Abstract

Background & Objectives: Despite decreasing the trend of coronary artery diseases in developed countries and outstanding improvements in clinical management of these patients, case fatality rate after an acute myocardial infarction (AMI) remains high in both genders. Identifying predicting factors of short-term survival in patients with AMI may play an important role in reducing mortality in these patients.
Methods: In this cohort study, all patients with acute myocardial infarction (AMI) admitted to all hospitals in Isfahan, Iran, during 2000-2008 which registered in Isfahan cardiovascular research Institute were analyzed. We used Cox regression models, uni- and multi-variable analysis. 
Results: Within the study period, 8800 AMI patients (73.6% male) were admitted with mean age of 61.85±12.5, and overall 28-day survival of 90.5%. Relative risk (RR) of death for 50-70 years old patients was 2.5 (CI:2-3.1), for over 70 years old RR=5 (CI:4-6.3), for women RR=1.7 (CI:1.5-1.9), for patients who had not received streptokinase RR=0.9 (CI:0.8-1.1), for inferior MI RR=4.2 (CI:2.2-7.8) and for anterior MI, RR was equal to 7.2 (CI:4-13.3).
Conclusion: Recognizing the predicting factors of short-term survival of AMI patients may help health professionals to provide better healthcare services for more at risk patients, i.e. elderly, women and patients with an anterior MI.

Abstract

Background and Objectives: Social capital consists of individuals' communicational networks, social norms such as mutual trust and cooperation in social networks. The aim of this study was to develop a model to assess the implication of different determinants such as age, gender, occupational status, mental and physical health on social capital components to draw a correlation network for social capital determinants.
Methods: For the purpose of this study, data was used from ‘social capital' section of Urban HEART-1 survey, which included 22,500 households from all 22 districts of Tehran, who were approached in a randomized multistage cluster sampling method. Path analysis is a statistical method to test hypothetical causal models, which requires various causal (path) diagrams. To demonstrate the causal models of social capital, the hypothetical paths of various components were developed and the final model of social capital was drawn using multiple regression analyses.
Results: Path analysis indicated that social capital components are influenced by various variables: A) Individual trust, by occupational status, marital status, and physical component of health-related quality of life B) Cohesion and social support, by education, age, and marital status C) Collective trust and associative relation, by family size, age and physical health. Direct effect of these variables on social capital components was more than their indirect effects (through mental health and physical health).
Conclusion: Social capital components are directly affected by occupational, marital, educational status, family size, physical health and duration of local residency. Planning to improve educational and occupational status, strengthening family bonds and provision of local facilities, may improve social capital.

Abstract

Background & Objectives: Chronic pain is one of main public and individual health problems and its epidemiological understanding needs reliable estimates of prevalence. The aim of this study was to investigate the epidemiology of chronic pain in all 368 neighborhoods of Tehran using small area estimation method.
Methods: The pain section from the second round of Urban HEART data from a selected individual of 23457 households in Tehran using a multistage randomized cluster sampling in 2011, were analyzed. In order to obtain reliable estimates for chronic pain prevalence at neighborhood level, a generalized linear mixed model and hierarchical Bayesian approach were used and the reliability of the estimates were evaluated.
Results: The average of estimated prevalence of chronic pain in neighborhoods of Tehran was 25.5% and a large heterogeneity was observed in its prevalence in neighborhoods of Tehran. Prevalence of chronic pain was significantly higher in married housewives, retirees and pensioners and was significantly associated with age, educational status, depression and anxiety (P<0.05). The reliability of Bayesian method was confirmed by evaluation methods in this analysis.
Conclusion: These results demonstrate prevailing amount of chronic pain at neighborhood-level in Tehran, which warrants careful attention to prevention, treatment, and rehabilitation by health care professionals.

