Abstract
Background and Aim: Lichen planus (LP) is a cutaneous disease characterized by violaceus flat topped papules in variable size. It may involve body surface areas such as mocus membranes. Some studies have reported an association between oral LP ad diabetes mellitus. The aim of this study was to compare the frequency of diabetes mellitus between patients with and individuals without LP.
Methods: A case-control study was done on 30 patients suffering oral LP as case group and 60 healthy individuals as control group. Diagnosis of oral LP was confirmed by typical clinical and histopathologic findings. Participants in control group were selected from general population after matching for sex and age. Venipuncture was done to take 5 ml blood sample for measuring fasting serum blood glucose levels in both groups. A repeated blood sampling was done for ones with abnormal results in the first sampling. Findings of the study were analyzed using SPSS 16.0 statistical software.
Results: Thirty LP patients [man 13 (43.3%), woman 17 (56.7%)] with mean age (mean±stabdard deviation) of 46±13.7 years and 60 healthy individuals [man 26(43.3%), woman 34 (56.7%)] with mean ages of 46±14 years were recruited to this study. In case group only one person (3.3%) suffered from diabetes mellitus, but none of healthy group had fasting blood sugar above the normal value. In quantative evaluation, mean level of fasting blood sufar was 102.5±33 mg/dL in case group and 111.1±49.3 mg/dLin control group (P=0.09).

Abstract
Background and Aim: C-reactive protein (CRP) is an inflammatory biomarker and its level increases in serum of psoriatic patients. Its level is also associated with Psoriasis Area and Severity Index (PASI) score.Methods: CRP serum levels in patients with psoriasis were measured before and after treatment with narrow-band ultraviolet B (NB-UVB) and the data were analyzed in relation the PASI score improvement.
Results: Baseline CRP levels among psoriatic patients were higher than normal. These levels decreased significantly after treatment. At the beginning of the study, patients with higher levels of CRP also had more extensive and severe skin involvement. Highest decrease in CRP was observed in patients who responded better to the treatment and acheived higher PASI 75%. There was an association between baseline PASI scores and CRP levels.
Conclusion: Patients with moderate to severe plaque-type psoriasis had systemic inflammation too, which was demonstrated by having higher levels of CRP. Skin disease activity also was associated with higher levels of CRP. Phototherapy healed the psoriatic skin lesions and also reduced inflammation and decreased CRP levels.

Abstract
Background and Aim: In addition to the age and immune suppression, several risk factors for herpes zoster have been studied. The aim of this study was to evaluate the family history of herpes zoster as a risk factor for this disease.Methods: This case-control study was conducted at Farshchian hospital, Hamedan, Iran. The cases were patients with confirmed diagnosis of herpes zoster. The controls were chosen from individuals with minor or chronic dermatologic diseases or their companions, who had no history of herpes zoster. Immune deficiency was the main exclusion criteria. Information was asked using special questionnaires administered by blinded investigators. Data were analyzed using chi squared test. Odds ratios (OR) with 95% confidence interval (95% CI) were calculated to compare proportions in two groups.
Results: Case and control groups included 217 and 200 participants, respectively. The mean ± standard deviation of ages in case and control groups were 49.08±15.59 and 49.96±15.54 years, respectively (P=0.936). 53.5% of cases and 54.5% of controls were women (P=0.845). The frequency of herpes zoster in first-degree relatives in cases and controls was 65/217 (30%) and 16/200 (8%) respectively (OR [95%CI] = 4.91 [2.73-8.85]). Positive history for second degree relatives was 36 (16.6%) and 8 (4%) in cases and controls, respectively (OR [95%CI] = 4.77 [2.16-10.54]). This proportion for at least one relatives was 101 (46.6%) and 24 (12%) in cases and controls, respectively (OR [95%CI] = 6.26 [3.79-10.36]).
Conclusion: This study suggests that positive family history is a risk factor for herpes zoster.

