Abstract

Background and Objective: Road traffic injuries are among the most important causes of death and disability in Iran, and the country has one of the highest prevalence of opioid drug use, especially among drivers. The effect of different situations related to opioid use needs great attention. The purpose of this study was to estimate the effect of driving in the withdrawal phase on the occurrence of traffic accidents leading to injury.
Materials and Methods: This is a Case-Crossover study on injured drivers of crashed motor vehicles in Kerman. Drivers having skipped one habitual drug dose within one hour of the driving session were considered as being in withdrawal. We compared the drivers' situation at the time of accident with their regular driving habits.
Results: Among 75 drivers who had history of regular use of opium, 15 were in withdrawal phase at the time of accident. The relative rate of occurrence of traffic injuries while driving in these circumstances was 2.67 (95% confidence interval: 1.52 - 4.68).
Conclusion: According to these findings we can conclude that habitual opioid users are at greater risk of traffic accidents while driving in withdrawal status this risk is more than two-fold relative to not being in withdrawal status.

Abstract

Background & Objectives: Considering the lack of adequate basic information on risk factors for road traffic injuries in Iran, a study was conducted to determine the association between potential risk factors and the incidence of injuries in motor vehicle drivers.
Methods: We performed a population-based case-control study on Qazvin-Loshan road. Risk factors related to injury incidence were compared between 175 cases and 175 controls. Cases were drivers recently injured in road accidents. Controls did not have any injury and were selected systematically while the study was being conducted. Injury was defined as trauma needing medical or surgical treatment. Information was obtained through police reports and interviews with cases and controls. Data were analyzed by two methods bivariate analysis (crude) and by a multivariate unconditional logistic regression model.
Results: In bivariate analysis, risk factors for road traffic injuries risk were failure to wear seat belts or protective helmets, ejection from vehicle, severity of collision, number of crashes, motorcycle vs. other motor vehicle accidents, fire, rain and fog, and collision with fixed objects. In the logistic model, factors such as seat belt or helmet use with OR= 0.619 (0.376-1.018), ejection from the vehicle with OR= 2.952 (1.235-7.056), severe collision with OR= 5.413 (2.865-10.224), motorcycle vs. other motor vehicle accident with OR= 3.164 (1.05-9.537) and raining and fog vs. good weather with OR= 4.562 (1.663-12.514) were found to influence injury incidence. There was an interaction between severity of collision and weather status.
Conclusions: Better speed control, use of seat belts or helmets, not using motorcycles for road travel and equipping vehicles with fire extinguishing capsules are recommended to reduce road accident injuries.

Abstract

Background & Objectives: In the chain of vehicles, human and environment, the human factors are the most complex element in the causes of road traffic injuries (RTIs). Unauthorized speeding and overtaking are considered as the main human factors resulting RTIs. This study estimated the population attributablefraction of two human factors, the most common risk factors of rural roads, of RTIs.
 Methods: To calculate the population attributable risk, the prevalence of unauthorized speeding and overtaking registered in Police data was employed. Effect size related to odds ratio was employed using Logistic regression. Joint effect of related risk factors also calculated and reported. Dasta were taken from national traffic police using the KAM 114 froms.
Results: The percentage of population attributable risk of unauthorized speeding for deaths at crash scene was 20.9 and for injured cases was 16.6. These values were 13.5 and 13.4 for unauthorized overtaking respectively. Corresponding for joint effects the population attributable risk was 31.6 for deaths and 27.8 for injuries.
 Conclusion: The results illustrate the importance of speed control and unauthorized overtaking as a priority of RTIs in Iran. It is recommended that related laws and legislations should be pursued more seriously, and followed by more effectively.

Abstract

  Background & Objectives : World Health Organization has recommended the main strategies for the prevention of road traffic injuries on some of the risk factors. This study aimed to prioritize the preventive interventions according to risk factors of road traffic injuries in Iran.

