Abstract

Background and Objectives: There are conflicting data on the effects of different types of fat on aortic atherosclerosis. This study used Wister rats to compare the atherogenic effects of a standard diet with a diet containing fish oil and another diet containing mixed oils (typical of the current consumption pattern in Iran).
Methods: Thirty female Wistar rats were randomly assigned to three diet groups: a standard diet (containing soy bean-oil as the only dietary fat), a diet containing fish oil and another diet which contained mixed oils and was designed on the basis of the current fat consumption pattern in Iran (butter, hydrogenated vegetable oil and sunflower oil). Mothers in each group were fed the same diet during pregnancy and lactation the litters were also weaned onto the same diet. Aortic samples were taken from the offspring at the time of weaning and puberty. Data were analyzed using SPSS software Kruskal-Wallis test was applied for comparison among groups.
Results: Rats that were fed soy bean oil showed less extensive fatty streaks and intimal thickening of aorta on weaning and at puberty. Rats fed fish oil and mixed oils showed more atherosclerosis lesions compared to those receiving soy bean oil.
Conclusions: It seems that diets with an omega-3 to omega-6 ratio of 0.6 can enhance aortic atherosclerosis. Diets with an Iranian fat consumption pattern also increase aortic atherosclerotic lesions because they contain substantial amounts of saturated fat.

Abstract

Background & Objectives: This study aimed to determine of the seroprevalence of visceral leishmaniasis to design a prevention and control program in Bojnoord district.
Methods: This was a descriptive and cross sectional study with multistage random sampling method. The study was carried out on children up to 12 years old and 10% of adult’s population in 8 rural areas of Bojnoord district in 2007. In total, 1608 blood samples were collected to detect anti-Leishmania antibodies. All the samples were tested by direct agglutination test (DAT). In order to determine Leishmania species, necropsy was performed on four suspected dogs in the studied areas. The species of Leishmania was determined by RAPD-PCR and PCR-RFLP methods using to amplify the ribosomal internal transcribed spacer 1 (ITS1).
Results: Thirty and eight (2.36%) out of 1608 collected blood samples had anti-Leishmania antibodies at titer 1:800 and nine (0.56%) were just positive at 1:3200 by DAT. There was no statistically significant difference between female and male seroprevalence (p<0/05). The seoprevalence in children <=12 years old compared to individuals greater than 12 years old did not show any statistically significant (p<0/05). Amastigotes were observed in all 4 necropsied dogs. The species of Leishmania isolated from 2 dogs, was determined as L.Infantum. Their ITS1 sequences were registered with Accession numbers EU810776 and EU810777 in NCBI.
Conclusions: These findings showed that visceral leishmaniasis has been circulated with low endemicity in Bojnoord district. Therefore an appropriate monitoring system is needed for health services in this area.

Abstract

Background & objectives: statistical modeling explicates the observed changes in data by means of mathematics equations. In cases that dependent variable is count, Poisson model is applied. If Poisson model is not applicable in a specific situation, it is better to apply the generalized Poisson model. So, our emphasis in this study is to notice the data structure, introducing the generalized Poisson regression model and its application in estimates of effective factors coefficients on the number of children and comparing it with Poisson regression model results.
Methods: Besides introducing Poisson regression model, we introduced its application in fertility data analysis. A sample of 1019 women in rural areas of Fars was selected by cross sectional and stratified sampling methods. The number of children of family was determined as a count response variable for model validation.
Results: The sample mean and sample variance of the response variable Y, the number of children, are respectively 4.3 and 8.3 (over-dispersion). Log-likelihood was -1950.93 for Poisson regression and -1946.93 for generalized Poisson regression model.
Conclusions: The results revealed that this data have over-dispersion. According to selection criteria, the suitable model for this data analysis was generalized Poisson regression model. It can estimate effective factors coefficients on the number of children exactly.

Abstract

  Background & Objectives : Suicide is a conscious attempt to end one’s life. Evidence suggests that nearly one million people die due to suicide worldwide each year. The aim of present study was to describe the epidemiological and demographic data of suicide victims and related factors in Fars Province.

  Methods: In this cross-sectional study, the demographic and epidemiological data of suicide victims in Fars Province during the 5-year period from April 2007 to March 2011 was collected by a questionnaire. Finally, the data was statistically analyzed.

  Results : The victims were 934 cases. The mean and standard deviation of age for the cases was 32.12 ± 81.29 years. About 72.5% & 27.3% were men & women, respectively. The most common methods of suicide were hanging (n=472, 5.50%) for men and self burning (n=103, 16%) for women. Most of the people who committed suicide (n=411, 44%) were in the third decade of life (20 to 29 years). The most common etiology (53.5%) was unknown.

