Abstract

Background and Objectives: Incidence of neurocognitive and psychologic disorders in elderly seems to become high There are numerous factors affecting neurocognitive functions like vascular factors, Vitamin B12 and folate and homocysteine levels. The aim of this study was to assess the relation between neurocognitive- psychologic markers and homocystein (Hcy), V.B12 (Cobalamin) and folate in serum.
Methods: Two hundred eighty elderly people admitted to Emam Reza Teaching Hospital in Mashahd, Iran. neurocognitive-psychologic disorders was diagnosed by a trained physician. Serum cobalamin, folate and homocystein measured by RIAand ELIZA methods, respectively.
Results: Based on Hcy>15 µmol/l level, cut off point <330 pg/ml for serum cobalamin and <6.5 ng/ml for folate deficiency defined as deficiency. The correlation between cobalamin and folate was statistically significant (p=0.000). Hemocystein correlated inversely to cobalamin (p=0.001) and folate (p=0.044). There were no statistical significant between Cobalamin, Folate and Hcv>15µmol/l and neurocognitive & psychologic markers unless Deep Tendon reflex test (P=0.04). All patients with dementia had hyperhemocysteinemia.
Conclusions: Despite the plausible biochemical mechanism, further diagnostic studies, based on clinical, neuropsychological, laboratory features will be necessary to better understand this fascinating biochemical challenge.

Abstract

Background & Objectives: Although the population of elderly is increasing in Iran, few studies carried out on this group. The aim of this study was to identify life expectancy and contributory risk factors for the Tehranian elderly of ages above 60 years.
Methods: Individuals above 60 years old whom were recruited in the primary phase of the Tehran Lipid and Glucose Study (TLGS) during 1998-2001 were followed up for 12 years and their vital status were registered (1998-2011). Age and sex mortality rates for age groups (60-69, 70-79, 80+) were calculated and by using Cox proportional hazard model the mean of survival time and hazard rates with respect to risk factors were estimated.
Results: Life expectancy for females and males after crossing 60 years of age reaches to 81 and 80 years, respectively without any statistically significant differences between these two groups. Cox model showed that diabetes, BMI>33Kg/m2 and non ischmecic heart disease reduced survival time in women significantly. While diabetes, smoking, hypertension, ischemic heart disease, history of MI, stroke or sudden death of father, brother or son, lack of physical activity and antihypertensive medications are among the hazardous risk factors for men.
 Conclusion: Among the variables studied, only three (ABC) of them were found as risk factors of women's life, while for men seven risk factors were identified. It seems that more studies are needed to determine the risk factors for women.

Abstract

  Background & Objectives : Suicide is a conscious attempt to end one’s life. Evidence suggests that nearly one million people die due to suicide worldwide each year. The aim of present study was to describe the epidemiological and demographic data of suicide victims and related factors in Fars Province.

  Methods: In this cross-sectional study, the demographic and epidemiological data of suicide victims in Fars Province during the 5-year period from April 2007 to March 2011 was collected by a questionnaire. Finally, the data was statistically analyzed.

  Results : The victims were 934 cases. The mean and standard deviation of age for the cases was 32.12 ± 81.29 years. About 72.5% & 27.3% were men & women, respectively. The most common methods of suicide were hanging (n=472, 5.50%) for men and self burning (n=103, 16%) for women. Most of the people who committed suicide (n=411, 44%) were in the third decade of life (20 to 29 years). The most common etiology (53.5%) was unknown.

  Conclusion: Due to the high incidence of suicide in young people and unmarried individuals, authorities need to consider these groups in social planning in Fars Province.

Abstract

Background and Objectives: Hypertension is a major global issue due to its consequences. Ordinary regression models have limitations in assessment of hypertension since the reference graph derived from a specific population may not be appropriate for another population. The polynomial quantile regression model is considered as a possible alternative. Hence, this study was conducted with the aim of determining reference values as well as blood pressure percentile curves in Mashhad.
 
Methods: This cross-sectional study was carried out in a random sample of 6949 individuals attending Samen health centers for diabetes screening in 2010. Different percentiles were analyzed using some variables such as gender, age, BMI, WHR, and systolic and diastolic blood pressure. The R software (version 3.0.1) was used for data analysis.
 
Results: In this study, 70.58% and 29.42% of subjects were men and women, respectively. The results of the quantile regression model showed that with an increase in age, BMI, and WHR, blood pressure increased in all percentiles. In all variables, subjects in the 75th and 95th percentiles had moderately high and high blood pressure while they had a normal blood pressure in other percentiles.
 
Conclusion: The model provided more information about blood pressure and its related patterns. According to the results, it seems that more attention should be paid to elderly and overweight individuals in the 75th and 95th percentiles.

Abstract

Background and Objectives: Investigation of child mortality is one of the most important strategies for improving children's health. The purpose of this study was to investigate the age distribution, trends, and projections of mortality in children under 5 years old in Khorasan Razavi province.
 
Methods: The study population included under-5 mortality data from Khorasan Razavi Province during 2012-2017 extracted from the Causes and Mortality Classification System of Vice-Chancellery of Health, Mashhad University as well as five universities and faculties. Cause of mortality was classified according to the ICD10 codes. Data were controlled using the ANACod software. Descriptive statistics methods and autoregressive integrated moving average (ARIMA) modeling were applied to explore the mortality trend during the time of study using the Minitab.15 and STATA16.
 
Results: According to the results, the highest mortality rate for children under five was in 2014 and the lowest in 2017. Using the differencing method, the data were stabilized. Finally, the ARIMA model (1,1,2) was identified as a suitable model using the MINITAB software.
 
