Abstract

Background: Finding effective drugs is considered for healing diabetic wounds by researchers. The purpose of this study is to evaluate the wound healing effects of palmatine in the healing of skin wounds in normal and diabetic rats. Methods: In this study 16 male Wistar rats were dهvided into four groups (Negative control, Positive control, The first experimental,The second experimental) . A 4 cm2 wound was created in the left side of the spinal cord in each of these groups and the process of wound healing was macroscopically evaluated. Results: The wounds of the diabetic groups (the diabetes was coused by streptozocin) showed longer healing process in comparison with the normal groups and the wound healing in the experimental groups which were treated by palmitine was faster in comparison with the control group. Conclusions: the results showed that palmatine speeds the wound healing up in normal and diabetic samples.

Abstract

Background: Diabetes Mellitus (DM) is one the most common chronic disease, with many complications including renal failure, blindness and non traumatic amputation. Prevention of complications is possible through monitoring and control of blood glucose levels. Considering how easy home blood glucose monitoring is, we decided to evaluate the performance of two available glucometers for detection of blood glucose compared with standard laboratory methods. Methods: In this study, 369 patients with diabetes who admitted to the Shahid Beheshti hospital laboratory in 2013 selected by simple way. After fasting blood glucose testing at the laboratory by lancet devices for ONCALL and GLUCOCARD Glucometers, capillary blood was taken, and glucose measurements were performed by a special strips. Finally, we compared of capillary blood glucose levels of 369 patients as determined by test strips (Glucometers) with venous blood glucose levels measurements by the enzymatic method (the standard laboratory kit), and the results was evaluated by statistical tests such as the mean ± standard deviation, and Pearson correlation. Results: The mean blood glucose measurements reported using the Glucocard glucometer, the ONCALL glucometer and the standard laboratory method were 127.0±51.6mg/dl, 117.9±42.1 mg/dL and 118.09±51.9 mg/dL. Results obtained using the ONCALL Glucometer had no differences with the standard method levels)p=0.901(, but obtained results from GLUCOCARD Glucometers had differences with the standard method levels(p≤0.001). Conclusion: Diversity glucometer devices used in various parts of the country highlight the need for similar studies. We recommend use the ONCALL Glucometer with standard principles for glucose monitoring routine.

Abstract

Background: Because of various health problems, the risk of the death in the elderly is higher than adults. The purpose of this study was to investigate the causes of the death, and comparing the cause of death that recorded on the death certificates and those were extracted from the medical records review. Methods: Variables included the demographic characteristics (age and sex), time of death and the cause of death were recorded in the death certifies and those were extracted from the medical records. Results: The mean of death age was 83.8 (8.50) years and in women and men were 84.2 and 81.5 years, respectively, (P < 0.01).The most prevalent cause of death was sudden cardiac death based on both death certificates recorded and review of medical records were 42.5% and 25.9% respectively. Sudden cardiac death in both methods was higher in women than men. Using the kappa, the highest degree of correspondence about the cause of death between two method was about gastrointestinal bleeding (K = 0.76) and then the cancers (K = 0.71) and respiratory infections (K =0.67). Total Kappa of two methods for causes of mortality was 0.50. The most cases of death were reported during 12:00 – 13:59 and 8:00 – 9:59am, time period (16.2 % and 14.1% respectively). Conclusion: the causes of deaths, which are recorded in the elderly nursing homes, do not have enough accuracy. Due to the high incidence of sudden cardiac death in elderly in nursing homes, resuscitation facilities and teams may be helping to reduce these types of death.

