Abstract

Social capital and altruism are the most effective factors on the cultural development of a society and can play an important role in promoting ethics within the health care system. The purpose of this study was to investigate the relationship between social capital and altruism and the ethical attitudes of physicians. This correlational and descriptive study was performed on 322 doctors working in medical centers in west Azerbaijan during 2013 who were selected by cluster-random sampling. Onyx and Bullen social capital scale, Carlo et al. altruism scale and Karamporian et al. ethics questionnaire were used to collect data. The results of this study indicated that there was a significant positive relationship between the ethical attitudes of physicians and altruism (R = 0.238), the structure subscale (R = 0.489), cognition subscale (R = 0.581) and relationship subscale (R = 0.554) of social capital. The significance level was set at p < 0.05 in this study. The multi-variable correlation coefficient and enter method indicated that social capital subscales can influence the ethical attitudes of physicians (0.457). The results showed that there was a correlation between social capital and altruism and ethical attitudes in the medical profession. Thus, it seems that promoting social capital and altruism can positively influence individuals’ ethical attitudes in the medical profession.

Abstract

The principal objective of the nursing profession is to provide evidence-based and competent care mainly based on humanitarian and ethical principles. Ethical care is contingent on a proper level of moral reasoning, which can be categorized into pre-conventional, conventional and post-conventional reasoning. At the pre-conventional level, individuals are mostly self-oriented and prefer obedience to avoid blame. At the conventional level people apply laws and social principles to decision-making, and at the post-conventional level they try to guide their actions and behaviors with regard to ethical principles and make humanist and ethical decisions. This study aimed to assess the level of moral reasoning in nurses and nursing students by providing an analysis of the existing literature on moral reasoning.For the purpose of this study, we conducted an extensive search of the papers published between 1980 and 2014 on international electronic databases including Scopus, Google scholar, Science Direct, PubMed, Proqust and Elsevier. We also reviewed papers published between 1985 and the autumn of 2014 on Persian electronic databases Sid, Magiran and Iran Medex. A broad range of search keywords were used such as: ethical growth, moral growth, ethical development, moral development, ethical reasoning, moral reasoning, nurse, and nursing student. In total, 35 studies were reviewed at this stage.Based on the results of the above-mentioned studies, most nursing students and nurses reason at the conventional and post-conventional level, and nursing students reason at a higher ethical level in comparison with nurses. We also found that teaching ethical concepts helps improve ethical reasoning, and that an inverse relationship exists between clinical experiences and ethical reasoning.Although in most studies, the level of moral reasoning in nursing students and nurses was found to be at the conventional and post-conventional level, this is not enough to provide superior professional care. It is therefore essential to apply the necessary measures such as improving clinical environments and the ethical education system to further promote the reasoning ability of nurses and nursing students, so that they can make their decisions based on ethical principles and at the post-conventional level

Abstract

The purpose of this research was to explore the components of nurses’ ethical conduct in public hospitals in Mashhad. This study employed an eclectic method for research and followed a mixed exploratory design. A qualitative study was first performed, and then based on the results the quantitative method was applied. The statistical population consisted of all the nurses in public hospitals in the city of Mashhad. In order to collect data, in-depth semi-structured interviews were conducted, and the data were then analyzed using thematic content analysis. A preliminary model was developed for the nurses' ethical conduct comprising three levels. The first level covered areas of ethical behavior toward patients, patients’ families and the core group. At the second level 13 dimensions and at the third, 51 components (themes) were abstracted. The questionnaire was then distributed among 176 experienced nurses employed in public hospitals in Mashhad. In order to assess the fit of the model in quantitative research, data were analyzed using confirmatory factor analysis and AMOS software. The results of the confirmatory factor analysis showed that all dimensions and components were acceptable with the exception of the component "patient education". Using AMOS software, the comparative fit index (CFI) was calculated at 0.9, which indicates a good fit of the model and validates the components and sub-components. The results of this study can guide human resource managers in public hospitals to promote ethical conduct in nurses.

