Abstract
Background: Adiponectin and omentin-1 are adipokines with insulin-sensitizing properties. The aim
of this study was to investigate the effect of resistance training on serum levels of adiponectin and
omentin-1 in Streptozotocin-induced diabetic rats.
Methods: Twenty four male Wister rats (12-14 weeks’ old) were randomly divided into three groups:
non-diabetic control, diabetic control, and diabetic training. The rats in diabetic training group were
subjected to a resistance training program (3 days/wk, for 4 wk) consisted of climbing a ladder
carrying a load suspended from the tail. Following four weeks resistance training serum glucose,
insulin, lipid profile, adiponectin, and omentin-1 concentrations were measured.
Results: Serum levels of omentin-1 and adiponectin were significantly lower in diabetic control group
compare with non-diabetic control group (P<0.001). After 4 weeks of resistance training serum
adiponectin levels was significantly higher in diabetic trained group compared with diabetic control
group (P= 0.028), but we did not find any significant difference in omentin-1 levels between two
diabetic groups. Morever, we did not find any significant difference in serum lipid profile among all
groups. Four weeks resistance training did not change serum glucose and insulin concentrations in
diabetic rats.
Conclusion: This study indicated that resistance training could increase serum adiponectin levels in
diabetic rats without significant changes in lipid profile, glucose, insulin, and omentin-1
concentrations. It seems low intensity and short term duration of resistance training had important
roles in failure of significant changes of omentin-1, glucose, and insulin concentrations.

Abstract
Background: Dementia is a common health problem in the elderly. Studies have shown that levels of pulse wave velocity (PWV) could predict cognitive impairment. The objective of this study was assessment of the relationship between arterial stiffness levels and cognitive status among the older people.
Methods: In this cross-sectional study, 60 individuals included 31 women and 29 men aged ≥ 60 years who were admitted to Khatam-Ol-Anbya hospital in Tehran and were eligible, were randomly selected according to admission code. Data such as demographic information, Global Deterioration Scale level, Geriatric Depression Scale -15 score, were collected by face to face interview. The arterial stiffness was measured using Sphygomocor system by calculation of carotid to femoral pulse wave velocity. Data analyzing was conducted using univariate and multivariable logestic regression models.
Result: The mean age of the participants was 72.62 (7.62) and it was significantly higher in cognitive impairment than those who had normal cognition [73.40(7.36) vs. 70.27(8.00) respectively, P = 0.05]. The average years of the schooling in individuals with normal cognitive status was higher than the other groups (P < 0.01). Average pulse wave velocity in the participants was 11.81(4.11) meters per second. The PWV was significantly increased with the exacerbation of the cognitive impairment (7.24 m/s in normal group vs. 15.52 m/s in the severe cognitive impaired group, P < 0.01).
Conclusion: The pulse wave velocity has an inverse relationship with cognitive status. It seems that arterial stiffness associates with lower levels of cognitive status independent to other cardiovascular risk factors.

Abstract
In medical studies, measures are required to reveal the effect of exposures and interventions and also the precision of measurements. This paper aimed to introduce the measures of effect and agreement and inferences about them in these studies. Principles and method of calculating measures of effect and agreement and inference about them were presented for all types of medical studies taking into account the relevant considerations. To assess the effect of risk factors on outcomes in case-control and cohort studies, and to determine the relevant effect, the attributable risk and fraction in the exposed group and population were used along with their confidence intervals. Also the relative risk reduction, absolute risk reduction and number needed to treat were applied as the measures of effect of intervention in the interventional studies especially in trails. The sensitivity, specificity and related measures along with their confidence intervals were computed for diagnostic accuracy and screening studies. In addition it is needed to evaluate the precision of measurements using standard error of measurements, ICC, Altman and Bland’s limits of agreement and Lin’s concordance correlation coefficient for quantitative variables and using kappa and weighted kappa for nominal and ordinal variables. In each of these situations the results of research based examples were presented along with the methods of their calculations. To assess the measures of effect and agreement, the mentioned analyses are recommended considering the situation and purpose of the study.

Abstract
Background: obesity is the major health problems affecting communities worldwide and the prevalence is rapidly rising. Obesity as a lifestyle-related factor increases the risk of many diseases. Omentin is a new adipocytokine that is abundantly expressed in visceral fat tissue and its expression levels reversely correlated with obesity. The purpose of this study was to investigate the association between Val109Asp genetic polymorphism of Omentin gene and obesity risk in women. Methods: This case - control study was done on 260 women, including 186 women with BMI<30 as a control group and 74 women with BMI&ge30 with obesity. Omentin genotypes were determined by the use of polymerase chain reaction (PCR) followed by restriction fragment length polymorphism (RFLP). The data were analyzed using the computer software SPSS for windows version17. Results: Genotype frequencies of the Asp/Asp, Asp/Val and Val/Val in the control group were 65.6%, 31.7%, 2.7% and obese patients were 51.4%, 39.2%, 9.5%, respectively. Comparison of genotype frequencies in the two groups showed that women with Val/Val genotype in compare to Asp/Asp had greater risk for complications of obesity (OR: 4.5, 95%CI: 1.3-14.9, P: 0.01). Conclusion: There is significant association between Val109Asp polymorphism in omentin gene and obesity in Iranian women.

