Normal 0 false false false EN-US X-NONE AR-SA Background & Objectives: Tuberculosis (TB) is an important issue which its control is still unsatisfactory at global level. Traditional diagnostic techniques for active TB diagnosis are inadequate: the diagnostic gold standard is the cultural exam which suffers from lengthy processing and requires highly specialized laboratories. Nowadays more specific tests have been recommended. The aim of this study is to evaluate the performance of Quanti FERON-TB (QFT)Gold In Tube-Test as a substitute for specific test tuberculin skin test for diagnosis of latent tuberculosis infection in high risk groups.
Methods: One hundred thirty four (134) individuals who worked in Bo-Ali hospital (Zahedan) enrolled in this study. They had no active tuberculosis. TST and QFT tests were performed. The cut-off point of TST was considered based on 15 (mm) or more indurations as positive. The result of QFT was evaluated by manufactured guidelines. Multivariate logistic regression was used to identify the putative risk factors of positive tests.
Results: Proportion of employees with latent TB were 111(82.8%) were positive by either TST or QFT, and 76(56.7%) were positive by both tests. Agreement between the tests was high (73.8%, k=0.39 95%CI:
0.21-0.44). Positive family history of Tuberculosis was significant risk factor for both positive tests. 
Conclusion: This study showed high latent tuberculosis infection prevalence in hospital workers and high agreement between TST and QFT. Decision to select one of the tests will be depended on the population, purpose of study and availability of resources. The results revealed that the QFT can be appropriate alternative test for high risk group.

Abstract

  Background and Objectives : Influenza is an acute, contagious, and zoonotic viral disease. It is caused by a virus of the Orthomyxoviridae family. This very infectious is caused by different subtypes of type A influenza virus in the poultry, turkey, and many other birds. In this study, the serum status of rural domestic poultry was investigated for influenza subtype H9N2.

 Methods: This cross sectional study was done from August to October in 2013 in Iranian villages through sampling domestic poultry. The sampling was accidental and was done based on the GIS 11-digit code. In each village, blood samples were taken from at least 28 birds from different species. Then, ELISA was used for screening followed by the HI test. A total of 397 villages and 11546 birds (10145 chickens, 1413 ducks, 397 turkeys, 10 pigeons, and 175 other species) were sampled.

  Results : Three hindered and forty nine (88%) out of 397 villages were positive on ELISA. In addition, 341 villages (86%) were positive and 56 (14%) were negative on the HI test for antibody titers. Also, among the considered variables, weather was a risk factor and the prevalence was significantly lower in villages near the rivers, lagoons and lakes (up to a radius of 3 Km).

  Conclusion: The high seroprevalence of influenza H9N2 in rural domestic poultry indicates that the disease is becoming endemic. As there is no eradication policy for influenza H9N2 in Iran, using effective vaccines can reduce the infection with influenza virus in domestic and rural poultry.

Abstract

Background and Objectives: Influenza is an acute, highly contagious disease of a variety of both domestic and wild bird species. The aim of this study was to estimate subclinical infections or previous exposure to H5 and H7 subtypes and to discover potentially important determinants of the prevalence of this disease in industrial and backyard poultry in Iran.

Methods: A survey was conducted from September to December 2014 in Iran using a cross-sectional design throughout the entire country. The Enzyme-Linked Immunosorbent Assay (ELISA) was used as the screening test and all ELISA-positive samples were examined with the HI test to differentiate H5 and H7. A total of 1378 places and 31547 birds were sampled.

Results: One place (bird garden) out of 1378 was positive for the H7 subtype and six places (2 bird gardens, 3 villages, and 1 ostrich farm) out of 1378 were positive for the H5 subtype on HI test. The results of this study showed that the presence of a lake (OR=12.20, CI 95% 2.19-68.09) and live bird marketing (11.73, CI 95% 1.32-104.42) were risk factors for the H5 subtype.

Conclusion: Sampling migratory waterfowl birds is recommended for early detection of probable infections and preventing the spread of infection to the backyard and industrial poultry because of their role in the transmission of the disease and probability of circulation of the virus.

Abstract

Background and Objectives: Foot-and-mouth disease (FMD) is one of major contagious animal. The purpose of this study was to evaluate the determinants associated with FMD occurrence in farms.