  Methods: The potential impact fraction (PIF) of the main strategies for prevention was analyzed from national traffic police data registry in 2011 in urban and rural roads. PIF was calculated for risk factors of road traffic injuries and joint effect at three counterfactual distributions: theoretical minimum risk level, plausible minimum risk level, and feasible minimum risk level. The priority setting of preventive intervention was performed based on contribution of each risk factor.

  Results : Based on the potential impact fraction of death (PIF calculated using odds ratio of death) the priorities were as follows in order: wearing a helmet by the motorcycle passenger, wearing a helmet by motorcycle rider, and lack of speed management. Risk factors in this study were more important in urban roads and joint effects of risk factors were close to 100 percent

  Conclusion: It is strongly recommended that future interventions focus on the use of the helmet by motorcycle passengers and riders, the use of the seat belt by the passengers, and speed management. Our findings indicated that laws and legislations should be pursued more seriously. Calculation of the potential impact fraction is better than the population attributable fraction and allows for more accurate prediction of the future.

Abstract

Background & Objectives: The number of road traffic accidents (RTAs) and their mortality are increasing in the world. In Iran, RTAs have caused life losses and economical damages. Death is the most important consequence of the accidents in terms of social, economic, and even political expenditures. This paper reports the statistics of the mortality status in the last decade.

Methods: A 10-year study was conducted on national and provincial data of the last decade collected by Legal Medicine Research Center. The population information provided by the National Center of Statistics was used. SPSS 15.5 and Microsoft Office Excel 2007 were used for data analysis.

Results: Between 2001 and 2010, the annual average of 34.6 per hundred thousand people were killed in traffic accidents, more than 80% of the casualties are men. The highest number of deaths and injuries was observed in 2005 and the lowest occurred in 2001. RTAs had a descending trend trough the last decade but the rate of injuries increased. The death rates in Semnan, Qazvin, and Kerman were the highest and in Tehran, Ardabil, and West Azarbaijan were the lowest.

Conclusion: Although mortality rates due to RTAs declined during the last four years of the study period, direct and indirect costs of deaths resulting from RTAs in Iran are a sign of crisis.

Abstract

Background and Objectives: Human errors are the most important factors for RTIs, which have been addressed in few quantitative studies. The objective of this study was to calculate the annual population attributable fraction (PAF) of the most important human risk factor for RTIs in the streets and highways of the Great Tehran.
 
Methods: In this study, the data of 124518 individuals registered in the COM114 form of the Police Information and Communication Technology (ICT- FAVA) data system of the country were employed in 2014. A regression model was applied to estimate the magnitude of the impact of traffic violations of drivers with RTIs using the adjusted relative risk ratio. To estimate the PAF, the prevalence and effect size were calculated based on the risk ratio (RR) using the baseline method or the Miettinen formula.
 
Results: The PAF of driving in reverse gear and failure to observe safe distance was 1.98% and 1.93%, respectively. Driving in the reverse gear and failure to observe the safe distance increased RTIs by 22% and 23%, respectively. On the highways of the Great Tehran, the largest proportion of PAF was related to driving in reverse gear (1.96%), sudden movement of the car (1.90%), speeding (1.87%). Speeding increased the risk of RTIs by 23%.
 
Conclusion: Monitoring traffic violations can decrease road crashes by 10% in the streets. Moreover, on highways, use of strategies to prevent traffic violations can decrease RTIs by up to 15%.

Abstract

Background and Objectives: Evidences show an increase in the occurrence of road crashes and causalities in early stages of economic growth. This study aimed to evaluate the correlation between the incidence of motorcyclists’ crashes and the level of development of the provinces of Iran in the year of 2015.
 
Methods: This was an ecological study of the motorcyclists’ crashes in all provinces of Iran. Motorcyclists’ injuries were obtained from the database of traffic police and the report of population census in 2011 was used. Given that more than 99% of motorcycle-related crashes occurred in male motorcyclists, the population of males was used to adjust the results. The Pearson correlation coefficient was also applied.
 
