Abstract

Background & Objectives: The summer of 2005 witnessed an outbreak of cholera in Iran due to V. cholera, Inaba serotype. During the outbreak, there were 16 registered cases in Markazi Province. The aim of this study was to determine the risk factors for cholera outbreak in this part of the country.
Methods: This was a case-control study in which cases were matched to controls on age and sex. All of the reported cases were compared with controls from the same neighborhood, with 2 controls selected for each case. We used paired odds ratios and conditional logistic regression to show the effect of each putative risk factor, and p values of < 0.05 were considered as statistically significant. Results: Multivariate results from 16 cases and 32 controls showed significant associations between the cholera outbreak and the following set of factors: eating raw vegetable (OR=9, 95%CI: 2.25-35.98), proper warming of refrigerated food (OR=0.12, 95%CI: 0.015-1.022), inadequate washing of fruits and vegetables (OR=4.29, 95% CI: 0.88-21.06) , and eating non-pasteurized ice creams (OR=4.96, 95%CI: 0.99-24.83).
Conclusions: Although the number of reported cases was small in Markazi Province, the results show that education in personal hygiene is important in the prevention of a cholera outbreak.

Abstract

Background & Objectives: Tuberculosis (TB) is the largest single cause of death from infectious diseases and has a ten rating of global burden of disease. Despite the availability of effective treatment for pulmonary TB, sputum conversion of patients affected by various factors. This study aimed to determine the time course of sputum conversion in patients and possible affected factors in this process. Methods: In this Retrospective cohort study, 440 patients with smear-positive pulmonary TB in Hamadan province from 21 March 2006 to 20 March 2012 referred to health centers were included. Demographic, clinical data and treatment status of patients, including the time of sputum smear negative were extracted using TB Register software from patient registry. Time to sputum conversion was considered monthly during the patient’s treatmentand shown by Kaplan-Meier survival curve. The effects of some determinants including gender, location, age group and number of bacilli in the sputum of patients at the beginning of treatment were determined using Cox proportional hazard regression model.
Results: From 440 patients with smear-positive pulmonary TB, 51% (221 patients) were male and 49% (219 patients) were female. Fifty seven percent (57%) were living in urban and others in rural. The median of sputum conversion was 3 months. Totally, sputum conversion rate at the end of month 2 and 3 were 69% and 88%, respectively.
Conclusion: Our findings revealed that there has been a considerable difference between the expected sputum conversion rate and the observed rate in Hamadan province, Iran.

Abstract

Background and Objectives: Epidemiology is a tool and a field of science which provides evidences necessary to make decisions. It is known as one of the basic sciences of public health. This study aimed to identify strategies for the promotion of epidemiology by Iranian Epidemiological Association so that people graduated in this field would become able to play an effective and positive role in different parts of the country.

Methods: This study was conducted in 2015. The views and opinions of the members of Iranian Epidemiological Association were collected by email. The views of the members were classified using qualitative methods.

Results: The results of this study helped to identify some of the strategies for the promotion of the position of epidemiology in Iran’s health system by the members of Iranian Epidemiological Association.

Conclusion: Iranian Epidemiological Association can play a more effective role in the promotion of epidemiology in the health system. Effective communication with the members, holding group meetings, advocacy and negotiation, reporting the activities of the Association continuously, identifying national problems and seeking solutions are some of the strategies that can help to enhance the role of the Association and promote the position of epidemiologists in Iran.

Abstract

Background and objective: Epidemiology is a tool and a field of Medical sciences  which provides evidences necessary to make scientific decisions.  This study aimed to identify strategies for the promotion of the epidemiology by Iranian Epidemiological Association (IrEA) so that people graduated in this field would become able to play an effective and positive role in different parts of the country.

Methods: This study was conducted in 2015. The views and opinions of the members of IrEA were collected by email. The views of the members  were classified using qualitative methods.

Results: The results of this study helped to identify some of the strategies for the promotion of the position of epidemiology in Iran health system by the members of IrEA.

Conclusion: IrEA can play a more effective role in the promotion of this field of study in the health system. Abstract

Background and Objectives: The cause of death in children under one year can be an important tool for designing prevention strategies and reducing the mortality rate. The aim of this study was to estimate the number of deaths in children under one year using the Mr. Murray’s estimation index in Mazandaran Province, and to compare this estimation with reported cases of civil registration organization.
 
