Abstract

Pseudo-Kaposi sarcoma (PKS) or acroangiodermatitis is a benign angioproliferative disease which shares many clinical and histopathological features with Kaposi sarcoma. Clinical presentations include violaceous macules, papules or plaques in the lower extrimities, which is usually associated with chronic venous insufficiency or congenital or acquired arterio-venous malformations. Its pathogenesis is not well recognized yet but the ischemia due to venous insufficiency or arterio-venous malformation is considered to play a role. The diagnosis is made through the histopathologic examination of biopsy specimens. Different treatment modalities have been suggested including conservative treatment or correction of vascular abnormalities.This paper introduces a 28-year old man with a brown linear plaque in a scar of 3-month duration due to trauma on the anterior of left thigh. A biopsy was performed and the histopathologic examination confirmed the clinical diagnosis of Pseudo-kaposi sarcoma.

Abstract

Background and Aim: Anti-inflammatory and hypo- pigmenting effects of topical tretinoin and alpha-hydroxy acids have been documented. This study was designed to compare the efficacy of 0.05% tretinion cream with 7% alpha-hydroxy acid in the treatment of ephelides.
Methods: In this clinical trial 80 patients with ephelides were selected and allocated into two groups. One group was treated with 0.05% tretinoin cream every other night. The other group was treated with 7% alpha-hydroxy acid cream in the same manner. Both groups were treated with antisolar creams and followed every month for 5 months.
Results: After 5 months excellent therapeutic response was achieved in 3 (7.5%) of patients who were treated with 0.05% tretinoin cream compared to 8 (20%) in alpha-hydroxy acid cream. Good therapeutic response was observed in 20 (50%) of tretinoin group versus 16 (40%) in alpha-hydroxy acid group. Patient's satisfaction from treatment was consistent with evaluation by dermatologists (P=0.384 and P=0.132).
Conclusion: Both 0.05% tretinoin cream and 7% alpha-hydroxy acid cream are equally efficient in the treatment of ephelides.

Abstract

Background and Aim: Acne vulgaris is a common disorder of adolescence which may lead to significant cosmetic and psychological problems. Different topical and systemic therapeutic modalities have been used to treat acne vulgaris. This study was designed to compare the efficacy of topical 0.05% isotretinoin gel with 1% clindamycin solution in the treatment of acne vulgaris.
Methods: In this randomized clinical trial, 60 patients with mild to moderate papulopustular acne vulgaris were randomly allocated into two groups of equal sizes. One group was treated with 0.05% isotretinoin gel every night. The other group was treated with 1% clindamycin solution twice daily. Both groups were followed every month for 3 months.
Results: The mean of total lesion count decreased significantly after treatment in comparison with its mean before the treatment in both groups (P<0.001). The mean of acne severity index significantly reduced after treatment in both groups (P<0.001). Satisfaction of patients in clindamycin group was higher (P<0.001).
Conclusion: Decrease in total lesion count and acne severity index in both treatment groups were statistically significant however, considering the minimal complications and satisfaction of patients in clindamycin group, 1% solution of clindamycin may be recommended in the treatment of papulopustular of acne vulgaris.

Abstract

Ichthyosis is defined as a group of diseases with keratinization disorder and diffuse scaling with highly variable degree of involvement. According to our knowledge, coincidence of ichthyosis and dermatophytosis, which both are very common disorders, is a very rare event. We report a young man with congenital ichthyosis that histological analysis of his skin biopsies and direct smear revealed PAS positive fungi. He had used topical steroids for generalized scaling erythematous patches and plaques for long time.

Abstract

Despite the large number of patients presenting with hirsutism and high prevalence of polycystic ovary syndrome, for which hirsutism is an important sign, no national guidelines are currently available to Iranian primary-care and family physicians for its diagnosis and management. We have integrated highest levels of evidence and expert consensus to provide a national guideline to standardize and unify the Iranian primary-care and family physician’s approach to hirsutism.

