Abstract

Nursing advocacy is an essential aspect of nursing professionals' ethics that promotes patients' autonomy, safety, and rights. It seems that nurses' attitudes toward nursing advocacy can affect their role as patients' advocates and improve the nurse-patient relationship. This study aimed to investigate nurses' attitudes toward patient advocacy in educational hospitals in Kerman.The data for this descriptive-analytical study was collected using a self-administered questionnaire by quota sampling among 385 nurses from four teaching hospitals of Kerman and analyzed using the SPSS software including descriptive and inferential statistics. Overall the nurses' attitude toward nursing advocacy was relatively positive. There was a significant relationship among nurses' attitude and age, sex, nursing experience and participation in workshops, while we could find no significant relationship between nurses' level of education and their attitude. The nurses' attitude in mental hospitals was more appropriate than others, and the majority of participants were confident that they were suited for the advocacy role.The cognitive aspect of nurses' attitude was stronger than the behavioral aspect. Despite the lack of significant effect of education, more nursing experience led to more willingness to play the advocacy role. In psychiatric hospitals, nurses with more independence and stronger relationships with the patients became more familiar with the needs of patients so these nurses made more effort to defend them. It seems Iranian nurses have felt morally obligated to defend their patients for many years. In order to support nurses' advocacy role in our country, we believe it is necessary to reinforce this attitude in nurses through holding ethical workshops.

Abstract

A model is a summarized representation of facts. Health-related models show values, perceptions and various understandings of health care. This study investigated the nursing advocacy models and the nurses’ protective role. In this review article, related literature and documents were searched in PubMed, Science Direct, Proquest, Google Scholar, Magiran, Iran Medex, and Scientific Information Database, using the general search engine Google. The search was performed by keywords “advocacy” and “nursing advocacy model”. In this stage, 89 sources including books, articles and dissertations on the subject of nursing advocacy written between 1991 and 2014 were collected. In the next stage, 43 sources were chosen for the review article. Finally, 8 models that were more consistent with the topic and objectives of the study were extracted. Considering the findings of this study and the benefits of nursing advocacy, we propose to design a model in order to promote the advocacy role of nurses in Iran. In this model, legal constraints, social factors and values, organizational culture, position of the nurse in the health care system and political‌ and economic considerations should be taken into account.

Abstract

Patient advocacy is an inherent element of professional nursing ethics that helps to protect the patient’s rights. Many codes of ethics in nursing are concerned with this particular role of nurses. The present study aimed to evaluate the protective advocacy beliefs and actions of the nurses employed in educational hospitals affiliated to Birjand University of Medical Sciences during 2014. Data in this cross-sectional study were collected using a demographic characteristics questionnaire and the protective nursing advocacy scale. Participants included 248 nurses working in six hospitals located in Southern Khorasan province selected by randomized stratified sampling. The Cronbach’s alpha for Protective Nursing Advocacy Scale was 0.74. Data were analyzed in SPSS version 16 using descriptive and inferential statistical tests.Moderate levels of protective advocacy were observed in the majority of participants (80.6%), with a total mean score of 137.39  13.65. Some of the protective advocacy components had significant correlations with age, work experience, type of ward, employment status, and history of participation in ethics education programs (P < 0.05). In other words, the total mean score of advocacy and some of its components were significantly higher among nurses who had attended ethics education programs than those who had not (P < 0.05). Moreover, nurses who were older had more work experience, had worked in the emergency ward, and had a permanent employment status had higher scores of advocacy than others. The total mean score of advocacy among the nurses in this study was relatively appropriate, although strategies are needed to improve the score. It can also be assumed that nurses’ involvement in ethics education programs can increase the level of nursing advocacy. On the other hand, changing nurses’ temporary employment status to permanent status and providing job security for them may reinforce protective advocacy beliefs and actions in nurses.

Abstract

Patient Advocacy is one of the vital duties of nurses. The purpose of this study was to determine the relationship between spiritual intelligence and patient advocacy in educational hospitals in Khorramabad during 2017. This was a correlational descriptive-analytic study that was performed on 250 nurses working in educational hospitals in Khorramabad in 2017. The criteria for entering this study included a minimum degree of nursing and one year of clinical work experience. Participants in this study were selected by random sampling method. Data collection tools consisted of Hank’s advocacy Questionnaire and King's spiritual intelligence. Data were analyzed using descriptive statistics (mean, standard deviation) and inferential (correlation coefficients and regression coefficients) using SPSS v.21 software. Sixty seven percent (164 persons) were female and 37.5% (79 persons) were male. Sixty three percent of participants had a work experience of less than 5 years. The findings of this study showed the mean score of spiritual intelligence of nurses was 80.88 ± 11.19. The average score for patient advocacy was 155.73 ± 21.41. Pearson correlation coefficient showed that there was a significant correlation between spiritual intelligence score and Advocacy and its components (p = 0.624). The mean score of spiritual intelligence in nurses was high in the King questionnaire. The average score for patient advocacy was Hanks' questionnaire in the medium range. The present study showed that there was a significant positive correlation between spiritual intelligence and patient advocacy. As spiritual intelligence is acquired, spiritual intelligence can be promoted through classes and workshops, thereby increasing the patient advocacy and increasing the safety of patients.

Abstract

Patient advocacy is a cornerstone of ethical healthcare practice, encompassing actions that protect patient rights, uphold their autonomy, and ensure they receive high-quality care. Nurses are often seen as being in the best position to play this vital role. This conference abstract aims to provide a comprehensive understanding of patient advocacy and identify strategies for enhancing advocacy practices in healthcare settings. In this comprehensive review, scientific literature, including peer-reviewed articles, professional guidelines, and relevant policies, was retrieved and reviewed. Data extracted from the literature were synthesized using quantitative content analysis to identify different forms of patient advocacy, including clinical advocacy and systemic or workplace advocacy, as well as strategies to improve the necessary skills for each. Clinical advocacy involves ensuring patient safety, supporting informed decision-making, and advocating for appropriate pain management. Improving clinical advocacy requires a multifaceted approach that prioritizes building trust and rapport with patients to foster effective communication and shared decision-making. Encouraging patient participation in their care, supporting self-management strategies, fostering effective communication within the healthcare team, and staying informed about relevant ethical, legal, and policy developments are essential components. Workplace or systemic advocacy, on the other hand, focuses on addressing broader issues within the healthcare system that impact patient care. Improving systemic advocacy involves identifying and analyzing systemic issues, such as healthcare disparities, resource limitations, and policy barriers, through data collection, stakeholder engagement, and thorough analysis. Strategies to address these issues include policy advocacy, improved resource allocation, and quality improvement initiatives. Building strong partnerships with other healthcare professionals, community organizations, and policymakers is key. Additionally, promoting education and awareness within both the healthcare workforce and the public is crucial. Leveraging technology to enhance data collection, communication, and mobilization efforts can further support systemic advocacy initiatives. By implementing these strategies, healthcare professionals can effectively advocate for systemic change, improve the quality of care for all patients, and create a more equitable healthcare system. Patient advocacy is a dynamic and multifaceted role that requires continuous learning and adaptation. By embracing the principles outlined in this synthesis, nurses can effectively champion patient rights, improve the quality of care, and contribute to a more equitable healthcare system.