Methods: All deaths of children under one year between 2011 to 2014 registered in hospitals across Mazandaran Province were included in this study. The cause of death as coded in the International Classification of Diseases (ICD-10) was converted to Murray classification. The coefficients in each of the Murray levels were used to estimate actual death cases. We compared this estimation with the number of deaths that is reported annually by civil registration organization. 
 
Results: Seven hundred and sixty four deaths occurred in this period. The leading causes of death in children under one year were conditions of the perinatal period, congenital anomalies and chromosomal disorders, respiratory diseases, and diseases of the cardiovascular system. The Murray method estimated 1711 deaths for the entire Province.
 
Conclusion: the Murray method predicted that from 2011 to 2014, 390-445 children under one year died in Mazandaran Province annually. There is a controversy between the estimates obtained in this study and the number of deaths reported by the civil registration organization, which may indicate a defect in a complete registration of deaths by this organization.

Abstract

Background and Objectives: an important problem challenging cesarean section is its extensive use as a common method of delivery. Due to the growing trend of cesarean section in Iran in recent years, the natural delivery promotion program was implemented as one the programs incorporated in the Health System Reform Plan in 2014. In this study, the trend of changes in the percentage of CS delivery in Qom Province following the implementation of this program was evaluated.
 
Methods: This trend analysis that was performed in all cesarean deliveries in Qom Province from 2005 to 2018 using a joinpoint regression method.
 
Results: These results showed an annual increase of0.4% in the CS percentage 95% CI: -0.5 to  1.2), which was not statistically significant. A significant decrease was observed in the rate of CS in governmental hospitals. Conversely, in non-governmental hospitals, the percentage of CS increased significantly.
 
Conclusion: According to the findings of this study, after more than 3 years of implementation of health sector evolution plan, overall implementation of this plan failed to significantly reduce the overall process of cesarean delivery during this period in Qom province and achieve the predetermined goals.

Abstract

Background and Objectives: Viral hepatitis is one of the most prevalent diseases and an important underlying cause of death and disability in the world. This case-control study was conducted to investigate the outbreak of hepatitis A in Boroujerd in 2019.
 
Methods: This case-control study was conducted to investigate the epidemic of hepatitis A in Boroujerd in 2019. Fifty hepatitis A patients were recruited. For each patient diagnosed, two controls with no history of the disease were selected from a neighboring village. Data were collected using a demographic questionnaire as well as the results of experiments. To investigate the association between the disease and risk factors, odds ratio and 95% confidence interval were used. 
 
Results: In this study, the odds ratios were significant for a number of risk factors including washing fruits and vegetables with a source other than tap water [OR = 3.2], washing dishes with a source other than tap water [OR = 3.7], non-sanitary toilets [OR = 8.3], using sources other than tap water when there is no tap water [OR = 4.4], being unaware of the disease and its transmission methods [OR = 4.8] and water well dug less than 15 m from the sewage well (OR = 4.1).
 
Conclusion: Increased public awareness and improved health status of the environment and rural sewage can preclude similar epidemics in the study area and other regions.

Abstract

Background and Objectives: The incidence of rectal cancer is increasing in developing societies, especially in younger age groups. The aim of this study was to evaluate the factors affecting the survival of patients with rectal cancer in the presence of competing risks.
 
Methods: In this retrospective cohort study, the data of 121 patients with rectal cancer during 2001-2017 were studied. Death related to cancer progression was considered as the interest outcome and other causes of death were considered as competing risks. Cause-specific and sub-distribution hazard models were used to investigate the factors affecting patient survival in the presence of competing risk.
 
Results: The mean (SD) age of the patients was 53.4 (13.9) years and 68 patients (56.2%) were male. The results of log-rank test showed that sex, age, metastasis, type of first treatment, rate of penetration into intestinal wall, tumor location, number of lymphomas involved and tumor size had significant effects on the patient survival (P<0.05). Based on cause-specific and sub-distribution hazard models, tumor stage, lymph node metastasis, and tumor grade had significant effects on death hazard due to the cancer progression (P<0.05).
 
Conclusion: Due to the need to consider competing risks, the results of both competing risk methods showed that tumor grade, lymph node metastasis and stage increased the instantaneous hazard and hazard of cancer death. Therefore, to determine the specific risk factors for each cause of death in the survival analysis, competing risk methods should be used if there is more than one cause of death.