Abstract

Background: Metabolic syndrome is a cluster of disorders which altogether increase the risk of cardiovascular disease and type 2 Diabetes. We evaluated the effect of consumption of Omega-3 Fatty Acids on different components of this syndrome. Methods: This was a double blind placebo controlled clinical trial on 199 elderly residents of Kahrizak charity foundation in Tehran. Participants were divided into two groups of intervention and control, consisting of 100 and 99 persons, respectively. The intervention group was given the Omega-3 Fatty Acid capsules contain 180 mg EPA and 120 mg DHA once daily. The control group was given placebo once per day. The duration of the study was 6 months in the beginning and after 6 months of the study, blood pressure, waist circumference and Serum levels of ESR, hs-CRP, APO B, APO A1 and Insulin were measured. Results: The study was completed in 89 and 87 participants in the intervention and control groups, respectively. The mean age of intervention and control groups were 74.13 ± 9.96 and 75.17 ± 8.70 years, respectively. Consumption of Omega-3 Fatty Acids could decrease diastolic blood pressure (P = 0.001) and serum Triglyceride levels (P = 0.01) significantly compared to control group. No change was observed in other measured parameters. Conclusion: Consumption of Omega-3 Fatty Acids with dosage used in this study could probably improve symptoms of Metabolic syndrome in the elderly.

Abstract

Background: increasing the population of aged, Consequently Increasing rate of chronic disease and to decreasing quality of life level caused many problems in different communities. The important aim of this study was surveying the effect of an educational intervention by use of the educational components of precede model on quality of life in the elders. Methods: This semi-empirical research was performed on 54 aged people in two group-case and control- for the Aged in 2009. In this study two questionnaires the designed questionnaire based on PRECEDE model and the questionnaire on the quality of life standard of the World Health Organization (WHOQOL) were used and the quality of life of the aged people in two groups before and after one month from the educational interference was evaluated. Independent t-test paired T-test, one way ANOVA, was used for data analysis. Results: The average total score of the quality of life of the aged people of test group, showed a meaningful difference before and after educational interference with a p quality less than 0.05, but there wasn’t a meaningful difference between total scores of two groups. Though the relationship of attitude (predisposing factors), enabling factors (skills) and behavioral factors with quality of life of a p quality less than 0.05 was meaningful. But knowledge (predisposing factor) and reinforcing factors didn’t show a meaningful relationship with the quality of life. Conclusion: It seems that the health, education and health promotion programs are effective on quality of life in the aged people.

Abstract

Background: Health staffs have different attitudes and beliefs toward elders that have effect on their behaviors in health service delivery to elders. This study was done for determination of Golestan province health centers general practitioners (GPs) attitude toward elders. Methods: In this cross sectional study, 152 general practitioners of Golestan province health centers were selected via random sampling. The Kogan's Attitudes toward Older People Scale (KAOPS) questionnaire was used as data gathering tools. Data analysis was done in SPSS 16 software using Pearson correlation coefficient, paired t-test, t-tests and one way ANOVA test. Results: Mean (standard deviation) of participants age was 34.73 (6.26) years. Mean (standard deviation) of total score of attitude toward elder scale was 115.48 (10.22) (score range in scale 34-204). 54.2 percent of GPs have lower score than mean. Negative attitude mean score were higher than positive attitude mean score which was statistically significant (P= 0.0001). Conclusion: According to our finding, only half of GPs had favorite attitude toward elder. Whereas negative attitudes were higher than positive attitude toward elder, designing some programs for change in attitude of heath care staffs is necessary.

Abstract

Background: Given diabetes mellitus has an adverse effect on spermatogenesis and male fertility, the aim of the present study was to investigate the effect of L-carnitine administration on spermatogenesis and testicular structure in diabetic rats induced with Stereptozotocin. Methods: In this experimental study, fifteen rats were randomly divided into 3 groups: control group was received citrate buffer. Case group was diabetic rats that received 40 mg/kg carnitine for 16 days. Third group was diabetic rats that did not receive carnitine. After sixteen days, morphometric study was performed. Besides, index of testicular weight was obtained. Then, data were analyzed using SPSS software and ANOVA. Results: In testicular sections of diabetic rats were observed a disruption and deformity in first layer of somniferous tubules. Statistical analysis showed a significant difference in carnitine group when compared to the control group (P<0.05). There was a significant difference between weight of diabetic rat compared to the control group (P<0.001). Besides, there was the most ratio of testis index in control group. Conclusion: Our finding showed that short-term L-carnitine administration was affective on the diameter and thickness of seminiferous tubules. Hence, L-carnitine treatment may cause an improvement in spermatogenesis of diabetic men.

