Abstract
Background: Diabetes is a hidden epidemic and the most common metabolic disorder in the world. Immune dysfunction (cellular and humoural) is one of the consequences of diabetes, including defects of phagocyte function, notably in chemotaxis, phagocytosis, and killing. Results of studies on phagocyte respiratory burst activity, however, have been contradictory. We studied the respiratory burst of peripheral blood neutrophils and monocytes separately in response to formyl-met-leu-phe (fmlp) and phorbol-12,13-myristate acetate (PMA).
Methods: 36 patients with type 2 diabetes (mean age 53±7 years) and 20 healthy controls (mean age 50±5 years) each provided a 15ml blood sample. Peripheral blood neutrophils and monocytes were separated and purified (>95%) using specific density gradients and short-term culture. We then separately assessed respiratory burst activity in response to fmlp and PMA by the semi-quantitative nitroblue tetrazolium (NBT) test.
Results: Following stimulation with PMA, diabetic neutrophils showed reduced respiratory burst activity compared with normal neutrophils (p=0.097). Following stimulation with fmlp, too, diabetic neutrophils showed reduced respiratory burst activity compared with normal neutrophils, which this time was statistically significant (p=0.027). There was no difference between diabetic and normal subjects with regards to monocyte response to either fmlp or PMA.
Conclusion: It appears that the diminished response and reduced effectiveness of the phagocyte system in people with diabetes can facilitate the onset, exacerbation, and persistence of infection.

Abstract
Background:Diabetes mellitus is regarded as a major health problem due to its complications that these,nonetheless could be prevented or delayed with changes and modifications in the patients’ behavior and dietby understanding their beliefs..The objective of this study was to explore the utility of the Theory of PlannedBehavior (TPB)and complications of diabetes perceived risk to find modifiable diabetes-related believes inorder to make behavior changes feasible.
Methods: The presentdescriptive-analytical cross sectional study was conducted in Semirom Diabetes Clinicbased on the TPBand complications of diabetes perceived risk, using a self-administered questionnaire. Atotal of 154 diabetic patients without complications of diabetes were participated in the study. Measured variables were patients’ attitude, subjective norm, perceived behavioral control, complications of diabetesperceived risk, intention to eat a healthful diet and nutritional style. Structural equation modeling was used toanalyze data.
Results:The results revealed that the TPBand complications of diabetes perceived risk fitted the data
acceptably well among the Type 2 diabetes and within dietary behavior. Perceived behavioral control,
subjective norm, complications of diabetes perceived risk and intention to eat a healthy diet were related tohealthy diet behavior. No relation was found between attitude and intention to eat a healthy diet.
Conclusion:The TPB and complications of diabetes perceived risk is a useful theory in determining
intentions of diabetic patients’ and their adherence to healthy diets. TPB and complications of diabetes perceived risk forms a framework for promoting dietary practice among Type 2 diabetic patien

Abstract
Background: Gestational diabetes in on of common complications of pregnancy which has different complications for fetus and mother. Aim of present study was to review the effect of six weeks aqua training in second trimester of pregnancy on visfatin and glucose tolerance of gestational diabetic women.
Methods: In this semi experimental research 24 gestational diabetic mothers who recourse to hafez clinic of shiraz city which were in 23- 24 week of pregnancy select as sample and divided in two groups of 12 persons of aqua training and control. Aqua training group trained aqua trainings 6 weeks, 3 sessions per week and 45 minutes per session. In this period of the time control group has just their daily activities. For measure the research variables fasting blood sample gathered from antecubital vein in pretest and post test.
Results: Six weeks aqua training in second trimester of pregnancy has significant effect on increase of visfatin and decrease of glucose tolerance of gestational diabetic women (p=0.001).
Conclusion: It seems that for improvement of visfatin levels and glucose tolerance of gestational diabetic women can use six weeks aqua training in second trimester of pregnancy.

Abstract
Background: Inflammation plays an important role in the incidence and development of metabolic disorders and exercises along with dietary restrictions for weight loss. It has beneficial effects on reducing inflammatory markers, especially in type 2 diabetes. The purpose of this study was to evaluate the effect of combined exercise (resistance-aerobic) with and without diet restriction on weight loss on some inflammatory markers (CRP, TNF-α and ICAM-1) in elderly men with type 2 diabetes.
Methods: 42 elderly men with type 2 diabetes (age: 67.6 ± 2.2, weight: 85.2 ± 7.4, BMI: 28.2 ± 0.2) after homogenization based on serum levels Glucose and mass index were randomly assigned to one of 3 groups: Combined exercise (15), combined exercise with weight loss diet (14), and weight loss diet alone (n = 13). The combined exercise program included aerobic training with 50-70 percent of reserved heartrate and resistance training included chest press, cable pull-down, machine leg extention and machine lying leg curl on 3 sessions per week for 12 weeks.
Results: After 12 weeks of intervention, there was a significant decrease in serum inflammatory markers levels (CRP, ICAM-1 and TNF-α) in the post-test compared to the pre-test in all three groups (P<0.001). However, bonferroni post-hoc analysis showed that combined exercise with diet compared to combined exercise and dietary restriction alone had more effects on reducing CRP, ICAM-1 and TNF-α (P<0.001). In addition, reduction of ICAM-1 (P<0.001) and TNF-α (P<0.001) after combined exercise compared to dietary restriction and CRP reduction after dietary restriction than combined exercise was significantly higher (P<0.001).
Conclusion: It suggested that in elderly type 2 diabetic patients, caloric restriction-induced weight loss with combine exercise is more appropriate for modulating of endothelial biomarkers and Inflammation dysfunctional levels than resisted exercise training or aerobic training alone.

Abstract
Background: Type 2 diabetes Mellitus (T2DM) is a multifactorial, polygenic disease caused by impaired insulin secretion, insulin resistance and beta-cell dysfunction. Melatonin is a circadian rhythm regulator and any imbalance in its levels can be related to various metabolic disorders. Melatonin and the genetic variants of MTNR1B gene are reported to be associated with T2DM susceptibility. We investigated the association between rs4753426 variant in the MTNR1B gene and the risk of T2DM in group of Iranian patients.
Methods: In this case-control study108 T2DM and 100 normal individuals were recruited to genotyping by PCR- RFLP.
Results: It was observed a significant difference in CC, CT, and TT genotypes distribution between T2DM and control groups (P<0.001). Frequency of C allele among cases was significantly lower than controls (8.3% vs. 42.5% respectively, P<0.001) and C allele carriers had a 88% lower risk of developing T2DM than T carriers.
Conclusion: Our results showed that the rs4753426 variant of MTNR1B gene could reduce the risk of T2DM developing.
