Abstract

Background & Objective: Malnutrition is one of the main nutritional problems among children under 5 years especially in developing countries. The effects of malnutrition in this range of age group includes disorder of mental and physical growth and development, increase in the morbidity and mortality, decrease of the learning capacity. A detailed epidemiological picture of the prevalent malnutrition status among children under 5 years in different regions of Iran enables health policy makers to identify priorities, establish political commitment and design appropriate interventional programs to prevent and control malnutrition and improve nutritional status of children. To determine the prevalence rate of malnutrition among children under 5 years.
Methods: Demographic data, weight and height of 34200 Iranian children between 0-72 months old in 28 provinces of the country in 2005 year based on a cluster sampling were gathered. The subjects' weight and height were measured by trained staff. Prevalence of underweight, stunting, and wasting indicators was determined by measuring the weight and height of subjects in urban and rural areas. EPI6 statistical software, version b6/04, was used for all statistical analyses.
Results: 4.7% of the children aged less than five years suffered from stunting (95% CI: 4.5- 4.9%) The prevalence of this type of malnutrition among urban children is significantly less than rural children (95% CI: 3.1- 3.5% and 6.5-7.1% respectively). Meanwhile the prevalence of underweight was 5.2% in the country (95% CI: 5.1-5.4% while the prevalence of this type of malnutrition was significantly less among urban children than those in rural ones. Moreover, the prevalence of wasting was 3.7% (95%CI: 3.5-3.9%) and there was a significant difference in this regard between the urban and rural children. Thus this indicator was significantly higher among urban children than those in rural areas (95%CI: 3.8-4.3% and 3.0-3.5% respectively).
Conclusion: The results of the study show that the prevalence of protein-energy malnutrition was low among Iranian children under five years old. In comparison with previous studies, it is felt that there has been a prudent decrease in all malnutrition in all levels. However, there were significant differences in the prevalence of malnutrition in different provinces, which has resulted in the differences in their developmental stages as well. It appears that subjective strategies are required to improve the nutritional and health status among children under five in all provinces.

Abstract

Background and objectives: People with thalessemia and chronic renal failure on maintenance hemodialysis are prone to blood-born infections, especially hepatitis C due to the long-term transfusion. Recently, hepatitis C has been one of the main health concerns in these patients. The aim of this study was to determine the prevalence of hepatitis C and its risk factors in these groups of patients in Kerman province of Iran.
Methods: HCV RNA in blood sample of 384 patients (203 hemodialysis cases and 181 thalassemia cases) was evaluated.
Results: One hundred thirty (130) out of 384 were infected by HCV. Infected male was predominant (83%).
Conclusions: It seems that the frequency of hepatitis C infection in Kerman is higher than the other provinces of Iran. Therefore more attention should be paid to screen of blood before transfusing for these group of patients.

Abstract

Background and Objectives: Anaphylactic reaction is one of the most important transfusion anaphylactic reactions in medicine. During transfusion if Individuals with IgA deficiency take blood which containing IgA, they would suffer from severe allergic reaction. Since there is a little known about this in different ethnic groups in Iran we have decided to evaluate this issue in Sistan & Baluchestan province in Iran. Methods: During one year, blood samples were collected from 3837 volunteer blood donors. Then the serum samples were tested for IgA titer by nephlometry and SRID techniques.
Results: According to our results only, one donor (0.026%) was found to have IgA less than 5mg/dl and 34(0.9%) cases had IgA: 5-30 mg/dl while 3798(99%) had IgA more than 30mg/dl.
Conclusions: With regards to very low prevalence of IgA deficiency in blood donors in Southeast part of Iran, it seems that anaphylactic reactions most likely be due to other factors in the individuals that receive blood products.