Conclusion: The mortality rate of children under five has declined sharply in the last four years in Khorasan Razavi Province. It is predicted that this reduction will continue according to fitted model. However, we are still far from mortality rates in developed and some developing countries; therefore, efforts should be made to reduce the under-five mortality rate by increasing the level of health services, the awareness level of families, and improving maternal and childbirth care.

Abstract

Background and Objectives: Due to the high prevalence of COVID-19 disease and its high mortality rate, it is necessary to identify the symptoms, demographic information and underlying diseases that effectively predict COVID-19 death. Therefore, in this study, we aimed to predict the mortality behavior due to COVID-19 in Khorasan Razavi province.
Methods: This study collected data from 51, 460 patients admitted to the hospitals of Khorasan Razavi province from 25 March 2017 to 12 September 2014. Logistic regression and Neural network methods, including machine learning methods, were used to identify survivors and non-survivors caused by COVID-19.
Results: Decreased consciousness, cough, PO2 level less than 93%, age, cancer, chronic kidney diseases, fever, headache, smoking status, and chronic blood diseases are the most important predictors of death. The accuracy of the artificial neural network model was 89.90% in the test phase. Also, the sensitivity, specificity and area under the rock curve in this model are equal to 76.14%, 91.99% and 77.65%, respectively.
Conclusion: Our findings highlight the importance of some demographic information, underlying diseases, and clinical signs in predicting survivors and non-survivors of COVID-19. Also, the neural network model provided high accuracy in prediction. However, medical research in this field will lead to complementary results by using other methods of machine learning and their high power.

Abstract

Background and Objectives: Cutaneous Leishmaniasis is a significant endemic diseases in Iran, leading to skin lesions, lifelong scars, and social stigma. This study aims to investigate the influence of climatic and ecological factors on the prevalence of cutaneous Leishmaniasis in Iran.
Methods: This study employed a narrative review approach. A comprehensive search was conducted using key terms such as 'skin Leishmaniasis', 'cutaneous Leishmaniasis', 'climatic factors,' and 'environmental factors' in both national and international databases. All relevant research was included without limitations on location, time, or research methodology.
Results: The initial search yielded 823625 articles. After refining the search keywords and conducting initial screening, 184 studies remained following the removal of duplicate articles. Subsequent screening for eligibility further narrowed down the selection to 30. The inclusion of 6 master theses brought the final number of studies for review to 36. A review of the studies showed a significant association between certain climatic factors, including temperature, humidity, hours of sunshine, and rainfall, and the incidence of cutaneous Leishmaniasis, across various geographical regions in the country. The relationship between factors such as vegetation, wind, and the number of disease cases had different results in different parts of the country.
Conclusion: The results of the present study show the effect of climatic and environmental factors on the rate of cutaneous Leishmaniasis in the country. Considering that the effect of these factors is not the same in all parts of the country, it is necessary to implement effective preventive measures to reduce the disease burden according to the needs of each geographical area.

Abstract

Background and Objectives: Evidence suggests that one of the behaviors impacted by the quarantine measures enforced due to the COVID-19 pandemic is sexual activity. This study aims to investigate this phenomenon.
Methods: A descriptive-analytical study with before-after measurement was conducted during both the COVID-19 pandemic and post-pandemic periods in 2021-2022, utilizing the standard Sexual Performance Index questionnaire administered to 250 women in Ilam province. Inferential statistics including paired t tests, correlation and chi square analysis were performed by SPSS software.
Results: The findings revealed that the COVID-19 pandemic had an impact on all six components of sexual function in women of reproductive age, including sexual desire, sexual stimulation, lubrication, pleasure, satisfaction and pain. The average overall sexual performance during the COVID-19 period (2.01±1.25) and in the post-COVID-19 period (2.94±1.2) was lower than the cutoff point (3.8), indicating poor performance across the entire scale of sexual function. The results showed that there is an inverse and significant (P<0.05) relationship between age, duration of marriage and number of pregnancies with sexual function index and its subscales.
Conclusion: The study findings suggest that research population experienced weaker sexual performance during the COVID-19 pandemic compared to the post-pandemic period, highlighting the impact of the mental and physical burdens associated with the pandemic on sexual function.

Abstract

Background and Objectives: Childhood obesity has become a global challenge today. Many studies have shown the relationship between obesity and socioeconomic factors. Therefore, this study aimed to evaluate socio-economic inequalities with nutritional patterns and obesity in children.
Methods: This study was conducted using the structural analysis methodology on 80 children from 6 to 13 years old, selected randomly from those referred to the health assessment centers of Sonqor and Kolyai, Iran. The body mass index was calculated using the standard method. Nutritional patterns were measured using the 24-hour food recall questionnaire, and socioeconomic status was assessed using related standard questionnaires. The data were analyzed using SPSS 24 and AMOS 24 software.
Results: The results showed that 28.75% of the subjects were obese or overweight. The structural analysis showed that the socioeconomic variable directly affected the nutritional pattern and body mass (-0.43) with an impact coefficient of as much as 0.65. The nutritional pattern variable affected BMI with an impact factor (-0.74). The bootstrap test results indicated that the significant effect of socio-economic status on BMI is mediated by nutritional pattern (-0.48). Prediction values show two economic-social and nutritional pattern variables predicted 0.16 and 0.29 BMI changes, respectively.
Conclusion: The findings of this study showed the effect of socioeconomic status on nutritional patterns and body mass index in the research community. Today, The issue of increasing body mass in the world and our country is one of the most important social challenges. Considering the heterogeneous effects of socioeconomic status on nutritional patterns and body mass index, it is necessary to formulate and implement preventive policies according to the conditions of the communities to achieve effective results.