Abstract
Background and Aim: Skin basal cell carcinoma (BCC) is the most common cancer in human beings. Prevalence of this tumor has increased over the past decades. The standard treatment is complete excision, however some risks for recurrence still exist. The aim of this study was to determine the frequency of margin involvement in skin BCC as well as to investigate the demographic characteristics of the patients.
Methods: This study was conducted from April 2011 to April 2012 on the patients who had skin lesions suspected for BCC. The patients were undergone elliptical excision surgeries for removing their suspected lesions with 5-millimeter margins of clinically normal skin in Farchian Hospital, Hamadan, Iran. Following data were collected and recorded: age, sex, anatomical location, duration of the disease, primary clinical diagnosis, histopathologic type, and margin involvement.
Results: In total, 125 lesions from 115 patients were investigated (70 males, 45 females). The range of the ages of the patients were from 32 to 93 years, with a mean of 64.6 years. The most common histopathological type was nodular type (61.0%). Twenty percent of the excised lesions showed margin involvement. Risk factors for incomplete excision were older age and anatomical locations (ears and peri-ocular lesions). There was no statistically significant differences between sex, histological type, primary clinical diagnosis, and duration of the disease. In almost all cases, the clinical and histopathological diagnoses were similar.
Conclusion: To avoid repeated surgeries and to reduce the risk of incomplete excision for lesions on high risk locations, particularly in elderly, it is recommended that BCCs be excised with wide margins or Mohs micrographic surgery.

Abstract
Background and Aim: Psoriasis is a common chronic inflammatory skin disease. The risk factors of metabolic syndrome have been reported with greater frequency in patients with psoriasis. In this study, we aimed to evaluate these risk factors and compare them with normal population, as the control group.
Methods: In this case-control study, 55 patients with psoriasis were studied from 2011 to 2012 and 55 matched (for sex and age) persons were considered as control group. Age, gender, body mass index, systolic and diastolic blood pressures, smoking, alcohol consumption, type of disease, and blood levels of triglyceride, total cholesterol, LDL, HDL, and fasting sugar were evaluated in both case and control groups and compared.
Results: Our findings showed that levels of TG (P=0.005,OR: 1.02, 95%CI: 1.0-1.34) and LDL (P=0.03, OR: 1.04, 95%CI: 1.0-1.09), the frequency of smoking (P=0.02, OR: 2.2, 95%CI: 1.1-3.2), and the mean of systolic blood pressure (P=0.004, OR: 1.07, 95%CI: 1.02-1.33) were significantly higher in psoriatic patients, compare with control group.
Conclusion: Higher frequency of hypertension and lipid abnormalities in psoriatic patients may enhance the risk of cardiovascular diseases in them. Therefore, we recommend to check all patients with psoriasis, regardless of their disease severity, for serum lipid profiles and blood pressure. In addition, we strongly recommend giving up smoking, as a part of psoriasis treatment.

Abstract
Background and Aim: Psoriasis vulgaris is a common chronic
dermatic disease impresses many aspects of patients’ life. Many clinical
researches focus on quality of life recently, to evaluate clinical
interventions and treatments. This study planned to recognize different aspects
of life in patients suffer psoriasis vulgaris.
Methods: This case-control study was performed on 100
patients suffer psoriasis vulgaris in 1391 year and 100 people who didn’t have
any specific disease as control group. Quality of life was measured by standard
quality of life questionnaire (SF-36) and data obtained were analyzed with chi
square and t tests.
Results: Average score of SF-36 in psoriasis patients group
65.05±15.51 was significantly less than control group 78.31±11.93 (P<0.001). Difference between patients and
control group was significant in all of aspects except physical and emotional
aspects (P<0.05).
Conclusion: According to the findings of this study, patients
with psoriasis vulgaris had a significantly lower quality of life, in
comparison with the control group. Therefore, these patients require special
attention and specific planning to improve and manage different aspects of
their lives.