Abstract
Background: One of the serious complications of type 1 diabetes is a sudden increase and drop in blood glucose levels causing risks of anesthesia and coma. Thus, an important step towards the optimal control of the disease is to use intelligent methods with low error rate and available information in order to predict and prevent such complications. In this paper, a combined Fuzzy SARSA algorithm was employed to design an expert system to predict fluctuations in blood glucose levels in patients with type 1 diabetes. The aim of this paper is to propose and implement a new algorithm based on reinforcement learning and fuzzy logic called FSA (Fuzzy SARSA Algorithm) to develop an expert system. Methods: The medical data used in this article were related to 3 Iranian female patients with type 1 diabetes. The data included parameters such as the type and dose of injected insulin and time interval between the hours of recorded blood glucose level and the one at the beginning of the period. A well-known reinforcement learning algorithm called SARSA was implemented with fuzzy logic. The Fuzzy SARSA algorithm was used in an expert system to predict fluctuations in blood glucose levels in patients with type 1 diabetes. Results: The results of the expert system with the proposed FSA algorithm for a learning factor included the parameters of blood glucose level at the beginning of the period (mg/dlit), short-acting injected insulin dose in to the body (unit), long-acting injected insulin dose (unit), stress level (unit), physical activity level (unit), used carbs (gr), and the interval between the beginning and end of the period (hour). They indicated that the factor managed to keep blood glucose levels at 85% in a normal range of 80 to 120, and it had 15% error. The error was observed in cases in which there was no proper exploratory factor for short-acting and long-acting injected insulin. Conclusion: According to the results, it was found that the proposed algorithm for intelligent system, with an approximate reduction of 15% in prediction error, can be used as a good model to predict fluctuations in blood glucose levels in patients with type 1 diabetes in comparison with other conventional methods including the ones based on reinforcement learning. In addition, other prediction methods based on artificial intelligence such as neural networks can be used to predict the results accurately.

Abstract
Background: Health locus of control is an important variable in diabetes. The aims of the present study were to investigate the role of health locus of control in predicting psychosocial adjustment to illness and compare psychosocial adjustment to illness and dimensions of health locus of control among male and female type 2 diabetic patients.
Methods: One hundred and eighteen patients were recruited via convenience sampling method from type 2 diabetic patients who were referred to the Shahid Motahari diabetes clinic in Shiraz. The participants filled Multidimensional Health Locus of Control (MHLC) and Psychosocial Adjustment to Illness Scale (PAIS).
Results: The results indicated that there were significant positive correlations between psychosocial adjustment to illness and internal health locus of control (r= .30, P &le .001) and God health locus of control (r= .18, P &le .05). Internal health locus of control (&beta= .28, P &le .01) significantly predicted psychosocial adjustment to illness. There were significant differences between male and female diabetic patients on the basis of chance health locus of control. Female patients got higher scores on chance health locus of control. Moreover, there were no significant differences between male and female diabetic patients on the basis of psychosocial adjustment to illness.
Conclusion: The findings highlighted that internal health locus of control and God health locus of control play significant role in psychosocial adjustment to illness. Therefore, it is recommended that psychologists and clinicians take the dimensions of health locus of control into considerations in the treatment of psychological problems of diabetic patients.

Abstract
Background: kallikrein 7 (KLK7), a serine protease with a chymotrypsin-like specificity, is able to cleave human insulin in the A- and B-chain. The aim of this study was to investigate the effects of aerobic exercise training on plasma KLK7 concentration and insulin resistance index (HOMA-IR) in normal and overweight sedentary women.
Methods: Twenty-eight sedentary women, aged 24-60 years, voluntary participated in this study and according to body mass index status divided into normal (n=15) and overweight (n=13) groups. All subjects completed an 8-week progressive aerobic exercise training program (running with 40- 80% Heart rate reserve). Metabolic and Anthropometric (body weight, BMI, body fat percentage) parameters in addition to plasma KLK7 concentrations were measured at baseline and end of training program.
Results: Body fat percentage significantly decreased (P<0.05) and maximum oxygen consumption increased (P<0.05) by this training program in both training groups. At baseline, plasma KLK7 concentration in overweight women was significantly higher compared with normal weight group (P<0.05). Plasma KLK7 concentrations significantly increased after 8-week aerobic exercise training only in normal weight group (P<0.05). Changes in plasma KLK7 concentrations were not correlated with changes in insulin concentration and insulin resistance index (P<0.05).
Conclusion: Aerobic exercise training could be an effective factor to increase plasma KLK7 concentration in sedentary women. These data do not support a role of plasma KLK7 in insulin resistance alterations.

