Abstract

Background & Objectives: Pediculosis is a ubiquitous and contagious parasitic dermatosis. Throughout the world, infestation by the head louse (Pediculus humanus capitis) is more common among schoolchildren, especially in those aged 6-11 years. This descriptive/analytical study was carried out in 2003 to determine the prevalence of pediculosis capitis and risk factors involved in the epidemiology of pediculosis in primary school pupils in Amlash, Gilan province.
Methods: The children were selected by cluster random sampling of schools and classes, and then examined for head lice using hair conditioners and a fine-toothed head lice comb. We screened a total of 4244 pupils from 95 primary schools. We used a comprehensive questionnaire to evaluate the effects of school type and location, education level, parent job, regular baths, number of persons sharing the same bed, combing frequency, host sensitivity, family size, hairstyle, gender, and hair length.
Results: Statistical analysis using χ2 tests showed that school type, school location, parent job, regular baths, number of person who share the same bed, frequency of combing, gender, host sensitivity, family size, and hairstyle are significantly associated with infestation. Infestation rate showed no correlations with hair color, season, age, or education level. Results showed that pediculosis is more prevalent in rural than in urban areas. The average rate of infestation for the area was 9.2%, indicating an epidemic situation by standard criteria.
Conclusions: The high prevalence of P. capitis infestation among these children was probably due to poor environmental hygiene and scarcity of water. Hygienic controls of schoolchildren by nurses are important for elimination of Pediculus humanus capitis. This is the first community-based study describing in detail the epidemiology of head louse infestation in the Amlash district of Gilan province in Iran and confirms that Pediculosis capitis is still a problem in many environments, particularly those with low life standards and poor health care.

Abstract

Background & Objectives: Rabies is one of the oldest and most lethal zoonoses in the world. Cases have been reported from most provinces of Iran, involving both domestic and wild animals. This study aimed to define the epidemiology of rabies in human and animal populations in the western province of Ilam and was conducted in all of the 7 cities of the province.
Methods: In this descriptive study we recorded data on the number of the persons bitten by animals, geographical location, age category, occupation and professional relationships, pre-exposure prophylaxis, treatment for rabies, conditions of the injured organs, and the mortality rate. Data were analyzed using the SPSS software package.
Results: There were 4 human deaths from rabies in this province (2 men and 2 women) three of these cases had been bitten by dogs and the remaining one by a cat. The mean age of these people was 25.2 years (SD=15.3y). A total of 2431 people had been bitten by animals over the preceding 10 years. Most such injuries happened in the 10-29y age group and the frequency was greater for males (P<0.01). About 64% of all cases had been reported from rural areas, and the greatest number of injuries occurred in winter and spring. The most common sites of injury were hands and feet (88.5%), trunk and the cervical region (9.3%) and head/ face (2.2%).
Conclusion: The increasing number of stray dogs and the movement of dogs and wild animals into cities and villages are important factors in the transmission of rabies. More research needs to be done on the eco-epidemiology of the disease in different cities of Ilam.

Abstract

Background & Objectives: Recognition of epidemiology of acute myocardial infraction (AMI) in every region could be a need for design the programs for prevention of AMI. In this study we assessed the epidemiology of changes in AMI from 2002-2006 in Birjand(east of Iran).
Methods: All of the patients which hospitalized by AMI diagnosis from 2002-2006 years, included in this study. We compared mean of age, gender, location of living, medications used, condition at discharge and cardiac risk factors in this period.
Results: From 1233 patients, 880 were male (71.4%). Overall, mean age of patients was 62/2±12/8 years and in five years was increased (from 61.7±13/2 in 2002 to 63.5±12/4 in 2006 & p=0.37). The most risk factors in patients were: Hypertension (HTN) (34/8%) ,cigarette smoking (32/1%) & dyslipidemia (24%). These risk factors did not show any significantly changes during 5 years. But the the prevalence of HTN (30/9% in 2002 to 38% in 2006, p=0/62) and DM (15/5% in 2002 to 21% in 2006, p=0/23) raised. Cardiac drugs use tended to increase during 5 years, however only this increase about STK & Statin was statistically significant. The hospital mortality rate was decreased in 5 years (11.3%) but it was not statistically significant (p= 0.48).
Conclusions: Regarding the accelerating the rate of AMI, designing an appropriate prograrme for prevention and modifying cardiac risk factors is recommended.