Abstract

Background & Objectives: Cholera is always being considered as a public health threat in poor and developing countries. However outbreaks of cholera are not very common in central area of Iran in 2008 district health authority reported a cluster of diarrhea cases. We investigated this cluster to identify the etiological agent, source of transmission and propose control measures.
Methods: We analyzed the data of total of 1219 patients with colorectal cancer who registered between 1 January 2002 to 1 October 2007. Data were analyzed using univariate and multivariate Accelerated Failure Time (AFT) parametric survival model with frailty, utilizing STATA statistical software.
Results: In the univariate analysis for age at diagnosis, gender, marital status, race and education level, the survival of patients with colon cancer were approximately between half to one fourth and for BMI, alcohol history, Inflammatory Bowel Disease (IBD), familial history of cancer and the pathologic stage of tumor, the survival of patients with colon cancer were significantly (between 0.12 to 0.56 times) shorter than those patients with rectal cancer. In the multivariate analysis, for age at diagnosis (45-65 years), there was significant difference between colon and rectum cancer. But for BMI, alcohol history, IBD and pathologic stage there were not significant differences. The adjusted survival and 1, 2, 3, 4 and 5 year survival of patients with rectal cancer were better than those with colon cancer.
Conclusions: Site-specific evaluation of colon and rectum could give a better perspective of factors affecting these cancers. It may help to design of clinical trials, better diagnosis of diseases and optimal administration of specific treatments.

Abstract

Background & Objectives: Prevention and control of brucellosis in animals is the main route of its prevention in human. After detecting a brucellosis outbreak in an industrial dairy cattle farm in Isfahan province in Iran, an epidemiological investigation was carried out to determine prevalence of seropositive cows and incidence rate inorder to eliminate positive couws.
 Methods: This study was carried out to determine abortion rates in caws. RBPT, SAT and 2ME serologic tests were used for detecting infected cows and Brucella agar for isolation of bacteria. Data analysis was performed by Chi-square test and calculation of cumulative incidence ratio.
Results: Of total 1395 female cows above 4 months age, 706 infected animals were found during investigation (March-December 2011). Incidence of abortion in infected cows was significantly higher than no infected (34% versus 14%, P<0.05). Culture for 47 milk samples was positive for brucella out of 100 milk samples.
Conclusion: Attention to using reduce dose of RB51 vaccine, controlling of import animal in farm besides biosecurity are the main factors for prevention of similar outbreaks in dairy farms.

Abstract

Background & Objectives: Chronic pain is one of main public and individual health problems and its epidemiological understanding needs reliable estimates of prevalence. The aim of this study was to investigate the epidemiology of chronic pain in all 368 neighborhoods of Tehran using small area estimation method.
Methods: The pain section from the second round of Urban HEART data from a selected individual of 23457 households in Tehran using a multistage randomized cluster sampling in 2011, were analyzed. In order to obtain reliable estimates for chronic pain prevalence at neighborhood level, a generalized linear mixed model and hierarchical Bayesian approach were used and the reliability of the estimates were evaluated.
Results: The average of estimated prevalence of chronic pain in neighborhoods of Tehran was 25.5% and a large heterogeneity was observed in its prevalence in neighborhoods of Tehran. Prevalence of chronic pain was significantly higher in married housewives, retirees and pensioners and was significantly associated with age, educational status, depression and anxiety (P<0.05). The reliability of Bayesian method was confirmed by evaluation methods in this analysis.
Conclusion: These results demonstrate prevailing amount of chronic pain at neighborhood-level in Tehran, which warrants careful attention to prevention, treatment, and rehabilitation by health care professionals.

Abstract

  Background and Objectives : I In the classification proposed by Holmes and colleague, the stress of divorce has been identified as the greatest one among all types of stresses in the world. This global stress is on the rise in Iran so that Iran is the fourth country in this regard. The purpose of this study was to present a comprehensive and systematic model of the factors affecting divorce in the country.

 Methods: The method used in this study was qualitative and a type of meta-study known as meta-synthesis. The study population consisted of all relevant papers with research topics. In order to perform the research, after designing research questions, a systematic search was carried out based on relevant key words (divorce, spousal conflict, separation) from the databases Science Direct, IEEE, Emerald, ISC, IRANDOC, SID, Civilica, Scopus and also Google Scholar between 1365 to early 1392 .