Abstract

The cultural competence, responsibility and ethical beliefs are influential factors in providing quality services by nurses and pay an important role in patient's right. The purpose of this research was to illustarte a predictive model of patient's right based on cultural competence, responsibility and ethical beliefs. The research method was correlational study conducted in 2016 among 300 nurses in west Azerbaijan in 2016, selected by multistage cluster sampling. For gathering the data, the Perng and Watson’s nurses’ cultural competence questionnaire, and the Mergler and Shield responsibility questionnaire as well as the Mahmoudi and et al questionnaire on ethical beliefs and Scale rights of patients were used. The data were analyzed by using Pearson correlational coefficient and bootstrap through SPSS software and amos-22. The results indicated that the model was fitted and has direct effect in cultural competence (0.11), responsibility (0.57) and ethical beliefs (0.24) on nurse’s attitude with the rights of patients were significant. The indirect effects of cultural competence (0.03) and responsibility (0/03) with mediating of forming the model of ethical beliefs were significant. In conclusion, %65 of the variance in nurses’ attitudes to the patients' rights was determined by the variables of this research model. Due to the direct and indirect effects of cultural competence, responsibility and ethical beliefs on the attitudes of nurses towards patients' rights, it is important to increase cultural competence relationship and responsibility with nurses’ attitudes towards   patient’s rights by focusing on ethical beliefs.

Abstract

Today, the role of spiritual leaders is to stimulate employees by employing a spiritual perspective and creating cultural backgrounds based on human values to raise capable employees. On the other hand, professional ethics is one of the variables that has a significant impact on individual and group activities and outcomes of the organization. The purpose of this study was to investigate the relationship between spiritual leadership and professional ethics with organizational performance and quality of work life with the intermediate role of organizational learning among employees of Imam Reza Hospital in Urmia. The design of this research was a descriptive-correlational cross-sectional design model of structural equations. The statistical population of this research included 759 employees, 240 of them were selected by random sampling method. For data collection, standard questionnaires were used. Pearson Coefficient and Structural Equation Model Finding were used to analyze and examine the assumptions and the relation between the variants. The results show that there is a significant relationship between spiritual leadership variables and Professional Ethics with organizational performance and quality of life of employees (P<0.01, r=0.42, r=0.51), (P<0.01, r=0.36, r=0.75). Furthermore, the results showed that the intermediate role of organizational learning in the association between spiritual leadership and professional ethics with organizational performance and quality of work life (β = 0.17, β = 0.18, β = 0.16, β = 0.17) ) is confirmed. According to the findings, it can be concluded that paying attention to spiritual leadership and professional ethics can improve the organizational performance and quality of work life, as well as enhance organizational learning among the staff of Imam Reza Hospital.

Abstract

 Corona is a global crisis that has affected government systems. The transition from crisis to crisis requires prudent measures. Governance is a continuous process through which conflicting interests are harmonized and cooperative actions are taken. The latest theory in this concept is good governance, which is doubly important in times of crisis. Good governance is the product of the favorable participation of the three institutions of government, civil society and the private sector. The components of good governance are numerous from the perspective of the World Bank, the United Nations, and thinkers in this field. The National Anti-Corona Headquarters was established by the order of the Supreme Leader and based on the approval of the Supreme National Security Council in March 2020 to take appropriate decisions regarding the treatment of Covid-19 disease, including announcing closures and other restrictive measures in case of violation of health instructions. In this research, with a descriptive-analytical method, an attempt has been made to explain the components of good governance in this period according to the unique legislative approvals in this field, while determining the competence of the mentioned headquarters in regulating. From the authors' point of view, the approvals of the Corona National Headquarters as one of the sub-councils of the Supreme National Security Council are beyond the ordinary law and among the existing components; Efficiency and effectiveness, transparency and accountability and pivotal consensus were recognized.

