Abstract

Background and Objectives: Injury by sharp instruments and needles are major occupational health hazard for healthcare workers. This study was conducted to determine the prevalence rate and causes of injuries due to needle stick in healthcare workers in Zahedan University of Medical Sciences.
Methods: In a cross-sectional study, 231 healthcare workers of university hospitals of Zahedan University of Medical Sciences were selected at random in 2007. Data were collected through questionnaire and were analyzed by SPSS 13.
Results: From all cases 77.1% were female 56.4% of them were expert, and 50.7% were nurses. Their mean age were 34±8.1 years. Needle stick prevalence was 64.9% during the whole history of career, and 57.2% of them had experiences of needle stick more than twice. Most cases of injuries were related to using needle 55.4%, and they were mostly prone to injury during injection and phlebotomy, 55.6%. The multivariate regression analysis showed that injury due to sharp instruments in university graduates was 60% less compared to those with high school or less, and the injury risk increases 5% in each year of increase in working experience.
Conclusions: The findings of this study highlight the importance of training and preventive measures in healthcare centers particularly among high risk group.

Abstract

Background & Objectives: The epidemic of HIV has been become worldwide. Infected people with HIV which are most intravenous drug users (IDUs) and/or people who have unsafe sex are important source of transmission of infection to other persons in a community. Since there is little known about the knowledge and attitudes of HIV positives about HIV in Iran, therefore we decided to measure the level of knowledge and attitudes of this group in Golestan province in compare with individuals with risky behavior.
Methods: This cross-sectional study was conducted on 54 HIV+ persons and 134 people with high risk behavior individuals in 2007 year. Results: Our study showed that HIV- (High risk group) cases had higher education level (P=0.034). Rate of jobless was higher in HIV+ persons (P<0.001). HIV+ cases had lower income (P=0.037). Both groups had moderate knowledge and poor attitude on HIV/AIDS. Level of attitude in HIV+ positive persons was lower than high risk group (P=0.05).
Conclusion: It is concluded that these susceptible persons and also infected individuals with HIV regardless of their socio-economic levels need more continuous education in structured programs.

Abstract

Background and Objectives: Ahvaz has become one of the most polluted cities in the world. Only on 11 November 2013, over one thousand residents in Khuzestan Province attended the nearest health center with severe symptoms, particularly shortness of breath, cough, wheezing, and high blood pressure. During a week, more than 7 thousand patients were registered with this syndrome. This study was conducted to investigate the evidence for the confirmation of an epidemic in acute respiratory syndrome among the residents of Ahwaz in November 2013.

Methods: In this cross-sectional study, we evaluated the data of the use of drugs related to acute respiratory attacks from November 2008 to 2013 in Ahvaz. Then, we conducted a descriptive study on the general population. The sample size was 120 and the study participants included different segments of the general population. The data collection tool was a researcher-made questionnaire and the data were analyzed using SPSS.18.

Results: The use of the respiratory drugs increased from 2008 to 2013. The drug consumption was markedly higher in November 2013. In the study of the general population, 93% of the participants declared that they had never seen a similar event, and 92.5% mentioned that they had never heard of a similar occurrence.

Conclusion: Due to the significant increase in respiratory medicine in November 2013 and the unexpected nature of the event, an epidemic seems to be obvious.

Abstract

Background and Objectives: Data plays a major role in a health care system in development planning and health services support if they are correct, timely and accessible. The data of the Ministry of Health are not readily available and the limited access reduces their value. The aim of this study was to explore the challenges of access to the data of the Iranian Ministry of Health.
Methods: This qualitative study was conducted in 2015. Twenty-three academic and administrative experts were selected purposefully. Semi-structured interviews were conducted to collect the data. The transcripts of the interviews were analyzed using content analysis.
Results: The results of this study provided 4 main themes (challenges of access to the data of the surveillance system, challenges of access to the data of national surveys and ordered projects, challenges of access to the data of electronic health records, and challenges of access to confidential data) and 15 sub-themes.
Conclusion: Given the multiple challenges of access to the data of the Iranian Ministry of Health, it is suggested to design access mechanisms in a systematic manner in the form of guidelines and organizational structures for data access management.