  Conclusion: Due to the high incidence of suicide in young people and unmarried individuals, authorities need to consider these groups in social planning in Fars Province.

Abstract

  Background & Objectives : The identification of risk factors and their interactions is important in medical studies. The aim of this study was to identify the interaction of risk factors of cerebral palsy in 1-6 years-old children with classification regression methods.

  Methods : The data of this cross-sectional study which was conducted on 225 children aged 1-6 years was collected during 2008- 2009. Classification regression methods (classification and regression tree (CART), adapting boosting (AdaBoost), bagging, and C4.5 algorithm) were used to identify interactions between risk factors. Data analysis was carried out with R3.0.1 software.

  Results : The identified interactions of the factors by a) the AdaBoost method were (consanguinity: sex, previous pregnancies: vaginal delivery, consanguinity: sex: preterm, history of the disease: preterm: asphyxia, consanguinity: sex: asphyxia, history of the disease: sex: small size relative to gestational age, neonatal infection: asphyxia: small size relative to gestational age, history of the disease: sex: asphyxia, preterm: asphyxia: vaginal delivery) by b) the bagging method were (consanguinity: asphyxia, consanguinity: preterm: asphyxia), by c) the C4.5 algorithm were (asphyxia: preterm, asphyxia: consanguinity: history of the disease: preterm), and by d) the CART method were (asphyxia: consanguinity). The sensitivity and specificity of the AdaBoost method was better than other methods (0.941±0.029 and 0.951±0.030, respectively).

  Conclusion : The AdaBoost method could better recognize and model potential interactions between risk factors of cerebral palsy.

Abstract

Background and Objectives: Breast cancer is one of the most common cancers among women and is the second main cause of death after lung cancer. The objective of this study was to use the Bayes model to analyze the prognostic effects on the survival of the women with breast cancer after surgery in the south of Iran.

Methods: The date was collected 1192 women who had breast cancer in Namazi Hospital Research Center between 2001 and 2006. The complete information of only 1148 of them was registered. Parametric Bayes and Bayes Cox methods were used. Considering 0.05 as the level of significance, the data analysis was done using the WinBUGS14 software.

Results: The mean age of the patients (at the time of diagnosis) was 47 years in this study. Cox one-variable analysis showed a significant relationship between survival and smoking (P=0.009), bone metastasis (P=0.01), the number of lymph nodes (P=0.001), the tumoral level of malignancy (P=0.001), the surgical method (P=0.015), financial status (P=0.025), and the tumor size (P=0.001). By fitting Bayes models the variables tumor size, level of malignancy and number of lymph nodes were significant.   

Conclusion: The results showed that clinicopathological features of cancer had a significant role in the survival of the patients.

Abstract

Background and Objectives: The cancer of the pancreatic head and Ampulla of Vater is a malignant disease usually seen in advanced stages with symptoms caused by stomach and biliary obstruction. Curative treatment is possible in the early stage. Unfortunately, most symptomatic patients are in the advanced stage and have an unrespectable tumor; therefore, they should undergo palliative surgery. This study was performed to determine the complications and survival rate of patients who underwent palliative surgery for advanced stages of cancer in the Ampulla Vater and pancreatic head.

Methods: In this study, 49 patients with advanced stages of cancer in the Ampulla of Vater and pancreatic head who had undergone palliative surgery from 2003 to 2014 at Baqiyatallah Hospital were studied and the complications and survival rate were determined.

Results: Eleven patients (22.44%) underwent biliary bypass, 6 (12.24%) underwent gastric bypass, and 32 (65.32) underwent both procedures. Twelve (24.49%) patients developed complications. Anastomotic leak and peritonitis were the most common complications seen in 6 (12.2%). Seven patients (14.28%) died in the hospital. The mean survival was 5.47 ± 8.38 months. Upon follow up, survival was significantly longer in older patients (P=0.01).

Conclusion: In one-fourth of the patients with advanced stages of cancer, palliative surgery of the pancreatic head and Ampulla of Vater caused complications, and the mean survival rate was less than six months. The results of this study recommend the use of less invasive procedures such as biliary stenting in the advanced stages of the disease.