Abstract

Background: Balance dysfunction is one of the problems in diabetic patients so that peripheral neuropathy and decreased somatosensory sensitivity are the most important causes for it. One of the supposed theories for diabetic peripheral neuropathy is reduction in blood flow secondary to pathologies of peripheral neural arterioles. Intermittent Pneumatic Compression, regarding to its effect on vessels hemodynamics and perfusion improvements, has been considered in recent years. The aim of this study is to evaluate the effects of this method on improvement of neuropathy signs and symptoms improvement in patients with type 2 Diabetes and neuropathy. Also, regarding to the role of neuropathy on balance impairment, other aim of this study is to investigate the effect of this method on improvement of dynamic balance in diabetic patients. Methods: This study is a clinical trial study. 39 patients with diabetes type 2 and neuropathy divided into intervention (20 patients) and control (19 patients) groups. The intervention group underwent 10 sessions of IPC treatment, with 45 minutes for each session and one day interval between them. Neuropathy severity changes (by Valk and Michigan Questionnaires), Proprioceptive sensation (assessed by Diapason), and balance (by Biodex system), were evaluated in both groups in first and final sessions. Results: Anterior-Posterior Stability Index and Overall Stability Index obtained from Biodex system in level 6 showed significant changes. Vibration sensation, and Valk and Michigan neuropathy questionnaires also showed significant improvements (P<0.05). Conclusion: This study showed that IPC treatment method has positive effects on improvement of neuropathy severity, Vibration sensation and dynamic stability (Biodex).

Abstract

Background: Vitamin D deficiency is an unrecognized epidemic and a common health problem worldwide. The objective of our study was to determine the prevalence of vitamin D deficiency and its relationship with overall and abdominal obesity in adolescent girls. Methods: This cross-sectional, descriptive study was conducted on girls aged 14-17 years whom were selected by multistage random sampling from four districts of Boukan. Questionnaires about general characteristics, dietary intake and physical activity were completed by interviewing with the subjects. Anthropometric measurements were performed. Fasting blood samples were collected for determining of serum of 25 (OH) D levels. Data was analyzed by using SPSS software and descriptive statistics tests and partial correlation test. Results: The Mean of serum 25(OH) D was 7.26 (SD 2.81) ng/ml and 100% of subjects had vitamin D insufficiency. According to CDC (Centers for Disease Control and Prevention) criteria the prevalence of overweight, obesity and abdominal obesity were 18.5%, 1.4% and 26.4% respectively. There were significant reverse correlation between serum 25(OH) D and WC (r= -0.112, p=0.047). Conclusion: The Results of this study indicate that vitamin D deficiency is a major health problem among studied adolescents. Given the reverse association between vitamin D status and abdominal obesity appropriate approaches are needed to improve vitamin D status and lowering the prevalence of obesity among adolescents. Such conditions may prevent prevalence of chronic disease in adolescents' lives over the coming years.

Abstract

In medical studies, measures are required to reveal the effect of exposures and interventions and also the precision of measurements. This paper aimed to introduce the measures of effect and agreement and inferences about them in these studies. Principles and method of calculating measures of effect and agreement and inference about them were presented for all types of medical studies taking into account the relevant considerations. To assess the effect of risk factors on outcomes in case-control and cohort studies, and to determine the relevant effect, the attributable risk and fraction in the exposed group and population were used along with their confidence intervals. Also the relative risk reduction, absolute risk reduction and number needed to treat were applied as the measures of effect of intervention in the interventional studies especially in trails. The sensitivity, specificity and related measures along with their confidence intervals were computed for diagnostic accuracy and screening studies. In addition it is needed to evaluate the precision of measurements using standard error of measurements, ICC, Altman and Bland’s limits of agreement and Lin’s concordance correlation coefficient for quantitative variables and using kappa and weighted kappa for nominal and ordinal variables. In each of these situations the results of research based examples were presented along with the methods of their calculations.To assess the measures of effect and agreement, the mentioned analyses are recommended considering the situation and purpose of the study.