Abstract

Background and Objectives: Domestic violence is the most common type of violence against women which leaves negative effects on the mental health of the mother, children, family, community, and the next generation directly and indirectly. Therefore, this study was conducted to assess the arte of domestic violence and some related factors in married women.
Methods: This cross-sectional descriptive research was carried out with participation of 623 married women attending health care centers of Abadan, Iran who were selected through random cluster sampling from in 2015. The data were collected by a demographic questionnaire and a women abuse scale checklist. Moreover, descriptive and analytical statistics (including T-Student Test, Pearson Correlation Coefficient, One-Way Variance Analysis, Chi-Square Test, and Fisher) were used for data analysis.
Results: The total prevalence of violence was 72.3%, and the porevalence of psychological, physical, life threatening, and sexual violence was 71.7%, 17.8%, 8.3%, and 7.1%, respectively. Moreover, a significant relationship was observed between violence and age, marital satisfaction, family behavior, financial situation, cigarette, alcohol, disease, domicile, occupation, place of birth, and criminal records.
Conclusion: Violence in the city of Abadan, eespecially psychological violence, is highly prevalent. Therefore, it is suggested that domestic violence against women be prevented using approaches such as life skills training starting before marriage such as anger management, proper communication, and problem solving.

Abstract

Background and Objectives: Breast cancer is the most common cancer in Iran. It can be prevented by rapid diagnosis of the disease. Thus, it is necessary to determine the causal relationships between variables related to breast cancer. Bayesian network is a data mining tool that shows the causal relationship between different variables. In this paper, a Bayesian network was applied to find causal relationships between breast cancer variables using a genetic algorithm in a graphical model. 
 
Methods: in this applied study, data were collected from 900 breast cancer patients in Kerman Province from 1999 to 2008. For data analysis, we used a probabilistic graphical model representing the causal relationship between variables.
 
Results: The results showed that surgery was the most important treatment for breast cancer. Based on the conditional and marginal probabilities, the women who underwent surgery had higher hopes of living longer. Moreover, 81% of the patients who did not undergo surgery only received chemotherapy or radiotherapy were less likely to have long lives.
 
Conclusion: People aged 40-65 years are more likely to have breast cancer. Moreover, the variables of age, surgery, chemotherapy, and radiotherapy had a direct effect on the status of the patients and there were direct edges from these variables to the status of the patients.

Abstract

Background and Objectives: Evidences show an increase in the occurrence of road crashes and causalities in early stages of economic growth. This study aimed to evaluate the correlation between the incidence of motorcyclists’ crashes and the level of development of the provinces of Iran in the year of 2015.
 
Methods: This was an ecological study of the motorcyclists’ crashes in all provinces of Iran. Motorcyclists’ injuries were obtained from the database of traffic police and the report of population census in 2011 was used. Given that more than 99% of motorcycle-related crashes occurred in male motorcyclists, the population of males was used to adjust the results. The Pearson correlation coefficient was also applied.
 
Results: Totally, 90,724 crashes occurred among motorcyclists. The highest rate was in the age group of 18 to 30 years with 46,944 crashes (51.7%). As for sex, men had 90363 crashes (99.6%). Among different provinces, Tehran and Ardabil had the highest and lowest incidence with 1129.84 and 77.37 per 100 thousand male populations, respectively. Significant correlations were found between the incidence of motorcycle crashes and Gross Domestic Product (GDP) (P value: 0.601) and Human Development Index (HDI) (P value 0.497), respectively (P<0.05).
 
Conclusion: Increased the incidence of motorcycle crashes is associated with the level of provincial economic development. In order to prevent the injuries and reducing its incidence, attention should be paid to the safety of roads and vehicles.

Abstract

Background and Objectives: Pulmonary consequences of exposure to low concentrations of ammonia have been less studied. The purpose of this study was to model the pulmonary consequences of exposure to low concentrations of ammonia.
 
Methods: This historical cohort study was conducted in the fertilizer industries in 2019. The participants were divided into two groups of exposed (n=98) and non-exposed (n=105). The exposure level, pulmonary symptoms, and pulmonary function parameters were assessed using the NMAM 6016 and American Thoracic Society and European Respiratory society (ATS&ERS) protocols. Data analysis was performed using the IBM SPSS v. 22.0, and multivariate linear and logistic regression models.
 