Abstract

Although ultraviolet rays constitute a small portion of the sunlight, they are destructive on skin. Ultraviolet rays cause sunburn, skin damage and skin cancers. Sunscreens are one of the most widely used cosmetic products which protect skin from cancer or other damages. There are two categories of sunscreen agents: chemical and physical. Physical sunscreens containing zinc oxide work through two mechanisms of reflection and scattering. Physical sunscreens are non-allergenic, non-irritating and non-comedogenic. Currently, by using zinc oxide nanoparticles in sunscreen formulation, both prevention of harmful sun rays and receiving cosmetic benefits are achieved.

Abstract

Background and Aim: Vitiligo is an skin condition that results in the skin depigmentation. The implementation of a basal cell layer suspension is a novel treatment in cases with recalcitrant vitiligo. In this study, the efficacy of topical calcipotriol on the implementation site with basal cell layer suspension versus basal cell layer suspension application alone was compared.

Methods: In this randomized, controlled clinical trial, 20 patients with recalcitrant vitiligo were randomly allocated in two 10-patient groups, undergoing the implementation of basal cell layer suspension alone (controls) or along with daily application of topical calcipotriol (intervention group). Results and possible complications were compared between the two groups 6 months after initiation of the treatment.

Results: Six months after initiation of the study, 2 patients (20%) in the control group had poor responses, and 8 patients (80%) had moderate/good responses. In the intervention group, 1 patient (10%) showed poor response, and 9 patients (90%) showed moderate/good responses (P>0.05). Complications were trivial and self-limiting.

Conclusion: Although topical application of calcipotriol did not signigicantly change the primary outcome of the study, it was associated with better results and no major side effects.

Abstract

Background and Aim: Dermatophytoses are the infections caused by the fungi that are capable to invade and multiply within keratinized tissues such as the skin, hair and nails. Topical azoles are considered the first-line treatment in dermatophytosis, but systemic therapy may also be required in some cases. Fungal skin infections have become more common in recent years, and injudicious use of conventional drugs has led to emergence of resistant species. This study compared the therapeutic effects of topical colloidal silver with topical clotrimazole in the treatment of  tinea capitis and tinea corporis.

Methods: In this double blind, non-randomized clinical trial, 40 patients with tinea capitis and tinea corporis were allocated into two groups, receiving either topical colloidal silver or topical clotrimazole, twice daily for at least 4 weeks. Systemic antifungals were also administreerd if needed. The response to treatment (negative KOH smear and resolved symptoms) was compared between the two groups at the end of the treatment period.

Results: The mean response time was 5.26±1.56 (range: 3-8)  weeks in colloidal silver receivers and 5.00±2.20 (range: 3-12) weeks in the clotrimazole group (P=0.67).  The response rates were 84.2% in the topical colloidal silver group, and 90.0% in the topical clotrimazole group (P=0.66). None of the patients experienced a side effect.

Conclusion: Topical colloidal silver may be as effective and safe as topical clotrimazole in patients with tinea capitis and tinea corporis. Further studies with larger sample sizes are recommended.

Abstract

Background and Aim: Acne vulgaris is a chronic inflammatory disease of the pilosebaceous unit. Retinoids are used as the first line therapy in acne. Systemic absorption and skin irritation including redness, pruritus and exfoliation are some of their side effects. In this study we compared the efficacy of isotretinoin 0.05% niosomal gel versus adapalen 0.1% gel in treatment of acne vulgaris.

Methods: In this randomized double blind clinical trial, 144 patients with mild to moderate acne vulgaris aged from 15 to 30 years were enrolled. The patients were randomly allocated into two groups of A, whom were treated with isotretinoin 0.05% niosomal gel and B whom were treated with adapalen 0.1% gel. The patients were evaluated for clinical reponse, decrease in the number of inflammatory and non-inflammatory lesions and side effects after 2, 4, 8, and 12 weeks of the initiation of treatment.