Abstract

Background & Objectives: The recent researches on mortality in Iran in the past 3 decades have obviously emphasized cardiovascular diseases, unintentional injuries, and neoplasms as the most important causes of shortening Iranian’s lives. We investigated the role of these major causes on the mortality patterns in Fars in 2006.
 Methods: The number of registered deaths by age and sex and the distribution of death by cause in Fars in 2006 were derived from the Death Register System of Ministry of Health (MoH). We used the 2006 census to obtain the information on the population exposed to death. The mortality of the people over 5 years of age was estimated and corrected using the Brass Growth Balance method. The mortality rate of the children under the age of 5 was estimated by indirect methods and the data were analyzed using the multi-decrement life table.
 Results: Life expectancy was 70.3 years in Fars, about 69 years in males and 72 years in females, in 2006. The results showed that 3 main causes of death were responsible for reducing life expectancy of about 11 years in men and women in Fars. Detailed results revealed that cardiovascular diseases and unintentional injuries played more important roles in the decrease. Unintentional injuries plays more major role in decreasing life expectancy in males than females.
Conclusion: Special attention should be paid to the risk factors of cardiovascular diseases in both genders and unintentional injuries in males in Fars to reduce the mortality level and increase the life expectancy.

Abstract

  Background & Objectives: Breast cancer is the most common cancer among women. Numerous factors are effective in the improvement of patients undergoing surgery in terms of preventing relapse or need for repeated treatment during 6 months after the surgery. This study aimed to determine the factors affecting improvement of patients undergoing surgery for breast cancer using a logistic regression model.

  Methods: In this study information of 150 patients were extracted from the surgical records of patients underwent surgery at the Cancer Institute of Tehran Imam Khomeini Hospital. Data were analyzed through SPSS software and using logistic regression models .

  Results: Of 150 studied patients, 18 patients (12%) were not fully recovered while 132 (88%) had complete remission. The mean age of the patients was 43 ± 7.5 years, 92.8% were married, and 87.3% had children. According to the results, the odds of non recovery increased by 1.26 times with each one-year increase in age, 5.3 times with each one lymph node involvement, and 8.67 times in the case of metastasis. Other variables, such as marital status, number of children, tumor size, and location of the metastasis showed no significant relationship.

  Conclusion : The Results showed that age at diagnosis, due to its relationship with metastases and number of involved lymph nodes can be directly or indirectly affect the outcome. Therefore, increasing the women's awareness about breast cancer seems to be necessary for early diagnosis which requires planning a sufficient screening program by health policy makers of the country.

Abstract

  Background & Objectives : Health risk behaviors in adolescence, such as alcohol consumption and smoking, can affect both short-term and long-term health. The aim of study was to estimate the prevalence of health risk behaviors among high school students of Qazvin.

  Methods : The target population of this descriptive study was all high school students of Qazvin who were enrolled in the 1390-91 academic year. The sample group included 160) 239 girls, 271 boys) students that were selected by randomly multistage sampling. The assessment instrument was the Youth Risk Behaviors Surveillance System (YRBSS) Questionnaire.

  Results: Findings indicated that use of hookah was the most popular in girls and boys (59%) as compared with cigarette smoking (32.7%), drug use (7.3%), and alcohol consumption (16.7%). The prevalence of physical violence in and outside the school in one year, carrying weapons to school in a month, cigarette smoking in the school in one month, smoking hookah, and alcohol and opioid use during one month were more frequently observed in boys than girls. The use of the seat belt was more common among girls than boys, and the total prevalence of using the seat belt was 23.7%.

  Conclusion : Given the high prevalence of high risk behaviors among high school students, it is necessary to develop a high risk behaviors surveillance system and use the obtained information to devise preventive measures at the individual, school, and family levels.

Abstract

Background & Objectives: Measuring the impact of various diseases on Life Expectancy( LE) is an important step toward prioritization in health. The present study was conducted to measure the impact of heart diseases, neoplasm, and respiratory diseases on life expectancy (LE) in 2010.

Methods: Data on death and population for all 22 districts of Tehran were obtained from the main cemetery of Tehran and statistical center of Iran, respectively. Age-specific mortality rates and consequently LE were calculated for all 22 districts and both genders. Finally, the death probability assuming complete elimination of the diseases was calculated and the resulting life tables were obtained.

Results: The LE at birth was estimated 74.6 and 78.4 years for total males and females in Tehran, respectively. The maximum and minimum LE at birth was 80 years in females and 72.7 years in males, respectively. Assuming complete elimination of heart diseases, the LE increased to 82.39 and 85.51 years in males and females, respectively while complete elimination of neoplasm resulted in an increase in LE to 76.27 years in men and 80.49 years in women. Finally, elimination of respiratory diseases increased the LE of men to 75.98 years and the LE of women to 79.97 years.