Results: The ammonia exposure level was 4.80±1.54 ppm in the exposed group. There was no significant difference in most of the individual variables between the two groups (P>0.05). The frequency of all pulmonary symptoms was significantly higher in the exposed group compared to the non-exposed group (P<0.05). The highest prevalence of pulmonary symptoms in the exposed group was for cough (25.51%), wheezing (19.39%) and shortness of breath (18.37%). All lung function parameters were significantly lower in the exposed group versus the non-exposed group (P<0.05). The values of lung function parameters in the exposed group including FVC, FEV1 and FEV1 / FVC were 86.0%, 82.47% and 81.97%, respectively. Regression modeling results showed that ammonia exposure had a significant relationship with all pulmonary symptoms and lung function parameters (P<0.05).
 
Conclusion: The findings indicated that exposure to low concentrations of ammonia should be considered as a major risk factor in the prevalence of pulmonary symptoms and reduced lung function parameters.

Abstract

Background and Objectives: The aim of this study was to investigate the epidemiological characteristics of patients with Covid-19 in Hamadan Province.
 
Materials and Methods: In this descriptive cross-sectional study, demographic and epidemiological data of all people who presented to hospitals in Hamadan Province from February 2019 to December 2020 were extracted using two checklists. Data were then analyzed using the Stata software.
 
Results: In this study, 9674 covid-19 positive patients were examined. According to results, 49.11% of the cases occurred in the elderly over 60 years. The rate of involvement was higher in females than males (51.57% vs. 48.43%). Moreover, 72.05% of the definitive patients lived in the city and 0.76% reported traveling to areas with a high prevalence of the disease two weeks before the onset of symptoms. The highest incidence of this disease per one hundred thousand population was in Malayer, Hamedan and Nahavand counties and the highest fetality was in Razan and Dargazin, Tuyserkan and Asadabad counties, respectively. The data of suspected, probable, and definite outpatients with Covid-19 presenting to medical centers were not evaluated in this study.
 
Conclusion: Due to the high death rate in the elderly, males, those with underlying diseases, and people living in rural areas, it is necessary to design and apply precautionary measures in these groups. Attention should be paid to these high-risk groups in the shortest possible time to reduce the burden of this disease on individuals as well as the health care system.

Abstract

Background and Objectives: Given the importance of the early detection of any outbreak or change in the trend of meningitis, this study was conducted to estimate the fixed and dynamic alarm threshold levels of meningitis and its outbreak in Iran.
 
Materials and Methods: In this study, the data of all patients from 20¬ March 2016 to 20 March 2019 were extracted from the National Meningitis Disease Surveillance System. Alarm threshold levels were calculated separately for different seasons and provinces. To determine the alarm threshold levels, four methods of Upper Control Limit (UCL), relative increase, statistical cut off points, and recommendations provided by the World Health Organization were used.
 
Results: The alarm threshold levels for confirmed cases were determined for Iran according to the upper limits of cumulative sum (CUSUM) algorithm and the statistical cut off points based on the 90th percentile, 2 cases per day for Iran. The fixed amount was 3 to 5 cases per week in populations under 30,000 and 5 to 7 per week in populations between 30,000 and 100,000. According to the relative increase method, a 30% increase in one week compared to previous weeks was considered as the threshold level.
 
Conclusion: The use of alarm threshold levels reported in this study can be a basis for detecting meningitis outbreaks.

Abstract

Background and Objectives: The mobile phone has become an indispensable tool for communication and interactions. This study aimed to investigate the relationship between excessive mobile phone use, depression, and social loneliness among students at Shahroud University of Medical Sciences.
Methods: This descriptive-analytical study, conducted in 2019, employed stratified random sampling to survey 313 students from Shahroud University of Medical Sciences. Data collection involved demographic questions, a harmful mobile phone use scale, and depression scale (Kavaja and Bryden). Collected data were entered into SPSS 18 and analyzed using descriptive and analytical statistics.
Results: The study included 313 participants with an average age of 22.16 ± 3.41 years. Pearson's correlation coefficient revealed a positive correlation between depression scores and various variables, including education level, interest in the field, native status, place of residence, history of taking antidepressants, employment status, financial problems, family life status, father's age, and excessive use of mobile phones. The results of linear regression analysis, as indicated by the coefficient of determination (0.603) demonstrated a high intensity of correlation between factors influencing depression and the actual depression scores.
Conclusion: According to the results of the study, there is a significant relationship between excessive use of mobile phones and depression and social loneliness in students, which requires proper planning for intervention.