  Results : By reviewing 61 articles out of 8477 primary ones, both foreign and domestic, researchers identified 15 dimensions and 64 components as the factors affecting divorce. Based on these factors, the final integrated model was presented and validated.

  Conclusion: The results of this study can be used as a basis for planning pre-marriage counseling programs by identifying factors affecting divorce on the basis of all available resources in the years 1365 to 1392.

Abstract

Background and Objectives: The effect of heart diseases on the quality of life is the issue needs attention of health care providers. Improving quality of life is considered as the goals of rehabilitative therapies. This study conducted to evaluate the McGill Quality of Life Questionnaire in patients with heart diseases.
Methods: In this cross-sectional study, 500 patients with heart diseases were recruited from BooAli Sina Hospital and Velayat Hospital affiliated with Qazvin University of Medical Sciences from May to August 2016. The participants completed the McGill Quality of Life Questionnaire. The construct validity (including convergent and discriminant validity) and Reliability using the Cronbach’s alpha, theta, and McDonald's Omega of the McGill Quality of Life Questionnaire were evaluated. The structure of the Questionnaire was assessed using factor analysis.
Results: Three factors, including overall view of the quality of life, physical aspect, and psychological dimension, were extracted. Model fit indexes confirmed a good fit of he McGill Quality of Life Questionnaire (Comparative of Fit Index: CFI=.918, incremental fit index: IFI=.919, Adjusted Goodness of Fit Index: AGFI=.844, RMSEA=.079, Minimum Discrepancy Function by Degrees of Freedom divided: CMIN/DF=2.97, Parsimonious Normed Fit Index: PNFI=.681, Parsimonious Comparative Fit Index: PCFI=.709). Convergent and divergent validity, internal consistency, and construct reliability of the questionnaire were confirmed.
Conclusion: The findings revealed that the three-factor model of the McGill Quality of Life Questionnaire has satisfactory validity and reliability. Thus, this questionnaire can be used in future studies to assess the quality of life of patients with heart diseases.

Abstract

Background and Objectives: Analysis of ordinal data outcomes could lead to bias estimates and large variance in sparse one. The objective of this study is to compare parameter estimates of an ordinal regression model under maximum likelihood and Bayesian framework with generalized Gibbs sampling. The models were used to analyze ovarian hyperstimulation syndrome data.
 
Methods: This study used the data from 138 patients of a clinical trial phase III to compare the efficacy of intravenous Albumin and Cabergoline in prevention of ovarian hyperstimulation syndrome. The original study was done between 2010 to 2011 in Royan institute. We compared maximum likelihood and Bayesian estimation with generalized Gibbs sampling for an ordinal regression model based on confidence intervals and standard errors. The model were fit through R 3.3.2 software version.
 
Results: Markov Chain Monte Carlo results reduced the standard errors for estimates and consequently, narrower confidence intervals. Autocorrelations for generalized Gibbs sampler reached to zero in compare to standard Gibbs sampler for shorter time.
 
Conclusion: It seems that confidence intervals of an ordinal regression model are shorter for generalized Gibbs sampler in compare to standard Gibbs and maximum likelihood. It suggests doing more studies to warrant the results.

Abstract

Background and Objectives: The aim of this study was to determine the predictability of the PLS-SEM model for injuries resulting in hospitalization in motorcyclists using a mediator variable in a case-control study.
 
Methods: In this case-control study, 300 cases and 156 controls were randomly selected from 150 clusters using random cluster sampling. The data were collected using the motorcycle riding behavior (MRB) questionnaire, adult attention deficit hyperactivity disorder (ADHD) questionnaire (subscales) and a checklist containing motorcycle related variables.
 
Results: The motorcycle riding behavior, adult ADHD, motorcycling related variables and some demographic variables were found to be the predictors of injury. There were significant positive relationships between injury and motorcycling related variables (B=0.20, P=0.001) and ADHD (B=0.33, P=0.001), between MRB and motorcycling related variables (B=0.51, P=0.001) and ADHD (B=0.52, P=0.001), and between ADHD and motorcycling related variables (B=0.39, P=0.001).
 