Abstract
Background and Aim: Melasma is a common acquired disorder characterized by symmetric, hyperpigmented patches with an irregular outline, occurring most commonly on the face. The goal of this study was to evaluate the efficacy of a cream containing 4% licorice extract with a novel formulation based on solid lipid nanoparticles technology in the treatment of melasma. To the best of our knowledge, it is the first study designed to investigate the efficacy of this novel formulation in the treatment of melasma all over the world.
Methods: In this randomized, double-blind, placebo-controlled clinical trial, 44 women with clinical diagnosis of melasma referred to the Afzalipour hospital dermatology clinic were randomly allocated into two treatment groups of equal size. All the patients were interviewed and examined every four weeks during the trial and their Modified Melasma Area and Severity Index (MMASI) score were evaluated at 4, 8 and 12 weeks. The efficacy of the interventions were classified in four levels: complete response, significant response, partial response and no response.
Results: Forty patients were enrolled in the study. At the end of the study (12 weeks), mean± standard deviation of MMASI score changed from 11.03±2.7 to 1.41±0.6 in the intervention group and from 11.25±2.9 to 2.37±1.2 in the placebo group, respectively (P<0.001).
Conclusion: Licorice extract can be used as a skin-lightening agent with minimal side effect in the treatment of melasma. Nano/micro solid lipid particles are used as carriers with unique properties like size, surface electrical bar. Moreover, a large amount of the drug might be loaded to increase the efficacy and decrease the adverse events.

Abstract
Angiolymphoid hyperplasia with eosinophilia (ALHE) is a rare disease due to benign proliferation of dermal and subcutaneous capillaries. It is most commonly seen in young to middle-age females. Skin lesions include single or multiple red papules, plaques and nodules. It most commonly involves head and neck. Its etiology has not been determined. ALHE lesions are usually resistant to commonly suggested treatment methods including surgery. Recurrence of treated lesions is also common. Herein, we report a 38-year-old pregnant woman who presented with multiple red papules and nodules on her periauricular and occipital skin. Her lesions were confirmed to be ALHE after histopathological assessment.

Abstract
Background and Aim: Atopic dermatitis is a chronic inflammatory disease that is caused by an inflammatory process stimulated by Th2 cells. Acne vulgaris is a chronic inflammatory skin disease. It is associated with an increase in sebum secretion, unusual pilosebaceous keratinization and an increased inflammatory immune response. Propionibacterium acne can induce IL-17 production and Th1/Th17 response. The present study was designed to determine the prevalence of atopic dermatitis in patients with acne.
Methods: In this case-control study, the case group consisted of 75 individuals with acne and the control group consisted of 75 individuals without acne matched in age and gender. Both groups were interviewed for atopic dermatitis, asthma and allergic rhinitis symptoms in the past and present.
Results: Prevalence of atopic dermatitis at present in patients who had acne was significantly lower than those who did not have acne. There was no significant difference between case and control groups in frequency of atopic dermatitis in the past and frequency of asthma and allergic rhinitis in the present and past time.
Conclusion: The inverse relationship between prevalence of acne and atopic dermatitis can be related to activation of different immune responses (Th1 versus Th2), but more studies should be done to confirm this relationship.

Abstract
Background and Aim: Alopecia areata is an autoimmune disease of hair follicle. Osteopontin is an early T lymphocyte activator that may play a role in some immunological diseases. The aim of this study was to evaluate the serum level of osteopontin in patients with alopecia areata and compare it with the level in normal subjects.
Methods: 54 patients with alopecia areata who presented to the Razi Hospital in Tehran in 2017 and 2018 were enrolled. Half of the patients had severe disease and half of them had mild disease. Also, 52 healthy subjects were selected as control group (age and sex matched). The severity of disease was determined using SALT Score. Serum osteopontin levels were measured by ELISA method.
Results: The results showed that serum osteopontin levels were significantly higher in patients with alopecia areata than healthy subjects. There was no significant relationship between osteopontin level and SALT score or duration of the disease.
Conclusion: The level of osteopontin is higher in AA patients than normal subjects but it does not correlate with the severity of disease.

