the results the cut of point of 130mg/dl, as recommended in previous studies, has more accuracy for screening.

Abstract
Background: Assessing the trend of ischemic heart diseases and the process of acute coronary care is one of the most important tools in monitoring the programs dedicated to control of ischemic diseases. The current project was developed to assess the feasibility of using routine data registered in clinical records for coronary event registration according to the standards of WHO/MONICA project.
Methods: Hospital records of 320 cases with primary diagnosis of acute coronary syndrome (80 cases from each quarter, July 2003-4) were evaluated according to sufficiency of data. Data were evaluated according to “internal consistency”, “change in the proportion of missing data in the time periods” and “the proportion of insufficient data”.
Results: Available data of hospital records were not sufficient to determine the diagnosis in 0.7% of cases In addition, they were resulted in a probable diagnosis in 11.2% of coronary events. Median percents of missing data regarding the prescribed drugs before event was more than 10% in both fatal and non-fatal coronary events (score 1 of 4). Median percents of missing data regarding the ECGs, cardiac enzymes and cardiac resuscitation was lower than 5% in non-fatal coronary events and lower than 2% in fatal cases (scores 2 and 3 of 4 relatively).
Conclusion: The quality of available registered data in the evaluated clinical records was comparable with many reporting units of MONICA project. Using the available clinical records seems to be effective and feasible for systematic registration of cardiac events.

Abstract
Background: Diabetes mellitus can have variable and sometimes profound effects upon the oral tissues. It has been shown that uncontrolled or poorly controlled diabetics have particularly a greater incidence of severe recurrent bacterial or fungal infections and periodontal diseases. The purpose of this study was to determine thee relationship between oral manifestations in diabetic patients and duration of disease, and glycemic control.
Methods: We studied 116 diabetic outpatients referred to Diabetes Clinic of Shahid Bahonar Hospital of Kerman Province. We evaluated the following parameters: dental caries, periodontal status, xerostomia, burning sensation and mucosal alterations. We also recorded information about history of diabetes and glycemic control. Data were subjected to SPSS and X, t- test.
Results: The most frequent alterations in this study were gingivitis (96.3%), periodontitis (90.2%), and xerostomia (97.24%). Burning sensation and periodontal diseases were significantly correlated to duration of diabetes (P=0.002, P=0.012 respectively). The results of the present study showed that periodontal diseases occur more frequently in moderately and poorly controlled diabetic patients than in those with good glucose control, but this difference was not statistically significant.
Conclusion: In this study, dry mouth and periodontal diseases are common complaints among diabetic patients. Therefore management of oral diseases in diabetic patients should include a comprehensive evaluation of salivary function and periodontal status.

Abstract
Background: Adiponectin and leptin are hormones which are related to body fat tissues and body weight. In pregnant women, conflicting associations have been reported between the role of serum concentrations of adiponectin and leptin with infant birth weight. The aim of this study was to determine the association between maternal and cord blood adiponectin and leptin concentrations with birth weight.
Methods: As a cross-sectional study 86 pregnant women referred to university hospital clinics were recruited. Maternal and umbilical cord blood samples were obtained in delivery room just after birth. The maternal and umbilical cord serum samples were analyzed for adiponectin and leptin. Their birth weight and height were measured at labor.
Results: The mean of maternal BMI, birth weight, and gestational age was 23.8±4.8 kg/m2, 3.13±0.14 kg, and 38.15±2.6 week, respectively. No correlation between adiponectin and birth weight were found. Birth weight positively correlates with leptin.
Conclusion: leptin concentrations in cord blood may be reflected the alternation of body fat tissue in infant and independently associated with infant birth weight.

Abstract
Background: Children with Type 1 diabetes were present one of the greatest challenges in the worldwide. Health-care providers must be sufficiently aware of diabetes so that such children make utmost benefit of their educational opportunities. The aim of the present study was to compare the effect of three educational methods in raising the level of knowledge of school health-care providers. Therefore the best method would be applied for this purpose.
Methods: In this interventional study 132 samples were randomly chosen from the list of school health care providers of Tehran and divided to the three groups (educated by posters, booklets, and group education). A questionnaire was completed for all the three groups before and after the education. Questionnaire consisted of two parts: -demographic information and 16 questions about diabetes information. Scores were analyzed by descriptive and inferential methods (T-test, ANOVA).
Results: The level of knowledge of school health-care providers was weak in 23.5% of the case, average in 59.8% of cases, and good in 16.7% of cases. The level of knowledge in the groups educated by booklets and group education increased meaningfully, while no obvious increase was observed in the group trained via posters.
Conclusion: From the three aforementioned training methods, group education and education by booklets have the most effect on raising the level of knowledge of health-care providers.

