Abstract

Background & Objectives: Orthodontic treatment need and demand in 12-year-olds in Abadeh city has not previously been analysed in relation to geographic origin. The purpose of this study was to assess the12 year old students need and demand for orthodontic treatment.
Methods: Four hundred seventeen 12-year-old students was selected from public and private schools in Abade (Fars province, Iran). All the students were examined according to the AC and DHC component of Index of Orthodontic Treatment Need (IOTN) by a trained dentist. Students' and parents' perceived needs were also assessed using AC component and their demand for orthodontic treatment was asked through a questionair .
Results: Twenty two percent of the students were in "no need of treatment" group when assessed by DHC component, 29.5 % were in "average need" and 48.2% were in "definite need" group. When assessed by AC score, these percents were 61.9%, 29%, and 9.1%. Parents and students percieved need for definite orthodontic treatment according to AC score was 8.6% and 7.7%, respectively. The students and their parents’ demand for treatment were 40.6% and 44.9%, respectively.
Conclusion: Orthodontic treatment need in Abade is higher in comparison with other reports according to DHC. DHC is not correlated with orthodontic treatment demand of 12 years old students, but AC had a strong relationship with treatment demand.

Abstract

 

Background & Objectives: Cutaneous leishmaniasis (CL) is one the major health problem in tropical and sub– tropical countries. Bam district in Iran is an old focus of anthroponotic CL (ACL). The aim of this survey was to assess the prevalence rate of CL by case– finding, treatment of cases and training of primary school children in the city and suburb of Bam in 2010.

Methods: All school children in three levels (18501 children) were included. Children were physically examined for presence of sores or scars. Suspected cases were referred to the center for cutaneous leishmaniasis control (CCLC) for diagnosis and the CL positive children were treated, accordingly. School children were trained and also educational and informative pamphlets were given to their parents.

Results: After initial physical examination ,117 suspected school children (6.9%) were referred to the CCLC and 48 children (28 per 10000) diagnosed as CL patients, comprising of 19 females (39.5%) and 29 males (60.5%). Fifteen (31.2%) school children were identified as lupoid leishmaniasis, failure to treatment or non – healing cases.

Conclusion: The prevalence rate was decreased about 18– fold as compared to the previous investigation in 2005. Establishment of the CCLC, active case– finding and subsequent treatment, seem to be a successful model for controlling CL in the endemic countries.

Abstract

Background and Objectives: Missing data is a big challenge in the research. According to the type of the study and of the variables, different ways have been proposed to work with these data. This study compared five popular imputation approaches in addressing missing data in the questionnaires.

Methods: In this study, 500 questionnaires were used for self-medication in diabetic patients. Missing in the observations was artificially generated by random selection of questions and then deleting them. Five imputation ways included: 1) the mean of the questions, 2) the mean of the person, 3) the mode of the person, 4) linear regression, and 5) EM algorithm. For each method, the mean and standard deviation were compared with imputation. The Spearman correlation coefficient, the percentage of incorrectly classified and kappa statistic were also calculated.

Results: A kappa higher than 0.81 represented almost perfect agreement at 10% missingness. The EM algorithm showed the highest level of agreement with the results of actual data with a Kappa of 0.886. With increasing missingness to 30%, the EM algorithm and the mean of  the person showed a rather similar agreement with a Kappa of 0.697 and 0.687, respectively.

Conclusion: In this study, the EM algorithm was the most accurate method for handling missing data in all models. The mean of the person method is easy for handling missing data, especially for most non statisticians.

Abstract

Background and Objectives: Productivity and efficiency are the most important and the most common mechanisms of evaluation and measurement of the performance of an enterprise including the Social Security Organization. In the past decades, performance evaluation of various economic sectors has been attractive to researchers in different disciplines. Thus, the aim of this study was to measure the efficiency of the direct treatment section of treatment management units of Social Security Organization by data envelopment analysis method. 

Methods: This descriptive- analytic study was conducted to measure the efficiency of the direct treatment section of treatment management units of Social Security Organization through the data envelopment analysis (DEA) method in 2014. Data and relevant statistics were collected from the Statistical Center of Social Security Organization. The Deap2.1 software was used to calculate the efficiency and the EMS software was used to calculate the super efficiency. Then, the hypotheses of the research were studied using the Stata software.

Results: The average technical efficiency, managerial efficiency, and scale efficiency in 2014 was 0.924, 0.992, and 0.932, respectively. Twelve decision making units (DMUs) had the maximum technical efficiency (1), 16 DMUs had technical efficiency between 0.8 and 1, and 3 units had technical efficiency less than 0.8.

Conclusion: This study introduces a functional pattern to managers of Social Security Organization that enables them to have more accurate planning for the development and saving of resources.

Abstract

Background and Objectives: Regular blood transfusions in patients with hereditary hemolytic anemia, especially thalassemia, increase the survival of most patients but also may result in infection with viruses like hepatitis. In order to identify these diseases and other complications in people with thalassemia, this study aimed to investigate epidemiological and clinical findings in patients with thalassemia in Dezful in 2015.
Methods: In this descriptive-analytic study, data were collected from medical records in Dezful thalassemia Clinic by the census method. The collected data included gender, age, location, ethnicity, blood type, transfused blood type, time between blood transfusions, medications, vaccinations, underlying diseases, and operations and their time.
Results: The records of 174 thalassemia patients with a mean age of 23.60 years were evaluated. Of these, 23 patients (13.2%) had thalassemia intermediate and 151 (86.8%) had thalassemia major. The last mean serum ferritin was 2760.60. Independent T test showed a significant relationship between the type of thalassemia and age at diagnosis (P=0.000). There was a significant positive correlation between age and blood transfusion intervals so that the intervals increased with age (P=0.004) (r=0.21).
Conclusion: The high mean age in the center compared to other studies as well as the low frequency of side effects indicate the effectiveness of new treatments and increased survival of these patients.

Abstract

One of the traditional methods used for the analysis of survival data is the Cox regression technique. This method calculates the conditional risk ratio. However, when the aim of the study is to estimate the effect of exposure in the total population level, using these conditional methods is not apposite. Furthermore, the hazard ratio has disadvantages of its own such as being non-collapsible, having the risk of structural selection bias and variability in time. Given the limitations, it is recommended to use the marginal hazard ratio, which estimates the average causal effect of exposure in the total population level.
This study introduces the inverse probability treatment weighting (IPTW) as a method of estimating the marginal causal effect. Finally, to illustrate IPTW method, we used Golestan Cohort Study and estimated the marginal causal effect of smoking on time to death due to the upper gastrointestinal cancer (esophageal-gastric).