Abstract

Background & Objectives: This study aimed to determine of the seroprevalence of visceral leishmaniasis to design a prevention and control program in Bojnoord district.
Methods: This was a descriptive and cross sectional study with multistage random sampling method. The study was carried out on children up to 12 years old and 10% of adult’s population in 8 rural areas of Bojnoord district in 2007. In total, 1608 blood samples were collected to detect anti-Leishmania antibodies. All the samples were tested by direct agglutination test (DAT). In order to determine Leishmania species, necropsy was performed on four suspected dogs in the studied areas. The species of Leishmania was determined by RAPD-PCR and PCR-RFLP methods using to amplify the ribosomal internal transcribed spacer 1 (ITS1).
Results: Thirty and eight (2.36%) out of 1608 collected blood samples had anti-Leishmania antibodies at titer 1:800 and nine (0.56%) were just positive at 1:3200 by DAT. There was no statistically significant difference between female and male seroprevalence (p<0/05). The seoprevalence in children <=12 years old compared to individuals greater than 12 years old did not show any statistically significant (p<0/05). Amastigotes were observed in all 4 necropsied dogs. The species of Leishmania isolated from 2 dogs, was determined as L.Infantum. Their ITS1 sequences were registered with Accession numbers EU810776 and EU810777 in NCBI.
Conclusions: These findings showed that visceral leishmaniasis has been circulated with low endemicity in Bojnoord district. Therefore an appropriate monitoring system is needed for health services in this area.

Abstract

Background & Objectives: Cytomegalovirus (CMV) is a one the cause of congenital illness and disability, including hearing loss and mental retardation. CMV is a herpesvirus which establishes a persistent life-long infection. Since the prevalence of antibodies against the virus varies in different geographical areas in the world, we decided to identify the seroepidemiology of CMV infection.
Methods: A descriptive study was conducted among 360 volunteer asymptomatic healthy female students in Islamic Azad University of Kazeroun. The men age of subjects was 20.8 years. For serological studies five (ml) of blood sample was collected and the serum was isolated by centrifugation. Enzyme linked immunosorbent assay (ELISA) was used for determination of immunoglobulin G (IgG) and immunoglobulin M (IgM) antibodies of CMV. Finally the results were analyzed by statistical methods.
Results: Overall, CMV-IgG antibody was positive in 340 persons (94.4%) out of 360 subjects and they have a previous infection. However, Only four persons (1.1 %) were determined to have IgM antibody. Also we couldn’t find any significant difference concerning their fields of study and their residency (p>0.05).
Conclusions: With considering the high rat of CMV infection rate in young female, it is suggested to have appropriate preventive programs for women particular in susceptible groups.

Abstract

Background & objectives: Head injuries are the most sever kind of injuries in accidents. It is an important factor of disability in society. Having information about present situation and its determinant factors is the first step of programming for head injury reduction. This survey aimed to have an epidemiological aspect of head injury in Arak.
Methods: Relevant informations about severity of the impact, causes, demographic information and treatment results were extracted from medical files between 2005-2006 years.
Results: Of total 1105 patients with head injury 25.8% were in their first decade of life and 11.8% were reported as sever head injury. The most common injuries (18.4%) caused by downfall from low height. Men had more head injured 64.7% compared with women. In the period of hospitalization 12.8% of moderate head injuries and 43.1of sever head injury caused to death.
Conclusions: It is concluded that applying and administration of the safety rules in the society as a compulsory rules and educating by mass media, are necessary. It should also be taken into account more effective hospital care for sever head injuries to reach better outcomes.

Abstract

Background & Objectives: the aim of this study is seroepidemiological survey of human Hydatidosis by ELISA method in East-Azarbaijan province. Methods: Totally 1500 serum samples were collected from patients referred to different health centers in 8 cities of East-Azarbaijan province. All the sera were examined using ELISA test.
Results: Nineteen cases (1.28%) were positive for human hydatidosis in East-Azarbaijan province using ELISA test. From these cases, 8 cases (0.93%) were from urban areas and 11 cases (1.8%) were from rural areas. 1.76% of females and 0.83% of males were positive and age group 30-40 years old had the highest rate of positivity (1.66%). There were no statistically significant different between gender and age groups and residency area.
Conclusion: According to results of this study the highest rate of positivity was seen in Sarab city (2.17%). The possible reason of this difference could be to live the highest rate of rural population of East-Azarbaijan province dwelling in this city.