Abstract
Background: Use of medicinal plants for attenuation of hyperglycemia and restoration of lipids to normal levels is very important. The effect of oral administration of Vaccinium myrtillus (VM) on serum glucose and lipids in diabetic rats was investigated.
Methods: Female Wistar rats were divided into 4 groups: control, VM-treated control, diabetic, and VM-treated diabetic groups. The treatment groups received oral administration of plant-mixed food (6.25%) for 4 weeks. Serum glucose, triglyceride, total cholesterol, LDL- and HDL- cholesterol levels were determined before the study, and at 2nd and 4th weeks after the study.
Results: Serum glucose level in diabetic group increased 2 and 4 weeks after the experiment as compared to data one week before the study (P<0.001) and VM treatment of diabetic rats did have a significant hypoglycemic effect (P<0.01). In addition, triglyceride level in diabetic group increased 4 weeks after the experiment in comparison with related data one week before the study (P<0.05) and there was a significant lower level of triglyceride in VM-treated diabetic rats (P<0.05). Furthermore, there was no significant changes regarding serum total cholesterol, HDL- and LDL- cholesterol levels in treated diabetic group as compared to untreated diabetic group.
Conclusion: Oral administration of VM has a significant hypoglycemic effects and leads to an appropriate changes only in triglyceride level.

compare the efficacy of the two traditional and electronic training methods in nutritional issues in order
to life style modification in old people to develop of healthy elderly.
Methods: This survey was a quasi-experimental method via applying the Pre-test and Post-test among
65+ years old people in a retirement association of Sari province, Mazandaran state. By simple random
sampling method, 60 old people were selected, and then trained in two separated classes to 15 persons as
women and men (30 persons) via film technique. The other method was a traditional which applied in the
two separated classes of 15 persons as women and men (30 persons) by book technique. The
questionnaire applied to measure of nutritional knowledge made up by the health center. Data were
described by using SPSS software in accordance to descriptive statistics indexes (mean and standard
deviation), and inferential statistics indexes (independent T and dependent T) were used in order to
generalize results.
Findings: The mean score of increasing knowledge in training group by film was 18/57 and the mean
score in training group by book was 16/90 (T =5/299, df= 58). To satisfaction evaluating, the mean score
in training group by film was 59 and the mean score in training group by book was 35/73 (T =1/96,
df=58).
Conclusion: Based on results training of old people by electronic method as a film is more effective than
traditional method to increasing of nutritional knowledge in elderly. On the other hand, they had more
satisfaction to life style modification by using film method in comparison with traditional method as
book reading technique. Also, the training cost of the elderly via educational film is lower than training
by book. So, should be suggested to consider in training by film rather than book as an efficient and
effective method to improve of elderly health.

Background: Diabetes is a chronic disease that can cause severe complications such as cardiovascular disease, blindness and kidney failure. Given the importance of self-care in diabetic patients, and considering that inadequate health literacy is a serious barrier for the management of diabetes that can affect patients' participation in self-care, Therefore this study aimed to assess the status of health literacy in patients with type 2 diabetes in three domains of functional, communicative and critical and Determine the factors associated with self-care behaviors in these patients. Methods: A cross-sectional study on 187 diabetic patients referred to Hazrat Ali diabetes clinic in Isfahan. The samples were selected by randomly Convenience sampling method. The level of health literacy and Status of self-care behaviors in patients was assessed by a standardized questionnaire (FCCHL) and (DMSES) respectively. Collected data were analyzed Using the ANOVA, t-independent, Pearson correlation coefficient at SPSS v.21 statistical software. Results: The mean age of the study population was 57.40 ± 11.07. Average health literacy score and self-care behaviors of the participants were 2.28± 0.57 and 3.84 ± 1.56 respectively. There was significant relationship between total health literacy, communicational and critical health literacy with self-care behaviors. There was no significant relationship between functional health literacy and self-care behaviors. Communicational health literacy was associated with diet, physical activity, blood glucose monitoring and foot care. Critical health literacy was associated with physical activity, blood glucose monitoring, foot care, and taking medication. There was no significant relationship between Communicational health literacy and taking medication and also between critical health literacy and diet. Conclusion: Considering that communication and critical health literacy was associated with the self-care of patients with diabetes so Emphasis and consideration should primarily be given on skills related to these domains of health literacy. Since that Patients’ functional HL capabilities that was associated with basic skills in reading and writing was not related to self-care, therefore assessment of education levels alone in the clinical setting is not recommended.