Abstract

Background and Objectives: Colon cancer is the third most common cancer in the world and the fourth most common cancer in Iran. It is very important to predict the cancer outcome and its basic clinical data. Due to to the high rate of colon cancer and the benefits of data mining to predict survival, the aim of this study was to survey two widely used machine learning algorithms, Bagging and Support Vector Machines (SVM), to predict the outcome of colon cancer patients.
Methods: The population of this study was 567 patients with stage 1-4 of colon cancer in Namazi Radiotherapy Center, Shiraz in 2006-2011. Three hundred and thirty eight patients were alive and 229 patients were dead. We used the Support Vector Machines (SVM) and Bagging methods in order to predict the survival of patients with colon cancer. The Weka software ver 3.6.10 was used for data analysis.
Results: The performance of two algorithms was determined using the confusion matrix. The accuracy, specificity, and sensitivity of the SVM was 84.48%, 81%, and 87%, and the accuracy, specificity, and sensitivity of Bagging was 83.95%, 78%, and 88%, respectively.
Conclusion: The results showed both algorithms have a high performance in survival prediction of patients with colon cancer but the Support Vector Machines has a higher accuracy.

Abstract

Background and Objectives: Congenital heart disease (CHD) is the most common type of birth defect that accounts for 25% of all congenital anomalies. This study was conducted to identify the risk factors od congenital heart disease in southeast Iran.
 
Methods: In this case-control study, 353 cases were selected from children aged 0 to 59 months who suffered from congenital heart disease and were referred to the Children’s Heart Clinic of Zahedan. Moreover, 353 controls were selected from healthy children aged 0 to 59 months who presented to health centers in Sistan and Baluchistan Province. The cases and controls were matched for age, sex and place of residence. The data were collected using interviews with children’s mothers and analyzed using independent t-test, chi-square test, and multiple logistic regression models.
 
Results: This study showed that the lack of folic acid consumption in pregnancy (OR =11.8), mot using multivitamins during pregnancy (OR = 4.1), history of CHD in first-degree relatives of parents (OR=3.4), history of abortion (OR =3.4), presence of telecommunication rig in the vicinity of the house (OR=3) and exposure to secondhand smoke (OR=2.9) significantly increased the chance of a CHD (P <0.05).
 
Conclusion: Effective planning, emphasis on the use of supplements during pregnancy, and improved awareness of the society, especially high risk women, can be helpful in decreasing CHD in this region. Providing education regarding preventive factors seems to be necessary for health and medical workers to control risk factors and reduce costs associated with CHD.

Abstract

Background and Objectives: On 12 November 2017, an earthquake with a magnitude of 7.3 on the Richter scale occurred in Kermanshah Province. Explaining the challenges and needs of the victims in such circumstances can be an important source of information for future planning and better management of such conditions. Thus, the purpose of this study was to explain the challenges and needs of the earthquake victims.
 
Methods: This study was a qualitative study. Twenty-nine semi-structured interviews were conducted with managers, policymakers, and health and emergency relief providers in earthquake areas, and 10 focused group discussions were held with earthquake victims from December to February 2016. The data were analyzed using conventional content analysis.
 
Results: The needs and challenges in Kermanshah earthquake were classified into three periods after the earthquake: 1. Challenges and needs in the first few days with three sub-categories including management, rescue services, and urgent care, 2- Challenges and needs in the first few weeks  after the earthquake with three sub-categories including housing and emergency assistance, public health services, and access to health care; and 3. Needs and challenges in several months after the earthquake with four sub-categories including economic, social, infrastructural and health service needs.
 
Conclusion: Lack of unity of command was the main challenge in this earthquake. Another point was the health needs of victim earthquakes over time. Therefore, dispatching of aid forces to the affected areas during natural disasters should be based on planning and needs.

Abstract

Background and Objectives: Reports indicates a higher mortality rate of Covid-19 in the elderly. A limited number of studies have been performed on elderly patients in Iran. In the present study, we investigated the clinical features and survival of elderly patients with Covid-19 in Baharloo Hospital, Tehran, Iran.
 
Methods: This retrospective cohort study was conducted on the data of elderly patients with Covid-19 admitted to Baharloo Hospital, Tehran, Iran. The data of the hospitalized patients were followed until discharge or death. Finally, survival-related factors were analyzed using Cox regression.
 
Results: A total of 522 patients were enrolled in the study, of whom 53.8% were male and 101 died. The mean age of the deceased was 76.3 years, with a standard deviation of 8.6 years. The most common clinical symptoms at the time of admission were respiratory distress (69.5%), cough (61.1%), and fever (51.1%). Cox regression results showed that the risk of death increased by 5% for each one-year increase in age. In addition, calcium supplementation reduced the risk of death by 35%.
 