Abstract

Although this concept is not specified to Medical Sciences and Epidemiology, the importance of this issue is more highlighted in the field of epidemiology. Causation is the most basic concepts in empirical sciences and is still under discussion because it is dependent on the basis of any scientific laws without acceptance something cease causality is impossible. With the increasing development of science as well as epidemiology, causality has found a broader concept and its application in analytical studies and logical interpretation of the results of this type of study, has a wider dimension. Due to developing new epidemiology courses at medical universities and increase the number of students, it is felt to talk more about the causality concept. In this review causality concepts in the humanities is overviewed, its history is briefly described, the causality of Medical Epidemiology and also Islamic religion is considered, then the causality framework, and models to interpret the conventional causality will be discussed.

Abstract

Normal 0 false false false EN-US X-NONE AR-SA Background & Objectives: Dental fluorosis is a kind of hypoplasia, enamel and dentin deficiency due to fluoride overuse. Since there is little known about this important dental problem in Southeast area in Iran we designed an analytic descriptive study to estimate the dental flurosis prevalence in 7-10 years old students.
Methods: Three hundred thirty four (334) children attending to community dentistry center during year 2006, were studied to evaluate the prevalence of dental fluorosis. Dental index (fluorosis rates) identified by a trained dentist. Criteria for definite diagnosis were existence of enamel defects with spread bilaterally and symmetrically. Children with hyperplasia tetracycline -induced, acute febrile illness in childhood and fetal birth  dental defects (Dentinogenesis imperfecta, Amelogensis imperfect) were excluded from this study.
Results: The prevalence of dental flurosis estimated to be 30.8 percent in this sample. Dental fluorosis protests in the subjects were so: enamel opacities (70. 6%),discoloration(14.2%), pit(1.36%), enamel opacities and discoloration(12.9%),enamel opacities and pit (1 %). Superior anterior teeth were the most effected by fluorosis. There are significant difference in dental fluorosis between boys and girls (P=0.004), and intensity was higher in boys (37.5% versus 22.6%). fluorosis pattern no clear difference in both sexes (P=0.27).
Conclusion: The prevalence of fluorosis was estimated 30.8% and intensity was higher in boys in Zahedan. Therefore it is essential that fluoride source and amount of fluoridation of drinking water be considered in Zahedan city.

Abstract

Background & Objectives: Burns is one the important injuries which may result vital impairment and disability. Burn injuries are still common particularly in developing countries. Assessment of the epidemiology of burns is very important to make effective preventive methods. This study was conducted in Fars province to assess and describe the epidemiology of burns including its main causes, and its demographic factors.
Methods: All epidemiological data of patients with burn admitted to the hospitals in Fars province during 2010 were analyzed in this descriptive study.
Results: Total of 328 patients with mean age 25.93±17.94 were included. It consisted of 178 male patients (54.3%) and 150 female patients (45.7%), The most common accident age was 26 years, the average percentage of burn was 36.63± 26.63%, average hospital stay in hospital was 13.40±12.72 days, 249 cases had unintentional injury (91/75%) and 79 cased had intentional (24.9%), The main reason for  intentional burns was divorce (16.76%), the most common month accident was December, the overall cumulative incidence of burn injuries was 7.28% in 100000 people (person per year) and the burn fatality rate was 29.88%
Conclusion: Epidemiology of burn injuries in Fars province indicates that effective public health programs on this issue would help to reduce the incidence of burn injuries in this region.

Abstract

Background & Objectives: Convulsion is the most common pediatric neurological problem that requires special attention in this group. The purpose of this study was to determine the frequency of seizures and to evaluate its risk factors in children in Bandar Abbas, Iran
Methods: A retrospective study was conducted on 222 children with seizures admitted to children hospital during 2008.
Results: Of total children hospitalized due to seizure, 64.5% were male with age range 1-2 years. Fever was the most cause of seizure. In 58.8% of cases, the child's age at the time of first convulsion was 1-3 years. The seizure attack occurred in wake up in 63.8% of cases, and 65.8% of the samples had normal consciousness at the time of admission. The mean duration of seizure in 64.1% of cases was less than 5 minutes and type of seizure in 80.1% has been reported as generalized tonic colonic. A history of convulsion in other family members was reported in 28.9% and in 19.36%, there was close family relationship between parents. There was no significant relationship between type of delivery, gestational age and birth weight with type and duration of seizure (P>0.05).
Conclusion: Particular attention should be paid to children with seizure during history-taking and be more vigilant to high-risk patients and those who might need prophylactic anticonvulsants.