Abstract

Background and Objectives: The aim of this study was to investigate the epidemiological characteristics of patients with Covid-19 in Hamadan Province.
 
Materials and Methods: In this descriptive cross-sectional study, demographic and epidemiological data of all people who presented to hospitals in Hamadan Province from February 2019 to December 2020 were extracted using two checklists. Data were then analyzed using the Stata software.
 
Results: In this study, 9674 covid-19 positive patients were examined. According to results, 49.11% of the cases occurred in the elderly over 60 years. The rate of involvement was higher in females than males (51.57% vs. 48.43%). Moreover, 72.05% of the definitive patients lived in the city and 0.76% reported traveling to areas with a high prevalence of the disease two weeks before the onset of symptoms. The highest incidence of this disease per one hundred thousand population was in Malayer, Hamedan and Nahavand counties and the highest fetality was in Razan and Dargazin, Tuyserkan and Asadabad counties, respectively. The data of suspected, probable, and definite outpatients with Covid-19 presenting to medical centers were not evaluated in this study.
 
Conclusion: Due to the high death rate in the elderly, males, those with underlying diseases, and people living in rural areas, it is necessary to design and apply precautionary measures in these groups. Attention should be paid to these high-risk groups in the shortest possible time to reduce the burden of this disease on individuals as well as the health care system.

Abstract


Background and Objectives: Breast cancer has the highest incidence in the Iranian women.

Methods: A cross-sectional study was conducted. All female with breast cancer during 2008-2015 were enrolled. Breast cancer registration is based on the pathology method in Iran. The information about female with breast cancer was collected from their files in the cancer registry department of Hamadan Health Center. The samples were divided into four groups according to age (<50 and> 50) and location (city, village). GeoBUGS was used to generate a map of high-risk areas in Hamedan Province based on the adjusted relative risk estimate (RR*) in OpenBUGS v 3.2.3 software.

Results: This study included 1316 females with breast cancer. The mean age of the patients was 50.38±12.98 years. The results of the study showed that high-risk areas of breast cancer for were Assadabad urban females aged over 50 years (RR*(i)=1.32, CI= 0.99,1.79) and Tuyserkan (RR*(i)=1.09, CI= 1.08,1.38) and Razan (RR*(i)=1.09, CI= 0.85,1.40) for females below 50 years. In addition, Razan for rural females over 50 years old (RR*(i)=1.18, CI=0.82,1.73) and Malayer for females below 50 years old (RR*(i)=1.08, CI= 0.81,1.45) were high risk areas for breast cancer in Hamadan Province.

Conclusion: The distribution of breast cancer is different at different ages and in the cities of Hamadan Province. Asadabad, Tuyserkan, Razan and Malayer were high risk areas for breast cancer in Hamadan Province.

Abstract

Background and Objectives: The purpose of this study was to ascertain the mortality rate and years of life lost (YLL) resulting from COVID-19 infection in Hamadan Province.
Methods: In this cross-sectional study, information regarding the number of deaths caused by COVID-19 infection was obtained from the Vice-Chancellor of Health at Hamadan University of Medical Sciences. The research period spanned from February 2020 to February 2021. The deaths recorded by the deputy health department encompassed both outpatients and inpatients. The calculation of Years of Life Lost (YLL) was based on the guidelines outlined in the Global Burden of Disease (GBD) 2010. All data analysis was performed using Excel software.
Results: During the study period, a total of 1556 deaths occurred due to COVID-19 infection in Hamadan Province. The years of life lost (YLL) due to premature death were 15783 years (49.99 per thousand) in men, 12794 years (38.43 per thousand) in women, and 28577 years (44.08 per thousand) in both sexes. Specifically, urban areas accounted for 19824 YLL, while rural areas accounted for 8753 YLL.
Conclusion: Based on the findings of this study, COVID-19 infection resulted in a significant loss of potential years of life, particularly among men, those over 60 years of age, and in urban areas. As a result, intervention programs should prioritize the importance of early detection of the disease, reducing its severity, and subsequently mitigating the number of fatalities. Additionally, better control of the disease in elderly populations, who represent the highest proportion of years of life lost, should be a focal point.