Methods: A case- control study was carried out in cattle farms.  The cases were farms with new reports of FMD and controls were units without any report of FMD in last 6 month. For assessing the risk factors, a researcher- made questionnaire was used. Statistical analyses were done with SPSS version 16 using the logistic regression test.

Results: Purchase of new livestock [OR: 14.69 (CI95%: 4.29, 50.36)] and passing migratory livestock [OR: 13.32 (CI 95%: 1.74, 101.87) were identified as the risk factors of the disease. Visiting the farm by the vaccinator in last 2 weeks [OR: 0.17 (CI 95%: 0.05, 0.63)] and buying new livestock from the same village/ city where the farm is located [OR: 0.16 (CI 95%: 0.04, 0.58)] were considered as protective factors.

Conclusion: Considering the role of infected livestock in the country, the most important steps for effective preventation are: areadherence to biosecurity as well as quarantining new purchased animals, and informing and training farmers to not purchase new animals from farms that are located in the infected areas and around the times of outbreak.

Abstract

Background and Objectives: Salmonella contamination of broiler breeder farms is of great importance regarding transmission of the bacteria through the production chain of poultry meat. The aim of this study was to determine the prevalence and management related factors of Salmonella contamination in broiler breeder flocks in Iran.

Methods: This cross sectional study was carried out between 2013 and 2014 in 23 provinces of Iran. Fecal samples were obtained from 139 broiler breeder farms and were subjected to standard bacteriological tests for Salmonella isolation. The serogroups of isolated Salmonella were determined using standard serological tests. Relevant information of sampled holdings was acquired from the GIS system and used for risk factor detection.

Results: The prevalence of Salmonella contamination in broiler breeder holdings was 7.9% and the more positive samples were from Tehran and Fars provinces. The flock age (P = 0.019) and the number of poultry houses in the farm (P = 0.037) were major risk factors of Salmonella contamination in broiler breeder farms.

Conclusion: This investigation showed that the prevalence of Salmonella contamination in broiler breeder farms was relatively high. It is necessary to establish an appropriate program for controlling Salmonella contamination in broiler breeder farms. Regular sampling for early detection of contaminated farms would be very important in this case. Sampling from old flocks should be the first priority.

Abstract

Background and Objectives: Data plays a major role in a health care system in development planning and health services support if they are correct, timely and accessible. The data of the Ministry of Health are not readily available and the limited access reduces their value. The aim of this study was to explore the challenges of access to the data of the Iranian Ministry of Health.
Methods: This qualitative study was conducted in 2015. Twenty-three academic and administrative experts were selected purposefully. Semi-structured interviews were conducted to collect the data. The transcripts of the interviews were analyzed using content analysis.
Results: The results of this study provided 4 main themes (challenges of access to the data of the surveillance system, challenges of access to the data of national surveys and ordered projects, challenges of access to the data of electronic health records, and challenges of access to confidential data) and 15 sub-themes.
Conclusion: Given the multiple challenges of access to the data of the Iranian Ministry of Health, it is suggested to design access mechanisms in a systematic manner in the form of guidelines and organizational structures for data access management.

Abstract

Background and Objectives: In Iran, rainbow trout farms exist in almost all provinces. Viral hemorrhagic septicemia is one of the most important infectious diseases of the rainbow trout which is a serious threat to the farming industry. This study was conducted to investigate outbreaks in 2014 and identify important determinants of the agent entry and disease occurrence in the farms.
Methods: Data were collected using a structured questionnaire and the disease was detected based on clinical signs and laboratory investigations by PCR methods.
Results: During 52 weeks of the study in 2014, 114 of 1140 (12.63%) farms were affected within 78 zones in 14 provinces. In multivariable analysis, illegal entrance of fries (odds ratio: 7.81, 95% CI: 3.63-16.8), illegal entrance of fish (odds ratio: 5.60, 95% CI: 3.03-10.35) and use of river as the water supply (odds ratio: 2.46, 95% CI: 1.51-4.02) were detected as risk factors associated with virus entry and disease outbreak in farms.
Conclusion: Observing biosecurity measures in the farm level, applying risk-based surveillance based on known risk factors, and assessing these factors on a regular basis are important in prevention and control of VHS.