Conclusion: The results indicated the high impact of the diseases on LE, especially the heart diseases. As a main result, LE will upgrade to more focus on this category.

Abstract

Background and Objectives: Epidemiology is a tool and a field of science which provides evidences necessary to make decisions. It is known as one of the basic sciences of public health. This study aimed to identify strategies for the promotion of epidemiology by Iranian Epidemiological Association so that people graduated in this field would become able to play an effective and positive role in different parts of the country.

Methods: This study was conducted in 2015. The views and opinions of the members of Iranian Epidemiological Association were collected by email. The views of the members were classified using qualitative methods.

Results: The results of this study helped to identify some of the strategies for the promotion of the position of epidemiology in Iran’s health system by the members of Iranian Epidemiological Association.

Conclusion: Iranian Epidemiological Association can play a more effective role in the promotion of epidemiology in the health system. Effective communication with the members, holding group meetings, advocacy and negotiation, reporting the activities of the Association continuously, identifying national problems and seeking solutions are some of the strategies that can help to enhance the role of the Association and promote the position of epidemiologists in Iran.

Abstract

Background and Objectives: Reproductive factors are in close relationship with breast carcinogenesis. This matched case-control study was conducted to study the association of reproductive risk factors with the risk of breast cancer (BC) among women in Tehran and northwest of Iran.

Methods: This hospital-based case-control study was performed on a total of 432 patients diagnosed with BC with confirmed histopathology who were recruited from hospitals affiliated with Shahid Beheshti and Tabriz University of Medical Sciences between 2007 and 2012, and 543 regionally matched controls without a prior history of BC.

Results: The average number of pregnancy and breast-fed children were significantly higher in cases than controls (P<0.01). The duration of breast-feeding was longer in patients (18.0±8.4 months) than controls (16.0±9.1; P<0.001). Most of the patients were diagnosed with BC above the age of 48 years old which was higher than the mean age of the recruited matched controls. Older age was associated with a 3.87 higher risks of BC development (95% CI: 2.94-5.10). The higher frequency of lactation in patients was significantly associated with the elevated risk (OR=2.22, 95% CI 1.62-3.04). The duration of breast-feeding within14-24 months was correlated with OR=0.52 to protect BC development during the reproductive age
(<48 years) (95% CI: 0.32-0.86). 

Conclusion: High frequency of pregnancy, lactation, and older ages at first pregnancy are associated with the risk of BC, while older age at puberty, menopause, and longer duration of breast-feeding (age above 48 years) have inverse associations with the risk of BC.

Abstract

Background and objective: Epidemiology is a tool and a field of Medical sciences  which provides evidences necessary to make scientific decisions.  This study aimed to identify strategies for the promotion of the epidemiology by Iranian Epidemiological Association (IrEA) so that people graduated in this field would become able to play an effective and positive role in different parts of the country.

Methods: This study was conducted in 2015. The views and opinions of the members of IrEA were collected by email. The views of the members  were classified using qualitative methods.

Abstract

Background and Objectives: The presence of biological aerosols in the air of medical centers is more than other enclosed spaces. Determining the role and importance of such factors has always attracted the attention of health and medical researchers. This study aimed to evaluate the bacterial density of surgical and infectious wards in two important hospitals of Kermanshah and environmental factors affecting them.
Methods: In this study, a total of 160 samples were selected. One hundred and twenty eight samples were related to the air of surgical and infectious wards in two stages before and after disinfection of wards and 32 samples were related to the hospital outdoor air as the control sample. MCS Flite (SKC) air sampling pump was equipped with a Biostage 225 (SKC) microbial sampling cassette made in the USA. Data were analyzed with SPSS 19.
Results: The highest bacterial density in the air was measured in Imam Khomeini Hospital (402.7± 200.3 CFU/m3) and the lowest was in Imam Reza Hospital (258.6 ± 90.5 CFU/m3). There was a significant inverse relationship between air temperature and total bacterial density and a significant direct relationship between the whole population and bacterial density.
Conclusion: It seems that reduced bacterial density in wards’ air with increased temperature occurs due to increasing the air volume and replacing air in wards. The presence of people in the ward plays an important role in increased bacterial density.