Results: Totally, 90,724 crashes occurred among motorcyclists. The highest rate was in the age group of 18 to 30 years with 46,944 crashes (51.7%). As for sex, men had 90363 crashes (99.6%). Among different provinces, Tehran and Ardabil had the highest and lowest incidence with 1129.84 and 77.37 per 100 thousand male populations, respectively. Significant correlations were found between the incidence of motorcycle crashes and Gross Domestic Product (GDP) (P value: 0.601) and Human Development Index (HDI) (P value 0.497), respectively (P<0.05).
 
Conclusion: Increased the incidence of motorcycle crashes is associated with the level of provincial economic development. In order to prevent the injuries and reducing its incidence, attention should be paid to the safety of roads and vehicles.

Abstract

Background and Objectives: This research was conducted to investigate the socioeconomic and geographical inequality in mortality from road traffic accidents in Iran in 2016.
 
Methods: In this descriptive cross-sectional study, the data of 16,584 people that died from road traffic accidents in 2016 were received from the Legal Medicine Organization. Theil entropy index was used to determine inequality in geographic areas. Moreover, relative and absolute concentration indices were used to measure inequality in mortality from RTAs across educational levels.
 
Results: The mortality rate from road traffic accident was 21.5 per 100,000 people. Theil index was 0.66 for traffic-related deaths among the provinces, indicating an unequal distribution of traffic injuries caused by traffic accidents among the provinces. When inequality was measured at smaller geographical levels, i.e., among cities in each province, the results indicated a fair distribution across smaller geographic levels. The focus index also indicated a fair distribution of traffic accidents among the deceased sub-classes (concentration index & 95% CI: -0.13 (-0.41; 0.16).
 
Conclusion: Our findings showed that the distribution of mortality from road traffic accidents was unequal at provincial positions. According to our findings, mortality from traffic accidents was distributed equally among the socioeconomic and urban levels. Therefore, health managers can use the findings of this study to develop interventions to reduce inequalities. In addition to targeting factors contributing to known social inequalities in the health and social status, other factors should be considered and applied to evaluate their interventions in the future.

Abstract

Background and Objectives: The aim of this study was to determine the predictability of the PLS-SEM model for injuries resulting in hospitalization in motorcyclists using a mediator variable in a case-control study.
 
Methods: In this case-control study, 300 cases and 156 controls were randomly selected from 150 clusters using random cluster sampling. The data were collected using the motorcycle riding behavior (MRB) questionnaire, adult attention deficit hyperactivity disorder (ADHD) questionnaire (subscales) and a checklist containing motorcycle related variables.
 
Results: The motorcycle riding behavior, adult ADHD, motorcycling related variables and some demographic variables were found to be the predictors of injury. There were significant positive relationships between injury and motorcycling related variables (B=0.20, P=0.001) and ADHD (B=0.33, P=0.001), between MRB and motorcycling related variables (B=0.51, P=0.001) and ADHD (B=0.52, P=0.001), and between ADHD and motorcycling related variables (B=0.39, P=0.001).
 
Conclusion: Considering the more accurate results of PLS-SEM, the intervention programs should especially address those who have hyperactive children, those who use the cellphone while riding, and those who ride in dark hours of the night.

Abstract

Background and Objectives: The Eastern Mediterranean Region (EMR) has the third highest road traffic fatality rate in the world. This article evaluates the epidemiology of road injuries and some of its main control factors in Iran and compares them with other countries of the EMR.
 
Methods: This study was conducted to analyze and compare traffic injuries resulting in mortality using secondary data. Data from 175 countries and 19 members of the EMR were analyzed in this study. A review study was designed to provide policy options to improve the status of the measures.
 
Results: Mortality from traffic injuries in the region according to road users was 39% for four-wheelers, 34% for pedestrians, 15% for two-wheelers, 2% for cyclists and 10% for other users. The traffic-related mortality rate was higher for users of four-wheel motorcycles and pedestrians in the EMR compared to the global average. Moreover, deaths from traffic injuries in users of four-wheel vehicles and two-wheelers in Iran were higher than the regional average.
 