Abstract

Background and Objectives: Brucellosis is one of the most important zoonosis diseases. The aim was to estimate the seroprevalence of brucellosis in rural cattle, sheep, and goat of Bushehr Province.
Methods: This cross-sectional study was conducted in 2012-2014. For the cattle population, a total of 118 villages and 891 cattle and for sheep and goat population, a total of 202 villages, 526 flocks, 3064 sheep, and 7182 goats were included. Samples were examined with Rose Bengal, 2-mercaptoethanol and wright tests.
Results: In the cattle population, 28 out of 118 villages (20.34%, CI 95%: 13.94%-28.73%) and 20 out of 891 cattle (3.36%, CI 95%:2.28-4.77%) were seropositive. Additionally, in the sheep and goat population, 89 out of 202 villages (44.06%, CI 95%: 37.10%-51.20%), 142 out of 526 flocks (27%, CI 95%: 23.25%-31%), and 335 out of 10246 sheep and goats (3.27%, CI 95%: 2.93%-3.63%) were seropositive.
The prevalence at the individual level of all animals was 3.53%in 2012 which was significantly higher than 2013 ( 2.59%) (P=0.02). Totally, in 3 years, the prevalence of brucellosis in sheep (3.88%) was significantly higher than goats (3%) (P = 0.02) and the prevalence in female animals (3.52%) was significantly higher than male animals (1.58%) (P <0.001).
Conclusion: Due to the reduction of the prevalence, the test and slaughter policy together with the could further reduce and control the spread of the disease in the animal population.

Abstract

Background and Objectives: HPAI has economic and public health importance. Aquatic and shore migratory birds are the main reservoirs and the cause of the spread of viruses across countries. The aim of study was risk assessment of the spread of the avian influenza H5 viruses.
 
Methods: In this qualitative study, structured interviews and focus group discussions were used to assess the risk of the introduction and spread of highly pathogenic avian influenza viruses (H5-subtypes) into Iran.
 
Results: Over 300 wetlands, natural and artificial lakes, ponds, and more than 517 bird species (swans, geese, ducks, and coots) are identified in Iran. Weakness in detecting virus entry windows, weakness in passive surveillance of migratory birds, inadequate supervision on hunting of migratory wild birds, movement of  hunted birds without any criteria without the control of the Veterinary Organization, inability to monitor the supply of migratory birds in the LBMs, law weaknesses in dealing with offenders, lack of training for people, presence of suitable conditions for close contacts between wild birds and backyard and industrial poultry, and weakness of research on AI in migratory birds were the most important factors influencing the spread of these viruses.
 
Conclusion: HPAI will be one of the most important challenges for the poultry industry and public health.  Solving this challenge requires national determination, overhead organizational views, collaborative and practical cooperation of related organizations, and short and long-term planning based on the realities of the country.

Abstract

Background and Objectives: Highly pathogenic avian influenza (HPAI) viruses are particularly important due to huge economic consequences and public health concerns. During recent years, because of the severity of epidemics and failure in the control and eradication of HPAI, vaccination has been introduced into the National Prevention and Control Program of some countries, including Iran. This article discusses the role of vaccination in the control of HPAI, the characteristics and constraints of the vaccine, types of commercial vaccines, and previous experiences.
 
Methods: Scientific databases were searched to collect essential evidence on the role of vaccination in controlling HPAI in a variety of poultry and birds with no time and language restrictions. The articles related to the efficacy and effectiveness of the commercial vaccines on at a national level or population-based studies in some farms were included in present study.
 
Results: Because of the high pathogenicity and variability of the influenza virus as well as the shortcomings of some commercial vaccines in prevention of virus transmission, vaccination should only be taken into consideration when all other methods of disease control are ineffective. The consequences of the use of vaccines containing the seeds non-antigenically matched with the circulating strains have been reported frequently.
 
Conclusion: In the national vaccination program, informed policymaking requires evidence based vaccinology that can facilitate development of the national infrastructure with awareness of the limitations of commercial vaccines and the realistic expectations of logistic, financial, and human resources.