Results: At the end of the study the clinical responses of comedones and inflammatory lesions in isotretinoin 0.05% niosomal gel group were 68% and 79%, in comparison with 65% and 76% in the adapalen gel group,  respectively. The differences were not statistically significant. At the end of the treatment period more decrease in number of inflammatory and non-inflammatory lesions were seen in group A patients (niosomal isotretinoin) than group B (adapalen 0.1% gel) (P<0.001). Also side effect of skin redness was found more in group B (P<0.001).

Conclusion: Isotretinoin 0.05% niosomal gel has less side effects and more efficacy in the treatment of mild to moderate acne vulgaris in comparison with adapalen 0.1% gel.

Abstract

Nowadays, in modern medicine, there is a great tendency to use non invasive or less invasive diagnostic and therapeutic methods with high precision and effectiveness. This is also true about diagnosis and treatment of non-melanoma skin cancers (NMSCs), that the prototypes are basal cell carcinoma (BCC) and squamous cell carcinoma (SCC) which also considered as the most common human cancers. Therefore, in this review, we present the value and diagnostic features of the dermoscopy (as an emerging and growing non-invasive dermatological evaluation tool) for approaching to NMSC. It is hoped that this study will be an infrastructure for the ever-increasing use of dermoscopy in evaluation of skin cancers, especially NMSCs.

Abstract

Notalgia paresthetica (NP) is a neuropathic complication which occurs within the innervating field of T2-T6 spinal nerves. The patient complains of burning and itching sensation in the lower part of the inner margin of the scapula, and because of frequent itching,signs of skinscratching and hyperpigmentation are observed in the affected area. The definite cause of this pathology is unclear. Available resources have suggested the followings as the possible causes of the disorder: entrapment of the sensory branches within surrounding muscles, postural abnormalities such as scoliosis and kyphosis and degenerative changes of the spine. NP is usually represented as a rare pathology, however, the relatively common clinical incidence suggests a lack of attention to appropriateidentification, diagnosis and treatment ofthe patients. Diagnosis is done through history taking and physical examination, and there is often no need to
additional studiesfor confirmation. A wide spectrum of treatment options have been recommended. Although there is often a tendency to prescribe topical and oral medications for treatment of NP, but application of non-pharmacological therapies such as therapeutic exercises to correct abnormal postures, physical agent modalities such as electrical stimulation and also acupuncture can be effective in accelerating the treatment process and increasing the duration of recovery. Given the undeniable importance of the disorder and its impact on patient's quality of life, a comprehensive approach to existing treatment options with an emphasis on the role of physical medicine and rehabilitation specialists will be discussed in this paper.

Abstract

Rosacea is an inflammatory, chronic and cosmetically annoying common dermatologic disorder which presents with cutaneous telangiectasia, sustained erythema with courses of flares and flushings, papulopustular lesions, ocular involvement or disfigurating phymas. Although there are many relative effective medical therapies, however there are not any promissable treatment. Due to chronic nature of disease and exhausting usage of topical or systemic therapies , we decided to review Pubmed’s literature for non-medical physical therapies, looking for more effective modalities and minimal side effects, particularly in intractable cases.

Abstract

Probiotics are living microorganisms that, when used adequately, have beneficial effects on the host, and their superiority is that they are safe and secure for the host. The use of probiotics in addition to the digestive tract, has been studied in immune function, skin diseases, diabetes, cancer, liver disease, hypertension, urogenital system and oral cavity. In general, probiotics seem to be a promising and safe treatment. Nevertheless, more extensive studies are needed to use probiotics as alternative treatments along with old treatments. In dermatology, there is also a widespread use of probiotics and prebiotics. The use of probiotics has been studied in the treatment of atopic dermatitis, acne, eczema, allergic diseases, skin aging, bacterial and fungal infections, chronic wound healing, including diabetic foot ulcers, with promising results. In this review, papers related to the use of probiotics in the treatment of skin diseases were searched in data bases of Pubmed, Google scholar and Medline. Relevant papers were selected and presented. It is expected that probiotics will be used as complimentary treatment in several skin disorders.