Abstract

Compassionate care is considered one of the important elements of patient-centered and oriented care that gives health to the patient. Therefore, this study was conducted with the aim of designing and explaining the model of compassionate care of nurses based on moral identity and compassion for the lives of others. The present research method was done according to path analysis. The statistical sample included 250 patients with covid-19 and 250 nurses working in the corona department of hospitals in West Azerbaijan province in 2021, which were selected by available sampling method. Rodriguez's compassionate care questionnaire, Black and Reynolds' moral identity questionnaire, and Chang's scale of compassion for others' lives were used to collect data. The data were analyzed using Pearson, Bootstrap and Sobel tests and also through SPSS and Amos software program, version 24. The results showed that the variable of moral identify in interaction with the mediating role of compassion for the lives of others in explaining the compassionate care of nurses. A total of 0. 41 of the variance of compassionate care was explained through model variables. The direct effect of moral identity (0. 47), compassion for the lives of others (0. 36) was observed and it was significant in explaining the compassionate care model. Also, the result showed that the indirect effect of moral identity was found to be significant through the mediation of compassion and also towards the lives of others (2. 96). Considering the variable mediating effect of compassion towards the lives of others in the relationship between moral identity and compassionate care of nurses, it seems that the growth or strengthening of compassion towards the lives of others and moral identity can promote the compassionate care of nurses.

Abstract

Moral intelligence encompasses an individual's ability to discern right from wrong, possess ethical values, and demonstrate ethical behavior in practice. Clinical competence, a crucial aspect of nursing practice, encompasses a combination of knowledge, skills, and attitudes, including adherence to ethical principles. This systematic review aimed to investigate the relationship between moral intelligence and clinical competence among nurses and nursing students. A comprehensive literature search was conducted using keywords related to "moral intelligence," "clinical competence," "nurses," and "nursing students" in major international databases, including Web of Science Core Collection, PubMed/Medline, Scopus, and Google Scholar, as well as national databases such as Irandoc, SID, and Magiran. The search included publications in Persian and English with no time limitations. After removing duplicates and screening the initial 150 identified studies, five studies met the inclusion criteria. Ethical considerations, including minimizing bias in the selection, extraction, and analysis of evidence, were adhered to throughout the review process. The abstract adheres to PRISMA guidelines for reporting systematic reviews. The review revealed a significant positive correlation between moral intelligence and clinical competence across various dimensions. Studies demonstrated that higher levels of moral intelligence were associated with improved clinical competence, including ethical reasoning and clinical self-efficacy. Factors such as age, work experience, educational status, and individual rank were found to influence this relationship. Additionally, some studies indicated that moral intelligence could predict clinical competence scores, while others demonstrated that it could be a significant predictor of clinical competence alongside other factors such as Grade Point Average (GPA). This systematic review provides evidence for a significant positive relationship between moral intelligence and clinical competence among nurses and nursing students. Enhancing moral intelligence through targeted educational interventions can contribute to improved clinical competence and ultimately enhance the quality of patient care. Future research should focus on developing and evaluating educational programs designed to enhance moral intelligence in nursing education and practice.

Abstract

Death anxiety, characterized by persistent fear of death, is a significant psychological burden. Spiritual care, encompassing compassionate listening, fostering trust, and addressing spiritual and existential concerns, has the potential to alleviate this anxiety. This systematic review aimed to investigate the impact of spiritual care interventions on death anxiety in various patient populations. A comprehensive literature search was conducted using keywords such as "spiritual care," "death anxiety," "spirituality," and "patients" in major databases, including Web of Science Core Collection, PubMed/Medline, Scopus, Irandoc, SID, Magiran, and Google Scholar. The search included publications in English and Persian with no time limitations. After screening and removing duplicates, eight studies met the inclusion criteria. Ethical considerations, including minimizing bias in the selection, extraction, and analysis of evidence, were adhered to throughout the review process. The review followed the PRISMA guidelines for reporting systematic reviews. The findings suggest a potential positive impact of spiritual care on reducing death anxiety. Studies conducted among patients with chronic kidney disease, multiple sclerosis, and stroke demonstrated that spiritual care interventions, including individual counseling and group support, can effectively alleviate death anxiety and improve overall well-being. However, some studies, particularly those involving patients with cardiac problems and gastrointestinal cancer, did not show a significant reduction in death anxiety. Furthermore, research conducted during the COVID-19 pandemic highlighted the potential effectiveness of tele-nursing interventions for providing spiritual care and alleviating death anxiety in older adults. This review provides evidence suggesting that spiritual care interventions may be effective in reducing death anxiety in certain patient populations. Further research is warranted to investigate the effectiveness of different spiritual care modalities across diverse patient groups, including those with chronic illnesses, terminal diseases, and those facing end-of-life care.