Conclusion: Considering the more accurate results of PLS-SEM, the intervention programs should especially address those who have hyperactive children, those who use the cellphone while riding, and those who ride in dark hours of the night.

Abstract

Background and Objectives: Progressive muscle relaxation (PMR) training as part of an educational program for patients involved in pathologic and physiological events (PPE) can have important physiological and psychological benefits for the patients, and can affect various dimensions of their lives including the quality of life. This systematic review and meta-analysis was conducted to determine the effect of PMR on the quality of life of patients involved in PPE.
 
Methods: Through searching the words “PMR” and “quality of life (QOL)” in Persian databases including SID, MagIran, IranMedex, and IranDoc and international databases including Google Scholar, Scopus, PubMed, Web of Science, ProQuest, and ScienceDirect, all studies published on the effect of PMR on the QOL were extracted. The methodological quality of the papers was examined using Cochrane risk of bias. Data analysis was carried out using a random-effects model and heterogeneity was assessed by I2. The data were analyzed using the STATA software version 14.0.
 
Results: Of 495 studies, only 10 explored the effect of PMR on the QOL of patients. The pooled mean of the QOL and all dimensions were statistically significant, including physical functioning (0.339), physical role limitation (0.378), bodily pain (0.341), general health (0.598), social functioning (0.873), mental health (0.736), emotional role limitation (0.791), vitality (0.706), total physical dimensions (0.652), total mental dimensions (1.316), and total QOF score (0.480). Only one study had a low risk of bias.
 
Conclusion: Implementation of PMR has a significant effect on the quality of life of patients involved in PPE. PMR is recommended to improve the QOL of the patients.

Abstract

Background and Objectives: The main purpose of this study was to compare the predictive power of the Inequality-adjusted Human Development Index (IHDI) with the Human Development Index (HDI) with regard to the share of deaths caused by Non-Communicable Diseases (NCD) among all deaths in the world and Iran.
 
Methods: The data required for this cross-sectional ecological study were extracted from the reports of the United Nations Human Development Program and the WHO in 2015. Pearson correlation test was used to investigate the correlation of HDI and IHDI with the share of deaths caused by NCDs and linear regressions models were used to determine the associations of IHDI and HDI with the dependent variable.
 
Results: At a significant level of P<0.01, the dependent variable showed a strong positive correlation with HDI (0.892) and IHDI (0.899). Simple linear regression showed that HDI alone predicted the dependent variable well (Adj.R2=0.794, P<0.001).However, according to the multivariate linear regression model, when IHDI and HDI were included in the model, IHDI was able to predict the dependent variable well (Adj.R2=0.809, P=0.001), while the relationship between HDI and the dependent variable was no longer significant.
 
Conclusion: Although HDI alone is an important predictor of NCD status, it loses its influence in the presence of IHDI. Therefore, in addition to HDI, IHDI that illustrates the impact of inequality on human development can provide more information on the status of deaths caused by NCDs.

Abstract

 
Background and Objectives: Understanding and analyzing the socio-economic factors affecting mental health is important for mental health policy-making in metropolitan areas. The aim of this study was to investigate the relationship between socio-economic factors and mental health of households living in Tehran.
 
Methods: This cross-sectional descriptive-analytical study was conducted on 650 households from 22 districts of Tehran using mixed sampling method. Mental health measures were collected using the standard SF-36 questionnaire and socioeconomic measures of the household were collected using a researcher-made questionnaire. The face validity of the questionnaire was measured by qualitative method and its content validity was confirmed by quantitative method. The test-retest method was used to test the questionnaire reliability. Regression models were estimated using ordinary least squares (OLS) and weighted least squares (WLS) methods. The Stata software was used for data analysis.
 
Results: Among the socioeconomic variables, four variables including years of education, health behaviors (regular physical activity, healthy eating habits), not-smoking, and social capital had a positive and significant relationship with all mental health measures (social role, emotional role, mental health and vitality ). The relationship between not-smoking and all mental health measures was more than other socioeconomic variables (P <0.05).
 
Conclusion: Mental health policymakers should place extra emphasis on the development of social capital in metropolitan cities, promotion of the citizens' health behaviors, and particularly on tobacco use and the related behavioral disorders.