Abstract

Acupuncture is an ancient medical procedure that is commonly used. It has been used in China for over two thousand years. Acupuncture has gained increasing interest from the public as well as health professionals for a wide range of disorders. This method works through regulation of qi current (life energy in body). Acupuncture has been introduced recently in dermatology for rejuvenation and treatment of facial wrinkles and improving muscle tone and elasticity. Acupuncture is used for treatment of many medical conditions including skin disorders. Tiny, sterile, needles are inserted along meridian lines and acupuncture points to increase blood circulation. Cosmetic acupuncture is a totally natural and holistic anti-ageing treatment that aims to reduce fine lines and wrinkles. Different mechanisms have been proposed for cosmetic acupuncture, including increasing muscle elasticity, and restoring muscle tone. There have been also evidence of increase in skin moisture and lipid.
 In this method, body points and local facial points are used and usually 10-12 sessions of facial acupunctures are recommended. With the increasing popularity of complementary therapies in the community, acupuncture is also considered as a new therapeutic approach.

Abstract

Background and Aim: Pruritus is one of the most common problems of chronic kidney diseases. Different treatment modalities, while extending the life span of patients, can lead to changes in their lifestyle, health status and ultimately change their quality of life (QOL). Understanding the different aspects of patients' QOL leads to better services provided by health care teams to improve the patients' QOL. The aim of this study was to evaluate the quality of life of hemodialysis patients with uremic pruritus in Mashhad in 1396 with a view to Iranian Medicine.

Methods: This descriptive-analytical study was performed on 52 patients. Data collection was done by the World Health Organization QOL and researcher-made demographic Questionnaire and SPSS 16 software was used for data analysis.

Results: The mean age of patients was 51.73 years and mean duration of dialysis was 41.42 months. Total score of life quality was 56. The lowest and highest scores were seen in physical and environmental dimensions. There was an inverse significant relationship between some aspects of QOL with hemoglobin and creatinine and a direct significant relationship with age, diabetes, hypertension and hyperlipidemia (P<0.005).

Conclusion: Hemodialysis patients' QOL is lower than others and several factors can improve it. Special attention to the individual characteristics of hemodialysis patients can be helpful in predicting their basic needs.

Abstract

Introduction: Onycholemmal carcinoma is a rare slow-growing malignant epithelial neoplasm originating from nail bed epithelium. Clinical presentations are nonspecific and include warty mass, ulceration, and crust, onycholysis, periungual erythema, edema, and pain.

Case Report: Herein, we report a 65-year-old woman with a mass lesion in the second finger of right hand that lasted for 3 years, which finally managed by finger amputation. Histopathology of the tumor revealed solid islands of atypical keratinocytes with abrupt keratinization without granular layer formation. Immunohistochemistry for epithelial and cell proliferation markers was performed.

Conclusion: Malignant epithelial neoplasms of the nail should be considered in long-standing nail problems and appropriate nail biopsy should be performed.

Abstract

Post herpetic neuralgia is a chronic neuropathic pain syndrome that it is characterized by pain in the area of ​​the nerve involved distribution after the disappearance of herpes zoster lesions and it may be persistent, refractory and severe. Various treatments including medication, Botox injections, nerve block and physical modalities have been used in the treatment of this syndrome, each has advantages and disadvantages. One of the most widely used methods today is the use of acupuncture. Acupuncture is a low-complication and reproducible treatment that is gaining more and more place in pain management, especially chronic pain. Studies have shown the effect of acupuncture on reducing pain in post-herpetic neuralgia patients. Different parts of acupuncture can be used to reduce pain. The recommended number of sessions is usually 10 to 12 sessions, lasting 20 to 30 minutes. Due to the beneficial effects of acupuncture and its low complication, this method is recommended as one of the treatment methods for postherpetic neuralgia. In this paper, we review the effect of acupuncture on post herpetic neuralgia.