Abstract

Background: The role of obesity and overweightness in emerging and enduring of serious medical and psychological disorders on one hand and the effect of psychological factors in obesity on the other hand, has made the surveys about psychological role in obesity of great importance. The aim of the present study was to compare the attachment styles and parenting styles of obese and non-obese adults. Methods: 144 individuals (72 obese patients: 48 females and 24 males) and (72 non-obese: 48 females and 24 males) participated in this study. Participants were asked to complete the Adult Attachment Inventory (AAI) and the Parental Authority Questionnaire (PAQ). Results: There was a negative relationship between secure attachment and Body Mass Index (BMI) of the obese patients, but no statistically significant relationship between insecure attachment styles (avoidant and ambivalent) was found. There was also a statistically significant relationship between the mean scores of secure and ambivalent attachment styles with BMI. Regarding the parenting styles and BMI, there was a statistically significant relationship between authoritative parenting style of the father and BMI, but no relation between authoritarian and permissive parenting styles was confirmed. Conclusion: Regarding the role of attachment styles and parenting styles of obese and non-obese adults, we can improve the physical and psychological public health of patients by changing the prevention and treatment protocols for the patients.

Abstract

Gangliosidosis GM1, is a lipid storage disorder due to deficiency of the lysosomal enzyme &beta-galactosidase. This deficiency leads to lysosomal accumulation of gangliosidosis GM1 to be, which contributes to swelling, cell damage and dysfunction of the organs. Gangliosidosis GM1a rare neurodegenerative disorder. Mongolian spots are significant clinical sign in some of the metabolic diseases, such as gangliosidosis GM1, but limited information is available. Hepatomegaly, hypotonia, edema, seizures, and skeletal malformations and Cherry red spot in the macula of the eye are of other symptoms of the disease. In this paper, we present a 10-month-old child with gangliosidosis GM1 type 1 with extensive mongolian spots who was referred the Hajar Medical Educational Therapeutic Center Shahrekord.

Abstract

There are situations in medical studies, wherein it is impossible to use the methods based on normal distribution (parametric methods). This paper objects to introduce common nonparametric methods and the inferences based on the methods in medical studies. Principles and method of calculations along with the software codes for common nonparametric methods and inference based on them were presented taking into account the considerations relevant to choose the nonparametric methods and their relative efficiency with examples in medical studies. In the situation where the assumptions are not satisfied, the nonparametric methods should be used without caution to lose the efficiency or even with higher efficiency of these methods. To compare a non-normal or ordinal variable between two groups Mann-Whitney test, to compare a non-normal or ordinal variable among more than two groups Kruskal-Wallis test, to compare a non-normal variable between two related situations or matched groups Wilcoxon test and to compare an ordinal variable between two related situations or matched groups Sign test should be used. In each of these tests the results of research based examples were presented along with the methods of their calculations. To assess the relation or difference in all types of medical studies, these tests are recommended considering the situation and purpose of study.

Abstract

Background: Hyperlipidemia is a major cause of cardiovascular diseases (CVD) and the most common problems in the community. The present study examined the effect of grape seed supplementation along with aerobic activity on lipid profile in elderly women Methods: In this experimental study, 40 obese women (BMI&ge30 kg/m2) over 60 years were selected and randomly divided into two groups (supplement + Exercise and control group). The exercise program included aerobic exercise with 50% of maximum heart rate for three times a week for 8 weeks plus Grape seed extract supplement 200 mg daily for 8 weeks. Blood sampling after fasting period for 12 hours in pretest and 48 hours after the last training session was taken. To compare data between groups t-test was used. Results: The results showed significant differences in the levels of cholesterol, triglycerides and LDL-C and pre-test and post-test group supplemented along with sports activities. Conclusion: Long-term aerobic exercise combined with grape extract consumption may reduce the lipid profile parameters.