Abstract

 
Background and Objectives: Accurate assessment of patients' death depression requires a specific tool that is appropriate to the culture of each community. Therefore, the aim of this study was to evaluate the psychometric properties of the Death Depression Scale in women with breast cancer.
 
Methods: In this methodological study, 246 breast cancer patients presenting to Qazvin Provincial Hospital completed the 17-item Templar Death Depression Scale. Psychometric properties of this scale were evaluated by exploratory and confirmatory factor analysis. Reliability was also assessed using Cronbach's alpha, omega and composite reliability.
 
Results: Exploratory factor analysis showed that six items in the Persian version did not acquire the required loading factor, and were omitted. The remaining eleven items as a single factor accounted for 66.91% of the variance of this concept. In confirmatory factor analysis, 11 items had appropriate fit indices (χ2 = 97.664, p <.001, χ2 / df = 2.504, GFI = .932, CFI = .972, IFI = .973, TLI = .961, SRMR = .030, and RMSEA = .079). A Cronbach's alpha coefficient of 0.95, Omega coefficient of 0.95, and composite reliability coefficient of 0.96 indicated the acceptable reliability of the 11-item version.
 
Conclusion: The results of this study showed that the 11-item Death Depression Scale was valid and reliable in Iranian patients with breast cancer. Given the appropriate psychometric parameters, this scale can be used to assess death depression in future studies.

Abstract

Background and Objectives: Colorectal cancer is the second and fourth most common cancer in Iranian women and men, respectively. This cancer ranks fourth and fifth in men and women, respectively, among Yazd population. This study aimed to estimate the burden of colorectal cancer in Yazd province in 2016.
Methods: This descriptive cross-sectional study was performed on the population of Yazd province in 2016. CanMod software was used to calculate the burden of colorectal cancer. The required information included the population of Yazd province, the incidence of colorectal cancer, the death rate due to colorectal cancer, the death rate due to all causes and the survival rate of patients; The Cancer Registration System, the Provincial Death Registration System and the opinions of provincial experts were collected and included, respectively.
Results: The total burden of colorectal cancer in Yazd province was 632 years (382 years in men and 250 years in women); The YLL and YLD of colorectal cancer in both genders were 478 years and 154 years, respectively. The burden of colorectal cancer in men was 65 years and in women 45 years (per 100,000 population). The peak age of disease burden was estimated in the age group over 60.
Conclusion: Due to the high incidence and treatment in the early stages of the disease, screening and diagnosis in the early stages can be one of the health priorities of the province. However, the final judgment is made when the burden of other diseases and injuries is calculated in the province.

Abstract

Background and Objectives: Fear of COVID-19 is common among older adults and negatively impacts on their physical and mental health. Self-regulation is an essential factor for controlling or managing excitement and anxiety. So, this study aimed to determine the relationship between fear of COVID-19 and death anxiety with the moderator role of self-regulation among the elderly residing in Qazvin.
Methods: This descriptive and cross-sectional study was performed on 430 elderly aged 60 and over living in Qazvin in 2021. Samples were selected by cluster sampling method. The demographic checklist, Templer death anxiety scale, fear of COVID-19 questionnaire, and Self-Regulation Inventory were used for collecting the data. The data were analyzed using the Path Analyze test.
Results: The mean age of the elder participants in the study was 66.88 ± 6.40 years old. The results showed a significant and positive relationship between fear of COVID-19 and death anxiety (β=0.60, P<0.001) and an inverse association between fear of COVID-19 (β=-0.24, P=0.021) and death anxiety (β=-0.10, P=0.021) with self-regulation. Furthermore, the results of the Path analysis confirmed the moderator role of self-regulation in the association between fear of Covid-19 and death anxiety among the elderly residing in Qazvin, Iran.
Conclusion: Given the mediator role of self-regulation in the relationship between fear of COVID-19 and death anxiety, it is suggested to use teaching strategies to the older adults to improve self-regulation to control and mitigate the negative impacts of the fear of COVID-19 in this vulnerable population.