Abstract

Background and Objectives: Iran is one of the Middle Eastern countries that implemented the HIV / AIDS control and surveillance program many years ago. The purpose of this study was to review the HIV / AIDS surveillance system in Iran.
 
Methods: This was a review research to assess the processes, structures and achievements of the HIV/AIDS surveillance system in Iran. The information sources of this study included data from the surveillance system, reports and documentation, and published guidelines, reviewing existing structures and views of managers and practitioners of the HIV/AIDS surveillance system in Iran.
 
Results: In Iran, all cases with HIV / AIDS as well as people with high risk behaviors are referred to behavioral disease counseling centers in order to receive health care services. Harm reduction in people with risky behaviors in the form of counseling centers, drug addicts' centers, and women's counseling centers in collaboration with governmental and non-governmental organizations, and attention to the second generation of HIV/AIDS surveillance, particularly conducting behavioral studies, are other components of the HIV / AIDS surveillance system in Iran
 
Conclusion: Although the HIV / AIDS surveillance system in Iran has a large structure with many achievements in reducing new HIV cases, especially in some high-risk groups such as injecting drug users, as well as reducing HIV transmission from infected mothers to their children, strengthening this surveillance system, especially for sexual high-risk groups, is essential for controlling HIV/AIDS in the country.

Abstract

 
Background and Objectives: Avian influenza (AI) is one of the most important respiratory diseases in the poultry industry that causes huge economic impacts and plays an important role in public health. H9N2 Avian Influenza Virus (H9N2 AIV) has a broad circulation and causes endemic infections in the poultry industry of the country.
 
Methods: A cohort study was conducted from July 2016 to November 2017 in broiler chicken farms of Qazvin Province, Iran to detect H9N2 AIV infection determinants in broiler chicken farms.
 
Results: Sixteen out of 34 units that were included in the study had positive PCR results. Some variables such as “type of fence around the unit”, “biosecurity”, “cooling system status”, “mean maternal antibody titers”, “location height”, “proximity to public traffic lanes” and “number of units within a radius of 1 km” had significant effects on the infection occurrence in poultry units according to univariate statistical analyses (P<0.05). However, only two of them (“biosecurity” and “cooling system status”) were statistically significant in multi-variable analyses (P<0.05).
 
Conclusion: Biosecurity measures should be implemented more seriously and strictly in broiler farms to reduce the impact of H9N2 AIV infection.

Abstract

Background and Objectives: The commercial layer chicken farming is one of the most important sectors of the poultry industry. This study was conducted to evaluate the status of biosecurity of commercial layer chicken farms in 9 Iranian provinces with the highest layer chicken population.
 
Methods: In this cross-sectional study, 202 commercial layer chicken farms were selected from nine Iranian provinces in 2019 using stratified random sampling. Then, the required data were collected using a questionnaire by presenting to the farms in person. The results are expressed as mean, standard variation, sum and adoption rate index.
 
Results: The results showed that the adoption level of total biosecurity measures was 68.18% in the commercial layer chicken farms in Iran. This figure was 72.11%, 75% and 60.82% for conceptual, structural and operational biosecurity measures, respectively. Among conceptual biosecurity measures, the lowest adoption level (29.20%) was for distance to the nearest garden and agriculture land. Among structural biosecurity measures, the lowest adoption level (28.96%) was for a hand disinfection spot before shed. Among the operational biosecurity measures, the lowest adoption level was for water filtrations (31.18%) and microbial testing of water (33.91%).
 
Conclusion: This study investigated the biosecurity status of commercial layer chicken farms in nine Iranian provinces with the highest layer chicken population and revealed some weaknesses in the implementation of these measures at conceptual, structural and operational levels.

Abstract

Background and Objectives: Drug use not only affects a person's physical and mental health, but also affects the health of others in the community. Various variables, including demographic and social factors, affect drug use. The present study was conducted to investigate the prevalence of drug use and related factors in the participants of Shahedieh Cohort Study.

Methods: A descriptive study was performed using the first phase of Shahedieh cohort study on 10194 adult residents of Shahdieh, Zarch, and Ashkezar in 2015-2016. The aim of the cohort study was to assess the prevalence of non-communicable diseases and their risk factors in adults aged 35-70 years. Data were analyzed with the SPSS 20 using chi-square and logistic regression.