Abstract

Background: One of the important goals in control diabetes to maintain normal levels of glycated hemoglobin, but different social and psychological factors such as social support and alexithymia play role on process of this control. The purpose of present study was to examine the role of alexithymia on the association of social support and glycated hemoglobin in patients with type 2 diabetes. Methods: This was a cross-sectional study. The study sample consisted of 100 (60% females, 40-60 years) type 2 diabetic out-patients visiting the Labbafinejad hospital in 2013. The sample of the present study was a convenience sample. Measures of data collection included demographic and anthropometric questionnaires, Multidimensional Scale of Perceived Social Support, Toronto Alexithymia Scale and test of glycated hemoglobin. The collected data were analyzed, using multiple regression analysis. Results: Pearson's correlation coefficients showed a significant association between better reported perceived social support and lower alexithymia and glycated hemoglobin. Results of hierarchical linear regression analyses indicated that social support explained 23% and alexithymia 31% of glycated hemoglobin variance. As well as, alexithymia moderated association between perceived social support and glycated hemoglobin in patient with type 2 diabetes. Conclusion: Based on the present data, alexithymia has moderate role on the association between perceived social support and glycated hemoglobin in patient with type 2 diabetes. The results of this study highlight effective psychological factors in glycated hemoglobin which can help in the conceptualization, planning and design of treatment policies for control and treatment of type 2 diabetes.

Abstract

Background: Apelin increases insulin insensitivity but its correlation with exercise training has less been taken in to consideration. The aim of this paper was to evaluation the effect of combined (resistance, endurance) and endurance training on plasma Apelin and in insulin sensitivity.

Methods: Forty -five volunteered students of eligible (BMI: 25-30,Health and inactive) were randomly divided into three groups, combined, endurance and control.The trainedgroups were trained forfour days per week, during 12 weeks. In combined group, endurance interval training was performed fromfivereplication with three minutebeginningand in final weeks reached to thirteen replications of 3 minutes with resting time of 1 to 2minutes. Intensity was 60-80% HRmax and resistance training planning: 2days per week,7 stations(Boat,leg press,buttreflys, Knee extension and flexion, forearm flexion and extension) 8-12 replication, resting time between the intervals and sets consequently:  30-60 ,60-90 second . The endurance group was trained with the same endurance section of combined group. Pre and post weeks, plasma Apelin,glucose and lipid profiles were measured in groups. Data were analyzed by One Way - Anova,significant level was P&le0/05, using SPSS, ver 18.

Results:  Plasma Apelin decreased significantly compared with the control group (P=0/0001).In training groups, HDL-c had in the training group increased compared with the control group (P= o/o43). Insulin Insensitivity wasn't different   among groups (P&ge0/05).

Conclusion: This paper showed that 12 weeks of combined or Endurance training had significantly decreased plasma Apelin and both training groups had an increase in their HDL-c but all of the exchanges were free of insulin sensitivity.

Abstract

Background: Type 2 diabetes is a major metabolic disorder. This study was aimed to evaluate the effect of probiotic soymilk fermented with Bifidobacterium lactis and containing omega-3 on hematologic, oxidative stress, anti-oxidant and inflammatory parameters in type 2 diabetic rats.

Methods: In this experimental manipulation study 65 rats were divided into five groups. Type 2 diabetes was induced by a single injection of streptozotocin and nicotinamide. Normal control (NC) and diabetic control (DC) rats received 1 mL/day of distilled water, and three groups of diabetic rats were administered 1 mL/day of soymilk (SM), fermented soymilk (FSM) and fermented soymilk fortified with omega-3 (FSM+omega-3) products by oral gavage for 28 days. Hematologic, oxidative stress, anti-oxidant and inflammatory parameters were measured at the end of experiment.

Results: At the end of experiment, in all treated diabetic rats compared with the DC group, red blood cell (RBC), hemoglobin (Hb), hematocrit and superoxide dismutase (SOD) concentrations increased significantly (p<0.05), and malondialdehyde (MDA) concentrations reduced significantly (p<0.05). The reduction of MDA and increasing in Hb, RBC, hematocrit and SOD concentrations in the FSM+omega-3 group was more than two other groups. In the FSM+omega-3 group, high sensitivity C-reactive protein (hs-CRP) concentrations compared with the DC and FSM groups were significantly lower (p<0.05).

Conclusion: The experiments suggest that combining soymilk, probiotics and omega-3 may be beneficial for the reducing oxidative-stress in type 2 diabetes.

Abstract

Background: Diabetes is such diseases that need high quality beside prevention such as correctly predict fluctuations in blood glucose levels. The main complications of the disease can be anesthesia, coma and even death. Today, in these patients, the correct dose of insulin determined based on experience or doctors knowledge, and interact between the patients and physician, although there is an inevitable human errors.