Background: The purpose of this study was the investigation effect of gender on plasma glucose, insulin and insulin resistance index in response to acute endurance activity and subsequent recovery periods in healthy individuals.
Methods: Eight females and seven males 20-30 years old (females 22.37±1.50 and men 23.42±1.27 years old, female weight 55.50±8.14 men and 79.85±15.16 kg, women height 160.12 ± 4.18 men and 179.71±6.57 cm) voluntarily participated in this study. Subjects performed two control activities during two consecutive weeks. The implementation of the sessions was randomly determined for each subject. Exercise program consisted of 45 min acute endurance at 75% of maximum heart rate on the treadmill and subsequently the subject was in a sitting position for one clock recovery. Four blood samples (6 ml) before and immediately after exercise, as well as 1 and 24 hours after the activity was collected. Two-way analysis of variance for data analysis was used.
Results: Regardless of gender, effects of activity and recovery on plasma insulin and insulin resistance index was significant (p <0.05) but not significant for glucose. Also when the data were corrected by changes in plasma volume, similar results were observed for insulin. Although changes of all factors (glucose, insulin, and insulin resistance) in response to activity session of acute exercise endurance was higher in men than in woman, gender has not significant effect on factors.
Conclusion: acute endurance activity and recovery induce change in insulin resistance index but these changes were not related to gender.

Abstract
Background: Although several tools for evaluating and screening of cognitive status have been introduced, most of them are not applicable in Iranian elderly population because of cultural differences and high illiteracy rate. This study is report of "Brief Cognitive Assessment Tool"(BCAT) was developed as a cognitive status-evaluating test and reported psychometric characteristics.
Methods: the BCAT tool includes memory assessment using three words recall test with a rating similar to Mini-Cog and the functional status including "ability to dress", "ability to use the phone," " ability to manage own medications", among both two sexes and financial management capabilities in men and ability to prepare food in women. Functional rating is different in educated and non-educated people. Diagnosis of dementia is considered, when memory impairment is accompanied by the dysfunction. Eighty-six residents of a nursing home (41 men and 45 women) among who had inclusion and no had exclusion criteria also consent to participate were randomly enrolled. Concurrent validity was shown as correlation coefficient between the BCAT categorization and score of mini meAntal state examination (MMSE). Criterion validity, sensitivity and specificity of BCAT were evaluated based on global deterioration scale (GDS). Reliability of two raters results within two weeks was indicated by Kappa coefficient.
Results: The sensitivity and specificity and accuracy rate of the BCAT based the GDS was 88.6%, 82.6%, and 84.9%, respectively. The correlation between the MMSE score and results of the BCAT classifying was significant. The Kappa coefficient between the results of two raters was 81.1%.
Conclusion: It seems that the BCAT has good psychometric properties to recognize cognitive impairment among Iranian older adults.

Abstract
Background: The adipokines omentin-1, vaspin, and retinol binding protein-4 (RBP-4) are mediators of body composition and adipose tissue that contribute to metabolic changes caused by obesity. The aim of this study was to determine the association between serum levels of these adipokines with body composition indices and metabolic status in different phenotypes of body size.
Methods: In the present cross-sectional study, a total of 350 women, including 127 metabolically healthy (MH) subjects and 223 metabolically unhealthy (MUH) subjects participated. The subjects were considered as MH in the presence of at least 4 of the five following metabolic indices: TG ≤ 1.70 mmol/L, LDL-C≤ 2.60 mmol /L, TC≤ 5.20 mmol /L, HDL-C≥1.30, and HOMA-IR≤ 1.95; otherwise subjects were considered as MUH.
Results: The results showed that serum levels of omentin-1 have a significant positive association with MHS (OR = 1.04, p <0.0001). Even after controlling the confounding factors of age, sex and weight, this relationship was still significant. Plasma levels of vaspin and RBP-4 were not significantly related to MHS. Furthermore, there was a significant correlation between omentin-1 and body mass index (r = 0.31, p = 0.02) and body fat percentage (r = 0.32, P = 0.02) in MH subjects. In the healthy group, serum levels of RBP-4 were positively correlated with fat percentage (r = 0.68, p <0.0001) and body fat mass (r = 0.74, P <0.0001) and were negatively correlated with fat-free mass (r = -0.74, P <0.0001) and total body water (r = -0.76, P <0.0001). There was no statistically significant correlation between serum vaspin levels and body composition indices in both groups.
Conclusion: Serum levels of adipokines may be an appropriate predictor for metabolically healthy status. Further studies in this field can reveal possible mechanisms.