Conclusion: Although Iran and EMR member states have implemented some important preventive measures recommended by the World Health Organization, significant efforts are still needed to optimize the enforcement of existing road safety laws. Sufficient attention should be paid to the needs of pedestrians, cyclists, and motorcyclists that comprise about 50% of all road traffic fatality.

Abstract

 
Background and Objectives: Violence and aggressive behaviors are major risk factors for traffic accidents. The drivers' attitudes and behaviors can also be influenced by personality traits. The purpose of this research was to identify the relationship between violent behaviors in driving and personality traits.
 
Methods: In this study, a 20-item researcher-made questionnaire was used to collect data related to violence and the revised 60-item Neo Personality Inventory was used to collect data about personality traits including extraversion, agreeableness, conscientiousness, neuroticism, and openness. Psychometric evaluation was performed for data collection tools from September to January 2018. Descriptive statistics and non-parametric statistical analysis including Mann-Whitney and Kruskal were used to analyze the data.
 
Results: In this study, 87.3% of the participants were male and 12.7% were female. The mean age of the participants was 42 years. The dominant personality trait was conscientiousness among the drivers (30.2%). The results showed a significant correlation between the degree of violence and neuroticism (P= 0.02). Based on regression analysis, 19% of violence could be explained based on the personality trait.
 
Conclusion: Neuroticism can potentially indicate driving behavior and the degree of violence. Therefore, it is essential to predict the drivers’ behaviors to improve road safety. It can provide useful information regarding road safety interventions for road users to target drivers who are more exposed to undesirable driving behaviors.

Abstract

Background and Objectives: Road accidents are a major public health problem and one of the leading causes of death in the world. Some survivors of traffic accidents experience acute stress disorder and post-traumatic stress disorder during weeks to months following the accident. These disorders can have disastrous effects on victims and their inability to return to normal living conditions.
 
Materials and Methods: This was a cross-sectional descriptive study. The Post-Traumatic Stress Disorder checklist (PCL5) was used to assess acute stress disorder and post-traumatic stress disorder. The sample size was 384 people and the data were collected one week and two months after the trauma in the orthopedic, emergency and inpatient wards of Taleghani, Shohadaye Tajrish and Haftom Tir hospitals. This study was conducted between October and January 2016.
 
Results: This study was conducted on 350 participants. About 49% of the participants (n = 172) met the diagnostic criteria for acute stress disorder and 40.6% (n = 142) met the diagnostic criteria for post-traumatic stress disorder. There was a relationship between demographic variables of age and death of a close family or friend and the severity of post-traumatic stress disorder.
 
Conclusion: According to the results, about half of the participants had acute stress disorder and met the diagnostic criteria of post-traumatic stress disorder. This result highlights the high prevalence of this disorder in traffic accident victims and the importance of its follow-up and control.

Abstract

Background and objectives: Considering the high contribution of unintentional accidents to mortality, particularly in males in Iran, and their changes over the last decade, a study was conducted to review the share of deaths due to unintentional accidents in sex difference in life expectancy at birth in the country over the last decades.

Methods: The mortality data for the years 2006 to 2015 were obtained from the Ministry of Health and Medical Education. Then, the child mortality was corrected using the intercensal generational method and adult mortality was corrected for underreporting based on the Bennett-Horiuchi method. The contribution of mortality changes due to unintentional accidents to increase in females’ life expectancy was calculated using the ARIAGA decomposition method.

Results: The sex differential in life expectancy was about 3.5 years in 2006 in Iran, which decreased to 2.9 years in 2011 and increased to 3 years in in 2015. The unintentional accidents in all three periods played a dominant role in sex differential mortality, but its contribution over time was significantly reduced, with a contribution of 60% to around 42% between 2006 and 2015.

Conclusion: Due to the slowdown in the increase in life expectancy at birth in Iran in recent decades, attention to differential mortality, including sex difference in mortality and identification of potentials to increase life expectancy according to different groups, can increase life expectancy.