Abstract
Background and aim: As the use of botulinum toxin to treat glabellar lines increases, particularly in Iran, where foreign products (e.g. Dysport) are relatively expensive, it becomes important to compare the efficacy, side effects, and duration of action of two locally produced botulinum toxin brands, Masport® and Dyston®. This study seeks to assess and compare the treatment results of these two medications.
Methods: In this cross-sectional, descriptive, retrospective study, the medical records of 81 patients who visited Razi Dermatology Hospital for the treatment of moderate to severe glabellar lines were reviewed. Data related to patient satisfaction, duration of action, and side effects of Masport® and Dyston® botulinum toxin were collected and analyzed via a questionnaire.
Results: The mean satisfaction score was 7 for the Dyston® group and 7.7 for the Masport® group, with no statistically significant difference between the two groups (P=0.142). Additionally, the mean duration of action was 3.5 months for Dyston® and 3.8 months for Masport®, with no significant difference observed. Headache was the most common side effect reported, with no other major adverse events noted. Furthermore, there was no statistically significant difference in the incidence of side effects between the two groups.
Conclusion: The findings of this study indicate that both Dyston® and Masport® have similar efficacy and safety profiles. The choice between them can be primarily based on cost and patient or physician preference rather than any significant therapeutic differences. Larger studies are recommended to further validate these findings.

Abstract
Background and aim: Pityriasis lichenoides (PL) is a rare papulosquamous disorder with an unknown etiology. Due to limited recent studies on this subject, the present research was conducted to describe the clinical manifestations and treatment course of these patients.
Methods: This retrospective cross-sectional study (2019-2023) was conducted on patients diagnosed with PL at Razi Dermatology Hospital, Tehran University of Medical Sciences, Iran confirmed by biopsy of skin lesions. Basic information was obtained from hospital records, and treatment follow-up was gathered from medical files and phone interviews with patients. Data were analyzed using R software, with a P<0.05 considered statistically significant.
Results: A total of 147 patients were evaluated. No significant difference was found between genders (P=0.358). The predominant subtype was PLC (91.8%), with a mean age of onset of 29.79 ± 18.73 years. The most common type of skin lesion was papules (48.4%), and generalized body involvement was the most frequent manifestation, observed in 58 patients (32.4%). The most commonly used treatment was topical corticosteroids, administered to 86 patients (52.4%). The average duration from onset to full recovery was 160.80±221.40 days. A total of 45 patients (30.6%) reported recurrent episodes of the disease. No significant association was found between treatment efficacy and gender (P=0.620), underlying conditions (P=0.293), or age (P=0.876).
Conclusion: Given the limited clinical data on patients with pityriasis lichenoides, establishing an electronic database and a national registry system for this condition is recommended.

Abstract
Background and Aim: Angiokeratomas are rare vascular lesions, typically presenting as small red to purple macules or papules on the skin. These lesions are usually benign, though they can undergo changes in thickness and color over time. Certain types of angiokeratomas may also signal underlying metabolic disorders, necessitating careful clinical attention. This study aims to analyze the clinical and demographic features of angiokeratoma in Iranian patients at Razi Dermatology Hospital.
Methods: This descriptive, retrospective cross-sectional study examined 199 patients diagnosed with angiokeratoma between 2013 and 2023. Data on demographics, lesion characteristics (location and size), family history, and treatment methods were collected from medical records and analyzed using SPSS. Both parametric and non-parametric statistical tests were applied to assess associations, with a significance level set at 0.05.
Results: The mean age of patients was 24.75 years, with the second and third decades showing the highest prevalence. Among the patients, 51.3% were male and 48.7% female. Lesions most commonly appeared on the lower extremities (40.7%) and genital area (18.6%). Treatment methods included cryotherapy (68.14%), laser (27.14%), and electrodessication (4.44%), with cryotherapy proving the most effective among them.
Conclusion: This study provides valuable insights into the prevalence, clinical characteristics, and preferred treatment approaches for angiokeratoma in Iranian patients. Broader studies across various centers, along with long-term follow-up, are recommended to further investigate the clinical course, therapeutic outcomes, and potential complications associated with different treatment methods.