Results: The prevalence of illicit drug use in the present study was 15.5% with a mean age of onset of 31.5 ± 9.2 years. The most common drug was opium (98.2%). The most common method of drug use was inhalation (98.1%). The logistic regression showed that male gender (P< 0.001), age 40 to 49 years (P<0.001), low education (below high school diploma) (P<0.001), positive history of smoking (P<0.001) and alcohol consumption (P<0.001) were the most important factors associated with drug use. In addition, a positive history of ischemic heart disease (P=0.007) and psychiatric disorders (P=0.02) were the diseases related to drug use.

Conclusion: The prevalence of drug use was high in the study population. There is an urgent need for intervention and preventive measures to solve this complex social problem.

Abstract

Background and Objectives: Colorectal cancer is the second and fourth most common cancer in Iranian women and men, respectively. This cancer ranks fourth and fifth in men and women, respectively, among Yazd population. This study aimed to estimate the burden of colorectal cancer in Yazd province in 2016.
Methods: This descriptive cross-sectional study was performed on the population of Yazd province in 2016. CanMod software was used to calculate the burden of colorectal cancer. The required information included the population of Yazd province, the incidence of colorectal cancer, the death rate due to colorectal cancer, the death rate due to all causes and the survival rate of patients; The Cancer Registration System, the Provincial Death Registration System and the opinions of provincial experts were collected and included, respectively.
Results: The total burden of colorectal cancer in Yazd province was 632 years (382 years in men and 250 years in women); The YLL and YLD of colorectal cancer in both genders were 478 years and 154 years, respectively. The burden of colorectal cancer in men was 65 years and in women 45 years (per 100,000 population). The peak age of disease burden was estimated in the age group over 60.
Conclusion: Due to the high incidence and treatment in the early stages of the disease, screening and diagnosis in the early stages can be one of the health priorities of the province. However, the final judgment is made when the burden of other diseases and injuries is calculated in the province.

Abstract

Background and Objectives: Today, overweight and obesity are among the significant challenges in the world. They can have adverse effects on quality of life. Quality of life is determined by social and physical environment. The present study investigated the construct validity of the Persian version of the Impact of Weight on Quality of Life (IWQOL-lite) questionnaire using multilevel confirmatory factor analysis.
Methods: The present study was conducted on 310 people over 18 years old living in Yazd city. The data were analyzed using m-plus6.2 software.
Results: The mean age of the participants in this study was 33.97± 4.70 years. Sixty percent (N=183) of subjects were male. The indices of the one-level model were more appropriate than those of the two-level model, and the one-level confirmatory factor analysis model had a good fitting to the data (CLI: 0.98, TLI: 0.98, RMSEA: 0.038).
Conclusion: The results of this study indicated the need for more effectiveness of urban areas on quality of life. The one-level confirmatory factor analysis model confirmed the construct validity of the IWQOL-lite questionnaire. This questionnaire can be used in the Iranian population.

Abstract

Background and Objectives: Identification of rotavirus genotypes in children is clinically important. This study aimed to determine the spectrum of rotavirus genotypes and assess their correlation with demographic variables and clinical manifestations in hospitalized children.
Methods: To determine rotavirus genotypes, rotavirus positive stool samples of symptomatic children were included in the study between December 2019 and March 2020. RNA extraction and cDNA synthesis for VP7 and VP4 genes were performed following standard protocols. Genotypes were determined using specific primers. Validation of results was done through sequencing and bioinformatic analysis. Data were statistically analyzed using SPSS version 20 and GraphPad version 9.5.0.
Results: Among the infected patients, three genotypes emerged as dominant in the studied population. The study demonstrated a significant correlation between genotype frequency and seasonal variations (p-value=0.0077), as well as between genotypes, hospitalization, and severity of diarrhea. While significantly more types of rotavirus group A were identified with increasing age, no correlation was observed between the genotypes and gender (p-value=0.473). Furthermore, there was no significant association between genotype, dehydration rates, and the presence or absence of fever.
Conclusion: This study revealed a relatively high diversity of rotavirus genotypes in children. The findings suggest the need for further research to validate the identified correlations between certain genotypes and age groups, seasonal variations, clinical symptoms, and the efficacy of available vaccines.