Abstract
Background: Resistin levels and insulin resistance increase in obese humans. Also, inactive life is associated with high risk of increased insulin resistance. The purpose of this study was to investigate effect of twelve weeks aerobic training on serum levels of resistin, and adiponectin-resistin (AR), homeostasis model assessment-adiponectin (HOMA-AD), and IRAR indices in sedentary obese men. Methods: In a semi-experimental study, twenty one sedentary obese men were randomly placed into two groups: aerobic training (n=10, 27.8±3.2 yr, 93.5±7.1 kg, 31.7±3.4 %, BMI 32.0±3.5 kg/m2) and control (n=11, 27.1±3.1 yr, 94.7±6.0 kg, 31.2±3.1 %, 32.2±3.3 kg/m2). General characteristics of subjects, serum levels of resistin, and HOMA-AD, AR, and IRAR indices were assessed before and after training. Aerobic training consisted of twelve weeks pedaling on cycle ergometer (3 sessions per week, intensity 60-70% of reserved heart rate, duration of each session 20-40 min). Data analyzed by SPSS16 software. Statistical significance was considered at P<0.05. Results: After the training period, serum levels of resistin (P=0.016), HOMA-AD (P=0.007), AR (P=0.020) and IRAR (P=0.003) were reduced, while none of mentioned parameters in control group did not change significantly (P>0.05). Conclusion: Circulating levels of resistin and HOMA-AD, AR, and IRAR, that are novel indices for insulin resistance, were reduced after twelve weeks aerobic training. Therefore, sedentary obese men can reduce risk of type 2 diabetes and cardiovascular diseases with performing aerobic training.

Abstract
Background: kallikrein 7 (KLK7), a serine protease with a chymotrypsin-like specificity, is able to cleave human insulin in the A- and B-chain. The aim of this study was to investigate the effects of aerobic exercise training on plasma KLK7 concentration and insulin resistance index (HOMA-IR) in normal and overweight sedentary women.
Methods: Twenty-eight sedentary women, aged 24-60 years, voluntary participated in this study and according to body mass index status divided into normal (n=15) and overweight (n=13) groups. All subjects completed an 8-week progressive aerobic exercise training program (running with 40- 80% Heart rate reserve). Metabolic and Anthropometric (body weight, BMI, body fat percentage) parameters in addition to plasma KLK7 concentrations were measured at baseline and end of training program.
Results: Body fat percentage significantly decreased (P<0.05) and maximum oxygen consumption increased (P<0.05) by this training program in both training groups. At baseline, plasma KLK7 concentration in overweight women was significantly higher compared with normal weight group (P<0.05). Plasma KLK7 concentrations significantly increased after 8-week aerobic exercise training only in normal weight group (P<0.05). Changes in plasma KLK7 concentrations were not correlated with changes in insulin concentration and insulin resistance index (P<0.05).
Conclusion: Aerobic exercise training could be an effective factor to increase plasma KLK7 concentration in sedentary women. These data do not support a role of plasma KLK7 in insulin resistance alterations.

Abstract
Background: Several studies have evaluated the effects of coenzyme Q10 on glycemic control, but there are large discrepancies between studies.
Objective: This meta-analysis of randomized controlled trials aimed to summarize the effect of Co-Q10 on diabetes’ biomarkers.
Methods: This systematic review and meta-analysis was conducted on studies published from 1998 until December 2015. We searched Pub med, EMBASE, Science direct, ISI web of science and Google Scholar to find relevant studies. Totally, our search resulted in 16 articles reporting the effects of Co-Q10 on fasting blood glucose, fasting insulin and HbA1c. Mean ± standard deviations (SD) were used for calculating mean differences. 95% confidence interval was considered between intervention and control treatments.
Results: The present meta-analysis revealed that Co-Q10 decreased fasting blood glucose (-0.20mg/dl, 95%: -0.38,-0.02). However, Co-Q10 supplementation could not effectively reduce HbA1c (0.05%; 95%: -0.22, 0.12) and fasting insulin (0.12pmol/l; 95%: -0.21, 0.44).
Conclusion: Co-Q10supplementation didn’t result in reduction in HbA1c and fasting insulin. However, it had a significant lowering effect on fasting blood glucose. It should be noted that significant changes in fasting blood glucose reported in this study was due to the extended sample size and this is not clinically significant in the present study.

Background: Irisin was identified as a myokine secreted by exercise which plays an important role in energy metabolism and regulation of metabolic diseases such as diabetes. The aim of the present study is to investigate the effects of high intensity interval exercise (HIIE) and moderate intensity continuous exercise (MICE) on serum irisin and Subcutaneous UCP-1 in diabetic male rats.
Methods: In this study, 29 diabetic Wistar rats (12 week- age, 220-240 gr- weight) were assigned to 5 groups: immediately and 2 hours after high intensity interval exercise (HIIE0, HIIE2), immediately and 2 hours after moderate intensity continuous exercise (MICE0, MICE2) and control (C). Both MICE groups performed on the treadmill with intensity 60-65% vo2max for 40 minutes and both HIIE groups with intensity 90-95% vo2max in the 12 interval-one minute period and 1 minute rest intervals. ELISA was used to measure serum irisin and subcutaneous fat UCP-1. One-way ANOVA and Tukey post hoc test has used to data analysis, the level of significance has been considered at P≤0/05.
Results: Data analysis showed serum irisin levels in the HIIE0 group and subcutaneous fat UCP-1 in the HIIE2 significantly increased compared to the control group (p˂0/05). However, there was no significant difference between other research groups (P>0/05).
Conclusion: It seems high intensity of exercise had important roles in significant changes of serum irisin levels and subcutaneous fat UCP-1in diabetic rats.