Abstract

Background & Objectives: One of the most common anxiety disorders in children is separation anxiety disorder (SAD). The purpose of the present study was to investigate the epidemiology of separation anxiety disorder in Isfahan primary school male student.
Methods:
The present study was a descriptive study. The statistical population was all the male students from 7 to 9 years old, studying in second and third grades in primary schools of Isfahan. The sample of study consisted of 1514 male student, who were selected randomly by multi-stage sampling method. The instruments of this study consisted of the Separation Anxiety Disorder Scale, Spence Children's Anxiety Scale and Clinical Interview. Data obtained were analyzed by using descriptive statistics and chi-square and logistic regression.
Results: The findings showed that the rate of separation anxiety disorder in male (7 to 9 year- old student) was %6/93. SAD were more common in 7 years old (7.4%). Children having record of hospitalization, immigration, single-parent households and close relatives of mortality had a higher rate of separation anxiety disorder.
Conclusion: This study shows that separation anxiety disorder among primary school male students is significantly high. Therefore it is important to consider the potential utility of early anxiety prevention/intervention programs, especially for children at this age group.

Abstract

Background & Objectives: The scorpionism is one of the important public health problem in several parts of the world becauseeither incidence or severity of envenomations is high and managed with difficulty by health services. The aim of this study was to describe the epidemiological and demographic information among humans stung by scorpion in Ramhormoz, Iran.
Methods: In this analytic-descriptive study relevant information were gathered from Imam Khomeini hospital in Ramhormoz, from April 2007 to April 2008.
Results: There were 3799 scorpion victims, 56% were from rural areas. Approximately 20 percent of the sting cases occurred in the month of July (the height temperature in this region). Near 50 percent of victims were between 11-30 years. Most of the stings were seen in exposed extremities (78.4%) mainly in the lower limbs (40.5%). Stings mainly occurred at night between 6 p.m. and 6 a.m (44.3%). The scorpions’ spices were, 53.9% yellow, 12.5% black and 34.6% unknown colors, respectively. The median time from sting to admission to the emergency room (patient delay) was 1.5 hour.
Conclusion: It is suggested that informing people with educational campaign about the importance of receiving prompt attention following a scorpion sting has potential value in reducing complications in the emergency room. We would advise that public awareness and physician readiness combined with the availability of effective antivenom significantly reduced lethality in this region.

Abstract

Background & Objectives: Chronic pain is one of main public and individual health problems and its epidemiological understanding needs reliable estimates of prevalence. The aim of this study was to investigate the epidemiology of chronic pain in all 368 neighborhoods of Tehran using small area estimation method.
Methods: The pain section from the second round of Urban HEART data from a selected individual of 23457 households in Tehran using a multistage randomized cluster sampling in 2011, were analyzed. In order to obtain reliable estimates for chronic pain prevalence at neighborhood level, a generalized linear mixed model and hierarchical Bayesian approach were used and the reliability of the estimates were evaluated.
Results: The average of estimated prevalence of chronic pain in neighborhoods of Tehran was 25.5% and a large heterogeneity was observed in its prevalence in neighborhoods of Tehran. Prevalence of chronic pain was significantly higher in married housewives, retirees and pensioners and was significantly associated with age, educational status, depression and anxiety (P<0.05). The reliability of Bayesian method was confirmed by evaluation methods in this analysis.
Conclusion: These results demonstrate prevailing amount of chronic pain at neighborhood-level in Tehran, which warrants careful attention to prevention, treatment, and rehabilitation by health care professionals.

Abstract

  Background & Objectives : Suicide is a conscious attempt to end one’s life. Evidence suggests that nearly one million people die due to suicide worldwide each year. The aim of present study was to describe the epidemiological and demographic data of suicide victims and related factors in Fars Province.

  Methods: In this cross-sectional study, the demographic and epidemiological data of suicide victims in Fars Province during the 5-year period from April 2007 to March 2011 was collected by a questionnaire. Finally, the data was statistically analyzed.

  Results : The victims were 934 cases. The mean and standard deviation of age for the cases was 32.12 ± 81.29 years. About 72.5% & 27.3% were men & women, respectively. The most common methods of suicide were hanging (n=472, 5.50%) for men and self burning (n=103, 16%) for women. Most of the people who committed suicide (n=411, 44%) were in the third decade of life (20 to 29 years). The most common etiology (53.5%) was unknown.

  Conclusion: Due to the high incidence of suicide in young people and unmarried individuals, authorities need to consider these groups in social planning in Fars Province.

Abstract

Background & Objectives: Keeping various species of reptiles as pets has become popular in Iran alongside other parts of the world. On the other hand, Salmonellosis is one of the most important zoonotic diseases and reptiles have been known as reservoirs of Salmonella. Therefore, this study was designed to assess Salmonella infection in reptile pets of Tehran.