Abstract

Background and Objectives: Accidents are one of the global concerns in public health. About 1.24 million people die in traffic accidents annually. Road accidents are the leading cause of years of life lost in Iran and motorcyclists are the riskiest group of road users. Drug abuse is one of the main causes of road traffic accidents and motorcycling is more popular in drug abusers. So, this study was conducted to compare the riding behavior of normal and drug abuser motorcyclists.
 
Methods: This historical cohort was performed in 411 motorcyclists in 3 study groups: 100 methadone consumer motorcyclists, 100 methamphetamine consumer motorcyclists, and 211 normal motorcyclists. The participants in three groups were matched for age and area of residence. A motorcycle riding behavior questionnaire was used to collect the data. Then, the collected data were analyzed with the SPSS (V-20) software using descriptive and analytic statistics. Multivariate linear regression was applied for statistical analysis.
 
Results: There was a significant difference in the riding behavior score between normal motorcyclists and methamphetamine user and methadone user motorcyclists (P<0.0001), but the difference between drug user motorcyclists was not significant (P=0.292). The most frequent violation in all three groups was speeding.
 
Conclusion: the results showed speeding was the most frequent violation in all three groups followed by traffic errors. It was found that drug abuse (methamphetamine and methadone) could have an impact on motorcyclists’ riding behaviors as it could increase the riding behavior score in drug user motorcyclists compared to normal motorcyclists.

Abstract

Background and Objectives: Cholera is one of the communicable diseases that should be reported immediately as a public health threat. This study was conducted to study the probable risk factors of cholera outbreak in Qom, central Iran, during 2017.
 
Methods: In a case-control study, 37 cholera patients diagnosed based on para - clinical tests and 37 control samples were evaluated. Charts, frequency tables, regression logistic, Chi-square and t-test in SPSS software ver.25 were used for data analysis.
 
Results: The causative agent in the Qom cholera outbreak was Vibrio cholerae serotype Inaba. Most of the patients (54%) were in the age range 21-40 years. Most of the cholera cases were males (87%), Iranians (81%), and school or college students (30%). The peak of the epidemic occurred in the third week of November 2017, coinciding with a religious event. Nearly half of the patients were identified at the cross-border surveillance centers. The most important risk factors for the outbreak were a history of travel to Iraq in order to attend the Arbaeen religious event (95%), (OR=75, P-value<0.001), and a history of consuming unreliable foods and water (94% and 50%, (OR=66, CI=8-410, P-value=0.00 and OR=11, CI=2.7-46)), respectively. 
 
Conclusion: Cross-border surveillance of cholera in common borders with Iraq, especially in the Arbaeen religious event, played a vital role in identifying patients suspected of cholera. The surveillance of communicable diseases should be strengthened when entering and leaving the Arbaeen event.

Abstract

Background and Objectives: During the COVID-19 pandemic, vaccination was crucial in preventing the spread of SARS-CoV-2 and saving numerous lives. Countries implementing COVID-19 vaccination programs have reported significant reductions in cases, ICU admissions, and COVID-19-related deaths. This study aimed to evaluate the effectiveness of vaccines used in Hamadan province, explicitly focusing on their impact on hospitalization and death caused by COVID-19.
Methods: A test-negative case-control design (TND) was conducted involving patients aged 12 and above who were admitted to hospitals in Hamadan province, Iran, and had symptoms of acute respiratory diseases. Data were extracted from hospital and health system databases. Multiple logistic regression analysis was performed to estimate vaccine effectiveness for the first, second, and reminder doses in prevention of hospitalization, and severe outcomes (ICU admission or death).
Results: The study was conducted on 3,702 patients, and the maximum effectiveness of vaccines against hospitalization was 50% for patients who received a booster dose. The effectiveness of the first dose of vaccine on severe outcomes (admission to ICU or death) was estimated as 42%, but the effectiveness of the vaccines in the second and booster doses were not significant.
Conclusion: Despite the predominant use of inactivated virus vaccines and delayed initiation of vaccination in Iran, this study shows the effect of vaccination on reducing hospitalization and improving the outcomes of COVID-19. The use of more effective vaccines at a more appropriate time plays an important role in reducing the burden on health services and preventing further transmission in future epidemics.