Methods: In this study based on applied method, 124 patients and 188 healthy subjects based on 12 features by Random Selection, Who had been referred to Research Center for Diabetic in Sabzevar university of Medical Science since 2006 to 2011 were studied. The proposed system has several subsystems, such as evolutionary algorithms (BPS 1) to select the most effective features, Data Mining Algorithms (SVM 2) to detect and classify the features from the non-effective features. Adaptive Neuro fuzzy systems (ANFIS 3) to estimate learn and adaptation in order to correctly predict have been used.

Results: In this study, we try to use artificial intelligence systems to determine the correct dose of insulin for diabetics. The proposed system combines the best attributes in the database in the form the interaction was able to achieve high accuracy with the lowest error. The proposed system based on best features in the database in the interaction form was able to achieve high accuracy with the lowest error. The proposed system in the form of composition and interaction with the subsystem was able to achieve carefully 84.1% in specificity, 91% in sensitivity and 92.9% in accuracy.

Conclusion: In this research, due to the importance of correct and timely determination of insulin for diabetics, a new method based on the combination of intelligent systems is presented. Thus, the results obtained in previous articles and studies provide significantly improved.

Abstract

Background: Neuropathic pain is associated with a profound impact on quality of life and daily activity in patients and caused high medical costs. Because of the nerves destructive nature in this disease, the neurotrophins are high regarded for their possible effects on survival, growth and development and neuronal functions as a possible therapeutic strategy. So the aim of this study is investigation of the chronic effects of decreased activity in neuropathic pain form on the NFG gene expression in the sciatic nerve of Spinal nerve ligated rats.

Methods: Ten adult male Wistar rats in the weight range of 250±30 gr randomly were divided into two groups including healthy control (C), ligation sciatic nerve ligation. Over the six weeks neuropathic pain behavior tests were conducted continually in groups. In the end change of NGF gene expression in sciatic nerve was measured with Real time PCR technique and calculated using the 2-&Delta&DeltaCT method.

Results: After 6 weeks, neuropathic pain behavior tests showed that pain threshold of thermal hyperalgesia and mechanical allodynia in the SNL group was significantly lower than the control group (P&le0.05). In addition, NGF gene expression in sciatic nerve ligation group compared to controls increased significantly (P&le0.05).

Conclusion: The findings of this study suggest that the development of neuropathic pain and decreased physical activity is associated with increased endogenous NGF. Although it is not clear that increase in NGF expression is due to the nature of neuropathic pain or nerve damage, it is clear that excessive expression of NGF is associated with neuropathic pain.

Abstract

Background: Diabetes is a metabolic disorder caused by insufficient production of insulin or insulin receptors deficiency. It is now the major cause of morbidity and hospitalization in patients with a significant financial burden to the society. The aim of this study was to evaluate the effect of Ziziphora ethanolic extract on active pancreatic beta cells on streptozotocin induced diabetic mice.

Methods: In this study, 36 mice were used and divided in to 6 group such as control group, diabetic control and experimental groups which were exposed to dose of 100, 150, 200 and 300mg/kg. STZ intraperitoneally at a dose of 70 mg / kg was administered. In order to determine the number of active pancreatic beta cells, the mice were anesthetized by diethyl ether after treatment with the extracts for 18days. The pancreases were removed from the mice and fixated in 4% formaldehyde afterwards, the pancreases were prepared for sectioning. Three-micron sections were prepared from the samples, and the number of active beta cells was evaluated by an immunocytochemistry kit.

Results: The results showed that the hydroalcoholic extracts of Ziziphora clinopodioides lam (100, 150, 200 and 300 mg/kg) increased the number of beta cells and insulin secretion in diabetic mice, compared to control diabetic mice. In other words, this plant could effectively increase the activity of beta cells in diabetic mice, compared to control mice. Therefore, Ziziphora clinopodioides lam could reduce serum glucose level in diabetic animals by increasing insulin secretion.

Conclusion: According to the results, the hydroalcoholic extract of Ziziphora clinopodioides lam had hypoglycemic effects in streptozotocin-diabetic mice by stimulating insulin secretion from pancreatic beta cells. Therefore, based on the obtained findings, application of this plant may be useful.