Methods: Fecal samples were collected and cultured for Salmonella isolation from 270 reptiles referred to the specialized veterinary clinics in Tehran. Statistical analysis was conducted on the data.

Results: Salmonella was cultivated from 142 samples (52.6%). Salmonella isolates belonged to a variety of serogroups however, more than half of them belonged to serogroups B and C. Most tested reptiles were healthy and most owners were unaware of the risk of Salmonella. Possible contact of these animals with immune-compromised people was recorded in many cases.

Conclusion: The results of this study revealed that considering the fact that reptile pets are becoming more popular in Iran, educating reptile owners who are mostly unaware of the reptile’s safe keeping methods is a necessity. Finally, more studies are suggested to further investigate the role of reptiles in the epidemiology of human salmonellosis in Iran.

Abstract

  Background & Objectives : Multiple Sclerosis is a chronic progressive disease with several disabilities for patients. Its distribution in the world can be an important clue to study racial, genetic, and environmental factors thus, epidemiologic studies have been a very attractive concept for recent papers.

  Methods: We conducted this retrospective study to determine the prevalence of multiple sclerosis in different geographic areas of Fars (south of Iran). We first evaluated 11 large cities for the prevalence of the disease. In the second stage, we searched for five clinical subtypes of the disease (relapsing remitting, clinical isolated syndrome, progressive relapsing, primary progressive and secondary progressive) in both men and women.

  Results : From 2535 patients, 523 (20.6%) were men and 2012 (79.4%) were women. The prevalence of the disease in the total population of Fars was 54.3 / 100,000. Shiraz had the highest and Firoozabad had the lowest number of patients for each sex. The most common clinical subtype was relapsing remitting in each city, as well. The female-male ratio was 3.8 / 1.

  Conclusion: Fars is a medium to high risk area for multiple sclerosis and women are affected more than men. The most common clinical subtype of multiple sclerosis is relapsing remitting (71%) followed by clinical isolated syndrome (22%).

Abstract

Recently, capture-recapture studies have been used and researchers tend to use these studies in the health field. Therefore, we discussed the basic concepts of these studies. First, we described capture-recapture studies. Then, the important assumptions and calculations were presented according to the close population assumption. Statistical formulas were presented for two-capture methods and dependency between the two lists was discussed. Then, we addressed more than two capture methods.

Abstract

  Background and Objectives : Knowledge transition is an important issue in social epidemiology. Taking into account the importance of knowledge translation network among vulnerable young people in closed environments, this study was conducted to recognize and survey knowledge transition in school lessons. The study aimed at teenagers residing in orphanages in the city of Kerman, using social network analysis.

  Methods: The study samples of this cross-sectional study were all teenagers aged 12-18 years old residing in orphanages. Data was collected via a checklist. In order to study the structural cohesion of social networks and the position of teenagers, density and centrality indices were calculated. Local structure of networks was studied using the triad census method. Finally, the effects of independent variables on indegree index were investigated using a mixed model.

  Results : The mean density in knowledge transition was 0.34 (0.42 and 0.27 in girls and boys, respectively. (P=0.2)). Completely null and completely mutual triads were 17.1% and 11.2% in girls and 33.7% and 0.09% in boys, respectively. Indegree increased with an increase in age (P<0.001), the average of the scores in school exams (P=0.002), and studying non school materials (P=0.04).

  Conclusion: Age, average score of school exams, and studying non school materials were important factors in indegree. In general, density of social network was rather low. The rate of knowledge transition was small. Therefore, activities are required to enhance knowledge sharing and transition.

Abstract

Background and Objectives: Brucellosis is a zoonosis disease that is important in humans and animals. Since the disease is important in ranchers and veterinarians, we conducted this study to estimate the seroprevalence of the infection in these groups in Kerman.

Methods: In this descriptive and analytical cross-sectional study, 187 industrial and semi-industrial dairy personnel and veterinary network staff of Kerman in 2012 were recruited. Rose Bengal, Wright and 2-Mercaptoethanol were done on serum samples. Demographic and occupational characteristics of the participants were recorded. Data was analyzed using descriptive statistics and Poisson regression via the Stata software. Results: The prevalence of brucellosis was 3.2%. In this study, the history of brucellosis in the family, working in the veterinary network, and working in semi-industrial versus industrial dairy farms were the risk factors of the disease.

Conclusion: Although the prevalence of the infection was lower than other studies, it was still considerable in these groups therefore, it is necessary to improve preventive measures in these groups.