Conclusion: The results of the present study showed that calcium supplementation could be associated with increased survival of the elderly and reduced mortality. Due to calcium insufficiency in the Iranian population and the protective role of calcium in Covid-19, this issue is essential in the elderly patients warranting further investigation in the future.

Abstract

Background and Objectives: Valid, reliable, short, sensitive and accurate tool is needed to diagnose pain in dementia patients. This study aimed to determine the psychometric properties and diagnostic accuracy of short-form Persian version of Doloplus-2 (P-Doloshort) in pain assessment in elderly with dementia.
 
Methods: In this cross-sectional study, 100 elderly were selected using convenient sampling from private clinics and nursing homes in Kashan in 2018-19. Construct validity was determined by exploratory factor analysis, known-groups approach and convergent validity. Also, reliability was evaluated by internal consistency, equivalence, and stability. P-Doloplus-2 and P-Doloshort were used as the reference standard and the index test, respectively. Data were analyzed by SPSS-v16 using independent T-Test, linear regression, Pearson and intra-class correlation coefficients (ICC), Cronbach's alpha, ROC curve, and diagnostic test parameters.
 
Results: Factor analysis identified the tool as a single-factor; it was able to explain 65.851% of total variance. P-Doloshort was able to differentiate between two groups with and without painful underlying diseases (P<0.0001). There was a positive and significant correlation between P-Doloshort score with P-Doloplus-2 score (r=0.975, P<0.0001) and PACSLAC-II-IR score (r=0.0001, P<0.0001). Cronbach's alpha, ICC and standard error of measurement were 0.898, 0.891 and ±1.183, respectively. The relative frequency of minimum and maximum possible score was less than 15%. At the cut-off point of 2.5, area under ROC curve, sensitivity, specificity, and overall diagnostic accuracy were estimated 0.985, 0.956, 1.000 and 0.970, respectively.
 
Conclusion: P-Doloshort can be used as a valid, reliable, sensitive and accurate tool for pain assessment in elderly with dementia.

Abstract

Background and Objectives: The disease surveillance system provides essential information about the population at risk and the disease pattern. This review aimed to describe the experiences of countries in establishing COVID-19 school-based surveillance systems.
Methods: We conducted a systematic review. Four databases were searched between January 2019 and December 2022 using relevant keywords. The studies were screened by two people according to the inclusion and exclusion criteria. The findings were extracted using a standard form and aligned to the objectives of the review.
Results: The data from 12 studies were extracted using the standard form. All studies related to the school-based surveillance system of COVID-19. Most of studies were conducted in the United States of America and England. The reports did not conform to the standard. The number of schools covered in surveillance systems ranged from 2 to more than 6000 schools. The age group in these studies was 0 to 19 years. Schools submitted data daily or weekly.
Conclusion: The results of the COVID-19 surveillance systems in schools should be reported according to standard Instructions. This is considered a necessity to monitor and evaluate the surveillance system. It also allows other countries and researchers to share and use the results. In addition, sensitivity, timeliness, and positive predictive value were not reported in implemented surveillance systems.

Abstract

Background and Objectives: During the COVID-19 pandemic, vaccination was crucial in preventing the spread of SARS-CoV-2 and saving numerous lives. Countries implementing COVID-19 vaccination programs have reported significant reductions in cases, ICU admissions, and COVID-19-related deaths. This study aimed to evaluate the effectiveness of vaccines used in Hamadan province, explicitly focusing on their impact on hospitalization and death caused by COVID-19.
Methods: A test-negative case-control design (TND) was conducted involving patients aged 12 and above who were admitted to hospitals in Hamadan province, Iran, and had symptoms of acute respiratory diseases. Data were extracted from hospital and health system databases. Multiple logistic regression analysis was performed to estimate vaccine effectiveness for the first, second, and reminder doses in prevention of hospitalization, and severe outcomes (ICU admission or death).
Results: The study was conducted on 3,702 patients, and the maximum effectiveness of vaccines against hospitalization was 50% for patients who received a booster dose. The effectiveness of the first dose of vaccine on severe outcomes (admission to ICU or death) was estimated as 42%, but the effectiveness of the vaccines in the second and booster doses were not significant.
Conclusion: Despite the predominant use of inactivated virus vaccines and delayed initiation of vaccination in Iran, this study shows the effect of vaccination on reducing hospitalization and improving the outcomes of COVID-19. The use of more effective vaccines at a more appropriate time plays an important role in reducing the burden on health services and preventing further transmission in future epidemics.
